Rehabilitation after musculoskeletal injury: Israeli and South African perspectives.

Rehabilitation for patients sustaining isolated and multiple musculoskeletal injuries due to trauma remains a mainstay of recovery. There are a wide variety of systems in place to manage the rehabilitation process. This article describes the post-traumatic rehabilitation procedures from 2 member countries of the International Orthopaedic Trauma Association, Israel and South Africa. The systems are reflective of the clear differences between these 2 countries with vastly different economic strata and health care systems. In Israel, the rehabilitation programs and resources are most likely the result of the mature social support systems and the trauma experience. In South Africa, the programs are reflective of a two-tier health care system, with patients exposed to varying levels of rehabilitation resources.

Toward a Framework to Assess the Financial and Economic Burden of Cervical Cancer in Low- and Middle-Income Countries: A Systematic Review.

To review the economic burden assessment of cervical cancer in low- and middle-income countries (LMICs) and use the findings to develop a pragmatic, standardized framework for such assessment. We first systematically reviewed articles indexed in scientific databases reporting the methodology for collecting and calculating costs related to the cervical cancer burden in LMICs. Data on study design, costing approach, cost perspective, costing period, and cost type (direct medical costs [DMC], direct nonmedical costs [DNMC], and indirect costs [IC]) were extracted. Finally, we summarized the reported limitations in the methodology and used the solutions to inform our framework. Cervical cancer treatment costs across LMICs vary greatly and can be extremely expensive, up to 70,968 International US dollars. Economic and financial assessment methods also vary greatly across countries. Of the 28 reviewed articles, 25 studies reported DMC for cervical cancer treatment by extracting cost information from billing or insurance databases (eight studies), conducting surveys (five), and estimating the costs (12). Only 11 studies-mainly through surveys-reported DNMC and IC. The economic burden assessment framework includes health care/payer and societal perspectives (DMC, DNMC, IC, and human capital loss) across the cervical cancer screening and treatment continuum. To assess health care/payer costs, we recommend combining the predefined treatment standards with actual local treatment practices, multiplied by unit costs. To assess societal costs, we recommend conducting a cost survey in line with a standardized yet adaptable protocol. Our standardized, pragmatic framework allows assessment of economic and financial burden of cervical cancer in LMICs despite the different levels of available resources across countries. This framework will facilitate global comparisons and monitoring and may also be applied to other cancers.

Effect of pH on Development of the Zebrafish Inner Ear and Lateral Line: Comparisons between High School and University Settings.

Increasing carbon dioxide levels associated with climate change will likely have a devastating effect on aquatic ecosystems. Aquatic environments sequester carbon dioxide, resulting in acidic conditions that can negatively affect fish development. Increasing climate change impacts in the coming decades will have an outsized effect on younger generations. Therefore, our research had two interconnected goals: 1) understand how aquatic acidification affects the development of zebrafish, and 2) support a high school scientist's ability to address environmental questions of increasing importance to her generation. Working with teachers and other mentors, the first author designed and conducted the research, first in her high school, then in a university research laboratory. Zebrafish embryos were reared in varying pH conditions (6.7-8.2) for up to 7 days. We assessed fish length and development of the inner ear, including the otoliths; structures that depend on calcium carbonate for proper development. Although pH did not affect fish length, fish reared in pH 7.75 had smaller anterior otoliths, showing that pH can impact zebrafish ear development. Furthermore, we demonstrate how zebrafish may be used for high school students to pursue open-ended questions using different levels of available resources.

