Grayish discoloration of the face – argyria

The 54 year-old vegetarian patient has taken oral colloidal silver for two years to stimulate his immune system. The silver intake resulted in diffuse grayish discoloration on the face. His laboratory values were in the normal range, and no internal organ disease was detected. The histology of the forehead skin confirmed the diagnosis of argyria. Argyria is a condition associated with chronic local or systemic exposure to silver-containing products. The silver is typically deposited in skin, fingernails, oral mucosa and conjunctival membranes. There is no effective treatment for this condition. With the availability of pharmacologic alternatives, physician-directed use of silver-containing products had significantly declined. We review the literature and call attention to the adverse effects and dangers of the widely used paramedicinal colloidal silver products.

Necrotizing vasculitis with a polyarteritis nodosa‐like pattern and selective immunoglobulin A deficiency: case report and review of the literature

Selective immunoglobulin A deficiency (IgAD) is a primary immunodeficiency disease characterized by low levels (< 7 mg/dl) of serum immunoglobulin (Ig) A and normal serum levels of IgG and IgM. Patients with IgAD have increased risk for recurrent respiratory and gastrointestinal infections, autoimmune disease, asthma and allergy. A 26-year-old woman was admitted with sudden onset of painful cutaneous lesions on her lower extremities, pyrexia and arthromyalgia. Her medical history was remarkable for recurrent respiratory tract infections, self-limited episodes of acute diarrhea, atopy, splenomegaly and a 4-year history of a lung granulomatous lesion. Laboratory and imaging tests ruled out severe life-threatening infection, connective tissue disease and neoplasm. Serum protein electrophoresis showed a low IgA serum level (6.67 mg/dl), with normal serum levels of IgG and IgM, conducting to a diagnosis of selective IgAD. A skin biopsy showed necrotizing vasculitis without any sign of internal organ disease. We report a patient with IgAD and granulomatous involvement of lungs, spleen and medium-sized arteries of the skin. Although IgAD results from a failure of B-cell differentiation, we propose that deregulated immune response with production of cross-reactive antibodies and hyperstimulation of T cells and macrophages could contribute to this widespread granulomatous reaction.

To administer the embedding method, one needs to utilize a embedding thread besides acupuncture apparatus. Sheep gut is commonly used in China and the surgical thread is the choice in Korea. 3. Embedding method may vary from the patient to patient, depending on the nature and location of the illness. Piercing, embedding, and tying are some of the possibilities. 4. Embedding method may have different arrangement of threads based on the choice of usage. 5. Embedding method is effective for various chronic illnesses such as aches, functional diseases, and the diseases of internal organs. 6. When using the embedding methods, cautions against infection and side effects due to strong stimulation are mandatory.

Early rheumatoid arthritis therapy: comparative characteristic of delagil, sulphasalazine and methotrexate

Objective. To assess early administration and efficacy of 3 disease modifying antirheumatic drugs. Material and methods. 92 pts with rheumatoid arthritis (RA) aged 17 to 45 years (mean age 34,9±8,5 years) were included. Disease duration did not exceed 6 months (mean 3,5±1,9 months. All had 2-3 activity degree and did not received glucocorticoid therapy (systemic or local) and did not have severe concomitant diseases of internal organs. Results. Diagnosis of RA was confirmed during follow up in 90 pts. In 2 pts after 6 months diagnosis was changed to systemic lupus erythematosus. 30 pts received delagil for 3 months without improvement. Treatment with sulphasalazine for 3 months was not effective in 23 from 30 pts. 7 pts had subjective improvement during first 3 months but at 6 month effect was lost. Methotrexate administration provided improvement (DAS change 1,6). Clinical and laboratory remission was achieved in 6 pts. Number of bone erosions in pts treated with methotrexate was significantly less and depended on time of therapy beginning and features of disease onset. Conclusion. Methotrexate was most effective from the 3 drugs in early RA particularly when administered during first 3 months of the disease.

Granuloma Mimics a Clinical Manifestation of Lumbar Spinal Stenosis after Acupuncture: A Case Report

Recently, the use of acupuncture has been increasing for the management of low back pain and herniated nucleus pulposus. There are many debates on its usefulness and associated complications. Complications such as hemorrhage, infection, direct internal organ or nerve injury and disease transmission have been reported. We report a case of dural sac and nerve root compression caused by chronic inflammatory granuloma. This chronic inflammatory granuloma, which might have been formed by local hemorrhage or lowgrade infection after acupuncture is highly suspected to produce the symptoms of spinal stenosis; moreover, after surgical excision of the granuloma the patient's symptoms were eliminated. Hence, we report this case and discuss the possibility of the acupuncture as a cause.

The paper describes the possibility of using acupressure on established points of stepped-up skin sensibility (puncta maxima - PM) for the purpose of diagnosing certain diseases of internal organs or the state of the bone-joint system in carnivora. We reviewed 33 cases of primary Sjögren's syndrome and peripheral neuropathy evaluated by neurologic examinations and EMG at the Mayo Clinic from 1976 to 1988, and studied sural nerve biopsy specimens in 11 of them. Symmetric sensorimotor polyneuropathy occurred most frequently, followed by symmetric sensory neuropathy. Autonomic neuropathy, mononeuropathy, or cranial neuropathy (especially trigeminal neuropathy) was superimposed on generalized neuropathy in approximately one-fourth of patients. The course generally was slowly progressive, except for a few patients who may have improved with prednisone therapy. Although spinal ganglion involvement might have accounted for some of the clinical and neurophysiologic findings, we found evidence that necrotizing vasculitis was involved in fiber degeneration. All nerve biopsies revealed perivascular inflammatory infiltrates and other vessel abnormalities, which were diagnostic in two cases and strongly suggestive of necrotizing vasculitis in six cases. Axonal degeneration predominated over demyelination and sometimes was focal or multifocal. There were 29 patients under observation: 21 patients were diagnosed with chronic bronchitis and 8 with bronchial asthma. Glucose tolerance was determined in all patients (50 g of glucose orally). Blood glucose content was determined by orthotoluidine method on an empty stomach, as well as 1 and 2 hours after glucose intake. At the same time, basal immunoreactive insulin secretion was studied.
