The Association of Epidemiological and Clinical Parameters with Diabetes and Hypertension in a Rural Area: A Mahbubnagar District Study

Back g round: Rapid development in a country like India has also brought an explosion in the prevalence of non-communicable disease. Affluence to western culture can be one of the reason. This might have encroached the rural India. A cross‑sectional survey was done to assess the socio-demographic, clinical and laboratory factors diabetes and hypertension among the study population. Subjects dan Method: This is a descriptive cross-sectional study. A sample of 244 diabetes and hypertensive patients registered under the subcentre were taken as study subjects. Sociodemo­graphic factor, clinical parameters and Laboratory parameters were taken independent variables. The dependent variables were diabetes and hypertension. Data analysis was done with help of SPSS version 26 software. Standard instru­ments were used to measure, weight, height, and waist circumference (WC), blood pressure. The random blood sugar, HbA1C and lipid profile values were taken from laboratory reports. The other variables were collected by questionnaire. Results: A total of 244 participants reported and responded to this study who were having diabetes, hypertension or both. About 35% of the population belonged to lower SEC. Amongst males almost 50% of the male had dual co-morbidity i.e Diabetes and Hypertension. About 85% of the population was Illiterate (p<0.05). Mean value of HbA1C was high among the diabetic patient who were under treatment (Mean= 7.10; SD=1.20). Lipid profile value were in normal range but higher compared to stats of other selected states. Conclusion: Study subjects even on treatment were unable to keep the laboratory parameters under control hence requiring indepth research. Study of Biochemical Parameters in Diabetic Patients with and without Diabetic Retinopathy – A Hospital based study

Background: Diabetic Retinopathy (DR) is a common, potentially blinding and visually disabling complication of diabetes. Early detection of retinopathy and its progression to severity with routine referral for screening by Ophthalmologist can save vision by timely management. Methods: This was a hospital based cross-sectional study done to study the association of glycated hemoglobin (HbA1c), fasting and postprandial blood sugar, serum lipid profile, serum creatinine and urine albumin in diabetic patient with and without DR. A total of 50 patients with Diabetic retinopathy and 50 patients without diabetic retinopathy were enrolled in this study. All patients included were Type 2 DM aged 35 years and above. Results: The mean HbA1c was 8.62±1.5 and 5.54±1.2, total cholesterol was 228.9±63 and 184.9±39.8 mg/dl, serum triglyceride was 226.6±80.7 and 160.8±45.1 mg/dl, LDL-C was 152.3±49 and 127.2±37 mg/dl and serum creatinine was 1.15±0.45 and 0.66±0.27 mg/dl in diabetic retinopathy group and no diabetic retinopathy group respectively. There was significant association of elevated HbA1c, serum triglyceride, LDL- C and total cholesterol with diabetic retinopathy in patients with type 2 DM. The mean values of serum lipoproteins were higher in the diabetic retinopathy group. Conclusion: Elevated fasting and postprandial blood sugar, glycated hemoglobin, total cholesterol, serum triglyceride, LDL-C, serum creatinine and urine albumin were significantly associated with DR in our study. So, all patients with diabetes mellitus should be screened routinely for serum lipoproteins along with blood sugar profile, as it would help in early detection of diabetic retinopathy.

Transient hyperglycemia, impaired glucose tolerance and gestational diabetes mellitus (GDM) in preterm pregnant women receiving antenatal steroids

OBJECTIVE:To determine the frequency of transient hyperglycemia, impaired glucose tolerance and gestational diabetes mellitus(GDM) in preterm pregnant women receiving antenatal steroids.
 METHODOLOGY:This descriptive cross-sectional study was carried out in Maternal and Child Health Center unit 1 (MCH-1) at Pakistan Institute of Medical Sciences Islamabad Pakistan from January 2017 till August 2017. A total of 365 pregnant women presenting to emergency and outpatient department with preterm labor (alive morphologically normal babies), with preterm premature rupture of membranes (PPROM) and other conditions which require early delivery including preeclampsia, IUGR requiring preterm delivery, severe oligohydramnios, antepartum hemorrhage(APH), women receiving two doses dexamethasone and all those with BSR>126mg/dl were included in the study. Multiple pregnancies, advanced preterm labor (cervix > 5cm dilated), gestational diabetes mellitus (GDM) or type I/II diabetes mellitus (DM), chorioamnionitis and taking any medication that affects glucose metabolism were excluded from the study. After ethical approval, informed consent was taken from study participants. Blood sugar levels before the commencement of 1st dose of dexamethasone were noted. Blood sugar profile (fasting, 2 hours after lunch, 2 hours after dinner) were carried out. 2nd dose of dexamethasone was given after 12 hours of 1st dose. Profile was carried out till euglycemia or 5 days if sugars remain deranged. Patients having deranged levels for greater than 5 days were advised 75 g oral glucose tolerance test(OGTT) and labelled as having impaired glucose tolerance or gestational diabetes mellitus.
 RESULTS: In our study, 57.57%(n=213) were between 18-30 years, 42.43%(n=157) were between 31-40 years of age, mean age was calculated as 28.92+5.54 while mean gestational age was 31.19+1.92 weeks. Frequency of transient hyperglycemia, impaired glucose tolerance and gestational diabetes mellitus(GDM) in preterm pregnant women receiving antenatal steroids revealed 62.16%(n=230) had transient hyperglycemia, 9.46%(n=35) had impaired glucose tolerance, 2.16%(n=8) had gestational diabetes and 26.22%(n=97) had no blood glucose abnormality.
 
