PREPARO DO CANAL RADICULAR COM SISTEMA PRODESIGN LOGIC - RELATO DE CASO CLÍNICO

The objective of this study was to describe the preparation of root canals with a rotary system in a maxillary premolar. A 26-year-old patient, classified as ASA I, attended the Faculty of Dentistry of Pernambuco – FOP due to the presence of spontaneous pain in the region of the upper left first premolar. After clinical, radiographic examinations and sensitivity tests with an intense response, it was concluded that the probable diagnosis was symptomatic irreversible pulpitis. The preparation of the root canal was carried out with the ProDesign Logic rotary system and for this purpose, the Glide Path 25.01 file was used in order to explore the buccal and palatal canals. Odontometry was performed with the Root ZX apical locator (J.Morita) and confirmed by the radiographic method. The filling of the root canal system was performed using the single cone technique, 30.05 (HB). The final x-ray of the case was performed. It was concluded that the ProDesign Logic system determined a good shape of the canals, thus facilitating the filling of the root canal system.

Comparative Evaluation of Dentinal Microcrack Formation During Root Canal Preparation Using Different Rotary Files: An In Vitro Study

Background:The success of endodontic treatment depends on completely removing all vital and necrotic pulp tissue, microorganisms, and microbial toxins from the root canal. During the cleaning and shaping process, the canal is shaped through the interaction between instruments and the dentin walls. These interactions can create several temporary stress points within the dentin, which may lead to the development of dentinal defects, craze lines, or microcracks. As a result, the techniques employed for shaping root canals carry the risk of starting crack formation, which, under functional pressure, could progress into complete fractures. This study was aimed to compare and evaluate the incidence of dentinal microcrack formation during root canal preparation using ProTaper Universal, ProTaper Gold, NeoEndo Flex and Hyflex EDM files. Materials and Methods: A total of 95 freshly extracted single rooted premolars with single straight canal were selected and stored in normal saline. Each specimen was examined using dental operating microscope to exclude cracked samples. Specimens then decoronated using a diamond disc and water as a coolant to a standardized length of 14 mm. Canal patency was determined by passing 10 K file passively and the working length was established 1 mm short of the file length at which the file become visible at the apical foramen. Canal was then enlarged upto15 k size file. Teeth samples were then divided into 5 groups having 19 teeth each. Group 1: Control Group (Uninstrumented teeth), Group 2 : Protaper Universal, Group 3 : Protaper Gold, Group 4 : NeoEndo Flex ,Group 5 : Hyflex EDM. After completion of cleaning and shaping procedure (to a standard apical diameter of 25), canal was rinsed with distilled water. Root was then horizontally sectioned at 3 mm, 6 mm and 9 mm from the apex with diamond disc at low speed under water cooling. All the sections were evaluated under stereomicroscope for presence of cracks and images were recorded. Cracks were observed as lines extending from the root canal lumen to the dentin or from the outer surface into the dentin. Results: All the file systems used in this study showed dentinal microcracks. ProTaper Universal exhibited the highest percentage of dentinal defects, while Hyflex EDM displayed the lowest among the file systems used (ProTaper Universal, ProTaper Gold, NeoEndo Flex and Hyflex EDM). The defects were present more in the apical third (3 mm) as compared to 6 mm (middle third) and 9 mm (coronal third) sections

Iatrogenic Root Perforations: Case Report

Root perforation, a connection between the root canal system and tooth surface, is a common cause of endodontic therapy failures. Caused by pathological conditions or operative errors, it can occur during access cavities, root canal preparation, or post-preparation. A 28-year-old patient was diagnosed with a vestibular medium-sized infra-osseous perforation during root post realization. Despite no systemic disorders, the patient showed physiologic movement and was slightly sensitive to pressure. A flap surgical treatment was chosen to remove excess material and enable tissue regeneration. Biodentine® was used to prevent future discolorations. The patient underwent an intrasulcular incision, flap raised, cavity cleaned, disinfected, revised, and sealed using an anterior Schilder plugger. Root perforations are common errors caused by factors like anatomy, caries, pulp calcifications, and intracanal posts. Diagnosis is based on clinical signs, radiographic examinations, and imaging techniques. The prognosis depends on preventing bacterial infection. The ideal reparation material should stimulate bone formation, be biocompatible, maintain sealing, and not disintegrate in tissue fluid. Equivalent materials like Biodentine® offer advantages in protocol, antibacterial power, and bioactive properties. Radicular perforations, a significant dental issue, can be prevented through early management using modern techniques and clinical training, ensuring long-term success in endodontic treatments.

