Suspected Case of Histoplasmosis-Related Immune Reconstitution Inflammatory Syndrome Manifesting as Unilateral Pleural Effusion

Background: Histoplasmosis is a prevalent endemic mycosis that frequently causes opportunistic infections among individuals living with HIV (human immunodeficiency virus), and acquired immunodeficiency syndrome (AIDS). The initiation of antiretroviral therapy (ART) can result in a rare histoplasma-associated immune reconstitution inflammatory syndrome (IRIS) that may be difficult to distinguish between active progressive disease and inflammatory reaction to the medication. Case report: A 35-year-old male with newly diagnosed HIV and cervical biopsy-confirmed histoplasmosis was admitted from the emergency department with disseminated histoplasmosis for HIV antiretroviral treatment and antifungal treatment. Discharged after a 2-week inpatient treatment the patient returned 1 week later with a large left pleural effusion. Two liters of serosanguinous fluid were removed and all fluid cultures and cytology were negative. An extensive work-up remained negative, and given the timeline of presentation in relation to the initiation of ART, it was felt that this isolated left-sided pleural effusion was a manifestation of IRIS. Eventually, corticosteroids were given for IRIS and the patient improved and was discharged on long-term treatment. Conclusions: Immunocompromised subjects are at serious risk for a virulent, aggressive, rapidly advancing histoplasma infection and should be counselled accordingly. Any clinical signs or symptoms of organ system deterioration or compromise should be reported to the provider and a workup for histoplasmosis considered. In patients treated with antiretrovirals, IRIS is particularly confounding since it is difficult to differentiate from active disease. Using corticosteroids in already immunocompromised patients with an established fungal infection requires caution. Compliance with treatment is one of the most important components of care, especially considering the months of protocol.

Grave’s Disease as A Manifestation of Immune Reconstitution Inflammatory Syndrome (IRIS) in HIV Patients Who are Later Diagnosed with Cervical Cancer: A Case Report

Grave’s Disease (GD) after giving Active Antiretroviral Therapy (ART) to HIV patients is one of the manifestations of Immune Reconstruction Inflammatory Syndrome (IRIS). Complaints and clinical symptoms of Grave’s IRIS usually appear in 8-33 months since the use of ART. A 40-year-old woman was diagnosed with HIV with comorbid pulmonary tuberculosis (TB) infection. The patient regularly took ART and had finished taking TB drugs for 6 months, and the pulmonary TB was declared cured. However, the patient began to complain and showed clinical symptoms of hyperthyroidism. After receiving anti-thyroid medication, the patient has no complaints related to thyroid disease. Six months later, the patient experienced frequent vaginal bleeding. After being evaluated by the Obstetrics and Gynecology section, the patient was diagnosed with cervical cancer. Abnormality of thyroid function test in HIV patients was common, but only 1-2% of HIV patients showed signs of thyroid disorder. However, few cases still show GD as a manifestation of IRIS in HIV patients receiving ART. Dysregulation of the immune system in IRIS-HIV condition and long-standing inflammatory response of HIV (innate and adaptive immune cells involvement, MCH class I & MCH class II expression, TGFβ effect, immune-modulating drugs effect, and effect of reactive oxygen species) promote progression of cancer. IMMUNE RECONSTITUTION INFLAMMATORY SYNDROME IN HIV INFECTION

Antiretroviral therapy (ART) leads to suppression of HIV replication, contributes to increase in the number of CD4-lymphocytes count and partial restoration or activation of the immune system. The consequence is a reduction of incidence of opportunistic diseases, increase in the duration and quality of life of people living with HIV (PLHIV). However, in some patients with severe immunosuppression, this may be accompanied by a worsening of the condition and risks of formation of the immune reconstitution inflammatory syndrome (IRIS), which manifests itself in the development of new or previously treated opportunistic, secondary and exacerbating non- infectious diseases against a virologically effective ART. The frequency of the development of IRIS varies widely, in cases of tuberculosis-associated manifestation it can reach 50%. Risk factors for the development of IRIS are low initial CD4-lymphocyte count and a high load of HIV RNA in the blood, the presence of opportunistic infections during the initiation of ART. Discussed terminology issues, other possible risk factors for the development of the syndrome, regularities of the pathological process are considered. Epidemiological statistics of IRIS, pathogenetic bases, variants of clinical and laboratory manifestations of complications are given. The criteria for diagnosis of the syndrome, as well as the necessary conditions for its occurrence, are considered. Particular attention is paid to the most common opportunistic infections that cause the manifestation of IRIS, the peculiarities and polymorphism of clinical manifestations and the prevention of their occurrence. Currently, there is an increasing incidence of HIV infection in the late stages. Laboratory and clinical differences in the manifestations of acquired immunodeficiency syndrome (AIDS) and IRIS have been sanctified. In view of the blurring of the diagnostic criteria, in the Russian Federation specialists rarely expose IRIS to clinical or pathological diagnoses, therefore it is rather difficult to trace the frequency of occurrence of this condition. Clinical and laboratory manifestations are systematized, which allows to formulate this diagnosis on the basis of their totality. Prevention of IRIS is the prudent prescription of antiretroviral drugs. It is neces sary to conduct a qualitative and timely diagnosis of concomitant diseases of infectious and non-infectious nature before the initiation of ART and during treatment, the appointment of effective etiotropic therapy for opportunistic and secon dary infections. In order to improve the prognosis of HIV infection, preferably early onset of ART with stable CD4-lym phocyte counts and low HIV RNA levels in the blood.

