Child Oral Health Management: Prevention, Early Diagnosis, and Intervention Strategies

This paper explores various aspects of childhood oral health management, including prevention, early diagnosis, and intervention strategies. This paper reviews the epidemiological data of pediatric oral diseases, highlighting the high incidence of dental caries and periodontal disease in the pediatric population. This paper details multiple preventive measures, such as the use of fluoride and the importance of oral health education, which can help to reduce the incidence of oral diseases. This paper analyzes early diagnostic techniques and therapeutic approaches, including digital imaging and laser diagnostic techniques, highlighting the key role of these techniques in early detection and intervention in oral diseases. Outcomes of very young women and adolescent (VYWAA) with breast cancer (BC) in rural and urban Saskatchewan: A population-based cohort study.

1571 Background: BC is uncommon in VYWAA (≤ 35 years) and accounts for about 1.8% of all BC. As BC screening is typically recommended for women aged 50-74 years there is a risk of delayed diagnosis of BC in VYWAA especially those with rural residence. The current study aims to assess outcomes of a cohort of VYWAA with BC in relation to their place of residence in the province of Saskatchewan over a period of 20 years. Methods: In this population-based retrospective cohort study all women ≤ 35 years with histologically documented epithelial BC during 2000-2019 were evaluated. Survival of the entire cohort and its subgroups was estimated using the Kaplan−Meier method, and the survival distribution of different groups was compared by the log rank test. A multivariate Cox proportional hazard model was performed. Various clinical and demographic variables were examined for their prognostic importance of overall survival (OS) for all stages and disease-free survival (DFS) for early-stage BC. Results: 248 eligible women with a median age of 32 years were identified, 24% were <30 years of age. Of all patients, 51% had node positive disease, 7% had DCIS, 15% stage I, 42% stage II, 22% stage III and 11% had stage IV disease at initial presentation. Among them 42% had hormone receptor +, 28% had HER2 + and 23% had triple negative BC (TNBC).16% developed BC within 1-year of pregnancy. 53% were rural resident and 47% were urban resident. Significant differences were noted between the two groups regarding smoking history (42 vs. 25%), HER2+BC (35% vs. 20%) and mean platelet count (285 vs. 258). Median follow up for all women was 91 months with a total follow up period of 294 months. Median DFS of VYWAA with stage I to III disease has not reached. 10-year estimated DFS of all women was 66%. 10-year DFS for stage I and II was 73% and 75%, respectively, vs. 43% for stage III (p<0.001). Median OS of entire cohort with stage 0-IV BC has not reached. 10-year OS of all VYWAA was 69%. 10 years OS of urban women was 79% compared to 65% for rural resident (p=0.041). 10-year OS of women age >30 years was 74% compared to 62% for age 26-30 years and 55% for age <26 (p=0.35).10 years OS of VYWAA with non-TNBC was 73% compared to 63% for with TNBC (p=0.028). On multivariate analysis for women with early-stage disease, stage III BC, HR, 3.0 (1.80-4.92) and grade III BC were significantly correlated with inferior DFS. On a multivariate analysis for OS of all VYWAA with stage 0 to IV disease, rural residence, HR, 1.75 (1.08-2.81); TNBC, HR, 2.25 (1.34-3.80), and stage IV disease, HR, 8.1 (4.72-13.94) were correlated with inferior OS. Conclusions: Very young women and adolescent with BC have a high incidence of node-positive disease and HER2+ and TNBC subtypes. TNBC and grade III disease were associated with inferior DFS in stage I-III disease whereas stage IV BC, TNBC and rural residence were correlated with inferior survival in women with stage 0-IV disease.

Aedes aegypti Knockdown Resistance Mutations and Dengue Virus Infection in Haiti.

