Prevalence and prediction of Lyme disease in Hainan province.

Lyme disease (LD) is one of the most important vector-borne diseases worldwide. However, there is limited information on the prevalence and risk analysis using correlated factors in the tropical areas. A total of 1583 serum samples, collected from five hospitals of Hainan Province, were tested by immunofluorescence assay (IFA) and western blot (WB) analyses using anti-Borrelia burgdorferi antibodies. Then, we mapped the distribution of positive rate (by IFA) and the spread of confirmed Lyme patients (by WB). Using ArcGIS, we compiled host-vector-human interactions and correlated data as risk factor layers to predict LD risk in Hainan Province. There are three LD hotspots, designated hotspot I, which is located in central Hainan, hotspot II, which contains Sanya district, and hotspot III, which lies in the Haikou-Qiongshan area. The positive rate (16.67% by IFA) of LD in Qiongzhong, located in hotspot I, was higher than that in four other areas. Of confirmed cases of LD, 80.77% of patients (42/52) whose results had been confirmed by WB were in hotspots I and III. Hotspot II, with unknowed prevalence of LD, need to be paid more attention considering human-vector interaction. Wuzhi and Limu mountains might be the most important areas for the prevalence of LD, as the severe host-vector and human-vector interactions lead to a potential origin site for LD. Qiongzhong is the riskiest area and is located to the east of Wuzhi Mountain. In the Sanya and Haikou-Qiongshan area, intervening in the human-vector interaction would help control the prevalence of LD.

A Rare Case of Mixed Type Severe Malaria Co-infection with Dengue Complicated by Expanded Dengue Syndrome

Background Malaria and dengue are the most prevalent vector-borne diseases worldwide. Both diseases are endemic in similar tropical regions. Each infection has a specific mosquito vector. Hence, overlapping of the habitat cannot be easily available. In co-infection, the clinical features were more like dengue mono-infection than malaria mono-infection. Therefore, clinically, it is difficult to diagnose co-infection dengue and malaria. Case Illustration A 42-yo Javanese man, presented with 10-days of fever that was clinically and serologically consistent with symptoms of vivax malaria. Plasmodium vivax was found in the ring, trophozoite and schizonts form in the thick blood smear examination with parasitemia index 84.410 parasite/µl. In the course of the disease, patients were found to have a mixed infection with Plasmodium ovale. The patients also experience complications of spontaneous bleeding, thrombocytopenia and worsening respiratory conditions leading to acute respiratory distress syndrome (ARDS) which increases suspicion of co-infection with dengue virus. Laboratory tests to enforce dengue infection are carried out with the results obtained positive IgG and IgM that indicate recurrent infections. The patient is then given the management of severe malaria and expanded dengue syndrome according to the Indonesia Ministry of Health of guideline. On the 11th day of hospitalizations, the patients showed a significant cure rate and continuing ambulatory therapy with Primaquine 15 mg oral until day 14th. Conclusion The incidence of heavy bleeding in malaria patients is very low. Thus, malaria patients who experience unclear fever patterns and heavy bleeding should be systematically investigated for suspicion of dengue virus co-infection.