 CONCLUSION: We concluded that the frequency of abnormal glucose levels increases in preterm pregnant women receiving antenatal steroids. Therefore, single blood sugar level done routinely before dexamethasone therapy are insufficient to judge the glucose metabolic status and should be closely monitored during the use of antenatal corticosteroids.

The Association of Higher Composite Biomarker Score of Antioxidant Vitamins with Lower Cardiovascular Diseases Risk: A Cross-Sectional Study

Biomarkers for the dietary system, which includes a mixed fruit and vegetable (FV), are needed to understand the association of FV intake with a reduction in cardiovascular diseases risk. This is a cross-sectional study that aims to find the potential relationship between a high composite score comprised of antioxidant vitamins and a lower incidence of cardiovascular diseases (CVD). A total of 94 (42 males and 52 females) participants (mean age ± SD: 51.7 ± 9.4 years) completed a specific questionnaire including a quick food scan, which has designed by the American National Institutes of Health. The vitamins A, C, and E were determined using high-performance liquid chromatography (HPLC). An enzymatic colorimetric method was used to determine other biomarkers [fasting blood sugar (FBS), haemoglobin A1c (HbA1c), and lipid profile]. A Composite Biomarker Score (CBS) comprising of the plasma vitamins (A, C, and E) have been derived. The results showed that male participants demonstrated significantly higher atherosclerotic cardiovascular disease (ASCVD) risk than female participants. Female participants with greater ASCVD risk were associated with significantly higher age, total cholesterol, and triglyceride concentrations. Additionally, there is a significant relationship between the CBS with lower ASCVD risk. Consequently, it can be concluded that higher concentrations of serum antioxidant vitamins are related to a reduction in cardiovascular diseases risk.

Brown Rice Consumption and Changes in the Metabolic Risk Factors of Non-communicable Diseases in Selected Overweight and Obese Filipinos

Human studies on the health benefits of brown rice (BR) are limited, especially in the Philippines, even though rice is the staple food. This is the first randomized controlled investigation on the effects of BR consumption on metabolic risk factors of non-communicable diseases (NCDs) in selected overweight and obese Filipino adults. Out of 60 eligible male and female participants (30–59 yr old) randomly assigned to the BR and white rice (WR) groups, 52 completed the study. This was organized in a semi-free-living condition with 6-wk dietary intervention and 6-wk followup. Participants were assessed at 0, 2, 4, 6, 8, 10, and 12 wk for overweight/obesity parameters and blood pressure (BP), and at 0, 6, and 12 wk for fasting blood sugar (FBS) and lipid profile. More metabolic risk factors were significantly reduced at post-intervention in the BR group [body mass index (BMI), visceral fat (VF), systolic BP (SBP), diastolic BP (DBP), FBS, total cholesterol (TC), and low-density lipoprotein (LDL)] than in the WR group [SBP, DBP, TC, LDL, and triglycerides (TG)]. Most of the parameters in the BR group significantly improved at follow-up including the high-density lipoprotein (HDL). Therefore, the BR-based diet was more effective in improving the metabolic risk factors status than the WR-based diet. Although a statistically significant difference was not detected between the groups, the percent change had small clinical significance – particularly in BMI, DBP, FBS, TC, LDL, and HDL. Hence, in clinical practice, BR could help ameliorate the metabolic risk factors of NCDs among overweight and obese Filipinos. Further investigations are needed to elucidate the effects of BR on metabolic risk factors.

