Esthetic Anterior Resin Restorations: To Bevel Enamel or Not?

Restoration of fractured anterior teeth presents a challenge to dental clinicians owing to its esthetic requirements and, more importantly, because of the young age of the patients. Thus, it is important to follow the most conservative protocol. Many professionals face the dilemma of whether or not to perform enamel beveling. This article briefly discusses the clinical perspective and evidence regarding this type of operative restoration procedure.

Assessment of the preferred restorative material – composite resin or ceramic – for anterior teeth restoration

Introduction and objective: To evaluate the perception of clinicians and undergraduate students concerning their preferences and experience in using composite resin and ceramic veneers to restore anterior teeth restorations. Material and methods: 60 participants including clinicians (n=30) and undergraduate students (n=30) volunteered for this study. The study consisted of two parts. Firstly, the participant answered questions about their use and possible failure of ceramic or composite resin for anterior teeth. In thesecond part, participants analyzed 3 photographs of the anterior smile and reported on the presence of healthy teeth or restored teeth, with composite resin or ceramic veneers. Results: For both groups, composite resin was frequently used over ceramic and the most important reported factor in this decision choice was the cost factor. Both groups presented a failure percentage higher than 50% for composite resin, and this percentage was higher forundergraduate students. In the visual analysis of the photographs, none of the participants identified all the restorations correctly. Conclusion: Composite resin is the most common material of choice for clinicians and undergraduate students. Distinguishing natural teeth from restored teeth has become increasingly difficult, despite the restorative material (composite resin or ceramic) used.

Pink esthetic defects of restoration in anterior teeth and the treatment protocol

How to achieve the esthetic results with the restoration in anterior teeth is key aims which both of dentists and patients concern about now. Previously, the shape, color and texture of the teeth, i.e. "white esthetics" , were main objectives that prosthodontists want to achieve. The development of dental materials has played a key role in this field. More and more, the prosthodontist has realized that the healthy soft tissue is also very important for the final esthetic results. The healthy periodontal tissue is not only the basis for the longevity of restoration, but also the basis of esthetic results. The shape, color and texture of soft tissue, the mesial and distal gingival papilla, the margin and symmetry of the gingival, i.e. "pink esthetics" , have ultimate relationship with final esthetic results. The objective of the article is to introduce the common pink esthetic defects for the restorations in anterior teeth, the treatment protocol and methods to improve pink esthetic results.

Novel approaches towards composite restoration of anterior teeth: a series of three case reports

Anterior tooth fractures are the most prevalent dental injuries. Most commonly involved teeth are maxillary central incisors. These fractures not only result in impaired aesthetics but also result in loss of function and phonetics. Apart from this, another major problem affecting aesthetics is midline diastema. In order to restore them, newer techniques should be considered along with conventional technique. Two cases with uncomplicated maxillary anterior teeth fractures were selected and another case with midline diastema was selected. Indirect Restorations and Fixed Prosthodontics: Materials and Techniques Used by General Dentists of New Zealand.

Background To investigate the selection and use of materials and techniques for core buildup, indirect restorations, and fixed prosthodontics by general dentists in New Zealand. Methods A questionnaire comprising 19 sections and 125 questions was distributed via mail to 351 general dentists in New Zealand who were selected from the Dental Council of New Zealand's 2016 register. Results The majority of the respondents (68.8%) reported using resin composite light-cured materials for the core buildup of vital posterior teeth. A large number of respondents (52%) did not use dentine pins, with the majority of them (25%) being recent graduates (<10 years). Fibre posts were used by 61.6% of the dentists surveyed. The majority of dentists (54.6%) reported using addition-cured silicone impression material for crown and bridge impressions. Glass-ionomer cements (37.5% of participants) and resin-modified glass-ionomer cements (35.8%) were the most common luting cements used. Direct resin composite veneers were the preferred material of choice rather than indirect restoration of anterior teeth (40.4%). Conclusions The study showed that New Zealand dentists surveyed are using current state-of-the-art materials and techniques, with their choice of material being greatly influenced by clinical indications and patients aesthetic demands.

Comparative evaluation of the effects of different methods of post space preparation in primary anterior teeth on the fracture resistance of tooth restorations.