High Aedes spp. larval indices in Kinshasa, Democratic Republic of Congo

BackgroundDengue, yellow fever, chikungunya and Zika are among the most important emerging infectious vector-borne diseases worldwide. In the Democratic Republic of Congo (DRC), increases in cases of dengue and outbreaks of yellow fever and chikungunya have been reported since 2010. The main vectors of these arboviruses, Aedes aegypti and Aedes albopictus, have been reported in DRC, but there is a lack of detailed information on their presence and spread to guide disease control efforts.MethodsIn 2018, two cross-sectional surveys were conducted in Kinshasa province (DRC), one in the rainy (January/February) and one in the dry season (July). Four hundred houses were visited in each of the four selected communes (N’Djili, Mont Ngafula, Lingwala and Kalamu). Within the peri-domestic area of each household, searches were conducted for larval habitats, which were then surveyed for the presence of Aedes larvae and pupae. A subset of the immature specimens were reared to adults for morphological identification followed by DNA barcoding of the specimens to validate identifications.ResultsThe most rural commune (Mont Ngafula) had the highest pupal index (number of Aedes spp. pupae per 100 inspected houses) at 246 (20) pupae/100 houses, and Breteau index (BI; number of containers positive for immature stages of Aedes spp. per 100 households) at 82.2 (19.5) positive containers/100 houses for the rainy (and dry) season, respectively. The BI was 21.5 (4.7), 36.7 (9.8) and 41.7 (7.5) in Kalamu, Lingwala and N’Djili in the rainy (and dry) season, respectively. The house index (number of houses positive for at least one container with immature stages of Aedes spp. per 100 inspected houses) was, on average, across all communes, 27.5% (7.6%); and the container index (number of containers positive for immature stages of Aedes spp. per 100 inspected containers) was 15.0% (10.0%) for the rainy (and dry) season, respectively. The vast majority of Aedes-positive containers were found outside the houses [adjusted odds ratio 27.4 (95% confidence interval 14.9–50.1)]. During the dry season, the most productive containers were the ones used for water storage, whereas in the rainy season rubbish and tires constituted key habitats. Both Ae. aegypti and Ae. albopictus were found. Anopheles larvae were found in different types of Aedes larval habitats, especially during the rainy season.ConclusionsIn both surveys and in all communes, the larval indices (BI) were higher than the arbovirus transmission threshold values established by the World Health Organization. Management strategies for controlling Aedes in Kinshasa need to target the key types of containers for Aedes larvae, which are mainly located in outdoor spaces, for larval habitat destruction or reduction.Graphical

Dengue Virus in Sri Lanka: An Observational Study From 2014-2018

Dengue virus is one of major public heath burden in several countries, it is described to be “a fast-emerging pandemic prone viral disease” by the WHO and is one of the most common form of vector borne diseases worldwide. The disease is transmitted through the female Aedes mosquito and most commonly found in tropical countries. A reported 400 million reported dengue cases occur yearly and an estimated 3 billion people could be affected by the disease in the upcoming decade making it a global crisis. The disease is caused by four serotypes of the dengue virus (DENV1-4) making it difficult for the treatment to work on the patients due to its different virulent mechanism. Using the data available from the Ministry of Health, Epidemiology Unit in Sri Lanka the dengue reported cases from 2014 to 2018 are analyzed over the five years to identify the trends and occurrence patterns [1,2].

Effect of a combination of permethrin and diesel oil on <i>Rhipicephalus sanguineus</i> and <i>Ctenocephalides canis</i> in experimentally infested dogs

Rhipicephalus sanguineus (Brown dog tick) and Ctenocephalides canis (dog and cat flea) cause heavy economic burden for dog owners in Nsukka, south-east, Nigeria, and are also major cause of vector-borne diseases worldwide. A total of 40 apparently healthy dogs were randomly assigned to 3 groups (A, B and C) and experimentally infested with Rhipicephalus sanguineus and Ctenocephalides canis. Each dog in Group A (n = 10) was treated with a mixture of permethrin and diesel oil while Groups B (n = 6) and C (n = 4) were treated with permethrin and diesel oil respectively. The remaining dogs in each group served as controls. Parasites were counted 48th hour post-treatment and efficacy estimated as percentage reduction in live parasites in treated groups compared to untreated groups. In Group A the average percentage efficacy was 99.7% while in groups B and C it was 98.3% and 72.1% respectively for R. sanguineus, Geometric mean of live parasites between treated and control groups was significantly different (p<0.05) for every day in Groups A, B and C. Tick and flea numbers can be reduced in dogs by the application of diesel oil as an acaricide.

Comparing the Climatic and Landscape Risk Factors for Lyme Disease Cases in the Upper Midwest and Northeast United States.

Lyme disease, recognized as one of the most important vector-borne diseases worldwide, has been increasing in incidence and spatial extend in United States. In the Northeast and Upper Midwest, Lyme disease is transmitted by Ixodes scapularis. Currently, many studies have been conducted to identify factors influencing Lyme disease risk in the Northeast, however, relatively few studies focused on the Upper Midwest. In this study, we explored and compared the climatic and landscape factors that shape the spatial patterns of human Lyme cases in these two regions, using the generalized linear mixed models. Our results showed that climatic variables generally had opposite correlations with Lyme disease risk, while landscape factors usually had similar effects in these two regions. High precipitation and low temperature were correlated with high Lyme disease risk in the Upper Midwest, while with low Lyme disease risk in the Northeast. In both regions, size and fragmentation related factors of residential area showed positive correlations with Lyme disease risk. Deciduous forests and evergreen forests had opposite effects on Lyme disease risk, but the effects were consistent between two regions. In general, this study provides new insight into understanding the differences of risk factors of human Lyme disease risk in these two regions.