Efficacy of sonic and ultrasonic activation on irrigant penetration in different tapered preparations: An in vitro study

ABSTRACT Aim: The present study aims to evaluate the irrigant penetration in extracted single-rooted teeth using various irrigant activation systems in different tapered preparations. Methods: One hundred and twenty-four single-rooted teeth were selected for the present study. They were standardized at the level of cementoenamel junction and shaped to 30 size using 4% and 6% taper rotary instruments under a standard irrigation regimen. Once the complete root canal preparations were done based on activation protocols, they were divided into four groups as Group 1: Four percent preparation sonic activation (n = 31), Group 2: Six percent preparation sonic activation (n = 31), Group 3: Four percent preparation ultrasonic activation (n = 31), and Group 4: Six percent preparation ultrasonic activation (n = 31). A radiopaque solution in conjunction with radiography was utilized to evaluate the penetration of irrigants into the canal. Radiographs were taken after dye injection to measure the length of irrigant penetration in each technique. Results were statistically analyzed using SPSS software. One-way ANOVA and Post hoc Tukey tests were performed. Results: The results of the study revealed that ultrasonic activation after 6% taper instrumentation was better followed by ultrasonic activation after 4% taper instrumentation (P < 0.05). The results revealed a statistically significant difference (P < 0.05) between the irrigation techniques. Conclusion: Ultrasonic irrigant activation proved to be a better and an effective technique for cleaning the root canal.

Comparison of cyclic fatigue resistance of four pediatric rotary file systems at body temperature: an in vitro study

BackgroundThe aim of this study was to compare the cyclic fatigue resistance (CFR) of the newly developed pediatric nickel-titanium (NiTi) rotary file systems for root canal preparation of primary teeth.MethodsEighty pediatric NiTi rotary file systems files were used in this study, including 20 EasyInSmile X-Baby (25/0.04), 20 Scope miniScope (25/0.04), 20 EndoArt Pedo Gold (25/0.04), and 20 EndoArt Pedo Blue (25/0.04) files. Static CFR tests; performed on a custom-made stainless steel block with an inner diameter of 1.5 mm, an angle of curvature of 60° and a curved artificial canal with a radius of curvature of 5 mm. The test system was filled with distilled water and the temperature was kept constant at 35 ± 1 °C. The files were rotated in the simulated canal until fracture occurred. The number of cycles to failure (NCF) data was recorded, and all the fracture surfaces of the files were evaluated using scanning electron microscopy (SEM). One-way ANOVA and Tukey tests were used for statistical analysis of the data, and the significance level was set at p < 0.05.ResultsEndoArt Pedo Blue showed the highest NCF values (2668.10 ± 755.26), while the miniScope showed the lowest NCF values (453.65 ± 72.51), with a statistically significant difference between all the tested file systems (p < 0.05). There was no statistical difference in terms of fractured fragment lengths among all tested files (p > 0.05).ConclusionThe EndoArt Pedo Blue file system showed the best CFR performance among the four file systems specifically designed for primary teeth.

Enhancing Root Canal Therapy with NIR-II Semiconducting Polymer AIEgen and Low-Concentration Sodium Hypochlorite Synergy.

