Hypertension and Diabetes Mellitus in Rural Haiti

Introduction: The problem of the prevailing occurrence of hypertension and diabetes cases quickly took the world by storm. Rural areas became even more exposed to the negative issues associated with these health conditions due to the lack of a skilled workforce and educational programs for patients. Within the framework of the current paper, the researcher utilized the data from two mission trips in Thomonde, Haiti, to gain more insight into chronic health issues among rural Haitian residents. Methods: This study employed a retrospective analytical cross-sectional retrospective, descriptive design utilizing data obtained from two mission trips in Thomonde, Haiti in the areas of Palmay, Savane Plate, and La Hoye. The information was analyzed with the help of the SPSS software. A total of 403 adult patient visits across the three sites and two visits were included in the analysis. Results: Gastroesophageal reflux disease (GERD) (25.8%) was the most common diagnosis found in the rural Haitian communes among adults in the total sample (N=403), followed by Hypertension (16.8%) and Dehydration (13.9%). Among women, significant differences were found by location for pregnancy. The greatest number of pregnancies were observed in La Hoye (19.0%). Conclusion: The core implication of these findings was the significance of disseminating knowledge across rural areas while conducting similar retrospective studies to check progress. Real-life application of relevant knowledge could be beneficial for both patients and care providers operating in rural locations that are the hardest to reach.

Commentary on the harm reduction reference group of justice health and forensic mental health network and corrective services NSW, Australia.

This commentary aims to reveal how a steering committee has effectively responded to advancing accessibility to harm reduction resources, hepatitis C virus (HCV) policy and health strategies within adult prison settings in New South Wales (NSW). By reviewing the audit approach taken by the of the Justice Health and Forensic Mental Health Network and Corrective Services New South Wales Harm Reduction Reference Group (JHFMHN/CSNSW HRRG), this commentary emphasizes the committee's success in identifying contemporary harm reduction issues that affect people in custodial settings. This commentary is a compilation of data gathered through the 2018 JHFMHN/CSNSW HRRG audit and corresponding program materials. Conclusions regarding the effectiveness of the working group's audit were drawn by critically appraising the JHFMHN/CSNSW HRRG's Final Audit Report (JHFMHN and CSNSW, 2018) with reference to current harm reduction literature. The HRRG has provided leadership, professional representation and strategic advice on the development, implementation, monitoring and evaluation of best practice harm reduction strategies in prison settings. The HRRG developed and maintained networks and information exchange between the state-wide HCV health network, corrections services and the NSW harm reduction sector at large. Public health partnerships and advocacy that involve all key players, such as the HRRG, will continue to be crucial to remove barriers to enhancing HCV harm reduction measures especially in NSW prison settings. Strategies such as primary prevention and treatment can mitigate the spread of HCV in the custodial system. This audit of access to harm reduction resources was conducted on behalf of the diverse group of professionals, scholars and stakeholders comprising the HRRG. This audit and other advocacy efforts of this committee can facilitate future access to quality healthcare and the necessary policies required to support a healthier prison population at large. Collaborating with health authorities, researchers and social service workers can enable prison health-care systems to be guided by wider health workforce programs and public health standards. This collaboration can reduce the professional isolation of custodial health-care staff and promote a balanced approach to harm reduction policies by ensuring an equitable focus on both health and security imperatives.

Facing COVID-19 in Liberia: Adaptations of the Resilient and Responsive Health Systems Initiative.

The 5-year Resilient and Responsive Health Systems (RRHS)-Liberia Initiative, funded by PEPFAR via HRSA, launched in 2017 and was designed to support the implementation of Liberia’s National Health Workforce Program as a means to improving HIV-related health outcomes. The COVID-19 pandemic, arrived in Liberia just five years after Ebola and during RRHS-Liberia’s fourth year, impacted educational programs and threatened the project’s continued work. This paper presents the challenges that the COVID-19 pandemic posed to the RRHS partners, as well as adaptations they made to maintain progress towards project goals: 1) contributing to Liberia’s 95-95-95 HIV targets via direct service delivery, and 2) building a resilient and responsive health workforce in Liberia via instruction and training. Direct health service impacts included decreased patient volumes and understaffing; adaptations included development of and trainings on safety protocols, provision of telehealth services, and community health worker involvement. Instruction and training impacts included suspension of in-person teaching and learning; adaptations included utilization of multiple online learning and virtual conferencing tools, and increasing clinical didactics in lieu of bedside mentorship. The RRHS team recommends that these adaptations be continued with significant investment in technology, IT support, and training, as well as close coordination among partner institutions. Ultimately, the RRHS Liberia consortium and its partners made significant strides in response to ensuring ongoing education during the pandemic, an experience that will inform continued service delivery, teaching, and learning in Liberia.