Efficacy of Extractions of Iranian Native Plants against Main Malaria Vector, Anopheles stephensi in Iran for Making Appropriate Formulation for Disease Control

Malaria is the main vector-borne disease worldwide. There are several reports of insecticide resistant in malaria vectors worldwide due to using different insecticides. The aim of this study was to evaluate different native plant extortions against main malaria vector, Anopheles stephensi in Iran for choosing the appropriate plant for formulation and use for vector control. The larvae of An. stephensi were reared in insectary, extraction of plants were carried out at department of Pharmacology. The standard WHO method for biological tests was used for calculation of LC50 and LC90. Probit regration lines were plotted for calculation of LC50 and LC90. In this study several plants including: Mentha spicata, Cymbopogon olivieri, Azadirachta indica, Melia azedarach, Lagetes minuta, Calotropis procera, Eucalyptus camaldulensis, Cupressus arizonica, Thymus vulgaris, Lawsonia inermis, Cedrus deodara, Cionura erecta, Bunium persicum, Carum carvi, Artemisia dracunculus, Rosmarinus officinalis were used. Results showed that Mentha spicata and Eucalyptus camaldulensis, had the lowest and highest LC50 respectively. Results indicated that Mentha spicata and Eucalyptus camaldulensis, had the lowest and highest LC50 respectively. Several other plant extract also showed significant mortality. The formulation of these plants should be prepared and evaluate at the field condition against malaria vectors.

Time to Micromanage the Pathogen-Host-Vector Interface: Considerations for Vaccine Development.

The current increase in vector-borne disease worldwide necessitates novel approaches to vaccine development targeted to pathogens delivered by blood-feeding arthropod vectors into the host skin. A concept that is gaining traction in recent years is the contribution of the vector or vector-derived components, like salivary proteins, to host-pathogen interactions. Indeed, the triad of vector-host-pathogen interactions in the skin microenvironment can influence host innate and adaptive responses alike, providing an advantage to the pathogen to establish infection. A better understanding of this “bite site” microenvironment, along with how host and vector local microbiomes immunomodulate responses to pathogens, is required for future vaccines for vector-borne diseases. Microneedle administration of such vaccines may more closely mimic vector deposition of pathogen and saliva into the skin with the added benefit of near painless vaccine delivery. Focusing on the ‘micro’–from microenvironments to microbiomes to microneedles–may yield an improved generation of vector-borne disease vaccines in today’s increasingly complex world.

Acute Hepatitis in Infections Caused by Dengue Virus in Southern Punjab, Pakistan.

BackgroundDengue is the most common vector-borne disease worldwide. It poses a significant health burden in tropical and subtropical countries. Common clinical presentations include retro-orbital pain, fever, headache, nausea, vomiting and aches and pains in the body. A severe form of dengue fever is known as dengue hemorrhagic fever (DHF) that includes signs of hemorrhage. Besides the typical signs and symptoms, atypical presentations of dengue include myositis, hepatitis and encephalitis. Hepatic involvement in dengue has varied presentations. This study aims to highlight the importance of acute hepatitis, an atypical presentation in dengue patients.Methods We conducted a descriptive, cross-sectional study in the Medical Unit-1 of Bahawal Victoria Hospital, Bahawalpur, a tertiary-care hospital serving the area of Southern Punjab, Pakistan. The relevant medical records of 63 patients admitted with dengue-associated hepatitis to the Medical Unit-1 of Bahawal Victoria Hospital, Bahawalpur, between January 1, 2015 and December 1, 2016, were reviewed. Informed consent was given. Information regarding demographic variables and disease course was collected and analyzed.Results This study included 55 men (87.3%) and eight (12.7%) women. Fifty (79.3%) patients were diagnosed with dengue fever (DF). Thirteen patients were managed on the lines of DHF. Out of the total 63 patients, only six were locals. The common clinical presentations in these patients included high fever, retro-orbital pain, severe headache, rash, dark-colored urine, bleeding problems and hepatomegaly. Higher levels of aspartate aminotransferase (AST) were noted in comparison to alanine transferase (ALT). Despite the complicated clinical course in some patients, all patients were managed successfully and discharged, except one.ConclusionThe frequency of acute hepatitis in dengue patients is high, especially in young men. Early diagnosis and prompt treatment are necessary for better prognosis. Although no specific treatment guidelines are available, supportive treatment in a timely fashion can prevent complications. Transfusion with packed cell volume (PCV) and N-acetyl cysteine (NAC) has produced promising results.