Haiti is home to approximately 11 million people and has a high incidence of vector-borne disease, including more than 70,000 cases of dengue per year. Vector control is difficult in Haiti and adulticide spray of malathion is the main method of control employed during the outbreak of disease although pyrethroids are used in both bed net campaigns and in widely available aerosol cans for personal use. However, limited pathogen or insecticide resistance surveillance data are available for making operational decisions. In this study, we assessed Aedes aegypti from serial surveillance collections from 3 locations for the presence of dengue virus serotypes 1-3 (DENV1-3) by polymerase chain reaction and assessed, by melt curve analysis, samples from 10 locations in 2 departments for the presence of two mutations (V1016I and F1534C), that in combination, are linked to strong pyrethroid insecticide resistance. Only one of the 32 tested pools was positive for the presence of dengue virus. The two knockdown resistance (kdr) mutations were present in all locations. The 1016I mutation frequency varied from 0.29 to 0.91 and was in all sites lower than the 0.58-1.00 frequency of the 1534C mutation. We also observed that the genotype homozygous for both mutations (IICC), which has been linked to strong pyrethroid resistance, varied from 13 to 86% in each population. Notably, 3 locations - Ti Cousin and Christianville in Ouest department and Camp Coq in Nord department had more than 30% of the tested population without the presence of kdr mutations. These results indicate that the kdr markers of pyrethroid resistance are present in Haiti, at high frequency in several locations and, based on previous studies linking kdr genotypes and phenotypic resistance, that operational interventions with pyrethroids are not likely to be as effective as expected.

Analysis of Underwater Melting Process and Leakage Plugging Performance of Phase-Change Materials.

Leakage is a high-incidence disease of embankment dams, and efficiently addressing this disease guarantees the safe operation of dams. Underwater leakage self-priming plugging technology is a new technology that utilizes the melting and solidifying characteristics of phase-change materials and the negative pressure in the leakage entry area to accurately plug the leakage. However, little is yet known about the underwater melting process of phase-change materials and how their characteristics influence the plugging effect. In this study, three kinds of phase-change materials, namely, paraffin, rosin, and stearic acid, were used to conduct underwater leakage self-priming plugging tests, observe and analyze the underwater melting process, and compare the plugging effects. The results showed that the underwater melting process of phase-change materials exhibited different plugging window periods depending on their melting points, specific heat capacities, and mobilities, which were the main factors affecting their plugging effects. In the final plugging stage, paraffin had the best plugging effect, but the material strength was low; rosin had good plugging compactness, but the fluidity performance was poor, and the material effective utilization was low; stearic acid had a low melting point but dispersed easily. Therefore, a blocking material with a suitable blocking window period can be produced by adjusting the material properties accordingly for an improved blocking effect.

#1395 Management of CKD patients and HK, as its main complication: degree of adoption of the multi-society consensus on CKD and HK in Spain

Abstract Background and Aims The high incidence, morbidity and mortality of Chronic Kidney Disease (CKD) makes it a public health problem, due to the difficulties in its early detection and management. Hyperkalemia (HK) is a common complication of CKD, also associated with high morbidity and mortality, impacting on the management of patients with CKD and leading sometimes to deoptimization of their treatment. It has been shown that the development of multidisciplinary consensus contributes to improving patient management. With this objective, the multi-society consensus documents –Information and consensus document for the detection and management of chronic kidney disease (2022)- and –Recommendations for the management of hyperkalemia in the emergency department (2022)- have been published. The aim of this study is to analyze the degree of adoption of the recommendations of both consensus in clinical practice in Spain, in order to identify potential areas of improvement in the management of CKD patients with HK. Method This study is based on the perceptions obtained through the discussion on the applicability of the national consensus documents for the management of CKD and HK. To this end, two medical education programs focused on CKD and HK, respectively, were carried out to discuss the applicability of both consensuses. In total, 41 meetings were held (14 from ERC and 27 from HK) from April 2022 to April 2023, involving a total of 542 healthcare professionals (HCPs) (nephrologists and emergency physicians). The main hypothesis was that neither the consensuses are being followed regularly in Spanish clinical practice. All responses were coded numerically, and a national mean was created based on the meeting means. Results The majority of nephrologists who participated in the discussion of the consensus analysis consider that CKD is underdiagnosed (77%), and that the detection of albuminuria is not routinely performed in Primary Care (PC) (72%). Furthermore, half of the nephrologists point out that referrals from PC do not meet the recommended criteria. In relation to treatment, almost 90% of nephrologists would follow the recommendations for the use of RAASi (renin angiotensin aldosterone system inhibitors) and Dapagliflozin very frequently. Regarding HK (a complication of CKD), 42% of HCPs considered that 30-40% of patients with CKD and/or HF suffered a second episode of HK. In relation to its management, only 21% of HCPs considered that the recommendations to maintain treatment with RAASis were followed. Of those who modified it, more than 50% did not consider reintroducing it or considered it little. 33% of HCPs did not show willingness to use the new antihyperkalemia treatments upon discharge. 50% of the participants said they did not have access to them. Regarding follow-up, 54% considered that it should be done by PC specialists. Conclusion The recommendations specified in the CKD and HK management consensuses are being applied less than desired in Spanish clinical practice. Especially highlighting the lack of detection of albuminuria and non-compliance with the referral criteria in patients with CKD, which is consistent with the high underdiagnosis described in Spain. Furthermore, the recommendation of not to modifying the RAASi treatment for the management of HK is not regularly adopted, despite of its impact on the morbidity and mortality of these patients. This information could help to guide future interventions that aim to improve the diagnosis and management of patients with CKD and/or HK.

