Rare Case of Vulvar Fibroid with Cystic Degeneration Mimicking Bartholin Cyst

Vulvar swelling is a common condition in the reproductive age group of women. The most common vulvar swelling is a Bartholin cyst or abscess, which is unilateral. The rarest presentation of vulvar swelling is Leiomyoma, which is like a Bartholin cyst. The common symptom of vulvar Leiomyoma is discomfort during walking, voiding, or coitus, otherwise asymptomatic. Here we present a rare case of vulvar fibroid mimicking a Bartholin cyst as the patient presented with swelling in the vulva, which was gradual in onset and associated with pain. Following which patient was investigated further and managed with surgical excision and marsupialization. Therefore, we should also rule out the rarest form of any condition. Here, we provisionally diagnosed a vulvar leiomyoma as a Bartholin cyst, which was confirmed by Histopathology of excised specimen.

Vulvar Leiomyoma Mimicking a Bartholin’s Gland Tumour

Bartholin's cyst is frequently the cause of unilateral vulval enlargement in a woman of reproductive age. We discuss a noteworthy occurrence of vulval leiomyoma that was initially misdiagnosed as a recurrent Bartholin's cyst in this article. Intraoperatively, it was observed that the 35-year-old lady who had been scheduled for marsupialization really had a tumour of vulvar or bartholin's gland origin rather than a cyst. It was discovered to be rather hard in consistency and well encapsulated intraoperatively. The vulvar leiomyoma was proven to be the diagnosis by histopathology. Extrauterine leiomyomas are uncommon and more challenging to diagnose. Bartholin's cysts, fibromas, lymphangiomas, soft-tissue sarcomas, and neurogenic tumours are among the differential diagnoses. Following surgery, it is advised that all patients have long-term follow-up. A rare case of vulvar leiomyoma masquerading as Bartholin cyst

Vulvar leiomyoma possesses a diagnostic challenge clinically due to its rarity. Usually, it is diagnosed as Bartholin cyst in clinical practice, until histopathological examination is done. Herein we report a 49-year female with a painless perineal swelling on the right side of the vulva for 5 months. Clinically, it was suspected as Bartholin gland cyst based on her anatomical location, clinical signs, and symptoms. Excision was done and histopathological study revealed a benign smooth muscle neoplasm with whorls and interlacing fascicles of spindle shaped cells without nuclear atypia. Hence diagnosed as vulvar leiomyoma histopathologically.

Vulvar leiomyoma in an adolescent girl: a case report and review of the literature

BackgroundVulvar leiomyoma is a rare soft tissue tumor, with only around 300 cases described in the literature. Owing to its low incidence of just 0.03% of all gynecological tumors, it often poses a great diagnostic challenge, especially in teenagers. We report this rare occurrence of vulvar leiomyoma in a teenage girl who was primarily left untreated due to cultural taboos and fear of loss of virginity. The main aim in presenting such rare case studies is to raise awareness and expand the diagnostic horizon of the surgeon for appropriate management.Case presentationWe describe a case of a 15-year-old North Indian, sexually inactive unmarried girl, who presented with a history of painless swelling in the left labia majora for the last 1 year, which gradually increased in size. There was no associated pain or any other difficulty. Local examination revealed a 14 × 10 cm solid, unilateral nonpedunculated mass on the left labia majora with superficial vascularity. Differential diagnoses of sarcoma, lipoma, Bartholin cyst, and fibroid were kept in mind. Ultrasonography revealed a solid mass with superficial vascularity with normal internal genitalia. The mass was enucleated with an intact capsule under anesthesia. Histopathology confirmed it to be benign vulvar leiomyoma. The patient was discharged after 3 days in a satisfactory condition.ConclusionLeiomyoma of the vulva is an exceptionally rare tumor and is seldom seen in teenagers. It is often misinterpreted as a Bartholin cyst and should be kept as one of the differential diagnosis in teenage girls presenting with unilateral vulvar swelling. Vulvar leiomyoma can be completely cured by surgical removal if diagnosed timely without compromising virginity, so should never be missed in adolescents.

