SUN-284 MINITHEATER: A VIABLE OPTION FOR INTERVENTIONAL NEPHROLOGY PRACTICE IN RESOURCE POOR SETTINGS

Interventional Nephrology (IN) is a new and emerging subspecialty of Nephrology that mainly deals with: renal ultrasonography and ultrasound-guided renal biopsy, dialysis access creation and dialysis access related percutaneous endovascular procedures. The dependence on other specialists to perform these procedures often resulted in delays in renal patient treatment, hence the birth of IN. Unfortunately, cost prohibits the establishment of an ideal IN centre in resource poor settings. To establish a viable IN practice in resource poor settings using minitheater. A minitheater space, trained nephrologist, couch, light source, trolley, surgical instruments, biopsy set, ultrasound machine and skin cleaning agents. UNTH Interventional nephrology minitheater At the Renal Unit of University of Nigeria Teaching Hospital Enugu, Nigeria, we set up a minitheater and organized an International Society of Nephrology (ISN) Continuing Medical Education (CME) training programme for nephrologists and nephrology trainees in May 2019. Seventy- seven participants from 17 centers in 4 out of the 6 geopolitical zones in Nigeria were in attendance. During the pilot study, 2 arteriovenous fistulae, 6 tunneled right internal jugular vein catheter placements, 5 adult and 4 pediatric real- time ultrasound- guided kidney biopsies were performed in the minitheater by the nephrologists. There was no procedural complication. Following this training programme, 3 centers have set up and are utilizing minitheaters for IN. It is feasible to set up and run a viable IN programme in resource poor settings using a mnitheater. This removes reliance on surgeons and radiologists making the nephrologist independent and ensuring prompt care of renal patients.

Data Analytics for Creative Processes: Designing the Next Great Product

Problem Definition: Over the past decades, firms that develop new flavor and fragrance products (a $30 billion per year market) have amassed large amounts of data from their own internal creation processes. Yet, new product creation still mostly relies on human expertise and arduous experimentation. We present a data analytics framework to aid in the flavor and fragrance creation process. Practical Relevance: We describe general and fundamental algorithmic challenges for creating recipes. We address these challenges and propose an analytics framework to help product developers navigate the space of potential products that arises from combining ingredients and accelerate the overall development process. Methodology: The framework consists of two steps. In the first step, we use data to fit a metric over products, a procedure we refer to as curation. The metric of choice is the Earth Mover's Distance (EMD), where distances between products are quantified in terms of the pairwise similarity between ingredients. The curation process is formulated as a metric learning problem, where the cost matrix used in EMD computations is directly fit from labeled data. In the second step, we develop analytical tools to aid in the design of new products by sampling and evaluating new products within an (EMD) neighborhood of a seed product, a procedure we refer to as creation. Results: We introduce efficient algorithms for the curation and creation procedures. We derive parallel metric learning and k-NN search algorithms, an efficient sampling procedure, and an appropriate evaluation criterion across multiple metrics. We illustrate our results and product creation pipeline on open-access datasets. Managerial Implications: The methods we propose are new formulations and algorithms that address challenges that we observed as common across the innovation process of different flavor and fragrance firms, and may serve as an inspiration for a broader computational creativity approach used in industry.

Surface Structures, Particles, and Fibers of Shape-Memory Polymers at Micro-/Nanoscale

Shape-memory polymers (SMPs) are one kind of smart polymers and can change their shapes in a predefined manner under stimuli. Shape-memory effect (SME) is not a unique ability for specific polymeric materials but results from the combination of a tailored shape-memory creation procedure (SMCP) and suitable molecular architecture that consists of netpoints and switching domains. In the last decade, the trend toward the exploration of SMPs to recover structures at micro-/nanoscale occurs with the development of SMPs. Here, the progress of the exploration in micro-/nanoscale structures, particles, and fibers of SMPs is reviewed. The preparation method, SMCP, characterization of SME, and applications of surface structures, free-standing particles, and fibers of SMPs at micro-/nanoscale are summarized.