#2183 Does thrombotic microangiopathy drive the occurrence and pattern of cerebrovascular disease in malignant hypertension?

Abstract Background and Aims Malignant hypertension (MHT) is characterized by diffuse microvascular damage caused by severe hypertension, often presenting with multiple organ involvement. Cerebrovascular injury, occurring in both acute and chronic forms, stands as a hallmark of the disease, being identified in up to half of patients. Thrombotic microangiopathy (TMA), defined as the association of mechanical hemolytic anemia with thrombocytopenia, also emerges as a frequent complication in MHT patients. However, the potential contribution of TMA to cerebrovascular disease remains unexplored. The secondary objective of this study was to provide a comprehensive description of the patterns of cerebrovascular injury occurring in MHT, with a focus on acute ischemic strokes (AIS). Method We retrospectively assessed the brain MRI of patients admitted to the kidney intensive care unit at Tenon hospital for malignant hypertension during the 2017-2023 period. Brain MRIs were performed on a systematic basis regardless of neurological symptoms and were independently reviewed by two trained specialists in a blinded fashion. Patterns of acute cerebrovascular disease, defined as AIS, intracerebral hematoma (ICH) or posterior reversible encephalopathy syndrome (PRES), were systematically assessed. Number of AIS lesions, their size and localization were recorded. Underlying chronic cerebral small vessel disease (CSVD) was defined as presence of lacunes, white matter hyperintensities, cerebral microbleeds or perivascular spaces. Severity of the chronic cerebrovascular disease was measured using a validated CSVD score. Results A total of 60 patients were admitted for malignant hypertension, with a mean age of 42.9 years. 40 patients (66.7%) presented with severe acute kidney injury. Cerebrovascular disease, either acute or chronic, was found in 42 patients (70.0%). Underlying chronic CSVD was present in 33 patients (55.0%). Acute patterns of injury occurred in 27 patients (45.0%). AIS was the most frequent (33.3%), followed by PRES (21.7%). Only one patient presented with ICH. Among patients with AIS, 14 (70%) presented with multiple simultaneous lesions. Of note, most patients with AIS exhibited diffuse encephalopathy syndrome (60.0%) rather than focal deficit (40.0%). Systemic TMA was present at admission in 34 patients (56.7%). In an exploratory data analysis, presence of AIS on brain MRI was associated with the presence of underlying CVSD (p = 0.013), but not with the presence of TMA at admission (p = 0.79). No association was found between TMA and PRES (p = 0.22). Conclusion Our study confirmed the high incidence of cerebrovascular disease during MHT. Most notably, 1 in 3 patients presented with AIS. TMA did not appear to increase the incidence nor to influence the pattern of brain injury.