Biomass Resources and Emission Reduction Potential of Agricultural and Livestock Residues in Mainland China from 2013 to 2022

Controlling carbon emissions is a global goal, and China is actively implementing carbon reduction measures. As a major agricultural nation, China has considerable potential for developing agricultural residues as renewable and environmentally friendly biomass energy. In this study, we obtained data on crop yields, crop-to-grain ratios, and livestock excretion coefficients to calculate the biomass resources of agricultural and livestock residues in Chinese provinces from 2013 to 2022. Crop residue biomass resources showed a distribution pattern with higher levels in the north than in the south and the east than in the west. Henan and Heilongjiang provinces consistently had the highest resource levels, exceeding 35 million tons annually for 10 years. The biomass resources from livestock residues were relatively abundant in Sichuan, Henan, Yunnan, Shandong, Hunan, and Inner Mongolia. Inner Mongolia, Sichuan, Shandong, and Henan had the greatest potential for CO2 emission reductions, primarily located in regions abundant in biomass resources and with high traditional energy consumption levels. ArcGIS was used to apply natural break classification to categorize the potential for emission reductions from agricultural and livestock residues across China from 2013 to 2022 into five classes. Based on factors such as crop planting area and livestock numbers, the spatiotemporal distribution of factors influencing the quantity of biomass resources was examined using Geographically and Temporally Weighted Regression. A tailored and integrated approach should be used for biomass, and the development of biomass energy should be promoted through policy support and technological innovation.

The impact of technical change on income inequality in Vietnam

PurposeThe primary purpose of this study is to explore the effect of technical changes on provincial-level income inequality in Vietnam. The authors also investigate whether the quality of institutions and human capital level moderate this relationship.Design/methodology/approachThis research applies the fixed-effect and random-effect models on a balanced panel data set of 63 Vietnamese provinces/cities from 2010 to 2020.FindingsThe study’s empirical results show that technical improvement has a nonlinear influence on income disparity in Vietnamese localities. When the local level of technology is limited, technological change can mitigate income disparity. However, as local technological levels increase, inequality tends to rise. Moreover, the study also reveals that the quality of a province’s institutions and the level of human resources are factors that moderate the correlation between technological change and income inequality. For provinces with better institutional quality and/or better human resources, inequality tends to decline under the impact of technological change.Practical implicationsThe results of this study suggest that while encouraging technology advancement, localities should also ensure sustainable development, reduce income inequality and focus on improving institutional quality and human resources development.Originality/valueThere are increasing concerns about the impact of technical change on inequality in income distribution; however, empirical evidence on this relationship in developing countries remains scarce. This study is among the few attempts to examine this issue at the provincial level of a developing country considering the moderation effect of institutional quality and human capital level.

Recruitment-Potential-Oriented Mechanical Ventilation Protocol and Narrative Review for Patients with Acute Respiratory Distress Syndrome.

Even though much progress has been made to improve clinical outcomes, acute respiratory distress syndrome (ARDS) remains a significant cause of acute respiratory failure. Protective mechanical ventilation is the backbone of supportive care for these patients; however, there are still many unresolved issues in its setting. The primary goal of mechanical ventilation is to improve oxygenation and ventilation. The use of positive pressure, especially positive end-expiratory pressure (PEEP), is mandatory in this approach. However, PEEP is a double-edged sword. How to safely set positive end-inspiratory pressure has long been elusive to clinicians. We hereby propose a pressure-volume curve measurement-based method to assess whether injured lungs are recruitable in order to set an appropriate PEEP. For the most severe form of ARDS, extracorporeal membrane oxygenation (ECMO) is considered as the salvage therapy. However, the high level of medical resources required and associated complications make its use in patients with severe ARDS controversial. Our proposed protocol also attempts to propose how to improve patient outcomes by balancing the possible overuse of resources with minimizing patient harm due to dangerous ventilator settings. A recruitment-potential-oriented evaluation-based protocol can effectively stabilize hypoxemic conditions quickly and screen out truly serious patients.

Exclusion and discrimination of the lgbtiq+ group in the health system of ecuador. Bibliographic review

Analyze the exclusion and discrimination of the LGBTIQ+ group that exists within the Ecuadorian Health System. Determine the incidence of violence, discrimination and exclusion by health professionals in the country. Materials and methods: Study based on the systematic review of the literature, in which the following databases were taken into account (Medline, PubMed, Scielo, Embase, Google Scholar, Scopus), where different articles based on the issue. Results: The articles were filtered taking into account inclusion and exclusion criteria, and finally there were 14 articles that met the initially established criteria. Conclusions: There are obstacles at the institutional and human resource level, which lead to some LGBTIQ+ people not being able to access comprehensive health care and, on the contrary, experiencing acts of discrimination. It is recommended that the INEC develop a statistical analysis again to know exactly the changes since 2013 regarding discrimination, violence and exclusion of the LGBTIQ+ group within the National Health System.