Cheek Dimpleplasty: A New Classification of Described Procedures

Cheek dimpleplasty has become a popular request amongst patients requesting cosmetic surgery. Since the first reported dimpleplasty in 1962, there have been many reported procedures in the literature for cheek dimple creation. Some of the procedures described by various authors as “novel” are actually similar if not identical to existing procedures. This study reviews the different procedures of cheek dimple creation and provides the first ever systematic classification for these techniques. EMBASE, Cochrane library, Ovid medicine, and PubMed databases were searched from its inception to June of 2019. We included all studies describing the surgical creation of cheek dimples. The studies were reviewed, and the different procedures were cataloged. We then proposed a new classification system for these procedures based on their common characteristics. The study included 12 articles published in the English language that provided a descriptive procedure for cheek dimple creation. We classified the procedures into 3 broad categories and subcategories. Type 1 procedures are nonexcisional myocutaneous dimpleplasties. In these procedures, the buccinator muscle is not excised. In type 1A, the suture used to create the adhesion traverses the epidermis. In type 1B, the suture does not traverse the epidermis, rather, the suture travels up into the dermis and returned back to the mucosa. Type 2 procedures are excisional dimpleplasties. In these procedures, the buccinator muscle is excised with (open) or without (closed) the excision of the mucosa. Type 3 procedures are incisional dimpleplasty. In these procedures, the muscle is incised and fixed to the dermis. Each of these groups of procedures has potential unique advantages and disadvantages. There are multiple procedures reported in the English language literature for the creation of cheek dimples. Most of the procedures are based on similar concept with minor variations. Our classification system, the Opoku-Simone Classification, will help facilitate communication when describing the different configurations of these procedures. Procedure within each group has similar potential advantages and disadvantages.

The paper shows the necessity for tracking the location and condition of both stationary and moving objects. The solution of this problem is possible by creating a domestic satellite system for data collection and transmission that provides automated collection of information on the state and coordinates of various objects serviced within this system regardless of their location on the globe. The choice of the optimal option for building a satellite data collection and transmission system is justified. The article presents the data on the existing scientific and technical groundwork of Joint Stock Company “Russian Space Systems” for creating a satellite data collection and transmission system. The main functions and principles of creation and tasks solved by the system for data collection, procession, and transmission are shown. The main design and technical solutions, composition, features of operation, and the main characteristics of such a system and its components are given. The procedure of creation and deployment of such a system is proposed.

НЕКОТОРЫЕ АСПЕКТЫ СОЗДАНИЯ УЧЕБНОГО ВИДЕОКОНТЕНТА В ПРОЦЕССЕ ОБУЧЕНИЯ

The study reveals the problem of organizing the efficient educational process using distance learning technologies based on video content. The paper presents the characteristics of e-learning and distance learning technologies. The study shows that e-learning, being flexible and variable, requires updating the content of education. Teachers need to master new telecommunications technologies, improve digital and information competencies. The authors show that video content genres being the synthesis of various types of visualization allow best solving teaching objectives within the educational process using e-learning technologies. The relevance of the latter is particularly obvious at present. The authors describe such types of video genres as a webinar, video-case, fast, conversation, interview, documentary webinar, and scripted video clip. The paper reveals the process of creation of a learning webinar with the participation of a lecturer and a student, describes the stages of script development, content design, video shooting, and editing. The development and rendering of learning programs to eContent causes the change in the role of a teacher; communication opportunities extend. The lecturer needs to customize innovative technologies to his/her methodological activity. The student’s participation in the process of preparing the materials supporting the teacher’s lecture gives an opportunity both to broaden knowledge in subject matter and to learn the fundamental aesthetic and technological principles and procedures of eContent creation. The presented material allows concluding that joint participation in the development of teacher’s and student’s webinars is an efficient means of formation of a set of skills of eContent communication and processing highly demanded in the modern educational process.