MiRNA polymorphisms affect the prognosis of gastric cancer: insights from Xianyou, Fujian.

Gastric cancer, characterized by high incidence and substantial disease burden, has drawn continuous attention regarding its occurrence and prognosis. Genetics plays a crucial role in influencing the prognosis of gastric cancer, and single nucleotide polymorphisms are closely associated with the occurrence, development, and prognosis of this malignant tumor. Our study aims to conduct survival analysis on patients carrying different single nucleotide polymorphisms, exploring the relationship between miRNA single nucleotide polymorphisms and the prognosis of gastric cancer. Genetic data from 344 patients in Xianyou, Fujian, formed the basis of our study. We delineated the survival rate and median survival time, utilizing the log-rank test and COX regression analysis as statistical tools. Upon stratifying the data by sex or operation, it was discerned that the GG genotype at MSH2 rs17502941 independently posed a heightened risk for gastric cancer. Other stratification analyses suggested that the subsequent single nucleotide polymorphisms were correlated with patient prognosis: rs17502941, rs884225, rs1468063, rs7143252, and rs2271738. The outcomes of this study strongly suggest that miRNA polymorphisms significantly influence the survival time of gastric cancer patients and can serve as effective predictors for the prognosis of gastric cancer.

Estimation of Yield Loss Due to Striped Disease in Peanut Plants in Palopo City, South Sulawesi

Palopo City is one of the peanut production centres in South Sulawesi. However, from 2008 to 2020, groundnut production in Palopo City has decreased. Recently, in South Sulawesi, it was reported that peanut stripe virus infection has spread widely and evenly in peanut plants. Symptoms of stripe disease include dark green and light green leaf colour, chlorosis, and can cause stunting. This study aims to determine the estimation of yield loss caused by stripe disease in peanut plants in Polopo City, South Sulawesi. This research was conducted in a farmer-owned groundnut field located in Purangi Village, Sendana Subdistrict, Palopo City, South Sulawesi, from January to May 2023. Parameters observed were plant height, disease severity and incidence, and yield loss. Observations were made 6 times on plants aged 4 to 9 weeks after planting. The results showed that the symptoms of striped disease showed that the growth of peanut plant height was inhibited, and caused a decrease in the number of pods, pod weight, and pod number. Disease severity increased to 57.2% and disease incidence to 46.6%. Yield loss caused by stripe symptoms in peanut plants was 43.78%, indicating that high disease severity and incidence can reduce yield.

SIRT1, resveratrol and aging.

Aging is linked to a time-associated decline in both cellular function and repair capacity leading to malfunction on an organismal level, increased frailty, higher incidence of diseases, and death. As the population grows older, there is a need to reveal mechanisms associated with aging that could spearhead treatments to postpone the onset of age-associated decline, extend both healthspan and lifespan. One possibility is targeting the sirtuin SIRT1, the founding member of the sirtuin family, a highly conserved family of histone deacetylases that have been linked to metabolism, stress response, protein synthesis, genomic instability, neurodegeneration, DNA damage repair, and inflammation. Importantly, sirtuins have also been implicated to promote health and lifespan extension, while their dysregulation has been linked to cancer, neurological processes, and heart disorders. SIRT1 is one of seven members of sirtuin family; each requiring nicotinamide adenine dinucleotide (NAD+) as co-substrate for their catalytic activity. Overexpression of yeast, worm, fly, and mice SIRT1 homologs extend lifespan in each animal, respectively. Moreover, lifespan extension due to calorie restriction are associated with increased sirtuin activity. These findings led to the search for a calorie restriction mimetic, which revealed the compound resveratrol; (3, 5, 4'-trihydroxy-trans-stilbene) belonging to the stilbenoids group of polyphenols. Following this finding, resveratrol and other sirtuin-activating compounds have been extensively studied for their ability to affect health and lifespan in a variety of species, including humans via clinical studies.