Smooth muscle tumor of uncertain malignant potential (STUMP) of Vulva

A 30-years old nulliparous lady presented with apparently recurrent leiomyoma of vulva and past history of repeated resections. She had firm bilobed non-tender vulval lump and underwent partial vulvectomy. Histopathology revealed Smooth muscle tumor of uncertain malignant potential (STUMP).
 Conclusion: Vulvar leiomyoma is fairly uncommon and can often be misdiagnosed. Surgical excision and histopathological analysis is helpful/ recommended for final diagnosis of Vulvar STUMP.

Vulvar Leiomyoma Presenting as a Painless Vulvar Mass

A 21-year-old woman with a painless 3-cm vulvar mass underneath the right labia minora (Fig. 1) had both a translabial ultrasound (Fig. 2A) and magnetic resonance imaging (Fig. 2B) that demonstrated a well-circumscribed, vascular nodule. A fine needle biopsy was not considered because of its vascular nature. The patient underwent excision of the lesion with an epithelioid leiomyoma of the vulva confirmed histologically (Fig. 3). Vulvar Leiomyomatosis: A Diagnostic Challenge Case Report

Introduction: Vulvar leiomyomas are rare and usually misdiagnosed as Bartholin’s cyst on initial presentation. Leiomyoma of vulva accounts for only 0.03% of all gynecological neoplasm. Definite diagnosis is based on surgical excision of the tumor. The aim of this report is to introduce a case of leiomyoma of the vulva which was initially misdiagnosed as Bartholin’s cyst. Case Presentation: We present a case of a 36-year old virgin female (BMI>30) who was referred to the clinic with a 5-year history of left labial mass which was initially diagnosed as Bartholin’s cyst. The mass was removed and the pathologist confirmed that the final diagnosis was vulvar leiomyoma. Conclusion: Vulvar leiomyoma is usually misdiagnosed with Bartholin cyst and distinguishing between benign and malignant forms is confusing, so it is a diagnostic challenge. Surgical excision is the treatment of choice in all smooth muscle tumors of the vulva. Continuing follow-up after treatment is highly recommended.

Giant Tumor of Vulva: Vulvar Myxoid Leiomyoma—A Rare Entity

Vulvar myxoid leiomyoma are very rare soft tissue tumors of vulva with less than 50 cases being reported in the literature. They are usually misdiagnosed as Bartholin’s cyst; however, it is important that they are diagnosed correctly owing to their ability to recur and metastasize. We present a case of a young multiparous woman, with a slowly progressive perineal swelling who was operated and diagnosed as a case of vulvar myxoid leiomyoma.

Review of the pathophysiology, diagnosis, and therapy of vulvar leiomyoma, a rare gynecological tumor.

The objective of this article is to discuss the pathophysiology, diagnosis, differential diagnosis, and therapy of vulvar leiomyoma. We performed a review of all English-language reports of vulvar leiomyoma published in PubMed from 1978 to 2015 using the following search terms: “vulval leiomyoma,” “vulvar leiomyoma,” “vulval smooth muscle tumor,” and “external genitalia smooth muscle tumor.” Vulvar leiomyomas, which are rare benign monoclonal tumors, most commonly occur in the fourth and fifth decades of life. The genetics of vulvar leiomyoma remain undefined. Three principal histological patterns have been identified: spindled, epithelioid, and myxoid. Imaging tests such as ultrasound, endoscopic ultrasound, and magnetic resonance imaging are used in diagnosis. Surgical excision is the only curative treatment for vulvar leiomyomas. Establishment of a full differential diagnosis list and correct final diagnosis before surgery are essential for optimal clinical management. Although recurrence of vulvar leiomyoma is extremely rare, long-term follow-up of all cases is advisable.

Leiomyoma of the Vulva: A Diagnostic Challenge Case Report.

Uterine leiomyomas are common and can affect up to 30% of women older than 35 years. Despite this, leiomyomas of the vulva are rare, masquerading, and usually misdiagnosed as Bartholin cyst preoperatively. These smooth muscle tumors are typically painless, solitary, and well circumscribed and can affect female of any age group. We present a case of a 46-year-old female that presented to the clinic with 2-year history of right labial mass and was diagnosed as Bartholin cyst initially. The patient underwent elective excision under spinal anaesthesia and the mass was removed. The final diagnosis after microscopy result showed benign vulvar leiomyoma.

Round ligament of uterus leiomyoma: an unusual cause of dyspareunia.