Despite significant efforts to eliminate bacterial biofilm within root canals, achieving effective disinfection remains challenging due to the complex anatomy and limitations of disinfectants. In this study, a second near-infrared (NIR-II) semiconducting polymer with aggregation-induced emission (AIE) properties, named PIDT-TBT, is deliberately designed and synthesized. This proposes an AIE luminogen-based sterilization strategy in synergy with a low concentration of sodium hypochlorite (NaClO). Water-dispersible PIDT-TBT nanoparticles (NPs) are prepared, demonstrating good biocompatibility, as well as photothermal and photodynamic properties. Subsequent antibacterial tests show that PIDT-TBT NPs exhibit excellent bactericidal effects against three bacterial strains: Staphylococcus aureus, Streptococcus mutans, and Enterococcus faecalis, upon 808nm laser irradiation. In synergy with a low concentration of NaClO (0.5%) solution, PIDT-TBT NPs significantly improves the outcome of root canal treatment under 808nm laser irradiation in a human extracted tooth root canal infection model. Additionally, it is found that PIDT-TBT NPs combine with a low concentration of NaClO solution could safely dissolve dentin debris and further increase the efficiency of root canal preparation by altering the elemental composition of the inner root canal wall.

Root canal transportation with ProTaper Next and WaveOne Gold: A systematic review

Root canal transportation is a significant concern in endodontic therapy, impacting treatment outcomes. ProTaper Next (PTN) and WaveOne Gold (WOG) are popular endodontic instrumentation systems used to shape root canals. This systematic review aimed to evaluate the literature on root canal transport with PTN and WOG. Fourteen studies were analyzed, highlighting the overall effectiveness of both systems. The PTN showed potential advantages in minimizing canal transport, while the WOG preserved the original canal shape. Operator experience and canal curvature influence performance. This review provides evidence-based insights to aid endodontic practitioners in making informed decisions for successful root canal treatments. The aim of this systematic review was to evaluate and synthesize the literature on root canal transportation with ProTaper Next (PTN) and WaveOne Gold (WOG) instrumentation systems in endodontic therapy. A comprehensive literature search was conducted using the PubMed Central, Scopus, EBSCO, Embase and Web of Science databases from 2000 to 2022. Studies that met the predefined inclusion and exclusion criteria were selected for analysis. Data on study design, participant characteristics, intervention details, comparison groups, outcomes, and results were extracted from the included studies. The methodological quality of the studies was assessed using appropriate tools, and the findings were synthesized qualitatively. A total of 14 studies were included in the systematic review. The analysis revealed that both the PTN and WOG systems demonstrated overall effectiveness in root canal preparation. PTN rotary files performed adequately with inexperienced operators and showed potential advantages in minimizing canal transportation. On the other hand, WOG files exhibited a conservative approach, preserving the original canal shape. However, variations in performance were observed depending on the instrumentation method, operator experience, and canal curvature. This systematic review provides valuable insights into the ability of the PTN and WOG to manage root canal transport. Both systems demonstrated satisfactory outcomes, with WOG often preserving the natural canal anatomy. The findings suggest that proper operator training and technique play vital roles in achieving optimal results. Clinicians should consider the unique characteristics of each system when selecting instrumentation for root canal treatments. Nonetheless, it is crucial to acknowledge the limitations of the included studies, such as variations in sample sizes and operator experience, which may impact the interpretability of the findings. Future research should address these limitations and further explore the efficacy of these instrumentation systems in various clinical settings. The evidence derived from this systematic review can aid endodontic practitioners in making evidence-based decisions for successful root canal procedures.

Regressão de lesão periapical após tratamento não cirúrgico – Relato de caso

The present study aims to present the clinical case report regarding non-surgical endodontic treatment performed on tooth 22, with diffuse periapical lesion, suggesting chronic dento-alveolar abscess. The methodology consists of an individual descriptive study of the case report type, which includes radiographic images of a 23-year-old female patient. Clinically, in addition to swelling, it was found that there was mobility, absence of pain in the thermal test (cold) and on palpation. After a complete clinical examination, endodontic treatment, coronary opening, root canal exploration, and root canal preparation were initiated using the modified Oregon crown-apex preparation technique, associated with 2.5% sodium hypochlorite as an irrigating solution. Calcium hydroxide paste was applied as an intracanal medication for a period of 30 days and the coronary access was sealed with glass ionomer. In the following session, after verifying the absence of clinical signs and symptoms, it was decided to fill the root canal using the Tagger's Hybrid technique associated with gutta-percha and AH Plus endodontic cement. After the 24-month preservation period, tissue repair and bone neoformation of the area were observed, as well as the absence of clinical signs.