HRSA’s PEPFAR-Supported Resilient and Responsive Health Systems Initiative in Liberia

During the 2014-16 Ebola outbreak, Liberia suffered 2,544 deaths, including 8% of its doctors, nurses and midwives. The Government responded in 2016 with a seven–year Health Workforce Program (HWP), the goal of which was “the creation of a fit-for-purpose, motivated, and highly-skilled workforce.” The US Health Resources and Services Administration (HRSA) with the support from the US President’s Emergency Plan for AIDS Relief (PEPFAR), established Resilient and Responsive Health Systems project to assist Liberia in strengthening health workforce and health systems. As the HWP approaches its final year, progress the numbers and skills of physicians, nurses and midwives have improved, through national and global support, and there have been some improvements in the overall health system. Improving health for everyone in Liberia is an ongoing process that requires continuing support and collaboration from national and global partners including US government agencies, UN agencies, academic and training institutions, private foundations, regional networks, and especially the people of Liberia.

Liberia's First Health Workforce Program Strategy: Reflections and Lessons Learned.

Following civil war and the Ebola epidemic, Liberia’s health workforce was devastated, essential health services and primary care were disrupted, and health outcomes for maternal and child mortality were amongst the worst in the world. To reverse these trends, the government of Liberia developed the Health Workforce Program (HWP) Strategy 2015–2021. With the goal of building a resilient and responsive health system to ensure access to essential services and the ability to respond to future crises, this strategy aimed to add 6,000 new professionals to the workforce. In the context of the COVID-19 pandemic, we share lessons learned from the program’s development and first years of implementation.

The Workforce Innovation and Opportunity Act: Supporting “Successful” Transitions into Adulthood

Completing education, obtaining employment, and securing an independent residence, tasks tied to an ideal of self-sufficiency are viewed as important hallmarks of adulthood. Yet, not all youth and young adults (YYA) have adequate resources and many require assistance to overcome transition barriers including lack of social support, poverty, and homelessness. This study analyzed the policy design and social justice values (equity, equality, and adequacy) of three federal workforce programs, authorized by the Workforce Innovation and Opportunity Act (WIOA), and designed to increase economic self-sufficiency among disconnected YYA. These programs lack stable and adequate funding and currently serve a small fraction of the YYA who could benefit. A value of equity is evident in the eligibility and financing criteria, while adequacy as it relates to programming—particularly benefits and services offered to address basic needs, economic stability, and healthy development—warrants further investigation. The programs’ focus on self-sufficiency through connection to education, training, and employment ignores research suggesting that at least six other dimensions are crucial for a successful transition to adulthood. Research on program entry and exit, primary services/benefits received, and life experiences of program participants is needed to determine current levels of support and the potential of these programs to enhance other components of a healthy transition including health, community engagement, and social support among disconnected YYA. Such research may also need to be focused regionally, given the highly localized control of programs.

Occupational Health Protection for Health Workers in China With Lessons Learned From the UK: Qualitative Interview and Policy Analysis.

BackgroundHealthcare settings have been recognized among the most hazardous places to work. Based on the five categories of occupational hazards that were identified by the ILO and WHO, this study aimed to analyze policy framework relevant to occupational health protection of health workers (HWs) in public health service in China, then discussed how to share the experience of the National Health Service (NHS) England for improvement.MethodsBased on policy learning theories, policy analysis and qualitative interview were used in this study.ResultsIn the Chinese public health service, at least five laws related to the regulation of occupational health protection for HWs; however, enforcement of relevant laws was separated and multi-centered; the national monitoring system, which targeted to occupational hazards and health outcome for HWs in China, had yet to be developed; the top three priorities were workplace violence, bloodborne pathogens, and musculoskeletal disorders; national strategies included Security Hospital, and Healthy China 2030. In NHS England, three laws were fundamental; several monitoring systems had been set up, including NHS Staff Survey, Commissioning for Quality and Innovation incentive scheme; mental health, musculoskeletal problem, and nutrition disorder and overweight were raised great concern; Health and Safety, and NHS Healthy Workforce Program were critical nationwide strategies.ConclusionThere were several similarities as well as differences between the Chinese public health system and NHS England, which laid foundation of learning by China. Recommendations of improving occupational health policies in China were provided, based on the lessons learned from the NHS England.

