The life of Wilhelm Conrad Roentgen.

This year, 1995, marks the centennial of the discovery of X-rays by Wilhelm Conrad Roentgen on the historical date of that discovery, November 8, 1895 [1]. After completing my radiology residency at Brooke General Hospital, I was assigned to the U.S. Army 5th General Hospital in Stuttgart Bad Cannstatt, West Germany. This period of payback was from October 1966 to August 1969. During these memorable years, the opportunity presented itself for me to make a pilgrimage to every location associated with the life of Roentgen. It is my pleasure to share that pilgrimage with you.

Operations management in emergency toxicology.

Dr. Lundberg holds degrees from North Park College in Chicago, Baylor Univ., and the Univ. of Alabama. He received his M.D. from the Medical College of Alabama in 1957. He completed a rotating cfinical internship at Tripler Hospital in Hawaii and a residency in anatomic and clinical pathology at Brooke General Hospital in San Antonio. In 1967, he joined the LAC/USC Medical Center, where he became Assoc. Director of Labs and Pathology and Professor of Pathology. In 1977, he became Professor and Chair of the Dept. o f Pathology at the Univ. of California Davis School o f Medicine. In 1982, he became Vice President of the AMA for Scientific Information, and Editor of JAMA, with editorial responsibility for nine journals. His primary goal at AMA, 'in addition to survival is to 're-establish JAMA as the foremost medical scientific periodical in the worM and to solidify each specialty journal among the respective top journals of their kind.

Abscess in the Nonlactating Breast-Reply

Reply <i>To the Editor</i>.—In answer to the letter by Drs. Caswell and Maier, I want to point out that this was a retrospective study of patients at Brooke General Hospital over a 20-year period, so that we cannot accurately state how many patients had communication between the abscess cavity and the base of the nipple. In many cases this was not recorded. Also, our study concerned itself with breast abscesses in the nonlactating breast, not with breast abscess due to inverted nipples, although from our material this did not appear to be an etiological factor. Simple mastectomy in the woman was performed early in the 20-year span, and only after multiple procedures had been attempted. The conclusions of the article did not recommend simple mastectomy as a mode of therapy.

Seminoma of the testis.

During the period of 1947 to 1969, 77 patients with pure seminoma of their testes were seen at Brooke General Hospital, Fort Sam Houston, Texas.Most of the patients were younger than reported by other authors.The right and left testis was involved with almost equal frequency.All of our patients except 2 had a Stage I disease. Orchiectomy on the affected side was the method of biopsy routinely in all patients.In this series no difference was noted in the survival rates of the patients who have had different kinds of treatments such as: retroperitoneal lymph node dissection and irradiation; irradiation to the abdominal paraaortic lymph nodes; and irradiation only of the abdominal paraaortic, mediastinal, and left supraclavicular regions.The retroperitoneal lymph node dissection in pure seminoma of the testis has no role in its management.There are also enough reasons to believe that routine irradiation of the mediastinum and the left supraclavicular region in a Stage I seminoma of the testis is not warrante...

Rectal carcinoma in patients in the second and third decades of life

The experience with carcinoma of the rectum in patients thirty years of age or younger at Brooke General Hospital is reviewed due to poor results in four recent cases. Nineteen patients, or 9.2 per cent of the total number registered, were found to have a five year survival of 16.7 per cent. Duration of symptoms, physical findings, tumor histology, and treatment were analyzed to determine the reason for the poor results. Delay in diagnosis was implicated as the important factor since those patients with tumors amenable to resection for cure had a survival rate of 37.5 per cent.

Seminoma of the testicle: prophylactic mediastinal irradiation versus periaortic and pelvic irradiation alone.

An analysis of 80 patients with seminoma of the testis treated at Brooke General Hospital, from July 1947 to March 1971, is presented. Forty-nine patients with Stage I disease were treated prophylactically to the mediastinum and left supraclavicular area in addition to receiving periaortic and pelvic irradiation. Twenty-two patients with Stage I disease received only periaortic and pelvic irradiation. No recurrences have been observed in Stage I patients with either method of treatment. The crude survival rates for the entire group of 80 patients at 5, 10, and 15 years were 97, 93, and 92%, respectively.

Remission induction of adult acute leukemia with combination chemotherapy.

Intensive chemotherapy was administered to 20 adults with acute leukemia for remission induction. A combination of 6‐mercaptopurine, methotrexate, vincristine, and predni50ne was used in 8 patients. Arabinosyl cytosine and cyclophosphamide in combination with or without vincristine and prednisone was used in 12 patients. Six of 8 patients with acute myelogenous leukemia and 5 of 8 patients with acute nonmyelogenous leukemia who received adequate therapy responded. The response rate for all patients including those who died without the benefit of adequate therapy was 60 per cent. Nonfatal hemorrhage occurred commonly during therapy. Gram‐negative infections were the most common complications causing death in the patients who did not achieve remission. Supportive care in the form of transfusion of platelets, packed red blood cells and prompt effective antibiotic therapy wm essential for the success of antileukemic therapy. These modalities of therapy have resulted in an improvement of the remission rate and survival in patients with acute leukemia at Brooke General Hospital.

Potassium Response to Succinylcholine

To the Editor:— I read with great interest the article by Birch et al and the companion editorial on hyperkalemia induced by succinylcholine chloride in patients with trauma ( 210: 490; 549, 1969). I am impressed by the thoroughness with which they investigated the mechanisms of potassium release. However, I feel that they have overstated the implications regarding curare as a blocker of this response. In their studies, they administered 6 mg of curare five minutes prior to a second dose of succinylcholine and blocked the abnormal elevations of serum potassium that had been seen with an initial dose of succinylcholine. From this they concluded that pretreatment with curare eliminated the risk of hyperkalemia associated with succinylcholine in patients with trauma. Data from a single patient with burns, treated at the US Army Institute of Surgical Research, Brooke General Hospital, San Antonio, Tex, contradict this conclusion. Multiple Halothane Anesthesia in the Burn Patient

Halothane is the anesthetic agent of choice for burn patients in the Burn Unit at Brooke General Hospital. It is used repeatedly—an average of four administrations per patient, with a range to 22. From 1964 through 1968, 408 patients received 1,770 halothane anesthetics. Two cases of liver necrosis occurred, one massive; neither is attributable to halothane. The evidence indicates that repeated administrations of halothane in a six-week period involve no additional risk to the patient.