Quantitative evaluation of the apically extruded debris from root canals prepared by single-file rotary and reciprocating file systems: An in vitro study.

The aim is to investigate and compare the amount of apically extruded debris after root canal preparation using single rotary and reciprocating file system. Forty single-rooted human mandibular premolars with straight canals were randomly assigned to four groups (n = 10). The root canals were instrumented according to the manufacturers' instructions using single-rotary file systems Hyflex EDM (HEDM), One Shape (OS) and single-reciprocating file systems Wave OneGold (WOG) and OneRECI (OR). The apically extruded debris was collected in preweighed glass vials using the Myers and Montgomery method. After drying, the mean weight of the extruded debris was assessed using a microbalance. Multiple comparison between groups demonstrated that the mean weight of apically extruded debris in the HEDM group was significantly less compared to all other study groups, and the differences were statistically significant (P < 0.001). Under the condition of this study, all file systems caused apical debris extrusion. The mean apically extruded debris was significantly least in HEDM, followed by WOG, OneRECI and highest in the OS group.

Impact of using single-file reciprocating system on the quality of root canal treatment treated by undergraduate students

BackgroundThis study aimed to evaluate the use of a single-file reciprocating system on the technical quality of root canal filling and treatment by radiographic assessment of cases treated by undergraduate dental students. Materials and methodsThis was a retrospective cross-sectional clinical study of endodontically treatments conducted by fourth-year undergraduate students during the academic year 2021–2022. Root canal preparation was performed using the WaveOne Gold system with matching single-cone gutta-percha. The collected data included sex, tooth position, number of canals, and treatment time (first or second semester). The quality of the root canal filling was assessed based on the root canal filling length, density, and taper. The presence of ledges, apical transportation, perforation, and instrument separation were recorded. Data were analyzed using SPSS version 28. Chi-square tests were used, and the statistical significance level was set at P < 0.05. ResultsA total of 601 teeth were included. The length of the root canal filling was adequate in 93.51 % of the teeth, underfilled in 2.82 %, and overfilled in 3.66 %. The density and taper were adequate in 96.5 % and 98.16 % of the teeth, respectively. There were no significant differences among the parameters of length, taper, density, or procedural errors relative to the arch type. However, there was a significant difference between the anterior and premolar teeth in terms of taper, density, and overall quality of the root canal filling. The overall quality of root canal treatment was acceptable in 527 teeth (87.68 %), with no significant difference between teeth (P = 0.256). ConclusionThe quality of root canal treatment performed by undergraduate students using a single-file reciprocating system is good or acceptable.

Efficacy of Different Irrigation Needles and Ultrasonic Activation on Calcium Hydroxide Removal: A Micro-CT Study Using 3D-Printed Endodontic Models

IntroductionThe aim of this study was to evaluate the efficacy of different irrigation needles and passive ultrasonic activation in removing Ca(OH)2 from an endodontic model that duplicated a root canal configuration of a human natural tooth. MethodsAn extracted human maxillary premolar was subjected to root canal preparation and scanned with microcomputed tomography. A 3-dimensional reconstruction model of the natural tooth was printed to endodontic models using a polyjet printer. The root canals of the models were filled with Ca(OH)2 paste and divided into 2 groups based on the irrigation protocol: conventional syringe-needle irrigation (conventional group) and passive ultrasonic irrigation (PUI) group. Each group was subdivided into 3 groups (n = 10) according to the type of needle: half-cut, side-vented, and TruNatomy irrigation needle. Microcomputed tomographyimaging was used to assess the percentage of reduction of Ca(OH)2. Data were analyzed using two-way analysis of variance test (α = .05). ResultsThe side-vented and TruNatomy irrigation needles showed significantly higher percentage reductions than the half-cut needle (P < .05) in the conventional irrigation group. The PUI group showed significantly higher percentage reductions of Ca(OH)2 than the conventional group regardless of the type of needle (P < .05). However, no significant difference was found among the needles in the PUI group. ConclusionsThe type of irrigation needle and the use of PUI influenced the removal efficacy of Ca(OH)2. PUI enhanced the removal of Ca(OH)2 regardless of the type of irrigation needle.

