Periodontitis is the most common inflammatory disease that leads to the destruction of the supporting tissues of the tooth. Therapeutic treatments aimed at reducing the effects of matrix metalloproteinases (MMPs) may be an effective adjunct to the treatment of periodontitis. The review examines the role of proteolytic enzyme inhibitors in the treatment of inflammatory periodontal diseases. General information about the ability of tetracyclines to inhibit MMPs is discussed. The purpose of the study was to examine the use of MMP inhibitors as a treatment for periodontal disease. Material and research methods. A scientific review of studies was carried out in Russian and English using information portals and platforms eLIBRARY.ru, Web of Science, PubMed and Scopus. The search was carried out using the following keywords: matrix metalloproteinases; oral diseases; tissue inhibitors of metalloproteinases. Of the 55 initially identified articles, after the initial analysis, 19 publications devoted to the study of MMP inhibitors associated with periodontal diseases were selected. Results of the study. Analysis of the literature allows us to conclude that the elimination of local irritants of periodontal tissues, the use of a therapeutic and prophylactic complex of measures using MMP inhibitors will have an advantage over traditional methods of treatment, will contribute to longer remission and prevent the progression of the inflammatory process in periodontal tissues, positively influence its course and reduce treatment time. The use of metalloproteinase inhibitors is an effective therapeutic strategy in the treatment of periodontal diseases. Conclusions. The therapeutic potential of metalloproteinase inhibitors may help prevent tissue damage in inflammatory periodontal diseases.

Optimisation of the composition of safe dental gel with IL-1β antagonist for the treatment of inflammatory periodontal diseases

Aim. The study aims to develop the composition of the oromucosal gel with the IL-1β interleukin antagonist for complex treatment of inflammatory periodontal diseases. Materials and methods. Experimental studies were conducted based on the Department of Medicines Technology of Zaporizhzhia State Medical and Pharmaceutical University and the Training Medical and Laboratory Center of Zaporizhzhia State Medical and Pharmaceutical University of the Ministry of Health of Ukraine. As an active component, in the recipe of oromucosal gel, used antagonist of the interleukin IL-1B. Excipients: D-panthenol (the plasticizer), carboxymethylcellulose sodium salt (the viscosity modifier and mucoadhesive component), Tween-80 (the enhancer absorption), benzalkonium chloride (the preservative), sodium hydrophosphate + citric acid (the phosphate buffer solution), purified water. The experiments used existing and auxiliary ingredients of pharmaceutical purity, which were obtained from Sinbias LLC, Istok-Plus LLC, LLC “Mobile Medical”. For the design of the experiment, the methodology of the response surface (Box-Behnken Design) is used: Na CMC, Tween-80, D-panthenol; and four answers: pH, viscosity test, system type, mucoadhesive properties. Research methods: rheological research (viscosity test, amplitude test, frequency test, mucoadhesive test, thixotropy test) performed in oscillation mode on the modular compact Rheometer Anton Paar MCR 302 (CP50-1 SN71317), which, compared to cylindrical devices, requires a much smaller amount of gel sample and allows the planned tests in the oscillation mode, the temperature in the experiments was provided with a built-in thermostat (Peltier Temperature Control, C-PTD 200). Pharmaco-toxicological studies were conducted on 46 white outbred rats of both sexes, weighing 160–180 grams, which were received from Vivarium by the Institute of Pharmacology and Toxicology. The studies were performed on enough animals, all manipulations were carried out by the provisions on the use of animals in biomedical experiments. The results of the study were calculated using the standard statistical package of the Statistica for Windows 13 (StatSoft Inc., No. JPZ804I382130ARCN10-J), as well as SPSS 16.0, Microsoft Office Excel 2003. Results. According to the design of the experiment, it is established that the components of the system do not change the value of pH in experimental oromucosal gels (6.5500 ± 0.0334). The Anova for Quadratic Model statistical analysis data certify the significant impact of Na CMC, and Tween-80 factors on the viscous characteristics of oromucosal gels (F-value > p-value). Comparison of mucoadhesive characteristics of the studied samples of oromucosal gels was made using dynamic mechanical analysis and the results of statistical analysis of ANOVA for Quadratic Model highlight the significant influence of Na CMC, D-panthenol, Tween-80. Optimized oromucosal gel test data is obtained that its structure is restored after the applied effort, namely the restoration of the structure after 10 seconds occurs by 69.5 %, after 30 s by 76.1 %, after 180 s by 85.4 %, after 180 s which allows to predict the stability of the dosage form, both after manufacture and after use. Conclusions. The composition of the oromucosal gel of anti-inflammatory action with the IL-1β interleukin antagonist for the complex treatment of inflammatory diseases of periodontal is developed. The optimized composition of the oromucosal gel obtained has satisfactory performance of kinetic stability and thixotropic properties. The developed gel for dentistry meets all the requirements for harmlessness and safety for dosage forms of this group toxicity, lack of local irritant and allergic action.