Immune Reconstitution Inflammatory Syndrome in children.

Immune Reconstitution Inflammatory Syndrome (IRIS) refers to a collection of inflammatory disorders, predominantly related to infectious processes that manifest after the initiation of antiretroviral therapy (ART) and can be classified as unmasking or paradoxical. The prevalence of IRIS in children in sub-Saharan Africa is low. Approximately half of all cases are associated with Mycobacterium tuberculosis. It may be difficult to distinguish IRIS from tuberculosis and other opportunistic infections radiologically; therefore, radiological findings must be interpreted with clinical and laboratory findings. In this review article, we describe the clinical and radiological manifestations of IRIS in children and provide illustrative radiological examples.

Immune reconstitution inflammatory syndrome (IRIS),often occurring in human immunodeficiency virus (HIV)-infected patients after antiretroviral therapy,is a pathologic inflammatory response to preexisting opportunistic infections or other antigens.The pathogenesis of IRIS remains unclear,and may be linked to multiple factors including dysregulated immune responses.Various manifestations of IRIS have been described.This article mainly summarizes skin manifestations,diagnosis,treatment and prevention of IRIS.Although IRIS was originally defined in HIV-infected patients,there have been reports of similar paradoxical manifestations in patients with other autoimmune diseases.Further work is needed to advance knowledge on IRIS. Immune reconstitution inflammatory syndrome observed in the setting of drug‐induced hypersensitivity syndrome/drug reaction with eosinophilia and systemic symptoms (DIHS/DRESS)

Immune reconstitution inflammatory syndrome (IRIS) is originally described in association with antiretroviral therapy (ART) for HIV infected patients. IRIS consists of a broad spectrum of inflammatory diseases, such as infectious inflammatory, neoplastic, and autoimmune diseases, that present after starting an effective ART, leading to CD4+ cells increase and plasma HIV-RNA reduction. IRIS reflects either worsening of an already-diagnosed infection or presentation of previously subclinical infection. Opportunistic pathogens include cryptococcus, mycobacterium, herpesviruses, or also auto-antigens. The paradoxical worsening of clinical symptoms as observed in IRIS is also the phenomenon of drug-induced hypersensitivity syndrome/drug reaction with eosinophilia and systemic symptoms (DIHS/DRESS). In the course of DIHS/DRESS, cytomegalovirus diseases and herpes zoster are observed coincidently with the increase in lymphocytes or rapid reduction of systemic corticosteroids. Based on the similar manifestations between IRIS and DIHS/DRESS, DIHS/DRESS can be seen in a broad context as another manifestation of IRIS. Although the mechanisms of IRIS is complex and variable, depending on the latent pathogens and shift of immune status, use of the concept of IRIS can help our recognition of various manifestations that occur in the setting of DIHS/DRESS. The understanding of IRIS may improve the morbidity and mortality rates of DIHS/DRESS.

Bilateral Chylothorax. An Unusual Presentation in the Course of Immune Reconstitution Inflammatory Syndrome, HIV Infection and Kaposi’s Sarcoma: A Case Report

Immune reconstitution inflammatory syndrome (IRIS) is a term used to describe the paradoxical worsening of a pre-existing infection or the presentation of a previously undiagnosed condition in HIV-infected patients soon after commencement of antiretroviral therapy (ART). We report a case of a patient with HIV infection and Kaposi sarcoma, who developed bilateral chylothorax as a manifestation of IRIS, after the initiation of highly active antiretroviral therapy. Bilateral chylothorax is extremely rare and has been associated with significant morbidity and mortality. We present here the successful management with bilateral video-assisted thoracic surgery. Hemophagocytic Syndrome Associated with Immune Reconstitution Inflammatory Syndrome in a Patient with AIDS Related Burkitt's Leukemia/Lymphoma.