Effects of the Irrigation Needle Design on Root Canal Disinfection and Cleaning

IntroductionThis ex vivo study evaluated the disinfecting and cleaning effects of root canal preparation using sodium hypochlorite irrigation with 3 different needle designs. MethodsMesial roots from extracted mandibular molars with Vertucci class II configuration were anatomically matched based on micro-computed tomography (micro-CT) analyses and distributed into 3 groups (n = 18/group). The canals were contaminated with a mixed bacterial culture for 30 days and then subjected to preparation using 2.5% sodium hypochlorite irrigation with open-ended flat needle (3 mm short of the working length - WL), closed-ended side-vented irrigation needle (1 mm short of the WL), or a closed-ended plastic needle with 2 lateral outlets (TruNatomy) (1 mm short of the WL). Bacteriological samples were taken from the canals before (S1) and after preparation (S2). After another micro-CT scan, the roots were sectioned and samples were also taken from the apical canal segment (S3). Bacterial reduction was assessed by quantitative real-time polymerase chain reaction. The amount of accumulated hard tissue debris was evaluated by micro-CT. ResultsA substantial reduction in bacterial counts was observed in all 3 groups when comparing S1 with S2 (99.95%, 98.93%, and 98.90% in the open-ended, closed-ended, and TruNatomy needle groups, respectively) (P < .01). No significant differences were observed between groups for bacterial reduction in the full and apical canal (P > .05). There were no intergroup differences in the amount of accumulated hard tissue debris either (P > .05). The open-ended needle group showed significantly more specimens with quantitative real-time polymerase chain reaction negative results for bacteria in S3 than TruNatomy (P < .05). ConclusionWhen used up to an appropriate insertion depth, the 3 needle types result in similar antibacterial and cleaning performance, provided variables such as needle size and irrigant type, volume and flow rate are controlled. Exclusive analysis of the apical segment, including the isthmus, revealed that the open-ended needle yielded more cases negative for bacteria.

Post-operative pain after root canal preparation with different apical finishing sizes a triple blinded split mouth clinical trial

BackgroundThis is a triple-blinded, prospective split-mouth clinical trial. It is important to shed light on the effect of different apical preparation sizes regarding postoperative pain within the same patient with the same pulpal histological status. The aim is to compare and evaluate the severity of postoperative pain following apical enlargement with two different sizes after the IBF using the visual analogue scale.MethodsFifty “teeth” in 25 patients were assigned into two equal groups (25 per group) using E3 Azure rotary files; Group A was prepared two sizes greater than the Initial binding file (IBF) (the largest K file to bind at the actual working length) mesial canals, which were enlarged to 35#/0.04 and 40#/0.04 for the distal canals. Group B was prepared in three sizes larger than the IBF: 40#/0.04 for mesial canals and 45#/0.04 for the distal canals. On a modified VAS form, patients were questioned to indicate the degree of their pain and assisted in narrating their pain intensity during the following periods: 12, 24, and 72 h, and after a week. VAS data were non-parametric and analyzed using the signed-rank test for intergroup comparisons, Freidman’s test, and the Nemenyi post hoc test for intragroup comparisons. The significance level was set at p < 0.05.Resultsshowed that regardless of measurement time, enlargement of apical preparation was significantly associated with higher pain scores (p < 0.001). Within both groups, there was a significant reduction of measured pain score with time, with values measured after 12 and 24 h being significantly higher than values measured at other intervals (p < 0.001) and with values measured after three days being significantly higher than 1-week value (p < 0.001).ConclusionThe size of apical preparation had a significant effect on postoperative pain.Trial registration number & dateNCT05847738, 08/05/2023.