РЕЗУЛЬТАТЫ ПРИМЕНЕНИЯ ПРЕПАРАТА НАПРАВЛЕННОЙ БИОКОРРЕКЦИИ И ПРЕПАРАТА, СОДЕРЖАЩЕГО ЛАКТОФЕРРИН, В КОМПЛЕКСНОМ ЛЕЧЕНИИ ХРОНИЧЕСКОГО ГЕНЕРАЛИЗОВАННОГО ПАРОДОНТИТА У ПАЦИЕНТОВ С ПЕРВИЧНЫМ ГИПОТИРЕОЗОМ

Subject In the traditional complex treatment of inflammatory periodontal diseases, antibacterial, antiseptic and anti-inflammatory agents are of paramount importance. These products allow you to obtain not only a positive clinical result, but can also manifest their negative properties, having an ambiguous effect on the microbiome of the oral cavity and periodontal tissue. The use of targeted biocorrection drugs and immunocorrectors has a significant local anti-inflammatory effect with minimal side effects. Their use in the complex treatment of chronic generalized periodontitis has not been studied enough.
 Aim. To evaluate the effectiveness of using a targeted biocorrection drug and a phytocomplex with lactoferrin in the complex treatment of chronic generalized periodontitis in patients with primary hypothyroidism.
 Material and methods. We observed 90 patients with the main diagnosis of primary hypothyroidism aged 44–59. The patients were divided into 3 groups: in group No. 1, 30 patients received traditional primary periodontal therapy, in group No. 2, 30 patients in whom this therapy was combined with a directed biocorrection drug, in group No. 3, 30 people for whom traditional primary periodontal therapy was combined with a directed biocorrection drug and a drug containing lactoferrin. The comparison group included 30 gender and age-matched patients with chronic generalized periodontitis without thyroid pathology. All patients underwent clinical, X-ray and index evaluation of periodontal tissues. 
 Results. In patients of group No. 1, after traditional treatment, clinical and index evaluation data indicated the persistence of inflammatory processes in periodontal tissues. In groups No. 2 and No. 3 on the background of traditional treatment and additionally administered.
 Conclusions. The addition of a traditional treatment regimen for chronic generalized periodontitis with a targeted biocorrection drug and a drug containing lactoferrin is justified and effective.

The use of low-intensity laser radiation and liposomal cream in the treatment of inflammatory periodontal diseases in patients after the coronavirus infection

