Psychiatric Advance Directives: Perspectives of Community Stakeholders

The present study compares the familiarity with and attitudes about psychiatric advance directives among multiple community stakeholders in two Ohio communities. One community was involved in a pilot psychiatric advance directive education project, and one was not. A total of 272 legal and law enforcement personnel, clergy, healthcare professionals, mental health professionals, consumers, and family members completed a newly developed measure of opinions about psychiatric advance directives. Results indicate significant differences in opinions among members of the various stakeholder groups, and implications for advance directive education are discussed.

Critical Issues in Bioterrorism Preparedness

The Bioterrorism Preparedness and Response Survey (BPRS) was a survey of Ohio local health departments' capacity to respond to bioterrorism. Soon after completion of the BPRS, the events of September 11 occurred, followed by the human cases of anthrax. The Ohio Response to Bioterrorism 2001 Survey (ORB) identified bioterrorism preparedness issues related to the suspected anthrax incidents. The BPRS measured capacity before September 11, 2001, and the ORB measured Ohio communities' response to white powder incidents. The BPRS and ORB provided independent and outcome measures related to the 2001-bioterrorism events. The significant bioterrorism response issues were: monitoring critical or unexplained deaths and clusters or symptoms; training on bioterrorism agents; integration of medical and criminal investigations of bioterrorism incidents; development of bioterrorism emergency response plans to include agencies to be contacted, management strategies for implementing mass vaccination, prophylaxis, treatment distribution and administration; and participation in a bioterrorism field or tabletop exercise. These results are confirmed and extended by studies by the US General Accounting Office, the Rand Corporation, Trust in the Future of America's Health foundation, and a follow-up survey of issues during a simulated covert smallpox attack.

Irish Farming Households in Eastern Canada: Domestic Production and Family Size

Irish farming households in Eastern Canada during postfamine period (1861-1871) are used as basis for a theoretical discussion of domestic production and family size. The purpose here is to extend discussion of Chayanov (1966) model of peasant economies, and offer modifications of this model based on empirical variations in Irish Canadian household composition, dependency ratios, and overall farm size. This article suggests that topic of choice and assumption of risk be made a more explicit part of Chayanov analysis, since farmers' decisions regarding productive capacity are apt to be made on a more holistic view of a farm's assets, and not just on basis of cultivated acreage per worker. (Irish Canadian households, domestic production, family composition, Chayanov model) ********** On subject of farm performance, or what Sahlins (1971) referred to as intensity of domestic production, previous studies by Kane (1968) and Symes (1972) have raised several important substantive issues concerning Irish households that have not yet been adequately resolved. (2) Kane (1968), for example, draws comparisons between rural households in southwestern Donegal and those of an Irish-American community in Ohio. Such studies of transcontinental relationships in Irish families are a valuable addition to literature because they illuminate structural similarities and differences in Irish farming as adaptations to new ecological and social settings. Thus, farm performance can be understood in a transitional sense, as an adaptive process. Differences between Irish American and Donegal households affecting performance include kinship structures that serve as a central distributing point for services, minor economic aid, and exchange of goods (Kane 1968:254). Based on ethnographic research in Ballyferriter, southwest Ireland, Symes (1972:25) concluded that the most important variable is structure of family unit itself [and these] variations occur both through time and through space. Examples of these variations include decreases in household size due to depopulation, transitional or structural factors such as those resulting in a change from stem to nuclear families, and a growing scarcity of farm labor. All in all, a fairly rapid of household size during dispersal [resulting from emigration and moves to urban centers] and increasing age and diminishing aspirations of farmer, level of farm production may be expected to decline (Symes 1972:35). This article on Irish farming households of Renfrew County (Admaston Township) in eastern Canada differs from those just mentioned in some significant ways. First, based on Canadian census data from postfamine period of 1861-71 (Canada 1861, 1871), farm performance is explored in context of population expansion, rather than depopulation, as is case with most previous Irish studies. Theoretically, it is beneficial to study farming communities undergoing growth and expansion as a balance to a focus on those communities in decline. Second, longitudinal approach taken here, extending over a decade in same geographical area, illuminates extent to which variations in household size and composition affect farm performance. IRISH EMIGRATION TO CANADA Prior to cataclysmic Great Famine (1846-49) in Ireland, emigration to Canada varied considerably from one decade to another (Elliot 1988; Houston and Smyth 1990; MacKay 1990; Mannion 1974; and Moran 1994). Migration from Irish ports to British North America (Canada) between 1825 and 1845 reached a high of 40,977 individuals in 1831 and a low of 2,284 in 1838. Curiously, corresponding figures for Irish emigration to United States for this period are much lower, reaching a high, for example, of only 6,199 in 1842, and a low of 1,169 in 1838. THE GAP DYNAMICS OF CANOPY TREES OF A TSUGA CANADENSIS FOREST COMMUNITY

Abstract We examined the correlation between canopy gap formation and the initial growth of forest trees by reconstructing the gap history of a Tsuga canadensis (L.) Carr. forest community in southeastern Ohio. We cored each tree (>10 cm dbh, n = 156) in a 40 × 90 m plot and examined the cores for release events, characterized by dramatic increases in radial growth. We identified 80 former gaps in the 79 yr sample period by clustering release events in time and space. Thirteen of the 80 former gaps coincided with the initial growth of trees. These 13 gaps were usually large gaps containing few established trees or gaps undergoing repeat disturbance. Of the 36 trees >10 cm dbh that began growth during the sample, 21 (58%) began growing inside a gap within 6 yr of gap formation-three times the rate predicted by chance (p = 0.001). Voices from the Delaware Big House Ceremony