Background. Severely damaged teeth do not have adequate structure to support the composite crown; therefore, use of the canal space has been suggested to increase retention. Furthermore, the effect of post space irrigation protocols on the fracture resistance of the primary anterior teeth has not definitely been studied in postoperative modalities. This study compared the fracture resistance of restorations of primary anterior teeth following irrigation of the post space with sodium hypochlorite and chlorhexidine with and without application of burs. Methods. Ninety-four extracted primary anterior teeth were selected. Root canal treatments were carried out, 4 mm of the post space was left empty and 1 mm was regarded as a bed. The post space was prepared with and without a bur and the irrigation procedures were carried out with 0.2% chlorhexidine and 2.5% sodium hypochlorite solutions. Composite posts were inserted into the canals and the crowns were restored. Following composite etching and light-curing, the fracture resistance of the restored teeth was determined by a universal testing machine. Results. In the sodium hypochlorite group, fracture resistance of the teeth was 376.8±107.29 N and 475.5±186.89 N without and with bur preparation, respectively. For chlorhexidine protocol, the values were 370.88±175.46 N and 430.85±178.22 N without and with bur preparation, respectively. The effect of irrigating material was not significant; however, the bur and irrigating preparation significantly increased the fracture resistance of the restored teeth (P=0.02). Conclusion. Post space irrigation with 0.2% chlorhexidine or 2.5% sodium hypochlorite did not significantly affect the fracture resistance of primary anterior teeth in the post treatment modality.

Patient's Perception on Esthetic Restoration of Primary Anterior Teeth

Back ground : The present study aimed to asses the social and self - perceptions of children ( 4 to 8 years) regarding the esthetics of restored primary anterior teeth. Materials and methods: A total of 135 children seeking dental treatment were asked questions about the dental restorations of primary anterior teeth after showing them pictures of restored carious lesions. Results: They showed increased acceptability for Zirconia crowns as compared to other forms of restorations Conclusion: The children can perceive esthetics of their anterior restored teeth as early as 4 years of age which is important when planning esthetic restorations for them.

Выполнение реставрации передней группы зубов при скученности и наличии кариеса как этап комплексной реабилитации имеет свои особенности. Описан клинический случай выполнения реставрации с применением реставрационных материалов украинского производства «Jen-Radiance» и «Jen Favorite» (ООО «Джендентал-Украина»). Effectiveness of Dental Bleaching With 37.5% and 6% Hydrogen Peroxide and Its Effect on Quality of Life.

This study investigated whether it is possible to achieve equally satisfactory results between 37.5% hydrogen peroxide (HP) gel and 6% HP gel. We also assessed the psychosocial impact and self-perception of esthetics generated by extracoronal tooth whitening. A prospective, double-blind, randomized clinical trial was carried out. A total of 33 patients were selected from the clinic of the Faculty of Dentistry at the University of Chile. The patients included men and women over 18 years old without prior tooth whitening treatments, tooth decay, or restorations of the maxillary anterior teeth. The patients had tooth colors of A3 or less according to the Vita Classical scale, which was determined with a Vita Easy Shade spectrophotometer. The study was carried out with a "split-mouth" design. One side of each mouth was randomly treated with 37.5% HP, and the other side was bleached with 6% HP. Each group received 3 to 12 minutes of treatment with the respective gel applications. Two sessions of bleaching were carried out each week. A spectrophotometer was used to measure the total variation of color (ΔE), and a subjective evaluation was made with Vita Classical scale (ΔSGU) between the baseline (session 1) and different measurement times. We compared ΔE and ΔSGU for both agents using the Mann-Whitney test ( α=0.05). In both groups, there was variation among the initial color and the color in the different measurement times. In the month after the treatment was completed, ΔE was 9.06 in the 37.5% HP group and 5.69 in the 6% HP group. The difference between the two groups was statistically significant starting in the second session ( p=0.000). There was a significant difference between the effectiveness of the bleaching gel concentrations of 37.5% and 6% HP according to spectrophotometer measurements and subjective evaluations. There was also a positive effect on psychosocial impact and esthetic self-perception among patients.

Direct resin composite restoration of maxillary central incisors using a 3D-printed template: two clinical cases

BackgroundThree-dimensional (3D) printing technology is used widely in dentistry for applications including implant surgery, oral and maxillofacial surgery, orthognathic surgery, endodontics and prosthodontics. Using a 3D-printed template makes performing the repair procedure faster and more convenient. The aesthetic restoration of anterior teeth can recover facial beauty, enhance speaking and chewing functions and improve the quality of life of the patient.Case presentationThis article describes two kinds of clinical cases including fractured teeth and dental caries. In both, a 3D-printed template was used for direct resin composite restoration of maxillary central incisors. A 3D-printed template was built using the following 3-step process: data acquisition was conducted via intra-oral scanning, virtual modeling was performed using an imaging process, and manufacturing was performed using a 3D printer. Aesthetically restoring the maxillary incisors with the assistance of the 3D-printed template achieved the anticipated results, and the patients were very satisfied with the effect.ConclusionsThe direct resin composite restoration of maxillary central incisors using a 3D-printed template represents a rapid, convenient, aesthetic and functional option for treating maxillary central incisors. A 3D-printed template is therefore an acceptable and reliable alternative to traditional direct composite restoration of maxillary central incisors including fractured teeth and dental caries.