A number of authors put forward a hypothesis about the role of systemic subclinical vascular inflammation characteristic of COVID-19 in the development of peripheral complications of this infection, including in the oral cavity. Notably, there is a high incidence of gingivitis and periodontitis in patients after COVID-19. Material and methods. 35 patients diagnosed with chronic generalized periodontitis and chronic gingivitis in the acute stage were followed-up on an outpatient basis. All patients had a history of COVID-19 in the past 2 to 6 months. The patients were divided into two groups: the study group (n = 18) and the control group (n = 17). All patients underwent professional oral hygiene. In the study group, low-intensity laser therapy procedures were performed with preliminary application of a liposomal gel. Clinical research methods included taking an anamnesis, examination, and determination of indices (CFE index, Green Vermilion index, PMA index, CI index, Russell’s index). Research results. After therapy, all patients of the study group had grade 1 CFE index up to 4 (compensated); in the patients of the control group, grade 1–2 CFE was determined (compensated — in 88.2 % of the patients and subcompensated — in 11.8 %). After therapy, the Green Vermilion index decreased in both groups by 40.5 % and 31.3 %, with no significant intergroup difference. The PMA index decreased by 88.8 % in the study group and by 71.5 % in the control group. The index of bleeding reduced by 86.4 % in the study group and by 60.9 % in the control group. The Russell’s index, which allows taking into account the presence of gingivitis, tooth mobility, and the depth of the periodontal pocket, was also considered. Its value decreased by 92.3 % in the study group and by 63.8 % in the control group. Conclusions. The developed therapeutic complex, which includes low-intensity laser exposure with preliminary application of the liposomal gel, is more effective than the standard approach to the treatment of this pathology.

Summary. The article covers the modern scientifically based treatment concept of gingivitis, associated with biofilm, in childhood and adolescence. It is emphasized that the priority direction of combating microbial biofilm is regular individual and professional hygiene of the oral cavity. Mechanisms of action and age criteria for choosing antibacterial and antiinflammatory drugs for treatment of inflammatory periodontal diseases in children and adolescents are described. The need for supportive therapy to ensure periodontal health in children and adults is substantiated. Key words: children, gingivitis, biofilm, professional hygiene, treatment.

To study the effectiveness of the drug Cholisal as part of the conservative treatment of chronic periodontitis. We selected 100 patients aged 35 to 65 years of both sexes with a diagnosis of moderate chronic periodontitis in the acute stage with a periodontal pocket depth of 3.5-5 mm. Depending on the tactics of conservative treatment of periodontitis, patients were divided into two groups of 50 people. In the main group, Cholisal dental gel was used as part of complex conservative treatment, and in the control group, Metrogil-denta gel was used. To assess the effectiveness of treatment, a dental examination of patients was carried out with an index assessment of the condition of periodontal tissues and a biochemical analysis of the content of arachidonic acid and prostaglandin E2 in gingival blood, comparing the indicators before treatment and 14 days after the start of treatment. When the drug Cholisal was included in complex treatment, 14 days from the start of treatment, patients experienced a statistically significant decrease in the depth of periodontal pockets from 4.7±0.32 mm to 3.6±0.19, and the Green-Vermillion hygiene index by 60.7%, Silness-Loe plaque index by 73.1%, PMA index by 68.8%, Muhlemann-Cowell bleeding index by 68.0% (p<0.001 compared to baseline). When Metrogil-denta gel was used in complex therapy, the effectiveness of treatment was lower: the depth of periodontal pockets did not change significantly (from 4.5±0.22 mm to 4.2±0.17 mm, p>0.05), reduction in the hygiene index Green-Vermillion was 51.9%, Silness-Loe plaque index - 64.0%, PMA index - 43.7%, Muhlemann-Cowell bleeding index - 45.8% (p<0.001 compared to baseline, p<0.001 compared to the main group). A laboratory study showed that in patients of the main group, after completing a course of conservative treatment, the content of biomarkers of inflammation significantly decreased compared to the initial level (p<0.05), while in patients of the control group the content of arachidonic acid and prostaglandin E2 in the gingival blood during the study period did not change significantly (p>0.05 compared to the initial level). The use of the drug Cholisal in the conservative treatment of chronic periodontitis has demonstrated more pronounced positive dynamics of clinical and biochemical parameters compared to traditional therapy, which suggests its high effectiveness.