Beyond biomass: Resource effects on primary production and consumer nutrient assimilation in streams

Abstract Primary production in aquatic ecosystems is strongly controlled by resource availability (bottom‐up). At the same time, grazers exert top‐down pressure on algae. The effects of both resources and grazers on algal communities have been investigated extensively for both benthic and planktonic systems. However, most studies focus exclusively on net effects on algal standing stock, while the underlying ecological functions and processes often remain unknown. Here, we tested the effects of the two resources, light (L) and the limiting nutrient phosphorus (P), on both primary production (net oxygen production) by benthic algae (periphyton) and matter assimilation by the next trophic guild (particularly nitrogen assimilation by grazers). For this, we grew natural periphyton communities in stream mesocosm flumes together with two native grazer taxa and 15N as a tracer. Both resources resulted in an initial increase in net primary production, but this effect equalised after 36 days in the more mature communities. While both resources still showed strong positive effects on algal biovolume, the biovolume‐specific net primary production decreased with resource level and was lowest in the double elevated (+L, +P) treatment. The response of nitrogen assimilation by grazers was more closely related to the response of periphyton quality (stoichiometric and taxonomic composition) than to its biomass. In addition, light affected the nitrogen assimilation of both grazers negatively. These findings demonstrate that the responses of ecological functions, here primary production and assimilation by grazers, are not necessarily proportional to algal biomass and are thus not easily predictable from the common use of biomass as a proxy for functions.

Looking at the problematic conditions of a school institution in this millennial era, it can still be found that several teaching staff or teachers have problems and shortcomings in teaching activities in school institutions. This is related to a teaching method or system in the classroom and there are still many school institutions that still have low levels of human resources in teaching at these institutions. Of course, from this incident, a school institution requires supervisors to supervise teachers or develop guides, and motivate them so that the quality of teachers improves much better and makes them professional teaching staff. Thus, this research aims to answer questions regarding the role of supervisors in the formation of teachers' professional competence. This research uses qualitative research methods with a descriptive nature. The data collection used is documentation sourced from literature, articles, books, and other scientific papers that are relevant and related to the theme in the title. The data analysis technique is obtained through data reduction, data presentation, and conclusion. Meanwhile, the research results explain as follows: 1) The role of supervisor is needed by teachers to improve the quality of human resources in teaching in school institutions. 2) A teacher's professional skills as a teacher influence students' teaching and learning activities so that they can master learning in depth. 3) supervisors have a significant role in forming teachers' professional competence in teaching in a school institution.

Unbiased analysis of spatial learning strategies in a modified Barnes maze using convolutional neural networks

Assessment of spatial learning abilities is central to behavioral neuroscience and a useful tool for animal model validation and drug development. However, biases introduced by the apparatus, environment, or experimentalist represent a critical challenge to the test validity. We have recently developed the Modified Barnes Maze (MBM) task, a spatial learning paradigm that overcomes inherent behavioral biases of animals in the classical Barnes maze. The specific combination of spatial strategies employed by mice is often considered representative of the level of cognitive resources used. Herein, we have developed a convolutional neural network-based classifier of exploration strategies in the MBM that can effectively provide researchers with enhanced insights into cognitive traits in mice. Following validation, we compared the learning performance of female and male C57BL/6J mice, as well as that of Ts65Dn mice, a model of Down syndrome, and 5xFAD mice, a model of Alzheimer’s disease. Male mice exhibited more effective navigation abilities than female mice, reflected in higher utilization of effective spatial search strategies. Compared to wildtype controls, Ts65Dn mice exhibited delayed usage of spatial strategies despite similar success rates in completing this spatial task. 5xFAD mice showed increased usage of non-spatial strategies such as Circling that corresponded to higher latency to reach the target and lower success rate. These data exemplify the need for deeper strategy classification tools in dissecting complex cognitive traits. In sum, we provide a machine-learning-based strategy classifier that extends our understanding of mice’s spatial learning capabilities while enabling a more accurate cognitive assessment.