Towards a LSRO model of industrial performance improvement: A case study of LSRO model implementation in an industrial SME

The globalisation recognised in this century engages the third world to a strange competitive economic environment. The Moroccan Government highlights some emergency decisions to support the small and medium enterprises (SME) like decrease taxes and facilitate enterprise creation cost and procedure however those actions remain insufficient toward the external promotion reflected on delay, quality and cost of products and services. Hence, emergence of lean manufacturing system (LMS) came to resolve this problem by improving productivity and management system. However, for many manufacturers, especially SME, these concepts stay very difficult to implement and maintain. In this perspective, this work aims to overcome the optimum steps to install the advanced LSRO model to improve both productivity and performance with low cost. These implementation steps of LSRO model has been proposed through a case study relating basic concepts of LMS and management by objectives (MBO). Nevertheless, few works related to this field make academic research and industry closer, our work falls within this context through the industrial productivity improvement. This paper also proposes some research perspectives aiming to complete this research axis by enhancing the presented model through other industrial experiments and studies.

Reflexivity and the use of the in-depth interview as a research method for family practices -based on research of socioeconomic strategies of action of families in Serbia

The paper has been conceived in the form of considerations on the process of research preparation and realization based on the in-depth interview as a research method. In the broader context of criticism levelled at qualitative research methods, all the phases related to data compilation and the phases of work done on the compiled material have been described and explained. An attempt has been made to use a specific example to point out all the principles which guide research and analysis, how researchers uses reflective research and the arguments in favor of certain choices, how systematicity is achieved, and to provide illustrations of the complexity and scientific relevance of research procedures in the creation of a highquality database. In addition, a discussion has been provided on the scientific basis of the entire process of database creation and the analytical procedures involved in working on it.

Hybridization of Galactic Swarm and Evolution Whale Optimization for Global Search Problem

The works presented in this paper addresses the robust population-based global optimization that is influenced by the simplicity and efficiency principles introduced in two new generation optimization algorithms. Galactic Swarm Optimization is inspired by the motion of stars, galaxies, and superclusters of galaxies under the influence of gravity. It acts well as a global controller of the whole optimization process by employing multiple flexible cycles of exploration and exploitation phases to find new, better solutions. However, the optimization process still suffers poverty in the exploitation phase, which is improved in this work by its hybridization with our evolution version of the Whale Optimization Algorithm. Concretely, the exploitation phase of Galactic Swarm Optimization is replaced by our Evolution Whale Optimization Algorithm to avoid early convergence. The Whale Optimization Algorithm mimics the unusual social behaviors and the hunting activities of humpback whales. However, it is not optimized for global optimization when the number of dimensions is increased. Hence its is evolved in our works by Levy-Flight trajectory for faster local search with adaptive step lengths and two-point crossover operator to reduce bias in the offspring creation procedure. The achieved results through extensive and careful experiments showed that our hybridization and evolution enhancements bring outstanding performance in terms of accuracy, convergence speed, and stability.

Design procedure and honeycomb screen implementation to the air transtport department’s subsonic wind tunnel

Wind tunnels are the most widely used tools when comes to validation of Reynold number. Most of wind tunnels use various air straighteners or flow conditioners to achieve as most unified air flow as it’s possible. This article deals with design and creation procedure of air flow straightener – honeycomb screen. Based on mathematical relations and empirical experience defines dimensions and characteristics for subsonic wind tunnel of the Air Transport Department of University of Žilina. Wind tunnel equipped with a suitable screen provides more relevant and accurate data, which are crucial for final validation of results of test objects.

Dynamické modelování ložisek kaolinu na příkladu ložiska Jimlíkov-východ

This paper focuses on research within the project TE02000029 Competence Centre for Effective and Ecological Mining of Mineral Resources, granted by The Technology Agency of the Czech Republic, and, more specifically, on the research within its work package WP4 – Spatial modelling of mineral deposits. The focus of this work package is digital modelling of selected non-energetic raw materials, which belong to the critical commodities, as defined by the European Union. For modelling these deposits, suitable mathematical procedures, based on study and re-evaluation of archived data, are needed. One of the selected deposits is a kaolin deposit near the village Jimlíkov near the city Karlovy Vary. In this paper, we show a step-by-step procedure for creation, visualization and evaluation of a 3D model of the deposit. This methodology, along with our recently developed soft ware allows a user to create a variant of this dynamic model for the same or similar types of deposits, enables rapid updating of these models when adding or changing the input data on the basis of new mining exploration or when changing modelling parameters, such as using multiple variations interpolation parameters. Our methodology leads to a more advanced deposit evaluation, including adaptive estimates of the reserves based on the usability requirements we choose.

