ABSTRACT Renal manifestations of plasma cell dyscrasias (PCD) are common. Most patients with multiple myeloma, for example, develop abnormal levels of proteinuria, and nearly one-half develop renal failure, a major cause of morbidity and mortality in these patients. The manifestations, causes, and treatment of renal disease associated with PCD are reviewed. Abnormal light chain production appears to be an especially important factor causing renal involvement in the setting of PCD, although not all light chains have demonstrable nephrotoxic properties. Renal failure, the most serious form of renal involvement in patients with PCD, should be managed aggressively, because even severe renal failure is reversible in some cases and need not preclude prolonged survival.