Improving education for incarcerated children and young people: policy recommendations from three transnational projects in Europe

Purpose The purpose of this policy piece is to present headline evidence from the equivalent of a decade of research across three projects focused on improving the educational experiences of incarcerated or justice-involved children and young people across 14 countries. The projects demonstrated a positive impact when using the specially designed innovative methods even with disengaged justice-involved children and young people. Each project had a different focus based on the needs of the children and young people, including (re)engaging them with education and learning opportunities re-engaging young offenders with education and learning (RENYO), developing identified social and emotional competencies through sport active games 4 change (AG4C) and developing skills for when transitioning into communities (S4L). Design/methodology/approach Each of the three projects designed bespoke interventions and materials based on the focus of the project. Combined, these were piloted across a total of 14 countries with 548 children and young people, 225 educators and 21 youth justice settings (custodial and non-custodial). Qualitative and quantitative data were gathered from educators, children and young people to better understand the effectiveness and limitations of the pedagogical approaches, the materials and experiences of the users. Findings The findings from across the three projects demonstrated that when custodial settings or youth justice settings have greater access to specifically designed tools and resources, with appropriate training to use them, it is possible to re-engage even the most disengaged children and young people with education and learning (RENYO). It was also possible to show a statistically significant increase in social and emotional competencies (social awareness, responsible decision-making, self-awareness, self-management, relationship skills) in participating country settings (AG4C) and to engage children and young people with a Skills4Life Curriculum in preparation for moving back into the community. Research limitations/implications Custodial and other youth justice settings vary widely in different jurisdictions with varying levels of resource. Whilst all programmes had success in implementation, this was to different degrees and extents in each of the country contexts. Much of this success will depend on the skills of educators and the willingness of the leadership. Practical implications The programmes and interventions in this policy piece can be used in a range of contexts and settings as shown by the wide range of sites they were piloted in. From a practical perspective, these programmes and interventions have been designed to fit into existing structures and timetables with custodial and youth justice settings, making them easily integrated into business as usual. Originality/value Planned and strategically delivered interventions by expert educators in a holistic way means there is real opportunity to better support justice involved children and young people to successful transitioning back into communities, thereby potentially reducing reoffending. This is done by (re)engaging them with education and learning on entry, using the authentic inquiry methodology, developing social and emotional competencies through the active games and sport programme, followed by a tailored Skils4Life Curriculum preparing them for transition back into communities.

Planning natural gas supply chain pathways for energy system decarbonisation: An Australian case study

The future role of natural gas systems in a deeply decarbonised world is very uncertain. This work seeks to characterise the changes that the clean energy transition is likely to induce in the natural gas chain, across extraction and transmission, distribution networks and gas consumption/use trends, for various scenarios with different demand and supply drivers. First, the results of a least-cost macro-scale energy system planning optimisation are explored to identify future natural gas demand and supply requirements. Second, those findings are used as inputs to a detailed analysis of the full gas supply chain, from extraction to final use, to examine the evolution of i) stored resource levels; ii) transmission and distribution networks operation and financing; and iii) required capacity and siting of gas generation; all, with a realistic account of the related infrastructure. Australia is investigated as a case study that relies on natural gas for both domestic and export purposes. Results project a shift towards a more localised gas consumption to be largely supplied by fewer basins, with associated potential transmission and extraction bottlenecks. They anticipate an overall less utilised, ageing distribution network with potential challenges to asset reliability and financing. Finally, they demonstrate both the scheduled and early retirement of fossil generation assets can allow the siting of considerable amounts of new gas power firming capacity on brownfield sites, thus offering an opportunity to reduce total system costs.