Cheek Dimpleplasty: A Review of the Literature

Dimpleplasty is the surgical creation of dimples on the face for aesthetic purposes. Dimples have been associated with a sign of beauty and even good fortune in some cultures and have also found themselves in Western cultures. Since the first described procedure of cheek dimple creation in 1962, there have been multiple studies on the topic. The aim of this article is to review the literature on the anatomy, creation, postoperative care, and complications associated with cheek dimpleplasty. The authors performed a literature review that focused on cheek dimpleplasty. EMBASE, Cochrane Library, Ovid Medicine, MEDLINE, Google Scholar, and PubMed databases were searched from its inception to July 2019. Peer-reviewed articles published in the English language were included. Our search in the various databases yielded multiple publications. There were different described techniques published on the determination of the ideal site for a cheek dimple. There were also multiple procedures described for the creation of cheek dimples. The basic tenet of cheek dimple creation is to create an adhesion between the dermis and buccinator muscle to create a dynamic dimple which is present on facial animation. Multiple procedures have been reported with few reported complications. Patient satisfaction is dependent on appropriate determination of the site and morphology of the dimple. Most of the complications associated with cheek dimple creation are minor, easily managed, and avoided by understanding the regional anatomy and attention to meticulous surgical technique.

Laparoscopic Creation of a Neovagina in Mayer-Rokitansky-Küster-Hauser Syndrome

Video Objective To show the step by step procedure of a neovagina creation in Mayer-Rokitansky-Kuster-Hauser Syndrome. Setting 22-year-old patient with primary amenorrhea, normal feminine phenotype development and vaginal agenesis on clinical examination. Pelvic ultrasound noted uterine agenesis. Mayer-Rokitansky-Kuster-Hauser Syndrome was diagnosed. Interventions A Neovagina was created with simultaneous laparoscopic and perineal approach. Surgery lasted 1 hour. Patient uses dilators to maintain a functional neovagina. Conclusion We believe this technique is simple, requires no special instruments and is fully reproductible with basic laparoscopic and vaginal surgical training. A correct understanding of surgical anatomy in each individual case warrants excellent results.

Arteriovenous fistula maturation rate is not affected by ipsilateral tunneled dialysis catheter

ObjectiveThe effect that ipsilateral tunneled dialysis catheters (TDC) have on arteriovenous fistula (AVF) maturation is unclear. We sought to define this association by comparing AVF maturation rates in patients with contralateral TDC with those with ipsilateral TDC. MethodsA review of a prospectively maintained database including all AVF creation procedures between 2009 and 2016 was performed. All patients with a TDC in place at the time of AVF creation were included in this study. Clinical and functional maturation rates were compared in patients with contralateral vs ipsilateral dialysis catheters. Categorical variables were analyzed by a two-tailed Fisher's exact test. A P value of less than .05 was considered statistically significant. ResultsThere were 187 patients who underwent fistula creation with a TDC in place during the study period. Of those, 137 patients had a contralateral TDC and 50 had an ipsilateral TDC. A greater proportion of contralateral patients were first-time dialysis access patients at the time of index AVF creation (67% vs 48%; P = .03). There was no difference in clinical (contralateral 73% vs ipsilateral 78%; P = .57) and functional (contralateral 64% vs ipsilateral 74%) maturation rates between the two groups. The rate of TDC removal after AVF maturation was also not different (contralateral 64% vs ipsilateral 72%; P = .30). There was also no statistical difference in the rates of thrombosis at less than 30 days, outflow stenosis, central stenosis, and steal syndrome. ConclusionsThere was no association between TDC sidedness and AVF maturation or early failure in our cohort. Planning for AVF creation should not be influenced by attempts to avoid an ipsilateral TDC.