Study of the antibacterial efficacy of an antiseptic drug activated by low-frequency and high-frequency ultrasound in an experiment

The aim of the work was an experimental study of the effectiveness of activation of an antiseptic drug, using low-frequency and high-frequency ultrasound, against pathogenic microorganisms obtained from periodontal pockets. As a result of automatic cultivation of clinical strains: S. sanguis, P. intermedia, C. albicans, revealed more pronounced antimicrobial effect of Miramistin activated by low-frequency ultrasound (26.5 kHz) compared with the activation of this drug by high-frequency ultrasound (880 kHz). In the treatment of inflammatory periodontal diseases, in order to obtain a pronounced antimicrobial effect, preference should be given to the combined effects of low-frequency ultrasound and the antiseptic drug Miramistin 0.01%.

Comparative evaluation of the effectiveness of light activation of photosensitizers in photodynamic therapy of periodontitis using different wavelengths

Dentists are always in quest of new methods for the treatment of periodontal diseases, and photodynamic therapy (PDT) is considered one of the most effective methods. A significant improvement in the clinical pic-ture was obtained in all patients throughout the control period of observation of the patients as a result of the treatment with the use of PDT. According to the data from research study it can be said that photodynamic therapy is an effective and gentle method of antibacterial treatment of inflammatory periodontal diseases, which can be effectively used to prevent exacerbations of chronic inflammatory periodontal diseases, to achieve a long-term positive effect and to refuse, in some cases, from surgical methods of treatment. There is a signifi-cant increase in the duration of remission of the disease, improvement of microcirculatory blood circulation in per-iodontal tissues and a small microbial contamination of periodontal pockets of patients treated with photodynam-ic therapy in the long term compared with groups of patients treated with only antiseptic and antibacterial drugs.

ОСОБЕННОСТИ ПРОФИЛАКТИКИ СТОМАТОЛОГИЧЕСКИХ ЗАБОЛЕВАНИЙ У ПАЦИЕНТОВ C САХАРНЫМ ДИАБЕТОМ

We consider interrelation of diabetes mellitus and dental health. Knowledge of dentists and dental patients about diabetes is insufficient. The role of a dentist in the early diagnosis of diabetes mellitus and the features of the treatment of dental diseases in diabetic patients are discussed. Diabetes mellitus is one of the diseases receiving much attention in modern scientific and medical societies. It is easy to explain such a high interest: diabetes mellitus ranks third among the direct causes of death after cardiovascular and oncological diseases, therefore, the solution of issues related to diabetes mellitus is put at the state level in many countries. According to various sources, there are from 120 to 180 million diabetic patients in the world, which is 2-3 % of the total population of the planet. The incidence of diabetes mellitus seemed to become endemic and a crisis in the health sector - according to forecasts of the World Health Organization, by 2025 the number of diabetic patients in the world will increase to 333 million people. Along with the increase in the number of patients with diabetes mellitus, the number of patients suffering from complications of this serious disease increases, which leads to rapid disability. The objective of this study was to increase the effectiveness of prevention and treatment of inflammatory periodontal diseases in patients with diabetes mellitus.

Dental gel Cholisal at the stage of conservative treatment of inflammatory periodontal diseases

To study the effectiveness of using the drug Cholisal as part of the conservative treatment of chronic periodontitis. We selected 100 patients aged 35 to 65 years of both sexes with a diagnosis of moderate chronic periodontitis in the acute phase with a periodontal pocket depth of 3.5-5 mm. Depending on the tactics of conservative treatment of CGP, patients were divided into two groups of 50 people. In the main group, in addition to standard treatment, the dental gel Cholisal was used, and in the control group, therapy was standard. 10 days after professional hygiene, patients in both groups were examined and underwent an index assessment of the condition of periodontal tissues and adherence to treatment. In patients of the control group, 10 days from the start of treatment, the depth of periodontal pockets in the control group decreased slightly from 4.7±0.28 mm to 4.2±0.21 mm (p=0.074), and the Green-Vermillion hygiene index decreased by 25.3±1.79% (p=0.041), Silnesse-Loe plaque index by 59.1±2.16% (p<0.001), PMA index by 51.5±1.92% (p<0.001) and Muhlemann-Cowell bleeding index by 42.2±1.75% (p<0.001). In the main group, the effectiveness of treatment of chronic periodontitis with conservative therapy using Cholisal was higher. There was a statistically significant decrease in the depth of periodontal pockets from 4.8±0.23 mm to 3.5±0.19 mm (p=0.043), the Green-Vermillion hygiene index decreased by 47.6±2.13% (p=0.0003), Silnesse-Loe plaque index by 78.2±3.05% (p<0.001), PMA index by 69.4±2.74% (p<0.001) and Muhlemann-Cowell bleeding index by 66.9±1.62% (p<0.001). The use of the drug Cholisal in the conservative treatment of chronic periodontitis has shown convincing positive dynamics in both subjective and objective assessments, which suggests its effective use.