Evaluation of antimicrobial and non-steroidal anti-inflammatory treatments for BRD on health and welfare in fattening bulls: a cross-sectional study

Our study aimed to evaluate the effect of different treatments for BRD on health and welfare in fattening bulls. A total of 264 bulls were enrolled. Welfare was assessed on day 2 (T0) and day 15 (T1) after arrival. A decrease in the welfare level was observed from T0 to T1. All bulls were inspected clinically at T0 and T1 revealing an increase of skin lesions and lameness in T1. In both periods, a high incidence of respiratory disease was observed. A prevalence of 79.55% and 95.45% of Mycoplasma bovis using RT-PCR and culture at T0 and T1 respectively was observed. Blood samples were collected for haematology at T0 and T1. At T0, 36 animals were individually treated for BRD with an antimicrobial (IT), 54 received a metaphylactic treatment with tulathromycin (M), 150 received a metaphylactic treatment with tulathromycin plus a second antimicrobial (M + IT) whereas 24 were considered healthy and therefore not treated (NT). Additionally, 128 were treated with a non-steroid anti-inflammatory (NSAID). Neutrophils of M + IT were significantly higher than groups NT and M and the lymphocytes of M + IT were significantly lower than that of IT. White blood cells, neutrophils and N/L ratio of animals treated with an NSAID was significantly higher than that not treated. Lung inspection of 172 bulls at the abattoir indicated that 92.43% presented at least one lung lesion. A statistically significant effect of the NSAID treatment on the lung lesions was observed. Our findings indicate that BRD was a major welfare and health concern and evidence the difficulties of antimicrobial treatment of M. bovis.

Impact of Soil-Applied Thyme Oil on Strawberry Yield and Disease Abundance

ABSTRACT Strawberry growers across the United States are looking for sustainable alternatives to conventional pesticides for management of soil-borne or crown-rot causing pathogens, such as plant-extracts like thyme oil. The objectives of this study were to determine the impact of soil-applied thyme oil fungicides on strawberry plant health, yield, sweetness, and foliar disease, and on the assemblage of soil microbial communities in strawberry plasticulture systems. In 2021 and 2022 the interaction of three soil-applied fungicides and three foliar fungicide programs were explored. Thyme oil was not found to improve strawberry plant growth or yield when compared to strawberries in the control. However, plants treated with mefenoxam exhibited an average winter crown weight of 2.11 g, significantly lower than control and thyme oil treatments at 3.14 g and 3.16 g, respectively. This translated into a significant yield loss where mefenoxam treated plants exhibited lowered yields at 416.04 g of fruit per plant compared to 523.57 g and 521.53 g per plant in the control and thyme oil treatments, respectively. A high incidence of foliar and fruit diseases was present; however, no interaction of soil and foliar fungicide treatments were observed for yield or fruit sweetness. Minimal root protection from pathogens was observed for all treatments and Fusarium, Pythium and Phytophthora fungi were regularly observed. Mefenoxam was observed to increase the recovery of oomycetes, which may cause Pythium and Phytophthora. No benefit from thyme oil was observed at our site and its addition as a soil fungicide would likely not benefit most growers.

Drastic reduction in the notification of acute cases of Chagas disease in the Northeast region of Brazil. Epidemiological evaluation in the period 2001–2021

Chagas disease (CD), caused by the protozoan Trypanosoma cruzi (T. cruzi), is a neglected disease endemic to some Latin American countries, including Brazil. Soon after infection, individuals develop an acute phase, which in most cases is asymptomatic and may go undetected. However, when CD is detected early, notification in the Notifiable Diseases Information System (SINAN), is mandatory. This study aimed to evaluate the information registered in the SINAN database and to determine the epidemiological profile of acute CD in Northeast Brazil, an endemic region, from 2001 to 2021. According to this survey, 1,444 cases of acute CD were reported in the Northeastern region of Brazil during this period. During the first six years, referred to as period 1, 90.24% of the notifications were registered, while the number of notifications significantly decreased in the subsequent years, referred to as period 2. Most individuals diagnosed with acute CD were Afro-Brazilian adults. All known routes of infection by the parasite were reported. Vector-borne transmission was predominant during period 1 (73.29%) and oral transmission during period 2 (58.87%). All nine states in Northeast Brazil reported cases in both periods. A higher incidence of disease was reported in Rio Grande do Norte (RN) during period 1, and in Maranhão (MA) during period 2. Our results show that CD remains a significant public health challenge.