Codesign for transitions governance: A mid-range pathway creation toolset for accelerating sociotechnical change

Vision building, pathway construction and experimentation are key processes in the management of long-term sociotechnical transitions. The need to accelerate transitions and to adapt transition management to new country contexts calls for new means to catalyse these processes. We improved the path creation toolsets and procedures of transition management to create more detailed pathways and analyses of pathway step interrelations. Our path creation system uses magnetic elements that could be easily moved around a large metallic board, a set of procedures and a digitalized counterpart of the board for out-of-the-workshop commentary and reporting. The system has facilitated and anchored well the discussions by participants with cross-sectoral backgrounds. Overall, the redesigned system underscores the potential that codesign for sustainability transitions holds.

Device for wideband ratio pyrometry

In this article we present procedures of creation, verification, and application of a device for contactless measurement of object’s temperature in vacuum. In contrast to conventional dual-wavelength pyrometers, it does not rely on spectral filtering and makes use of the intrinsic bandwidth parameters of photodetectors. In theory the device is capable of measuring values of temperature ranging from room temperature to several thousands kelvins. The verification was performed using a reference thermometer. Also, in-field measurements of the sample’s temperature in vacuum were conducted, which show efficiency of the proposed system.

Current state of dialysis treatment and vascular access management in Japan.

According to the data from the Japanese Society for Dialysis Therapy, the number of dialysis patients was about 330,000 at the end of 2016. The mean age of newly initiated patients was 69.4 years and that of maintenance was 68.2 years. And, diabetic nephropathy is the most common primary disease, with an incidence rate of 43.2%. These results mean that the systemic vascular condition is getting worse. In spite of these backgrounds, the patients of 97.3% were treated by hemodialysis; therefore, careful management of vascular access is essential to better maintain the condition of patients. The Dialysis Outcomes and Practice Patterns Study shows that vascular access modalities are an important factor in determining prognoses of patients and that prognosis in Japan is one of the best worldwide. In Japan, the use of arteriovenous fistulae accounts for 95% of vascular access modalities. However, a statistic by Japanese Society for Dialysis Therapy suggests that the use of arteriovenous graft has been increasing. In 2005, Japanese Society for Dialysis Therapy Guidelines recommended percutaneous transluminal angioplasty be the first choice for the treatment of vascular access stenosis. Since then, percutaneous transluminal angioplasty has become an important procedure for long-term maintenance of the morphology and function of vascular access. In Japan, approximately 60% of percutaneous transluminal angioplasty are conducted by nephrologists and urologists; in addition, arteriovenous fistulae creation procedures are also performed by them. According to my private opinion, such conditions above show that even in the absence of standardized training on vascular access management, doctors on site perform their duties in an appropriate manner. However, the problems of how we evaluate the specificity in Japan and pass it down the generations still remain.

Whether an Enormously Large Energy Density of the Quantum Vacuum Is Catastrophic

The problem of an enormously large energy density of the quantum vacuum is discussed in connection with the concept of renormalization of physical parameters in quantum field theory. Using the method of dimensional regularization, it is recalled that the normal ordering procedure of creation and annihilation operators is equivalent to a renormalization of the cosmological constant leading to its zero and nonzero values in Minkowski space-time and in the standard cosmological model, respectively. It is argued that a frequently discussed gravitational effect, resulting from an enormously large energy density described by the nonrenormalized (bare) cosmological constant, might be nonobservable much like some other bare quantities introduced in the formalism of quantum field theory.

Te questionnaire designed to measure structure and severity of emotional burnout among mothers of adolescents with schizophrenia

Tis work is devoted to the problem of emotional burnout in the context of parent-child relationship. Te possibility of applying the concept of burnout is being considered in order to describe the processes that occur in parents whose children suffer from a chronic psychiatric disorder, the author’s point of view on the matter is being presented. Te necessity of creating a questionnaire to measure emotional burnout among mothers of children with schizophrenia is being justifed, the procedure of creation of the new questionnaire is described, and the results of its approbation are performed.