A Comparative Study of Relative Translucency of Six All-Ceramic Restorations

Purpose: Compared relative translucency of IPS e.max CAD,Vita Suprinity,Lava Zirconia,Cercon Zirconia,Zenostar and upcera Zirconia, to provide a reference for the clinical aesthetic choice. Method: Relative translucency were calculated from the luminous reflectance (Y) of the specimens with a black (Yb) and a white backing (Yw) to give Yb/Yw with Minolta CR-321.One-way analysis of variance and Tukey’s multiple comparison test were used to analyze the data (P&lt;0.05). Results: Relative translucency of control group was 0.577-0.843,while experimental group was from 0.798(IPS e.max CAD) to 0.947(upcera Zirconia),there were significantly different between the groups. The relative translucency of the six systems are as follows: IPS e.max CAD &gt;Vita Suprinity &gt;Lava Zirconia &gt;Cercon Zirconia &gt;Zenostar &gt;upcera Zirconia. Conclusion: There were significantly different between the six groups, for patients with anterior tooth areas restoration and high aesthetic requirements, IPS e.max CAD and Vita Suprinity are recommended; For patients with poor transmittance of adjacent teeth, and the abutments are dark which need to be covered can choose Lava Zirconia and Cercon Zirconia; For low transparency like posterior areas and severe tetracycline teeth, upcera Zirconia and Zenostar ceramic blocks may be considered.

Clinical effect of zirconia all-ceramic crown on repairing the anterior teeth and its influence on the quality of life of patients

Objective To investigate the clinical effect of zirconia all-ceramic crown for anterior teeth restoration, and its influence on the quality of life. Methods Eighty-four patients with anterior teeth repair[4 cases were lost during the follow-up, and there were 80 cases (106 teeth)] were selected.According to different surgical methods, they were divided into control group(56 teeth) and study group(50 teeth). The control group used Ni Cr alloy porcelain crown, the study group used two zirconia all-ceramic crown.The patients were followed up for 1 year, the repair effect, incidence rate of adverse reactions and quality of life score were compared between the two groups. Results After 1 year of crown restoration, the color, integrity, edge coloring in the study group were significantly better than those in the control group(all P 0.05). The quality of life scores in the study group after repair were significantly higher than those in the control group(t=18.623, 22.389, 20.724, 10.190, all P<0.05). Conclusion The clinical effect of zirconia all-ceramic crown for anterior teeth restoration is better.It can improve the aesthetic effect, safety and quality of life score of patients.It is worthy of popularizing and application in clinic. Understanding the interaction of resin composite with light for predictable aesthetic results in anterior teeth: A case report

This case report illustrates a conservative technique for reintervention on anterior composite restorations in a young patient with composite veneers. The treatment incorporated the use of photos and digital planning to establish a harmonious smile design. In order to conserve the dental tissues, existing resin composites restorations in maxillary anterior teeth were carefully removed to minimize enamel reduction, followed by a study of color and shade of teeth considering factors such as hue, chroma, luminosity, opacity, translucency and light effects. Eight direct veneers were built up with resin composite taking the optical properties of the material into account and their influence on the thickness of the layers that create specific light effects when superimposed on each other. To obtain excellent results with a predictable working protocol, nanohybrid composite systems facilitate good aesthetics, avoiding more invasive treatment in an economic manner.KeywordsDental aesthetic; Optical proprieties; Resin composite.

Clinical Evaluation of Maxillary Anterior Teeth in Relation to Golden Proportion, Red Proportion and Golden Percentage

Golden proportion, recurrent esthetic dental proportion and golden percentage are the widely accepted tools to measure perceived mesiodistal width of maxillary anterior teeth. Our study was aimed to identify appropriate tool among the above mentioned proportion to measure perceived mesiodistal width. A cross-sectional observational study was conducted in National Academy of Medical Sciences, Bir Hospital, Nepalese Army Institute of Medical Sciences and Patan Academy of Health Sciences between March 2014 to March 2015.Sixty-three respondents were recruited in the study by the convenient sampling technique. Photographs of the maxillary anterior teeth were taken using digital single reflex camera with macrolens. Data analysis was done using SPSS version 17.0 and paired sample t- test was performed. In our study the golden proportion was 14.28% between central and lateral incisor and 12.69% between canine and lateral incisor. Therefore, golden proportion in our scenario doesn't exist. Recurrent esthetic dental proportion in female Aryans was 71 and 71 on left side and 71and 75 on right side. Golden percentage of respondents was near to values 11, 15, 22, 22, 15, and 12 %. Golden percentage is an appropriate anterior tooth proportion which may serve as a guideline for the restoration of anterior tooth. RED proportion is applicable only in Mongoloid female population.