Rapid Realist Review of School-Based Physical Activity Interventions in 7- to 11-Year-Old Children.

Meta-analysis of physical activity interventions in school settings have revealed low efficacy and that there is a need to explore implementation fidelity. The aim of this rapid realist review was to determine, what physical activity interventions in school settings for children aged 7- to 11-years-old works, for whom, and in what circumstances. The realist synthesis was conducted following RAMESES guidelines. Relevant studies were identified following a systematic search process and data from 28 studies was extracted for evidence to form context-mechanism-outcome configurations that were clustered and refined. Using the five-level socioecological model, the program theories were classified into the levels of intrapersonal (child), interpersonal (teachers), institutional (program content, school administration, and school environment), community (home and neighborhood), and policy. The school level led to most context-mechanism-outcome configurations related to school leadership and policy, workforce structure, program characteristics, and school environment. At each level, we identified features of interventions, alongside implementation considerations that might work to promote efficacy and sustainability. The need to recognize the school environment as part of a complex system with multi-level interaction and influences was a key finding. In line with realist philosophy, the researchers encouraged primary research to confirm, refute, and refine the program theories presented.

Improving Geropsychology Competencies of Veterans Affairs Psychologists.

Older adults are more likely to seek mental health care through integrated care settings such as primary care. Currently, there exists a significant shortage of mental health providers trained in geropsychology and integrated care competencies. To address this need within the Veterans Health Administration, a national workforce development program was extended to include psychologists, which is called the Geriatric Scholars Program-Psychology Track (GSP-P). The GSP-P has two overarching educational program aims: (1) to improve geropsychology competencies of practicing VA psychologists, particularly those working within integrated settings (e.g., primary care) and (2) enrich psychologists' abilities to enact change in their clinical settings. Ninety-eight VA clinicians participated in the GSP-P, which includes a multi-day in-person course, from 2014 to 2018. Participants completed measures assessing confidence and self-reported knowledge in geropsychology and integrated care competencies pre-course and 3-months post-completion. Two-weeks post-course participants responded to open-ended survey questions regarding their perceptions of the course and potential applications of learning. Significant improvements in confidence in and knowledge of geropsychology and integrated care competencies emerged from pre-course to 3-months post-completion. Qualitative findings demonstrated that participants valued the face-to-face, integrated multimodal educational program. Findings provided insights regarding clinicians' planned application of the knowledge acquired, such as modifying treatments for older patients. Specialized workforce programs such as the GSP-P have a significant, positive impact on the care of older Veterans.

AbstractPregnant mothers class to learn about health for pregnant women in the form of face-to-face in groups who want to improve the knowledge, attitudes, and skills of mothers and families regarding assessment, care, childbirth, childbirth care, illness, or situations during pregnancy-childbirth and childbirth, care newborn babies use the Mother and Child Health book (MCH) as the main ingredient. High-risk pregnant women in Puskesmas putri ayu experienced an increase in 2018 (January-December) increased by 146 high-risk pregnant women and in 2019 (January august) there were 152 high-risk pregnant women. The purpose of this study was to study the effect of education on information and communication on the ability of pregnant women to face the challenges of pregnancy, childbirth and childbirth in pregnant mothers working area of the Putri Ayu Public Health Center in 2019. The method of this study used a quasi-study with one group pretest-postet design. The sampling technique in this study was a total sampling of 50 respondents. analysis using the Wilcoxon test. Research results obtained by pregnant women p value of .000, childbirth with a p value of .000 and puerperium p value of .0009 means related to education education about health and safety for pregnant women who associate the problem of childbirth danger in the class of pregnant women at the working area of Putri Ayu Health Center Jambi in 2019. It is expected that midwives' special health centers will improve their communication skills, through health counseling, this can be used as a health workforce program in providing health education.