Review of extended-release drug delivery systems for non-surgical treatment of inflammatory periodontal diseases

Relevance. The onset of periodontal pathogen resistance to systemic antibiotics revealed a new direction in periodontics. The characteristics of local drug delivery systems allow for a prolonged impact on periodontal pocket microbiota.Materials and methods. The non-interventional study was carried out by an analytical review of Russian and international scientific publications of independent clinical and laboratory research databases PubMed, ResearchGate, ScienceDirect, and eLIBRARY.RU for the last decade. The search was performed using the keywords: "periochip", "periodontal treatment", "the matrix", "tissue engineering", and "bioactive cryogels". The review included randomized controlled studies, controlled clinical trials, prospective and retrospective comparative cohort studies of drug delivery systems and systematic reviews.Results. In contrast to systemic drug administration, biodegradable local drug delivery systems provide long-lasting and effective treatment at the lesion site at much lower doses and in a shorter period of time. The prolonged drug activity time combined with non-surgical treatment of inflammatory periodontal diseases results in faster soft tissue regeneration. Gels, foams, fibres, bioactive cryogels, micro- and nanoparticles, and chips may be biodegradable matrices for the extended-release drug delivery system. The basic polymer for all these forms can be of natural origin (proteins, collagen), semi-synthetic (cellulose derivatives) or synthetic.Conclusion. Films, chips and bioactive cryogel are nowadays more common forms. Due to its unique structure, the bioactive drug cryogel can serve as a controlled and multifunctional system for drug delivery to the periodontal tissues, which determines its importance in periodontics and high interest f or further research.

Study of the clinical efficacy of the crystalline form of the immunomodulatory drug based on antimicrobial peptides and cytokines complex in the treatment of patients with inflammatory periodontal diseases

On the basis of clinical periodontal indices of Muhlemann, RMA and Silness-Loe, the effectiveness of a new water-soluble form of the immunomodulatory drug Superlimf L in the treatment of inflammatory periodontal diseases was studied.&#x0D; Material and methods. 2 groups of patients with moderate periodontitis were formed: the experimental group (Group A) and the control group (Group B) (30 patients in each group). Before the start of treatment, all patients were taught the rules of correct brushing. Piezon-Master-400 and polishing the surfaces of the roots of the teeth in patients of the experimental group (A) were introduced into the periodontal pockets on cotton turundas with a solution of Superlimf® L. Previously, the pulvis of the Superlimf® L was dissolved in 5 ml of distilled water. Cotton turundas soaked in the preparation solution were introduced into periodontal pockets and soaked swabs were applied to the surfaces of gum — for 40 minutes. The procedures were carried out daily for 2 weeks. In the control group, the intervention was limited to the removal of dental deposits and subsequent controlled brushing of the teeth once every 3 days. The duration of the study was 2 weeks. An index assessment of the periodontal condition in patients of both groups was performed before treatment and, 3, 7 and 14 days after treatment.&#x0D; Treatment results. Clinically visible improvement in the condition of periodontal tissues after mechanical treatment occurred in patients of both groups after 3 days. The dynamics of the Silness-Loe index was very demonstrative in patients of both groups. Being initially almost equally high (2.35±0.45 units), after 3 days these values decreased by more than half. However, the difference in the decrease in the value of the Muhlemann index and PMA in patients of groups A and B was very significant. If in the control group (group B) after 3 days the intensity of bleeding during a probe test decreased by no more than 20%, then in patients in group A it decreased by almost half. In subsequent periods of observation, the decrease in inflammation and bleeding indices was also almost 3 times more intense in patients in group A than in patients in the control group. It is characteristic that the Silness-Loe plaque index already on the 7th day in patients in group B was significantly higher than in patients in group A. By the end of the study, both clinically and according to the index assessment of the condition of the periodontium in patients who received Superlimf® L, the condition of periodontal tissues was significantly better than in patients in the control group (Group B).&#x0D; Conclusion. Pronounced efficacy, the absence of adverse effects when using the product, good organoleptic indicators — all of the above allows specialists to use the powder form of the new drug Superlimf® L for the treatment of inflammatory periodontal diseases both in the conditions of dental units, as well as recommend to patients after their preliminary instruction and training apply the remedy at home.

