A 36-year-old male presented with a complaint of glare since 2 months. He had a history of trauma to the left eye with copper wire 2 years back. At presentation, the visual acuity was 20/20 in both eyes. Anterior-segment evaluation of the left eye revealed peripheral corneal scar, anterior uveitis, and iris hole. Dilated evaluation revealed sunflower cataract, focal cortical cataract, and an intralenticular foreign body (ILFB). The posterior segment was normal. Inflammation was controlled with topical steroids, followed by ILFB removal, phacoaspiration, and intraocular lens implantation. A thorough dilated ocular examination is essential to rule out an intraocular foreign body in all the cases of penetrating ocular trauma.