This research was motivated by the implementation of the Village Cash Workforce Program(PKTD) As a priority program for the use of village funds in 2018 in Stowe villageBrang, Utan District, as one of the villages that has become a pilot project implementationPKTD program that has not been going well. This research aims toknow the strategic strategies for developing the Village Cash Workforce Program (PKTD),to find out what obstacles in the PKTD program and to find outefforts to overcome obstacles in the implementation of the PKTD program in Stowe VillageBerang, Utan District. This study uses a theory based onthe SWOT analysis presented by Siagian in determining Strategic Alternative.This study uses interviews, observation and documentation. Data analysisused is the Miles & Hubberman model. The result of the research is that the strategyappropriate to implement the Village Cash Workforce Program (PKTD) in the village of Stoweberang kecamatan utan is a strategy to use village funds for improvementcommunity capacity so that they have competence, strategies for the use of village funds andprofessional assistants in providing socialization to the community, andstrategies to increase coordination or cooperation between village governments, implementersactivities and society.

Social Justice and Inclusion Within the Right-based Universal Workfare Programme with Self-selection: Lessons and Ways Forward from the Participation of SCs and STs in MGNREGA in Karnataka

MGNREGA is a right-based universal, demand-driven workforce programme with self-selection, providing 100 days of guaranteed employment and productive asset to the poor households (HHs). Its universalism assured unlimited resources to satisfy any demand and hard manual labour ensured that only poor would like to access it by design. The programme earmarked 33 per cent share for women but did not as a right or policy incorporate any definite share or affirmative action for the Scheduled Castes/Scheduled Tribes (SCs/STs) like other flagship programmes of Pradhan Mantri Awas Yojana (PMAY) or Deen Dayal Upadhyaya Grameen Kaushalya Yojana (DDU-GKY). In the light of the above paradigm, this study explores as to how far this programme had accommodated the interests and requirements of the SCs/STs in Karnataka, providing equitable wage work and productive sustainable assets by building their awareness, providing them equitable space in participatory planning and grassroots monitoring, augmenting their capability to demand work and enabling them to self-select, and in case of breach of their rights approaching accountability through social audit or Ombudsman. The study finds low awareness, low grassroots participation in planning and monitoring, and inequitable participation of SCs/STs in both wage employment and assets. The study finds exclusion of SCs/STs from the assets sharper and more pronounced than their exclusion in wage employment. The study finds evidence that a right-based workfare programme with self-selection alone was incapable of being a substitute for earmarking (reservation, affirmative action) and protecting the interests of SC/ST HHs. The study finds that promotional goals included in the guidelines do not by themselves translate into formal actions, monitorable outputs and outcomes. Our primary data also show that the socio-economic and political predicament of SC/ST HHs justified a case for definite affirmative action for them. The study gathers primary and secondary evidences to show that in Karnataka, the programme failed the test of social justice and inclusion proportionate to the disabilities and need of SCs/STs and required immediate attention and policy responses.

University partnership in child welfare workforce preparation and program implementation

ABSTRACTIn this article, we present a university partnership with public and private child welfare organizations that simultaneously transformed MSW field curricula and informed installation and implementation of new child welfare practice models. Anchored in implementation science and frameworks, the partnership’s plan-do-study-act feedback loops identify challenges and opportunities, integrate lessons for students, instructors, and participating organizations, and prepare future workforce and leaders to deliver evidence-informed practice. Its focus, processes, structures, and tools offer academic professional degree programs an example of how workforce preparation and the implementation of service programs can be mutually reinforcing and beneficial.

Maka dari itu revolusi industri 4.0 di sektor pelabuhan merupakan hal baik untuk menuju smart port dan smart supply chain. Seiring dengan revolusi industri 4.0 dan teknologi digital, persaingan bisnis dan pembangunan yang semula bertumpu pada pemanfaatan sumber daya alam akan bergeser pada penguasaan teknologi informasi dan kompetensi angkatan kerja. Di sinilah pentingnya investasi SDM. Era revolusi industri 4.0 membuka kesempatan bagi sumber daya manusia (SDM) di sektor manufaktur untuk memiliki keahlian yang sesuai dengan perkembangan teknologi terkini. Untuk itu, diperlukan pelaksanaan program peningkatan keterampilan (up-skilling) atau pembaruan keterampilan (reskilling) para tenaga kerja berdasarkan kebutuhan dunia industri saat ini.