Clinical spectrum of Lyme disease.

Lyme disease (borreliosis) is one of the most common vector-borne diseases worldwide. Its incidence and geographic expansion has been steadily increasing in the last decades. Lyme disease is caused by Borrelia burgdorferi sensu lato, a heterogeneous group of which three genospecies have been systematically associated to Lyme disease: B. burgdorferi sensu stricto Borrelia afzelii and Borrelia garinii. Geographical distribution and clinical manifestations vary according to the species involved. Lyme disease clinical manifestations may be divided into three stages. Early localized stage is characterized by erythema migrans in the tick bite site. Early disseminated stage may present multiple erythema migrans lesions, borrelial lymphocytoma, lyme neuroborreliosis, carditis, or arthritis. The late disseminated stage manifests with acordermatitis chronica atrophicans, lyme arthritis, and neurological symptoms. Diagnosis is challenging due to the varied clinical manifestations it may present and usually involves a two-step serological approach. In the current review, we present a thorough revision of the clinical manifestations Lyme disease may present. Additionally, history, microbiology, diagnosis, post-treatment Lyme disease syndrome, treatment, and prognosis are discussed.

Medical Entomology: A Reemerging Field of Research to Better Understand Vector-Borne Infectious Diseases.

In the last decade, the Chikungunya and Zika virus outbreaks have turned public attention to the possibility of the expansion of vector-borne infectious diseases worldwide. Medical entomology is focused on the study of arthropods involved in human health. We review here some of the research approaches taken by the medical entomology team of the University Hospital Institute (UHI) Méditerranée Infection of Marseille, France, with the support of recent or representative studies. We propose our approaches to technical innovations in arthropod identification and the detection of microorganisms in arthropods, the use of arthropods as epidemiological or diagnostic tools, entomological investigations around clinical cases or within specific populations, and how we have developed experimental models to decipher the interactions between arthropods, microorganisms, and humans.

Tick-borne diseases in dogs

Tick-borne diseases (TBDs) can have serious impact on the health and welfare of dogs, and have been described in all continents. The expanding number of tick-borne pathogens, the broad geographic range of many tick species, the ability of tick-borne pathogens to induce infections, and the highly zoonotic potential of some of these pathogens make TBDs the most important subcategory of canine vector-borne infectious diseases worldwide. Indeed, emerging TBDs have become a concern for pet owners and veterinary professionals. The occurrence of babesiosis in a cluster of dogs from Harlow, Essex in 2016 has raised some concerns regarding the inevitable increase in the risk of TBDs particularly after the relaxation of pet travel rules. In addition to babesiosis that has dominated recent headlines other TBDs such as Lyme borreliosis have more quietly expanded to many parts of the country. The large number of tick-borne pathogens, the diversity of tick vectors, the broad range of animal reservoir hosts, limitations associated with diagnosis and treatment, and the ecological complexity of tick-borne pathogens make effective control of TBDs a challenging task. Therefore, it is important for veterinary professionals to be able to detect TBDs early and accurately in order to minimise the morbidity and mortality of these diseases. This article provides an update on some of the most common TBDs in dogs, namely babesiosis, hepatozoonosis, borreliosis, anaplasmosis, and ehrlichiosis. The key roles that veterinary nurses can play to support pet owners in recognising and dealing with ticks and TBDs are also discussed.