Highly active antiretroviral therapy (HAART) has markedly decreased human immunodeficiency virus- (HIV-) related mortality and the incidence of opportunistic infections. The dramatic reduction in HIV-1 RNA and increase in CD4 lymphocyte count mean a recovery in immune function. This restoration in immune function may be associated with paradoxical deterioration in subclinical opportunistic infections in some patients, a condition called immune reconstitution inflammatory syndrome (IRIS). IRIS, a “paradoxical” inflammatory response to either previously treated or subclinical infections or noninfectious diseases, can manifest during the restoration phase of immunity hemophagocytic syndrome (HS) which is a very rare complication in patients with acquired immune deficiency syndrome (AIDS). We describe a case of hemophagocytic syndrome associated with IRIS in a patient with AIDS related Burkitt's leukemia/lymphoma (BL). IRIS was probably the cause of hemophagocytosis for our patient. Zoster infection may facilitate to IRIS. With the increasing number of people with HIV infection and the accompanying use of HAART, much more clinical manifestations of IRIS will be experienced especially in patients given high dose chemotherapy, just like in our case.

The use of highly active antiretroviral therapy (HAART) has significantly reduced the incidence and progression of HIV-associated cryptococcosis. However, an early complication of HAART is the immune reconstitution inflammatory syndrome (IRIS), which may affect the CNS. The authors report a patient successfully treated for cryptococcosis and HIV who presented a late manifestation of IRIS. Neuroimaging aspects and management of CNS-IRIS in this patient are discussed in this paper.

Pathogenesis of the immune reconstitution inflammatory syndrome in HIV-infected patients

The immune reconstitution inflammatory syndrome (IRIS) is an important clinical complication in HIV-infected patients initiating antiretroviral therapy. This review focuses on the latest literature pertaining to the pathogenesis of IRIS. The clinical manifestations of IRIS are heterogeneous due to the variety of opportunistic infections that are associated with this inflammatory syndrome. However, the disproportionate inflammation is a defining hallmark for which common mechanisms are suspected. Lymphopenia-induced proliferation in the context of systemic immune activation, presence of high antigenic exposure and a wider availability of interleukin-7 contribute to the exacerbated immune response underlying IRIS. Defect in pathogen clearance by phagocytes might favor high pathogen burden, which in turn is thought to activate both innate immune cells and pathogen-specific T cells upon correction of the CD4 T-cell lymphopenia, predisposing to IRIS. This common scenario might be further invigorated by functional impairments among regulatory T cells. Further insight into the cellular mechanisms driving IRIS is urgently needed. Understanding the relative contribution of distinct effector and regulatory T-cell subsets, and innate immune components to IRIS is required to inspire future therapeutic approaches.

Head and Neck Manifestations of the Immune Reconstitution Syndrome in HIV‐Infected Patients

To describe head and neck manifestations of immune reconstitution inflammatory syndrome (IRIS) in a cohort of HIV-infected patients receiving combined antiretroviral therapy (cART). After initiation of cART, some HIV-infected patients present a paradoxical worsening and clinical deterioration due to pathological inflammatory reactions to infectious or noninfectious antigens, a condition known as IRIS. Prospective study with a follow-up period of 6 to 24 months. Tertiary referral center in Mexico City. Our cohort was integrated by 165 patients who had started cART within the past 2 months prior to study entry. Patients underwent a complete ear, nose, and throat examination (ENT). Laboratory tests (hematology and blood chemistry), cultures from body fluids, and biopsies were performed. Of the 165 patients studied, 21 (12.7%) presented IRIS in the head and neck region. Kaposi sarcoma was the most common presentation, observed in 7 patients. Tuberculosis-associated IRIS was observed in 6 patients with scrophulas, lymph node enlargement, or retropharyngeal abscess. Other manifestations included herpes simplex I infection and unilateral vocal fold palsy secondary to Mycobacterium avium intracelulare paratracheal abscess and scrophulas, as well as cervical lymph node histoplasmosis and facial palsy. To our knowledge, this is the first prospective study describing the different manifestations of IRIS in the head and neck region.

Immune reconstitution inflammatory syndrome: the trouble with immunity when you had none

Some individuals who are infected with HIV rapidly deteriorate shortly after starting antiretroviral therapy, despite effective viral suppression. This reaction, referred to as immune reconstitution inflammatory syndrome (IRIS), is characterized by tissue-destructive inflammation and arises as CD4(+) T cells re-emerge. It has been proposed that IRIS is caused by a dysregulation of the expanding population of CD4(+) T cells specific for a co-infecting opportunistic pathogen. Here, we argue that IRIS instead results from hyper-responsiveness of the innate immune system to T cell help, a mechanism that may be shared by the many manifestations of IRIS that occur following the reversal of other types of immunosuppression in pathogen-infected hosts.