Correlation between HbA1C and D-dimer in Type 2 Diabetic with COVID-19 Patients

Background: Coronavirus disease-19 (COVID-19) is the disease caused by 2019-nCoV/SARS-CoV-2, a novel β corona- virus of group 2B . COVID-19 has been reported to be associated with severe condition with type 2 diabetes mellitus (T2DM). D-dimer is a by product of fibrin degradation. It is widely recognized as a biomarker for thromboembolism and as a prognostic marker for critical patients.
 Methods: This study uses secondary data, which is obtained by looking at the contents of patient medical records at H. Adam Malik Medan Hospital between 2020-2021. The population of this study was all inpatients confirmed COVID-19 through an RT-PCR SARS-CoV-2 swab examination. The study sample was obtained by the total sampling method, where all populations that meet the criteria of inclusion and exclusion are used as research samples. Patients were triaged as per severity on basis of national guidelines: asymptomatic, Mild (respiratory rate < 24/min, SpO2 >94% at room air), moderate (respiratory rate: 24-30/min, SpO2 90-94% at room air) and severe (respiratory rate >30/min SpO2 < 90%), ARDS and septic shock [6]. HbA1c implementation using HPLC methods and D-dimer is measured by ELISA method.
 Result: There were 163 patients consisting of 92 men and 71 women, the average age was 56.9±10.3 years, and HbA1c, FPG and PPG remained uncontrolled. There were no differences in age parameters of blood sugar profiles and D-dimers between men and women. D-dimers are significantly negatively correlated with PPG.
 Conclusion: Age, blood sugar profile and D-dimer did not differ significantly between men and women, and D-dimer had a correlation significantly with PPG on T2DM with COVID-19 patients.

EARLY CONSUMPTION OF HIBISCUS SABDARIFA ATTENUATES THE SEVERITY OF TYPE 2 DIABETES IN A WISTAR RAT MODEL

Diabetes constitute a serious challenge for many health system and families due to the cost of its care. Alternatively, traditional plants offer a huge potential for health care. Thus, many plants have been used in form beverage including the use of Hibiscus sabdarifa (HS) as tea. Recent data suggested the beneficial effect of HS cardiometabolic diseases models. In this study, we evaluated the preventive and curative effects of crude extract of HS in a type 2 diabetes rat model. Through in vitro complexion and/ or precipitation reactions, we qualitatively assessed the phytochemical composition of the crude extract of HS for different groups of secondary metabolites. The antiradical scavenging activity was assessed through hydroxyl radical test. Type 2 diabetes was induced by high fat diet (HFD) and single dose streptozotocine (STZ) injection. Body weight change and blood biochemical analysis were carried out. Data were statistically analyzed. HS contains different phytochemical polyphenolic compounds such as tanins and flavonoids and presented an interesting antiradical scavenging activity. Early intake from experimental day (ED1) of crude extract of HS significantly prevented gain in body weight (P < 0.05), reduced T2D induced elevated glucose (P < 0.01) and lipids (P < 0.01) and has better outcome as compared to late intake (from ED14). These findings confirm and supports the use of HS as tea and may offer protective effect to consumers by regulating blood sugar and lipid profile.

A cross-sectional study on the association of single nucleotide polymorphism of leptin receptor (Gln223Arg) and insulin resistance in gestational diabetes mellitus

BackgroundThe role of genetics in pathophysiology of gestational diabetes mellitus (GDM) is least explored. Objective of the study is to find the association of leptin receptor (LEPR) gene polymorphism with GDM and its role in altered leptin levels, insulin resistance, and dyslipidemia in GDM. MethodsHundred GDM patients and an equal number of BMI and gestational age matched normal glucose tolerant pregnant women were recruited as cases and controls. Five milliliters of venous blood was drawn for biochemical and genetic analysis. Genotyping of LEPR (Gln223Arg) was performed by PCR-RFLP. Fasting blood sugar, leptin, insulin, C-peptide, and lipid profile estimations were done. Various insulin resistance models were constructed using suitable formulae. The statistical analysis was performed using SPSS 23.0 software. ResultsThere was no significant association found between LEPR gene polymorphism with leptin levels and insulin resistance in GDM. However, Odd’s ratio showed that individuals with the “A” allele were at 1.25 times higher risk of developing GDM. HOMA B-cell significantly varied among LEPR genotypes (p < 0.0001), values being double in AA genotype, compared to AG (p < 0.05), ten times higher in AA compared to GG (p < 0.0001). There was no significant deviation of the genotype frequency distributions for rs1137101 variants from HWE in GDM cases, suggesting that alleles were in equilibrium. ConclusionThe study established a cycle of gene polymorphism altering leptin levels which in turn can alter insulin secretion and insulin resistance, contributing to dyslipidemia of pregnancy as well as gestational diabetes.