Research progress of gingival aesthetics evaluation in aesthetic restoration of anterior area

Gingival aesthetics is one of the factors that affect the clinical effects of prosthesis, and is particularly important in the aesthetic restoration of anterior teeth. However, gingival aesthetics involves a number of parameters for which there is no standardized evaluating criteria available so far. In addition to implant, other methods of restoration still lack recognized aesthetic evaluation systems for clinical use. At present, there are lots of solutions to improve anterior gingival esthetics, and most of the solutions can effectively improve the aesthetic effect in clinical treatment. This article reviews the aesthetics criteria of the anterior gingival area and the aesthetics restoration methods.

Factors Influencing Patients\' Satisfaction with Anterior Teeth Restorations in Ha\'il City, Kingdom of Saudi Arabia

Kidney disease, increased body mass indexes, and anterior teeth treatment needs

Objective: The purpose of this study was to test the hypothesis that individuals with kidney disease will have more dental issues affecting their anterior teeth. This effect on oral health would be independent from the effects of their higher frequency of obesity. Methods: Using the University of Pittsburgh School of DentalMedicine Dental Registry and DNA Repository project, anterior teeth treatment history was evaluated for patients with kidney disease. Four thousand nine hundred and eighty-three individuals were evaluated. Individuals with kidney disease were compared to individuals who did not present the condition. Results: One hundred and three individuals reported kidney disease and 1,424 had history of treatment of anterior teeth. Individuals with kidney disease had significant more anterior teeth treatments (p=0.001). Individuals who reported kidney disease also had a higher frequency of overweight individuals, but anterior teeth restoration needs were higher in all individuals with kidney disease, independent of their body mass index. Conclusion: Chronic kidney disease increases treatment needs of anterior teeth and potentially leads to worse oral health outcomes and warrants changes in protocols for treatment of individuals with chronic kidney disease.

Role of Reinforcement Methods in Retention of Composite Restorations of Primary Anterior Teeth

Objectives This study aimed to evaluate the retention of composite restorations of primary anterior teeth reinforced with composite posts, glass fiber posts, para-pulpal pins with composite posts, and para-pulpal pins alone.Methods Forty freshly extracted sound primary canine teeth with at least two-third of the root length remaining were selected. After disinfection, the tooth crown was cut perpendicular to the longitudinal axis. Root canals were prepared and filled using zinc oxide eugenol paste. The teeth were randomly allocated into four groups and restored using one of the following: Composite posts, glass fiber posts, para- pulpal pins with composite posts, and para-pulpal pins alone. Composite cores were constructed while a preformed U-shaped orthodontic wire was placed in the composite tip, samples were then placed in a universal testing machine for measurement of retention. The minimum force required to dislodge the restoration or cause fracture was considered as the retentive strength. The collected data were analyzed using one-way ANOVA and Tukey’s test (P 0.05).Conclusion para-Pulpal pin alone may not help in improving the retention of full crown restoration of primary anterior teeth. However, when used along with a composite post, it appears to improve the retention of restoration.

Clinical observation of the effect of immediate and delayed implant repair for the surrounding tissue

Objective To compare the effect of immediate implantation and delayed implantation on the surrounding tissue. Methods The clinical data of 98 cases of anterior teeth with single implant repair were analyzed retrospectively, and they were divided into study group(n=50) and control group(n=48) according to the method of implantation.The patients in the study group were implanted in the tooth socket immediately after the minimally invasive tooth extraction, and the patients in the control group were implanted after 12-16 weeks of minimally invasive extraction.All the two groups were repaired in coronal at 6 months after implantation.The success rate of implant, implant bone resorption, periodontal probing depth, gingival bleeding index, plaque index and red aesthetic score were compared between the two groups. Results The success rate was 100% in the two groups after 12 months, the difference was not statistically significant(P>0.05). The implant surrounding bone absorption in the study group was lower than that in the control group, the differences were statistically significant(t=2.132, 2.241, 2.421, all P 0.05). Conclusion The clinical effect of immediate implant restoration of anterior teeth is better than delayed implantation. Key words: Dental implantation; Periodontium; Incisor; Cuspid