A Case of Quadrivial Pattern of Right Upper Lobe Bronchus

Abstract The knowledge of the tracheobronchial tree is fundamental for a bronchoscopist, especially for diagnostic, therapeutic, and surgical options that are under consideration. Among all lobes, the right upper lobe (RUL) has a relatively high incidence of tumor and airspace diseases. Most bronchial anomalies are on the right side. Bifurcation is the most common anatomical variation. Quadrification has a very low prevalence. We report the case of a 54-year-old male who came with complaints of hemoptysis and showed a quadrivial type of bronchial division of RUL during a bronchoscopic examination after which three-dimensional computed tomography bronchography showed a supernumerary lateral or axillary segment bronchus. Awareness of this variant may explain otherwise confusing findings and its clinical implication is important for every pulmonologist.

Maternal Exercise Improves Infant Mesenchymal Stem Cell Lipid Storage and Oxidation, and Decreases Infant Adiposity

Rates of pediatric obesity are continuously rising and are likely to translate into high incidence of metabolic disease later in life. Maternal exercise during pregnancy has been established as a useful non-pharmacological intervention to improve infant metabolic health; however, mechanistic insight behind these adaptations remains mostly confined to animal models. Infant mesenchymal stem cells (MSCs) give rise to a multitude of infant peripheral tissue (e.g., skeletal muscle) and remain involved in mature tissue repair and maintenance. Importantly, these cells preserve the phenotype of an offspring donor and represent a model for the investigation of mechanisms behind infant metabolic health improvements. We used undifferentiated MSC collected during a randomized controlled trial to investigate if maternal exercise, regardless of exercise mode, affects infant adiposity and fatty acid oxidation. Compared to control, we observed >160% higher neutral lipid content in Ex-MSCs (p<0.01). However, this finding was coupled with significantly higher complete fatty acid oxidation to CO2 and lower incomplete fat oxidation (ASM production) in Ex-MSCs (p≤0.05). Infants in the exercise group had significantly lower body fat percentage compared to same age infants of controls at one month of age (p≤0.05). There was no difference in lean body mass volume (p>0.05) or infant one-month BMI (p=0.069). Together, these data indicate that maternal exercise exposure in utero is still associated with decreased infant adiposity one month after birth. MSC lipid content was positively associated with maternal fitness and exercise volume (p<0.01) during pregnancy. Finally, an increase in average maternal exercise volume (MET*min/wk; r=-0.4, p≤0.05) and total gestational MET*min (r=-0.36, p≤0.05), but not VO2max (p=0.07) was associated with infant 1-month body fat percentage. Together, these data showcase the influence of maternal exercise exposure on improvements in cellular metabolism and at 1 month of age. Collectively, our data suggests that exposure to maternal exercise could contribute to lower adiposity in early infancy. Multilocus Gene Analyses Indicate Tamarix aphylla as Reservoir Host of Diverse Phytoplasmas Associated with Witches' Broom and Yellowing Symptomatology.