Voices from the Delaware Big House Ceremony examines and celebrates the Big House ceremony, the most important Delaware Indian religious observance to be documented historically. Edited by Robert S. Grumet, this compilation of essays offers diverse perspectives, from both historical documents and contemporary accounts, which shed light on the ceremony and its role in Delaware culture. As Grumet says, The many voices brought together in this book produce something more akin to a chorus than a chant. The annual fall festival known as the Gamwing (Big House) was the center of life for Delaware Indian communities in Pennsylvania, Ohio, and Indiana west to Ontario and Oklahoma. The last ceremony was performed by the Eastern Oklahoma Delaware community in 1924. Determined to preserve their traditions for future generations, Delaware Big House followers have worked with anthropologists to preserve Big House texts, rituals, songs, and sacred objects. Including commentaries by Delaware traditionalists from communities in Oklahoma, Wisconsin, and Ontario, where most descendants of the Big House Church live today, the volume also features an ethnographic description of the Big House ceremony and historical accounts dating from 1655 to 1984. Spectral differences in /ai/ offsets conditioned by voicing of the following consonant

Two experiments were designed to investigate the observation that offsets of pre-/t/ /ai/ (e.g., tight) show lower F1and higher F2than those of pre-/d/ /ai/ (e.g. tide) in some dialects. The first experiment examined production of /ai/ in minimal pairs like tide... tight by non-Hispanic whites from two central Ohio communities and Mexican Americans from southern Texas. Results confirmed that the spectral difference occurs in the speech of the central Ohioans. Although it was anticipated that the south Texas subjects would not exhibit the spectral difference, they did produce it, but to a lesser degree than the central Ohioans. The second experiment examined perception of stimuli as tight, tide, or tie by non-Hispanic whites from central Ohio and Mexican Americans from southern Texas to investigate whether the observed patterns in production are reflected in perception. It was confirmed that speakers of the two dialects under study can access the offset spectral difference as a perceptual cue. Moreover, it was found that speakers of the two dialects differ in perception of the spectral difference. The cross-dialectal discrepancies show that the spectral difference is part of a speaker's grammar, even though it is not contrastive.

DIVERSIFICATION TRENDS OF THE REGIONAL ECONOMY OF MILL-TOWN COMMUNITIES IN NORTHEAST OHIO, 1980-1991

This study evaluates industrial diversification trends of four county communities in northeastern Ohio after more than ten years of attempts to stimulate the regional economy following steel mill closings of the mid-1970s. With the application of a multiple-model replicants technique, employment in 39 compatible sectors of the region's 2-digit industrial sectors were analyzed. The study shows that while a few sectors grew above the state and national averages, a majority experienced modest to declining growth rates. In particular, the analysis shows that the general regional growth trends are predominantly skewed towards low-technology service-producing industries with little or no value-added activities—consistent with the lack of a diversified economic base. The region needs to attract a blend of high value-added manufacturing and service-producing industries if it is to withstand future structural/cyclical shocks from the national economy as well as remain competitive in the global economy.

The least restrictive alternative in the postinstitutional era.

The legal concept of the right of psychiatric patients to treatment in the least restrictive alternative has come to mean treatment anywhere but in the state hospital, as illustrated in two large communities in Ohio and Massachusetts. Oversimplified application has led to the use of treatment setting as the only measure of restrictiveness. Patient's characteristics and needs have been ignored, along with the possibility of wide variation in program quality within each class of facility. The role of the state hospital has become uncertain, making it difficult to attract and keep qualified professional staff. The authors argue for an expanded view of the concept that includes consideration of patients' needs. They contend that the concept should not be used as a sole reason to avoid admitting patients to state hospitals or to continue the down-sizing of public institutions.

The relation between caries prevalence and strontium concentrations in drinking water, plaque, and surface enamel.

Samples of plaque and surface enamel were collected from 80 boys aged 14 years living in five communities in Ohio (USA). Mean (+/- SE) strontium concentrations in plaque were 1.43 +/- 0.18 micrograms/g (wet) and 12.34 +/- 1.63 micrograms/g (dry). For surface enamel, the mean concentration was 421 +/- 31 micrograms Sr/g. Concentrations of strontium in both plaque and enamel were significantly related to strontium levels in the drinking water. Caries prevalence as DMFS was inversely related to strontium levels in water, plaque, and enamel.

Reservoir Impacts: A Synthesis of a 10‐Year Research Project

The purpose of this paper is to present a synthesis of research findings derived from longitudinal monitoring of a reservoir affected community group located in the urban fringe of a major metropolitan community in central Ohio. Data were collected at three time periods to assess the social impacts of rapid change on local people. Data were collected over a 10‐year period using standardized measurement devices to ensure comparability of the information collected. The research findings demonstrate that extensive modification of social and physical environments did not result in the destruction of the social fabric of the affected group. Observations over time suggest that the affected group has accommodated many of the changes introduced in the community and has accepted the lake project.

Until fairly recently the new labor history has devoted relatively little atten tion to coalminers, despite the fact that by the end of the nineteenth century they constituted the largest body of workers employed in industry on either side of the Atlantic. Thus on the English side the standard history of the miners until the 1960s was R. Page Arnot's useful but narrowly institutional The Miners. In America McAlister Coleman's Men and Coal and McDonald and Lynch's Coal Miners' Unions had a similar appeal. There have been a number of U.S. area studies, such as R. D. Ward's and W. W.
