To review the outcome of intracytoplasmic sperm injection (ICSI) pregnancies complicated by spontaneous loss of a co-twin compared with singleton ICSI pregnancies managed at our hospital. Başkent University Ankara Hospital Delivery Records of years 2012 and 2014 were reviewed to find out the singleton ICSI deliveries. Patients' files were reviewed afterwards in order to investigate the ICSI pregnancies complicated with spontaneous loss of a co-twin and they were categorised in which trimester loss of the co-twin occurred. Mann Whitney U and Chi-square tests were used for the statistical analyses. Twenty dichorionic ICSI twin pregnancies complicated by spontaneous loss of a co-twin in the I, II or III trimester (study group) and 217 singleton ICSI pregnancies (control group) were seen during the study period. Spontaneous pregnancies were excluded. Maternal demographic features, prematurity, gestational age at delivery and intrauterine growth restriction (IUGR) were analysed. A total of 2,361 deliveries occurred between years 2012 and 2014. Among those, 366 (15.5%) cases were ICSI pregnancies. In the study group, 12 co-twin fetal deaths occured in the I trimester, 3 of them in the II and 3 of them in the III trimester. There were no significant differences between the study and control group according to parity and maternal age. Gestational age at delivery was significantly lower in the study group (p < 0.05), but when subgroups analysed, the difference was caused from II and III trimester co-twin losses. Mean birthweight did not differ significantly between the study and control group (p > 0.05). Mean birthweight differed significantly between II and III trimester co-twin loss cohort and control group. IUGR rate was similar between control and study group, 11.5% and 10 % respectively.