A Patient with Multiple Immune Reconstitution Inflammatory Syndrome (IRIS) Following Initiation of Antiretroviral Therapy

The Immune Reconstitution Inflammatory Syndrome (IRIS) is an exaggerated pathological inflammatory reaction which occurs after the initiation of the antiretroviral therapy, due to the exuberant immune responses to the occult or the apparent opportunistic infections. The hallmark of the syndrome is the paradoxical worsening of an existing infection or a disease process or the appearance of a new infection or a disease process soon after the initiation of the antiretroviral therapy.The most common forms of IRIS occur in association with tuberculosis and chronic viral and invasive fungal infections. Multiple IRIS in a patient is extremely rare. Our patient had multiple manifestations of IRIS, in the form of cryptococcal meningitis, toxoplasmosis and Cytomegalovirus (CMV) retinitis after the initiation of highly active antiretroviral therapy.

Incidence and risk factors of immune reconstitution inflammatory syndrome in HIV-TB coinfected patients

Tuberculosis is one of the leading causes of development of Immune reconstitution inflammatory syndrome (IRIS) in HIV patients receiving antiretroviral therapy (ART). OBJECTIVE: To determine the incidence of IRIS in HIV-TB coinfected patients, and to find out the possible risk factors associated with IRIS. MATERIALS AND METHODS: Study commenced with 96 patients adhered to standard antitubercular therapy (ATT) and ART without defaultering, and followed up for six months. RESULT: The mean (± SD) CD4 count and CD4 percentage at baseline was 59.16 (± 24.63) per mm³ and 4.59% (± 1.73) respectively. Only 18.75% developed IRIS after 57.05 (± 14.12) days of initiation of ART. Extrapulmonary tuberculosis was the most significant factor associated with IRIS (83.33%) than those without IRIS (44.87%) (p = 0.0032). Specifically, tubercular lymphadenitis (38.88%, p = 0.0364) and disseminated tuberculosis (33.33%, p = 0.0217) were significantly associated with IRIS. The other risk factors associated with appearance of IRIS were higher CD4 count (p = 0.0212) at three months after initiation of ART and increment of CD4 count (p = 0.0063) and CD4 percentage (p = 0.0016) during this period. The major manifestations of IRIS were fever (40%), followed by lymphadenitis (38%). The mortality rate in IRIS was not higher than those without IRIS. CONCLUSION: Patients with extrapulmonary tuberculosis, especially tubercular lymphadenitis, were more likely to develop IRIS and fever was associated in most of them. Higher increment of CD4 count may indicate development of IRIS in presence of new or worsening tuberculosis lesion.

Cryptococcal lymphadenitis and immune reconstitution inflammatory syndrome: Current considerations

HIV-related immune reconstitution inflammatory syndrome (IRIS) is increasingly being recognized in regions with a high burden of opportunistic infections. We describe an unusual manifestation of IRIS- 'unmasking' cryptococcal lymphadenitis in a patient shortly after initiation of antiretroviral therapy. Epidemiology, clinical features and management of cryptococcal IRIS with reference to lymphadenitis presentation are briefly reviewed. Early and accurate diagnosis is essential for appropriate management. Standard antifungal therapy along with continuation of antiretroviral therapy is recommended. Cases with severe inflammation may benefit from anti-inflammatory drugs/corticosteroids. Unusual manifestations like lymphadenitis pose diagnostic difficulties in regions with a high prevalence of tubercular lymphadenitis.

Neurologic Consequences of the Immune Reconstitution Inflammatory Syndrome (IRIS)

Although major advancements have been made in the treatment of HIV infection, graft-versus-host reactions, and autoimmune diseases, an unexpected consequence of treatment has been the emergence of a devastating inflammatory syndrome, termed the immune reconstitution inflammatory syndrome (IRIS). The pathophysiology of the syndrome is poorly understood, and the syndrome poses unique challenges for diagnosis and treatment. We have reviewed the neurologic manifestations of IRIS in the context of HIV infection as well as in the setting of treatment of autoimmune diseases such as multiple sclerosis, in which compartmental immune suppression may occur without an obvious underlying immune suppression. The purpose of this review is to identify common themes that may assist in the diagnosis and management of these IRIS syndromes.

Highly active antiretroviral therapy (HAART) has dramatically reduced HIV-related morbidity and mortality. The complications of HAART include drug-related toxicities, drug interactions, and immune reconstitution inflammatory syndrome (IRIS). IRIS encompasses a diverse range of pathologic inflammatory manifestations that occur in the days to months after HAART is commenced. Many infectious and noninfectious dermatologic manifestations of IRIS have been described. We report the case of an HIV-infected patient who developed Karposi's sarcoma with atypical presentation as the first manifestation of IRIS.