Exploring access cavity through non-carious cervical lesions: Impacts on instrumentation, obturation, and fracture resistance in mandibular incisors.

To compare the effects of Cervical Access Cavity (CervAC) and Traditional Access Cavity (TradAC) on root canal preparation, root canal filling, and fracture resistance in mandibular incisors. Twenty recently extracted and intact mandibular incisors were initially scanned using a micro-CT device. The specimens were anatomically matched to create two groups (n = 10). A wedge-shaped non-carious cervical lesion (NCCL) was created on the buccal aspect of all teeth using a double-faced segmented diamond disc. In group 1, a TradAC was prepared, whilst in group 2, access was created through the simulated NCCL using a round diamond bur 1012. After root canal preparation with Rotate instruments sizes 15/0.04, 20/0.04, and 25/0.04, the teeth were scanned again and evaluated for root canal volume and surface area, static voxels, volume of removed dental tissue, and dentine thickness. After that, the root canals were filled using the single-cone technique, teeth were restored with composite resin, and subsequently rescanned to calculate the volume of filling materials and voids. Then, the restored teeth were subjected to fracture resistance tests using a universal testing machine. Statistical comparisons between groups were performed with the Mann-Whitney test or the Student's t-test, with a statistical significance level of 5%. Statistical comparisons indicated no significant differences between groups in terms of root canal volume, surface area, static voxels, removed root dentine, root canal filling volume, percentage of voids, or fracture resistance load (p > .05). Conversely, teeth prepared with TradAC showed a significantly higher percentage of dental tissue removed from the crown (p = .001) and a greater volume of filling material remnants in the pulp chamber (p < .001) compared to the CervAC group. A significantly lower percentage reduction in dentine thickness was observed only on the mesial aspect of root at Level 1 in TradAC group (p = .008). CervAC can be considered a viable alternative approach in the presence of NCCL in mandibular incisors.

Evaluation of Volumetric Changes between Pediatric Rotary Files and Manual Files during Canal Preparation of Primary Mandibular Canine: A Nano-CT Analysis

Abstract Objective Pediatric endodontics has become popular due to advancements in cleaning, shaping, and irrigation systems, resulting in faster and effective removal of infected pulp, saving time, and creating a pathogen-free environment. The patented rotary file system, Kedo S, designed for primary teeth, introduced a single file generation for efficient pulp therapy. However, there are currently no studies assessing canal preparation in primary mandibular canine using nano-computed tomography (nano-CT). This study aims to evaluate the efficacy of rotary file systems (Kedo S plus, Kedo SG blue) against traditional hand files in root canal preparation of primary mandibular canines using nano-CT. Materials and Methods This in vitro study was performed in extracted primary mandibular canine based on certain inclusion and exclusion criteria. Samples were prepared and working length was determined before the preoperative scan using a high-resolution nano-CT device (SkyScan 2214, Bruker, Kontich, Belgium). A single well-experienced pediatric dentist prepared the canals using three file systems: Kedo S plus, Kedo SG blue, and hand K-files. A postoperative scan was performed similar to preoperative scan. Image reconstruction was performed with NRecon software for three-dimensional volumetric visualization and analysis of the root canals. Results Kedo S plus displayed significant alterations in volumetric, surface area, and instrumented areas postpreparation compared with Kedo SG blue and hand files. Statistical analysis revealed significant differences in volumetric and surface changes between Kedo S plus and hand files. Conclusion Rotary file systems, especially Kedo S plus, resulted in increased canal volume and surface area with minimal uninstrumented areas, showing promise for primary dentition root canal preparations. Further clinical assessments are warranted to validate these findings.