Tamarisk witches' broom, yellowing, and little leaf symptoms were observed during 2018-2023 surveys of rural deserts in central regions of Iran with the highest disease incidence up to 72% in Chah Afzal (Yazd province). A verification of the presence and identity of phytoplasmas associated with these symptoms was then performed. Tamarisk tree branch cuttings obtained from symptomatic plants sprouted up to 90.3% but with 15-25 days' delay compared to the asymptomatic ones and showed internode shortening and witches' broom, while the branch cuttings from asymptomatic plants had normal growth and sprouted up to 97.8%. Phytoplasma transmission by dodder bridges to periwinkle did not succeed, while nested polymerase chain reaction on the phytoplasma ribosomal gene followed by RFLP and phylogenetic analyses revealed the presence of 'Candidatus Phytoplasma asteris', 'Ca. P. australasiae=australasiaticum', and 'Ca. P. trifolii' (ribosomal subgroups 16SrI-B, 16SrII-D, and 16SrVI-A, respectively) in the samples from symptomatic plants only. Further amplifications were performed on selected phytoplasma-positive samples on tuf and secA genes, and the produced sequences indicated the presence of mixed phytoplasma infection in some of the samples. In particular, in the tuf gene, a mixed infection of 'Ca. P. australasiae=australasiaticum' and 'Ca. P. trifolii' was detected, while in the secA gene, the presence of 'Ca. P. asteris' or 'Ca. P. tritici' strains was identified. The first-time detection of diverse phytoplasma strains in symptomatic T. aphylla suggests that this species represent a relevant source of infection for the agricultural crops and for landscape plants especially when temperature allows insect vector transmission, and therefore, it represents a risk in every environment especially in the frame of climatic changes.

Clinicopathological Characteristics of Everolimus-Associated Interstitial Lung Disease: A Single-Center Consecutive Analysis

Everolimus, a mammalian target of rapamycin inhibitor used as an antineoplastic drug, is associated with a remarkably high incidence of interstitial lung disease (ILD). The clinical and pathological characteristics of ILD caused by everolimus have not been thoroughly investigated; therefore, we aimed to elucidate the features of everolimus-associated ILD. We retrospectively reviewed the medical records of patients who received everolimus for cancer treatment at our hospital. Patient backgrounds were compared between the ILD and non-ILD groups. Chest computed tomography (CT), changes in biomarkers, and lung histopathological features were analyzed for ILD cases. Sixty-six patients were reviewed, and ILD developed in 19. There were no differences in patient demographics between the ILD and non-ILD groups. The severity of ILD was grade 1 (G1) in 9 and grade 2 (G2) in 10 cases. Chest CT showed organizing pneumonia (OP) or a hypersensitive pneumonia pattern. The levels of lactate dehydrogenase, C-reactive protein, Krebs von den lungen-6, and surfactant protein-D (SP-D) at the onset of ILD were significantly higher than those at baseline. Analysis of G1 and G2 ILD subgroups showed a higher SP-D levels in the G2 subgroup. Five patients underwent lung biopsies; all specimens demonstrated alveolitis with lymphocytic infiltration and granulomatous lesions, and some had OP findings. Everolimus-associated ILD is mild and has a favorable prognosis. Patients with symptomatic ILD were more likely to have higher SP-D levels than those with asymptomatic ILD. Granulomatous lesions are an important pathological feature of everolimus-associated ILD.

MiR-214-PTEN pathway is a potential mechanism for stress-induced immunosuppression affecting chicken immune response to avian influenza virus vaccine

Stress-induced immunosuppression (SIIS) is one of common problems in the intensive poultry industry, affecting the effect of vaccine immunization and leading to high incidences of diseases. In this study, the expression characteristics and regulatory mechanisms of miR-214 in the processes of SIIS and its influence on the immune response to avian influenza virus (AIV) vaccine in chicken were explored. The qRT-PCR results showed that serum circulating miR-214 was significantly differentially expressed (especially on 2, 5, and 28 days post immunization (dpi)) in the processes, so had the potential as a molecular marker. MiR-214 expressions from multiple tissues were closely associated with the changes in circulating miR-214 expression levels. MiR-214-PTEN regulatory network was a potential key regulatory mechanism for the heart, bursa of Fabricius, and glandular stomach to participate in the process of SIIS affecting AIV immune response. This study can provide references for further understanding of stress affecting immune response.