Families and inequalities

Abstract This article examines inequalities between families with children in the UK. Families are central to the nurturing and raising of children, yet not every family is able to provide the same level of resources, support and opportunities. Moreover, in recent decades, changing partnership and fertility behaviours has led to greater diversity, fragility and complexity between families. Here we examine whether there are discernible socio-economic gradients and geographical variation in the recent changes in partnership and parenthood behaviour. We also assess the extent to which these family developments and the attributes of the families in which children are born and reared contribute to disparities in their lives and their future life chances, with a particular foci on education, income, mental well-being, parenting and parental relationships. To this end, we provide new analyses and draw on an extensive literature from a range of disciplines.

Biodiversity Offset Mechanisms and Compensation for Loss from Exceptional to Popular: Rediscovering Environmental Law

The use of compensatory mechanisms for biodiversity conservation, also known as biodiversity offsets, has increased significantly in recent decades. The Kunming Montreal Global Biodiversity Framework mentions them as an innovative scheme in support of substantially and progressively increasing the level of financial resources for biodiversity conservation. This article traces the origin of compensatory mechanisms in international environmental law and their development in transnational biodiversity governance. The article points to the shifts in the application of the biodiversity offsets: from the context of wetlands to other habitats and ecosystems; from its use in intergovernmental conventions to an increasing number of transnational (business) networks; and from an instrument of last resort to a source of additional funding for biodiversity conservation. In the evolution, compensatory mechanisms have been decoupled from their original purpose as an exceptional mitigation measure and a strong focus of environmental law on the preventive function. The increased rhetoric of commitment to no net loss, net gain, restoration, and the mitigation hierarchy has not been matched by an improved status of wetlands and other ecosystems. The processes within the biodiversity conventions (Ramsar and CBD) have accepted an ongoing destruction of nature and limited the role of environmental law to minimizing harmful impacts on nature and consolidating the decline, rather than shaping socio-ecological outcomes. An ambiguous position about the spread of compensatory mechanisms has been part and parcel of this; biodiversity conventions have neither endorsed nor distanced themselves from the application, promotion, and justification of compensatory mechanisms. To maintain the integrity of environmental law, the rules that prevent biodiversity loss need to be emphasised and enforced.

Fiscal resilience over time and its management in the context of multi-risks: an application to the Danube Region

Multi-hazards as well as multi-risk management are increasingly gaining importance in research, policy, and practice, but present a challenging task. Focusing on governments as key risk bearers, we assume a multi-hazard and multi-risk perspective and address the question of how different natural hazards can influence fiscal risk and how fiscal risk can change over time due to other risk realizations (e.g., pandemics). We employ a risk-layer approach to analyze the changes in fiscal risk, comparing the fiscal stress associated with different hazards during distinct time periods and scenarios. In doing so, we address the question under which circumstances risk reduction or risk financing may be needed as well as how an iterative approach can account for changing financing resource levels for different hazards under different scenarios. We apply this methodology to the Danube Region which is exposed to different natural hazards and encompasses countries with different levels of fiscal resilience. Furthermore, the countries in the Danube Region were affected by Covid-19, which acted as an additional stressor and caused large economic costs. The analysis should demonstrate the flexibility as well as the relevance of the presented methodology to address multi-risks within a coherent framework. One of the main outcomes of the study is the appreciation of different fiscal resilience levels for different countries and the different types of disasters they are exposed to, which can inform the diverse strategies needed on a case-by-case basis but within a common framework to tackle current and future risks. The analysis should be therefore not only informative for the Danube Region and respective countries but also regarding the more general question under which circumstances risk reduction or risk financing may be needed as well as how an iterative approach can account for changing financing resource levels against multiple risks.