Round ligament of uterus leiomyoma is a rare, benign tumor of the vulva. Its incidence is not known exactly, and the mean age ranges from 13 to 70. Although clinical properties of benign and malignant diseases in the vulvar area are frequently similar, early diagnosis and treatment are essential. Local excision is recommended as definitive therapy. We present an 28-year-old female without any birth with a mass in anterior vaginal wall diagnosed as vulvar leiomyoma. In conclusion, a brief review of relevant literature emphasizes that leiomyomas are quite rare outside of the uterus but they might occur in any tissue or organ containing smooth muscle, spontaneously or parasitically after the spreading effect of an accident or surgical trauma. Clinicians should be alert especially for the diagnosis in a tissue with smooth muscle content.

Myxoid Epithelial Leiomyoma of the Vulva: A Case Report and Literature Review.

Leiomyoma of vulva is rare, and usually misdiagnosed clinically as Bartholin cyst. It usually presents spindle-shaped tumor cells, but some rare cases consisted mainly of atypical epithelioid tumor cells. We report here a case of 30-year-old woman consulting with a vulvar mass of 7 cm in the Bartholin glands area. The lesion was surgically excised with its capsule completely. Pathological examination and immunochemistry showed characteristic of epithelioid leiomyoma with myxoid stroma with both estrogen receptor (ER) and progesterone receptor (PR) staining negative, which was really rare as only 2 cases of vulvar leiomyoma with both ER and PR were reported before.

Cryosurgical Treatment of Equine Maxillo-Facial Sarcoids and Vulvar Leiomyomas

The present study was undertaken to evaluate the efficacy of cryotherapy for the treatment of equine tumours. In the present study, nitrous oxide cryoprobe was used to excise five tumours: three sarcoids and two leiomyomas at the maxillo-facial and vulvar region under general anesthesia and epidural anesthesia respectively. Complete healing without any post-operative complication was observed in all the cases. No recurrence was observed after 1 year of surgery. It may be concluded that cryosurgery can be used for treatment of certain types of external tumours in equines.

Smooth Muscle Neoplasms of the Vulva Masquerading as Bartholin Gland Duct Cysts

Smooth muscle neoplasms of the vulva can be mistaken for Bartholin duct cysts, which can lead to a delay in diagnosis. We present a case of vulvar leiomyoma and a case of leiomyosarcoma that clinically mimicked Bartholin duct cysts. Identification of leiomyosarcomas in this region is particularly important; due to the risk of recurrence, patients may need radiation and/or chemotherapy in addition to adequate surgical treatment and appropriate follow up. Prior series have shown that risk of recurrence is related to inadequate resection and not to the size or grade of tumor. It is critical that pathologists recognize smooth muscle tumors of the vulva and communicate to clinicians the importance of clear margins and wide local excision in cases of malignancy.

Androgen Insensitivity Syndrome with Gynandroblastoma and Vulvar Leiomyoma

This study aimed to describe a rare case of androgen insensitivity syndrome (AIS) in association with gynandroblastoma and vulvar leiomyomata. The patient was a 64-year-old phenotypically female person with 46 XY chromosomal karyotypes who presented with a chief complaint of abdominal distension of 1-month duration. She had 2 vulvar masses for more than 20 years and had experienced life-long (untreated) amenorrhea. History and physical examination findings showed the patient to be 175 cm in height, with normal breast development, and a lack of pubic or axillary hair. Serum levels were as follows: testosterone, 1,980 ng/dL; estradiol, 1,380.8 pg/mL. In combination with clinical findings, a diagnosis of complete AIS was made. The patient subsequently underwent surgery, during which the bilateral vulvar masses and the rudimentary uterus, right sex gland, and left side of the tumor were resected. Histological examination of the tumor revealed a mass composed primarily of juvenile-type granulosa cells, admixed with a Sertoli cell component that comprised less than 50% of the tumor. The right side of the gonad had an ovarian-like cortex and was considered to be an undeveloped ovary. The left and right side of the vulvar masses were diagnosed with leiomyomas. Patients with AIS should be monitored closely because these patients may also experience gonadal tumors. When confronted with gynandroblastoma, close attention should be paid to the patient's endocrinologic status, and comprehensive endocrinologic analyses should be conducted to make correct treatment decisions.