Клінічна ефективність місцевого лікування запальних захворювань тканин пародонту в осіб підліткового та юнацького віку, що палять

The high prevalence of periodontal disease requires the development of effective treatment and prevention measures in teenagers and young adults who smoke. Purpose - to study the effectiveness of the proposed local treatment of inflammatory periodontal disease in teenagers and young adults who smoke. Materials and methods. To achieve this goal, changes in periodontal tissues before and after treatment were studied in 123 teenagers and young adults aged 15 to 24 years, which were divided into groups: group I involved 31 people who regularly smoke traditional cigarettes; to group II - 26 people who regularly smoke electronic cigarettes (Weipi); to group III - 30 people who regularly smoke tobacco heaters (IQOS); to group IV - 36 people without a bad habit of smoking. The dynamics of OHI-S, PMA and PBI indices before and after treatment with the proposed method of treatment - a combination of Stomolik solution and Holisal gel in comparison with the generally accepted treatment was analyzed. Results. During the treatment process, a positive trend of the studied indices was registered in all observation groups, but the best results were registered in the groups where the proposed local treatment was used, which is confirmed by the data of the objective survey. Conclusions. Comparison of the results of local treatment of inflammatory diseases of periodontal tissues in the observation groups with different methods indicates the advantages of the proposed method of treatment - a combination of Stomolik solution and Holisal gel. The research was carried out in accordance with the principles of the Helsinki Declaration. The study protocol was approved by the Local ethics committee of the participating institution. The informed consent of the patient was obtained for conducting the studies. No conflict of interests was declared by the author. The use of an adhesive balm with magnetic polymers, hexethydine, cytylpyridinium chloride and hyaluronic acid in the treatment of inflammatory periodontal diseases

The article presents the results of a study of an adhesive balm with magnetic polymers, hexethidine, cytylpyridinium chloride and hyaluronic acid in the treatment of inflammatory periodontal diseases. Revealed and scientifically proven its effectiveness in acute and subacute periods of the course of gingivitis and periodontitis. The study involved 40 patients aged 20 to 60 years, in whom the anti-inflammatory and hemostatic effect of the adhesive balm on the inflamed periodontal soft tissues was studied. During the monitoring of index indicators, it was revealed that the PMA index decreased from 50,72 to 16,31 by the end of the study. After 14 days, the hemostatic efficiency was 56,25%. Local allergic reactions and irritation were not observed during the study. It has been proven that an adhesive paste with magnetic polymers, hexethydine, cytylpyridinium chloride and hyaluronic acid has a bactericidal effect on the microflora of the oral cavity.