An open-source parallel EMT simulation framework

As the integration level of inverter-based resources (IBRs) increases, ensuring the reliable operation of the bulk power systems requires the use of electromagnetic transient (EMT) simulation tools to identify and mitigate system-wide stability risks. Conducting EMT studies for large-scale, IBR-rich grids, however, is challenging due to the inherent computational bottleneck caused by the underlying high-fidelity models and required small time steps. This paper introduces ParaEMT: an open-source, generic EMT simulation framework designed to accelerate simulations by leveraging advanced parallel computational technologies, such as high-performance computers. This paper presents a comprehensive exposition of ParaEMT, covering its modeling library, simulation strategy, framework structure, operational procedures, and auxiliary features, alongside its extensible parallel computational architecture. Notably, ParaEMT is a publicly accessible and modularized framework written in Python, thereby facilitating future development and the integration of new models and algorithms. The accuracy and efficiency of ParaEMT are demonstrated by rigorous validations via multiple case studies.

Biodiversity and Vulnerability of Fisheries Resources in Berau Bay, West Papua Province

Berau Bay, located in West Papua Province, is one of Indonesia's marine areas rich in biodiversity. This aquatic ecosystem is a crucial habitat for various fish species, which hold ecological and economic value for the local communities reliant on fisheries. However, increased human activities, including intensive fishing, pollution, and climate change, pose severe threats to the sustainability of fish resources in this region. This research aims to assess the biodiversity and vulnerability levels of fish resources in the waters of Berau Bay. The methods used include field surveys, fish sampling, species identification analysis, and calculating the diversity index. The vulnerability levels of fish resources are determined by categorizing their conservation status according to the IUCN Red List. The study results indicate that Berau Bay has a relatively high level of biodiversity with low dominance. The highest biodiversity index value in Berau Bay waters is 3.35, dominated by the families Carangidae and Scombridae. The vulnerability analysis results show that 85.71% have a low vulnerability level. However, several species exhibit high vulnerability due to excessive fishing activities, pollution, and climate change. These findings emphasize the importance of implementing effective conservation measures to ensure the sustainability of fish resources in this area. Efforts to protect habitats, manage sustainable fishing practices, and educate the local community are expected to reduce vulnerability and enhance the sustainability of the Berau Bay aquatic ecosystem.

SOCIO-ECONOMIC FACTORS AND STUDENTS LEARNING OUTCOMES IN TWELVE YEARS BASIC EDUCATION IN RWANDA

This studys objective was to examine the socio-economic factors affecting student learning outcomes in Twelve Years Basic education in Rwanda. The research question was: What are the socio-economic factors that affect students learning outcomes in Twelve Years Basic Education, in Rwanda? The researcher used descriptive survey research design. 182 respondents constituted the studys population. The researcher used census and purposive sampling techniques to find the sample of respondents. Questionnaires and documentation research techniques were used as tools for data gathering. Based on the responses from the respondents, the analysis of the findings using SPSS Version 21 showed a significant correlation with a p-value of 0.000, which is less than the usual significance level of 0.05 and indicates a very strong positive association between the students learning outcomes and the socio-economic factors of family income, parental level of education, school resources, and food security. The Karl Pearson correlation coefficient (r), of 0.870, provided additional evidence of this strong positive relationship.

Job insecurity and (un)sustainable well-being: unravelling the dynamics of work, career, and life outcomes from a within-person perspective

ABSTRACT The present study investigates longitudinal relationships between job insecurity and a set of work, career, and life outcomes. Using an integrative theoretical framework that combines the principles of conservation of resources and sustainable career development, we aim to unravel the scope and dynamics of stressor-strain pathways that undermine employee well-being in these domains. To that end, latent curve modelling with structured residuals was performed on a large heterogeneous sample (N = 1134) of individuals living and working in Switzerland. Based on seven annual measurement points, the results revealed yearly within-person dynamics through which job insecurity may hinder sustainable well-being in different areas of life. Furthermore, our findings allow for contrasting short-term (yearly) versus long-term (7-year) effects. They uncover potential protective mechanisms through which between-person differences in the initial levels of sustainable well-being resources might mitigate the risks of being increasingly exposed to job insecurity over time, as reflected in within-person growth patterns. Research, policy, and practical implications are discussed based on these findings.