Assessing the impact of air pollution on short-term hospital visits for respiratory diseases in Lampang, a city heavily affected by biomass burning in Mainland Southeast Asia: Sex and age considerations

Mainland Southeast Asia has long been confronted with severe air pollution issues caused by open biomass burning. The analysis is conducted from January 2014 to December 2017 in Lampang located in central of Mainland Southeast Asia. The analysis of air pollution revealed significant exceedances of the PM10 (32%) and O3 (29%) air quality guidelines set by the World Health Organization. The major source of PM10 in Lampang is biomass burning, accounting for 97%, and it also experiences a high respiratory disease incidence of 7,276.61 per 100,000 population. A time-series analysis was conducted using generalized linear models based on Poisson regression, considering various lag times, to assess the relative risk (RR) among all age groups (0–14 years, 15–64 years, ≥65 years) and genders. A 10 μg/m3 increase in PM10 and O3 was associated with statistically significant percentage increases at lag 0. Specifically, there was a 0.65% (95% CI: 0.38–0.92) increase in respiratory diseases for PM10 and a 0.72% (95% CI: 0.43–1.01) increase for O3. The impact of PM10 and O3 significantly affects respiratory diseases in children, adults, and the elderly. Elderly males and females both experience the highest impact of PM10, whereas the greatest impact of O3 is observed in elderly males and adult females. Biomass burning is a major source that should be controlled to reduce the health risk of people. Protecting vulnerable groups like adult females, elderly males, and elderly females from high air pollution concentrations is crucial, especially with many countries experiencing an aging demographic shift.

Adequacy and Acceptability of the Self-Collected Anal Pap Smear in People Living With Human Immunodeficiency Virus in the Infectious Diseases Clinic.

Background Anal Pap smears are imperative to screening for human papillomavirus (HPV)-associated anal squamous cell cancers, particularly in patients living withhuman immunodeficiency virus (HIV) given a higher incidence of disease. Self-collection of specimens may be favored by patients and more feasible to collect, increasing screening. Methods This was a single-center observational cohort study at a single academic medical center Infectious Diseases clinic from October to December 2021. We aimed to improve compliance of anal Pap collection documentation of "self-collected" versus "physician-collected" as well as verify if self-collected specimens (SCS) were adequate for interpretation equivalent to physician-collected specimens (PCS). Additionally, we aimed to evaluate patient and provider satisfaction with self-collected anal Paps. Results Sixty anal Pap smears were available for evaluation. The rate of documentation of the collection method (self-collected vs. physician-collected) was 88% during the intervention. A total of 75% of patients opted for self-collection, and 35/45 (78%) of these samples were adequate for interpretation. There was no difference in the adequacy of specimen (the ability of a cytopathologist to interpret the specimen) between the SCS and PCS. Conclusion Limited prior data suggest self-collected anal Pap specimens are adequate for interpretation only slightly less often than PCS. In our small cohort, there was no statistically significant difference between collection methods. Satisfaction with self-collection of specimens was high for both patients and providers. Additional validation in more diverse/larger clinical settings may be helpful to support this practice.

Peculiarities of adaptation of the cardiovascular system of combatants after exposure to combat stress and trauma

Abstract. Military operations are generally considered to be a pronounced psycho-traumatic situation due to specific factors that lead to persistent mental and somatic health disorders and a slowdown in the rate of further social adaptation of military personnel.
 Post-traumatic stress disorder (PTSD) is described as a set of complex somatic, cognitive, affective, and behavioral effects of psychological trauma.
 One of the essential features of combatants' PTSD is that after the end of the period of stay in the conditions of hostilities, a high incidence of chronic somatic diseases is manifested. There is a correlation in the neurotic symptoms (in all cases) of deterioration of the somatic condition, and exacerbation of somatic pathology aggravates mental disorders.
 The problem of morbidity and mortality from circulatory system diseases, blood poisoning and injuries is of particular relevance to combatants, as the onset of cardiovascular disease is closely related to the traumatic impact of wartime or PTSD. Particular attention to the medical and psychological consequences of the effects of extreme factors associated with combat on the body of military personnel is associated with both the prevalence of PTSD, which reaches 70-85%, and the duration of its symptoms.