CLINICAL AND LABORATORY EVALUATION OF A NOVEL DENTAL GEL FOR TREATMENT OF CHRONIC CATARRHAL GINGIVITIS

This article presents the study of a dental gel developed by us, containing, as an active ingredient, sanguitrithrin and oil extract from calendula flowers and yarrow grass. As a result of our studies of the dental gel on 1,844 children and adolescents, we revealed that it has antiinflammatory and wound-healing activities and produces anti-inflammatory, reparative, hemostatic and trophic effects. The results of the study make it possible to use the dental gel in clinical practice for treatment of inflammatory periodontal diseases in children. We substantiated the prospects of using the phytocomplex in the form of a dental gel and proved it as effective and safe. Besides, its dosage form is convenient for use in dental practice and even provides advantages over known domestic analogues.

ОЦЕНКА КОМПЛЕКСНОГО ЛЕЧЕНИЯ C ИСПОЛЬЗОВАНИЕМ СТОМАТО-ЛОГИЧЕСКОГО ГЕЛЯ ОРИГИНАЛЬНОГО СОСТАВА НА ПОКАЗАТЕЛИ ПСИХОЭМОЦИОНАЛЬНОГО И СТОМАТОЛОГИЧЕСКОГО ЗДОРОВЬЯ КУР-САНТОВ ВЕДОМСТВЕННОГО ИНСТИТУТА

Subject. Dental diseases are a frequent companion of distress, since the negative impact of psychoemotional stress on the immune status of cadets is reflected in changes in the microflora of the oral cavity and periodontal [4, 9]. &#x0D; The aim of the study is to evaluate the results of using the developed gel for the preven-tion and treatment of inflammatory periodontal diseases in cadets who are under psychoemotional stress.&#x0D; Methodology. The group of examined persons consisted of 245 people-cadets of 1–4 courses of the Ministry of Internal Affairs of the Russian Federation. After the dental exami-nation, the cadets were divided into 2 groups: 26 cadets were found to have gingivi-tis/periodontitis of mild severity (group I), and 45 cadets without periodontal lesions (group II) served as a comparison group. The study included psychological testing, clarification of the state of their vegetative reactions and cortisol content in saliva, molecular genetic deter-mination of the composition of the most common periodontal pathogenic bacteria of the dentoalveolar furrow and enzyme immunoassay of the content of pro-inflammatory cytokines in saliva, cell-regulatory and immunosuppressive effects. &#x0D; Results.The application of the gel helps to correct the phenomena of depression, while it affects the frequency of relapses of diseases.The effect of the application on the frequency of detection of periodontal pathogenic bacteria in cadets is manifested in relation to Tanerella forsythia and Aggregatibacter actinomycetemcomitans in the absence of depression, but in the presence of periodontal lesions. When the phenomena of gingivitis/periodontitis of mild se-verity occurred against the background of depression, the effects of using dental gel were in the form of a decrease in the occurrence of these bacteria. There was a significant drop in the oral fluid of IL-12 in the group with depression and periodontal disease, as well as IL-10 in the group without depression, but with mild gingivitis/periodontitis.&#x0D; Conclusions. The developed dental gel is recommended both for the phenomena of de-pression and in cases of mild gingivitis/periodontitis against the background of psychoemotional stress.

ИЗМЕНЕНИЕ ФАКТОРОВ НЕСПЕЦИФИЧЕСКОЙ ЗАЩИТЫ РОТОВОЙ ПОЛОСТИ У ДЕТЕЙ С ГИНГИВИТОМ НА ФОНЕ ПРИМЕНЕНИЯ ЭФИРНЫХ МАСЕЛ И БЕНТОНИТОВОЙ ГЛИНЫ