Cost-utility analysis of dengue vaccination in a country with heterogeneous risk of dengue transmission

BackgroundDengue is one of the most important vector-borne diseases worldwide, and annually, nearly 390 million people are infected and 500,000 patients are hospitalized for severe dengue. Argentina has great variability in the risk of dengue transmission due to eco-climatic reasons. Currently no vaccines are available for dengue even though several vaccines are under development. ObjectiveThe aim of this study was to estimate the cost-effectiveness of a dengue vaccine in a country with heterogeneous risk of dengue transmission like Argentina. MethodsThe analysis was carried out from a societal perspective using a Markov model that included both vaccine and disease parameters. Utility was measured as disability adjusted life years (DALYs) averted, and the incremental cost-effectiveness ratio (ICER) of the vaccination was expressed in 2014 American dollars (US$) per DALY averted. One-way and probabilistic sensitivity analyses were performed to evaluate uncertainty in model outcomes, and a threshold analysis was conducted to estimate the highest possible price of the vaccine. ResultsThe ICER of the vaccination program was found to be US$ 5714 per DALY averted. This value is lower than 3 times the per capita GDP of Argentina (US$ 38,619 in 2014); 54.9% of the simulations were below this value. If a vaccination program would be implemented the maximum vaccine price per dose has to be US$1.49 for a vaccination at national level or US$28.72 for a targeted vaccination in high transmission areas. ConclusionsThese results demonstrate that vaccination against dengue would be cost-effective in Argentina, especially if carried out in predetermined regions at high risk of dengue transmission. However, these results should be interpreted with caution because the probabilistic sensitivity analysis showed that there was considerable uncertainty around the ICER value. The influence of variations in vaccine efficacy, cost and other important parameters are discussed in the text.

P. vivax Malaria and Dengue Fever Co-infection: A Cross-Sectional Study in the Brazilian Amazon

BackgroundMalaria and dengue are the most prevalent vector-borne diseases worldwide and represent major public health problems. Both are endemic in tropical regions, propitiating co-infection. Only few co-infection cases have been reported around the world, with insufficient data so far to enhance the understanding of the effects of co-infection in the clinical presentation and severity.Methodology/Principal FindingsA cross-sectional study was conducted (2009 to 2011) in hospitalized patients with acute febrile syndrome in the Brazilian Amazon. All patients were submitted to thick blood smear and PCR for Plasmodium sp. detection, ELISA, PCR and NS1 tests for dengue, viral hepatitis, HIV and leptospirosis. In total, 1,578 patients were recruited. Among them, 176 (11.1%) presented P. vivax malaria mono-infection, 584 (37%) dengue fever mono-infection, and 44 (2.8%) were co-infected. Co-infected patients had a higher chance of presenting severe disease (vs. dengue mono-infected), deep bleeding (vs. P. vivax mono-infected), hepatomegaly, and jaundice (vs. dengue mono-infected).Conclusions/SignificanceIn endemic areas for dengue and malaria, jaundice (in dengue patients) and spontaneous bleeding (in malaria patients) should raise the suspicion of co-infection. Besides, whenever co-infection is confirmed, we recommend careful monitoring for bleeding and hepatic complications, which may result in a higher chance of severity, despite of the fact that no increased fatality rate was seen in this group.

Lyme disease: Emerging and Re-emerging Metazoonoses of Global Importance

Lyme borreliosis is a tick-transmitted multisystem inflammatory disease caused by the spirochete Borrelia burgdorferi. Lyme disease has a worldwide distribution including India. Warmer weather expected to be increase emergence of common vector-borne diseases worldwide without an exception of Lyme disease. Several hundred cases are reported each year. About 10% of cases involve the more serious symptoms of arthritis and 5% experience facial paralysis, in addition to the skin rash that is characteristic of the disease. There is paucity of data on Lyme disease in India. Some study was under taken to study the seroprevalence of Borrelia burgdorferi infection in north eastern parts of India. Therefore, India represents a particularly interesting scenario for the study on Lyme disease. Vaccines against the condition are still not very successful. Hence, the importance of recognizing the cutaneous manifestations early, to prevent systemic complications which can occur if left untreated, can be understood. Here in; we review the problems and opportunities for the incidence of Lyme disease in India. Control measures are discussed in the review in the area of ticks and tick-borne zoonotic diseases (TTBDs). The disease warrants constant monitoring and surveillance because once introduced to human population it would be a herculean task to eradicate it.