A quantitative comparison of apically extruded debris during root canal preparation using NiTi full-sequence rotary and single-file rotary systems: An in vitro study

ABSTRACT Aim: The purpose of this study was to compare quantitatively the apically extruded debris during root canal preparation using NiTi full-sequence rotary and single-file rotary systems. Methods: Sixty extracted mandibular premolar teeth with single canals and similar lengths were used and divided into four groups. Groups A and B are NiTi full-sequence rotary systems and Groups C and D are single-file rotary systems. In Group A, the canals were prepared using V-Taper 2H file (up to 25/0.06), Group B: TruNatomy file (up to 26/0.04), Group C: OneShape file (25/0.06), and Group D: OneCurve file (25/0.04). The initial weight of each Eppendorf tube to be used was calculated using an analytical balance before instrumentation. After instrumentation, the tubes were then stored for 15 days in an incubator at 37°C. The Eppendorf tubes were weighed again to determine their final weight which contained the extruded debris. The data were statistically analyzed using a one-way ANOVA test and Tukey’s post hoc test. Results: The OneCurve single-file system resulted in statistically significant less debris extrusion than the OneShape single-file system, TruNatomy multifile system, and V-Taper 2H multifile system (P &lt; 0.001). Among the full-sequence rotary file systems, the TruNatomy rotary file system produced statistically significantly less debris extrusion than the V-Taper 2H rotary file system (P = 0.01). Conclusion: All the instrumentation systems caused apical extrusion of debris. OneCurve system showed least debris extrusion, followed by OneShape, TruNatomy, and V-Taper 2H systems. Thus, single-file rotary instruments performed better with the less extrusion of debris when compared to multifile rotary systems.

Evaluation of Immediate and Delayed Placement of Post on Sealing ability of Periapical Area: An Original Research

ABSTRACT Objective: The purpose of this research is to compare the effects of post placement—immediate and delayed—on the periapical area’s capacity to close after root canal therapy (RCT). Techniques: A random split of sixty recently removed human single-rooted teeth was made into two groups: Group A received immediate post placement, whereas Group B received post placement after seven days. Standard guidelines were followed in the preparation of root canals and the placement of posts. A dye penetration method was used to measure microleakage, a measure of sealing capacity. Findings: Compared to the immediate post placement group (0.62 ± 0.08) (P &lt; 0.001), the delayed post placement group had a considerably lower mean microleakage score (0.35 ± 0.06), showing improved sealing capacity. In conclusion, post implantation done later enhances the periapical area’s sealing capacity after RCT. This research underscores the significance of scheduling endodontic treatments to maximize treatment success and improve tooth retention, as well as the possible therapeutic advantages of postponing post implantation.

Propagation of preexisting dentinal microcracks after root canal preparation using rotary, reciprocating, and adaptive instruments: An ex vivo microcomputed tomographic analysis

ABSTRACT Aim: To evaluate the propagation of preexisting dentinal microcracks after instrumentation with ProTaper Universal (PTU), ProTaper Gold (PTG), Twisted File Adaptive (TFA), and Reciproc Blue (RB) instruments by micro-CT imaging. Methods: The mesial roots of 60 mature human mandibular molars were cut out approximately 10 mm from the apex. The roots were then divided randomly into four groups (n = 15) based on file system used: PTU, PTG, TFA, and RB. Before and after canal shaping, roots were scanned with micro-CT. The preinstrumentation cross-sectional slices were examined to ascertain the presence of preexisting dentinal microcracks in apical 10 mm of radicular dentine. Postinstrumentation, the extension of preexisting dentinal microcracks was assessed and compared with corresponding preinstrumentation image to determine the grade of propagation. Postinstrumentation, the propagation of preexisting dentinal microcracks were categorized into three grades: nonpropagated dentinal microcracks (Grade 1), incomplete propagation (Grade 2), and complete propagation (Grade 3). Results: The PTU system showed a significantly higher propagation of preexisting dentinal microcracks compared to PTG, TFA, and RB system (P ≤ 0.05). All the instrumentation systems showed propagation of preexisting dentinal microcracks into Grade 2 cracks. Grade 3 propagation was seen in PTU, PTG, and RB systems. The number of Grade 2 dentinal microcracks were significantly higher than Grade 1 dentinal microcracks for the PTU system (P ≤ 0.05). Conclusion: Root canal instrumentation promotes propagation of preexisting dentinal microcracks. As compared to conventional nickel-titanium file systems, reciprocating and thermally treated file systems may less likely result in propagation of dentinal microcracks.