Introduction. The prevention and treatment of inflammatory periodontal disease in children remains a topical issue in dentistry. This is due to the increasing prevalence of this disease in the early age groups. Remission periods of diseases in children are short. Therefore, treatment and preventive measures of periodontal tissue diseases in this population should be primarily focused on the regulation of nonspecific protection system at the local level. The components of the system are secretory immunoglobulin A, lysozyme and modified albumin. Using means of natural origin as regulators will allow to optimize treatment regimens and reduce allergic reactions. Aim. The aim of the study was to study the effectiveness of the complex for applications to the gingiva, consisting of means of natural and mineral origin of the Crimean deposit, in the regulation of local oral immunity in children with chronic catarrhal gingivitis. Material and methods. Twenty-eight children aged 11–12 years with moderate chronic catarrhal gingivitis were examined. The control group consisted of 12 healthy children of the same age. We used applications of phytopreparations on the gingiva in the treatment of chronic catarrhal gingivitis daily for 20 minutes, No 10. Complex for applications was essential oils of peppermint, pine, fennel, eucalyptus (1:1:1:1) on the basis of bentonite clay (10:1). Clinical and laboratory parameters were determined in the dynamics. Statistical processing was performed using the standard method of variation statistics. Comparison of the mean indices was conducted using Student’s t-criterion. Differences were considered reliable at p&lt;0,05. Results and discussion. The criterion for the effectiveness of treatment was the positive dynamics of paraclinical indices (p&lt;0,001). Indicators of oral dysbiosis decreased and reached values of oral microbiocenosis of healthy children. The content of sIgA increased 1,8 times (p&lt;0,001). The level of salivary lysozyme activity increased 1,7 times (p&lt;0,001) and reached normal values. The concentration of modified albumin decreased 1,6-fold. Conclusion. Inclusion of applications of the combination of essential oils based on bentonite clay of the Crimean deposit into the plan of treatment of chronic catarrhal gingivitis in children contributes to a pronounced relief of the signs of inflammation in the gingiva. The proposed complex has a pathogenetic orientation in the treatment of chronic catarrhal gingivitis, which is confirmed by the pronounced dynamics of nonspecific immunity parameters.

In this article the review of foreign and domestic literary sources, which are devoted to the actual problem of modern dentistry – the treatment of inflammatory diseases of periodontal tissues: gingivitis and periodontitis are presented. The complex approach to their treatment involves the appointment of a significant amount of pharmacotherapeutic drugs. Therapeutic failures and iatrogenic complications have led to the fact that today the interests of doctors and population to medicinal products significantly increased. The purpose of the study is to analyze the data of scientific literature on the use of plant-based medicinal products for the treatment of periodontal inflammatory diseases over the past 10 years. Materials and methods. Comprehensive and systematic analysis of literature. Review and discussion. The analysis of information sources on the use of plant-based medicinal products in dentistry both independently and in the composition of medical and prophylactic means has established that the modern assortment of plant-based preparations in the pharmaceutical market of Ukraine to a certain extent is limited. The emergence of new plant-based species that have been tested in conditions of experimental pathology and require an evidence-based clinical base is noted. The composition of plant-based preparations used for the treatment of inflammatory periodontal diseases include vitamins, biologically active substances, glycosides, alkaloids, in connection with a wide range of action: antiseptic, anti-inflammatory, regenerating, hemostatic, antioxidative. The data on plant-based preparations that are most often used such as chamomile extracts, calendula, hypericum, plantain, kalanchoe, aloe, eucalyptus, milfoil, nettle, calamus and plant-based species are summarized. The medicinal agents considered are mainly recommended for local treatment of periodontal diseases in the form of dental care means, mouth rinse, gel, chewing gum, herbal liquer. It is known that the complex treatment of periodontal diseases includes a general influence on the body. The properties of green tea with its wide range of actions are investigated. With antioxidant properties, it can be a healthy alternative for controlling destructive changes in periodontal diseases. Attention is drawn to the proposed unique natural complex “Resverazin” due to a wide range of pharmacological action, low toxicity and relative safety. The drug produces antioxidant, anti-inflammatory, immune stimulating, vasodilative, neuroprotective action. Conclusion. Based on the literature analysis, it can be concluded that the accumulated experimental and clinical data on the therapeutic properties of plants prove perspective of their use in the complex treatment of inflammatory periodontal diseases. Future studies are mandatory for further confirmation of the effectiveness of these medicinal plants