The pyrethroid resistance status and mechanisms in Aedes aegypti from the Guerrero state, Mexico

Dengue is one of the most important vector-borne diseases worldwide and is a public health problem in Mexico. Most programs in dengue endemic countries rely on insecticides for Aedes control. In Mexico, pyrethroid insecticides (mainly permethrin and deltamethrin) have been extensively used over a decade as adulticides and represented a strong selection for insecticide resistance for dengue vectors in several parts of the country. We studied the type, frequency and distribution of insecticide resistance mechanisms in Aedes aegypti from six municipalities in the state of Guerrero selected on the basis of historically intense chemical control and a high risk for dengue transmission. Ae. aegypti eggs were collected from October 2009 to January 2010 using ovitraps. F1 adults, emerged from these collections, were exposed to permethrin, deltamethrin and DDT in WHO diagnostic tests and showed high resistance levels to both pyrethroids and DDT. This was consistent with the presence of increased metabolic enzyme activities and target site insensitivity due to kdr mutations. Biochemical assays showed elevated esterase and glutathione S-transferase activities in the six municipalities. The V1016I kdr mutation on the IIS6 domain of the sodium channel gene was present in an overall frequency of 0.80. A second mutation, F1534C on the IIIS6 domain of the same gene was also detected, being the first report of this mutation in Guerrero. The multiple resistance mechanisms present in Ae. aegypti from Guerrero state represent a warning for the efficacy of the pyrethroid usage and consequently for the success of the dengue control program.

Comparison of the FDA-Approved CDC DENV-1-4 Real-Time Reverse Transcription-PCR with a Laboratory-Developed Assay for Dengue Virus Detection and Serotyping

Dengue virus (DENV) is the agent of the most common vector-borne disease worldwide. Using 199 clinical samples collected from Nicaragua and Sri Lanka, a laboratory-developed DENV multiplex real-time reverse transcription-PCR (rRT-PCR) proved more clinically sensitive than the FDA-approved CDC assay for DENV serotypes 1 to 4 when measured against a composite reference standard, with sensitivities of 97.4% versus 87.1%, respectively.

Estimates of Dengue Force of Infection in Children in Colombo, Sri Lanka

Dengue is the most important vector-borne viral disease worldwide and a major cause of childhood fever burden in Sri Lanka, which has experienced a number of large epidemics in the past decade. Despite this, data on the burden and transmission of dengue virus in the Indian Subcontinent are lacking. As part of a longitudinal fever surveillance study, we conducted a dengue seroprevalence survey among children aged <12 years in Colombo, Sri Lanka. We used a catalytic model to estimate the risk of primary infection among seronegative children. Over 50% of children had IgG antibodies to dengue virus and seroprevalence increased with age. The risk of primary infection was 14.1% per year (95% CI: 12.7%–15.6%), indicating that among initially seronegative children, approximately 1 in 7 experience their first infection within 12 months. There was weak evidence to suggest that the force of primary infection could be lower for children aged 6 years and above. We estimate that there are approximately 30 primary dengue infections among children <12 years in the community for every case notified to national surveillance, although this ratio is closer to 100∶1 among infants. Dengue represents a considerable infection burden among children in urban Sri Lanka, with levels of transmission comparable to those in the more established epidemics of Southeast Asia.

Predicting ectotherm disease vector spread—benefits from multidisciplinary approaches and directions forward

The occurrence of ectotherm disease vectors outside of their previous distribution area and the emergence of vector-borne diseases can be increasingly observed at a global scale and are accompanied by a growing number of studies which investigate the vast range of determining factors and their causal links. Consequently, a broad span of scientific disciplines is involved in tackling these complex phenomena. First, we evaluate the citation behaviour of relevant scientific literature in order to clarify the question "do scientists consider results of other disciplines to extend their expertise?" We then highlight emerging tools and concepts useful for risk assessment. Correlative models (regression-based, machine-learning and profile techniques), mechanistic models (basic reproduction number R0) and methods of spatial regression, interaction and interpolation are described. We discuss further steps towards multidisciplinary approaches regarding new tools and emerging concepts to combine existing approaches such as Bayesian geostatistical modelling, mechanistic models which avoid the need for parameter fitting, joined correlative and mechanistic models, multi-criteria decision analysis and geographic profiling. We take the quality of both occurrence data for vector, host and disease cases, and data of the predictor variables into consideration as both determine the accuracy of risk area identification. Finally, we underline the importance of multidisciplinary research approaches. Even if the establishment of communication networks between scientific disciplines and the share of specific methods is time consuming, it promises new insights for the surveillance and control of vector-borne diseases worldwide.
