1) Norfloxacin was given to 53 patients with otitis media. It was effective in 95.0% of those with acute suppurative otitis media, and in 63.6% of those with acute exacerbation of chronic otitis media.2) Cultures showed elimination of bacteria in 92.3% of those with acute suppurative otitis media, and in 62.5% of those with acute exacerbation of chronic otitis media.3) The antibacterial activity of Norfloxacin was excellent against both gram-positive cocci and gram-negative rods.4) No side effect was noted except for diarrhea in one patient.These results indicate that Norfloxacin is a useful antibacterial agent in the treatment of otitis media.

Respiratory Viruses and Mycoplasma Pneumoniae Infections at the Time of the Acute Exacerbation of Chronic Otitis Media

The present study was undertaken to ascertain whether or not patients with chronic otitis media are infected with viruses or Mycoplasma at the time of sudden increase in otorrhea.From 26 patients with acute exacerbation of chronic otitis media, sera were collected at the time of sudden increase in otorrhea and three to four weeks later. These paired sera were examined for antibody titer to respiratory viruses (21 species) and Mycoplasma pneumoniae. Of them, influenza B virus and RSV infections were demonstrated in four and two cases, respectively. Examinations showed no infection in 10 control cases without acute exacerbation.In 36 cases of acute exacerbation of chronic otitis media, attempts were made to isolate viruses and Mycoplasma pneumoniae from the pharynx and otorrhea. Consequently, influenza B virus was detected in pharyngeal mucous scrapings in two cases and RSV in one.The probability of respiratory virus infection leading to acute exacerbation of chronic otitis media appears to be lower than that provoking acute otitis media in children and infants. However, the present data suggest that the development of respiratory virus infection in patients with chronic otitis media may cause an increase in the otorrhea, eventually resulting in an acute exacerbation of inflammation.

TA-058, a new penicillin derivative, was administered to 21 patients with otorhinolaryngological infections.1. It was effective in all eight patients with tonsillitis, in six of nine patients with acute exacerbation of chronic otitis media and in the other cases.2. The bacteriological efficacy rate was 85.7% (18/21).3. TA-058 was moderately or markedly effective in all four patients who had not responded to previous medications.4. Drug eruption was observed in one of the 21 patients treated with TA-058.5. TA-058 is very useful antibiotic agent for patients with otorhinolaryngologic infections.

In order to evaluate the clinical efficacy and safety of AM-715 on acute suppurative otitis media (ASOM) and acute exacerbation of chronic otitis media (AECSOM), a double blind trial was carried out comparing with ABPC as the control drug.800mg/day of AM-715 and 2, 000mg/day of ABPC were administered four times a day.Results obtained were as follows:1. Clinical effectiveness(1) The effectiveness rates for the patients with ASOM evaluated by the attending physicians and the evaluation committee were 76.9% and 65.4% in the AM-715 group and 76.7% and 73.3% in the ABPC group.(2) The effectiveness rate for the patients with AECSOM evaluated by the attending physicians and the evaluation committee were 55.2% and 42.9% in the AM-715 group and 41.5% and 34.1% in the ABPC group.(3) In the cases of AECSOM, the AM-715 group was significantly superior to the ABPC group in the single infections and in improvement of swelling of the tympanic cavity mucous membrane.2. Bacteriological effect(1) The bacteriological effect of the AM-715 group was significantly superior to the ABPC group.(2) In the rate of eradication of S. aureus in single and mixed infections, the AM-715 group was significantly superior to the ABPC group.3. Adverse reactionsAdverse reactions were observed in 8 of the 147 cases (5.45%) of the AM-715 group and in 17 of the 151 cases (11.3%) of the ABPC group. The incidenec of adverse reactions for the AM-715 group was less than that for the ABPC group (P<0.10).No abnormal laboratory findings were observed in either of the groups.

OTITIS MEDIA; CARCINOMA OF ANTRUM; TUMOR OF PHARYNX. DR. JOSEPH C. BECK. The first patient was a woman, aged 23, who was first seen sixteen years before presentation. At that time she had an acute exacerbation of chronic otitis media, with subacute mastoiditis and cholesteatoma. The condition required immediate operation, and with the assistance of Dr. William Ballinger a Heath operation was performed on one side and a radical mastoid operation on the other. The patient was presented to demonstrate the effects of the treatment. She could hear the whispered voice at a distance of 20 feet and the high tones with the audiogram were preserved. This was one of the few cases in which the Heath operation gave permanently successful results. The case was interesting also because ether anesthesia was used, and the patient developed an infective bronchitis which persisted at the time of presentation. She