Retinal artery occlusion as a probable idiosyncratic reaction to topical minoxidil: a case report

BackgroundMinoxidil hair formulation is commonly used for the treatment of male or female androgenic alopecia. This over-the-counter product is considered safe, but should be used with caution. Ocular side effects following topical minoxidil use are rarely reported. In this paper, we report a rare case of inferior hemiretinal artery occlusion possibly caused by topical 5% minoxidil treatment.Case descriptionA 21-year-old Asian Indian male presented to the retina clinic with sudden onset blurring of vision and superior visual field loss in the right eye since morning. He was diagnosed with androgenic alopecia and was on treatment with topical 5% minoxidil spray twice a day for the last 3 years. On examination, his corrected distance visual acuity was 6/6, N6 in both eyes. Anterior segment examination and intraocular pressure in both eyes and left eye fundus were within normal limits. Right eye fundus examination showed features suggestive of inferior hemiretinal artery occlusion, which were confirmed on fluorescein angiography and optical coherence tomography. A detailed systemic evaluation and investigations (blood pressure, random blood sugar, hematological and coagulation profile, serum homocysteine level, Mantoux test, chest x-ray, cardiac two-dimensional echography, thyroid function test, and immunological profile) did not detect any abnormalities. The ocular condition and its visual prognosis were explained to the patient, and he was asked to review after 4 weeks.ConclusionThough there is no definite cause–outcome relationship between topical minoxidil use and retinal artery occlusion development, this possibility should be kept in mind when observing retinal vascular occlusion cases with concurrent use of topical minoxidil.

Effect of Consumption of Whole-Wheat Breads on FBS, HbA1c, and Blood Lipids in Patients with Type 2 Diabetes.

Whole-wheat can have positive effect on blood parameters, such as blood sugar and fat profiles, in patients with diabetic. The aim of this study was to assess the effect of whole-wheat breads on hemoglobin A1c (HbA1c) and blood lipids in patients with type 2 diabetes (T2D). The study population included men and women with T2D aged 30 years old and over in Arak, Iran. Using random sampling method, 60 patients with T2D were selected and randomly divided into control and intervention groups. After taking blood samples from the patients, the intervention group was administered 180 g of wholegrain wheat for 12 weeks. The participants were asked not to change their diets and amounts of physical activity during the study period. After 12 weeks, blood samples were taken and insulin resistance indexes, blood glucose levels, and lipid profiles were examined. The results indicated that consumption of whole-wheat bread for 12 weeks can significantly reduce body weight, and HbA1c, triglyceride, total cholesterol, and low density lipoprotein cholesterol (LDL-c) levels, and increase high density lipoprotein cholesterol (HDL-c) levels (P<0.05). However, in the control group, changes in body weight, body mass index, HbA1c, total cholesterol, HDL-c, and LDL-c levels did not significantly differ before and after the intervention period (P>0.05). As consumption of whole-wheat bread has positive effects on control of various physical and biochemical indictors in patients with diabetes, its consumption should be included in educational programs at health centers across the country.

Screening for Comorbid Cardiovascular Risk Factors in Pediatric Psoriasis Among Iraqi Patients: A Case-Control Study.

Background: Psoriasis is a polygenic multifactorial immune-mediated skin disease associated with comorbidities. As one-third of adult psoriasis starts during childhood, early detection of these comorbidities might help to mitigate their impact on future health.Objectives: To investigate the risk for cardiovascular events and their relationship with psoriasis severity among Iraqi children and adolescents.Patients and methods: A prospective, case-control, cross-sectional study on 150 patients with psoriasis and 150 age and sex-matched individuals. The study was carried out at the Department of Dermatology/Basra Teaching Hospital from December 2018 to December 2020. Psoriasis severity was assessed by PASI (psoriasis area severity index) score, and in both groups, blood pressure and body mass index (BMI) were measured. Laboratory tests including fasting blood sugar (FBS) and lipid profile were also done.Results: More patients were overweight and obese in the psoriatic group compared to the control group (26.7% and 40% versus 11% and 8%), 5.3% of psoriatic patients who had stage 2 hypertension (defined as any blood pressure [BP] measurement higher than 99th plus 5 mm of mercury applied to BP levels for boys and girls by age and height percentile charts), none of the control group was hypertensive, and the difference was statistically significant (p-value<0.05). A significantly higher proportion of the psoriatic patients had abnormal lipid profiles compared with the control group, 62% versus 30% (p<0.05), 15.3% versus 6.7% had elevated cholesterol (p<0.05), 24.7% versus 8% had raised low-density lipoprotein (LDL, p<0.05), 18% versus 8.6% had low high-density lipoprotein (HDL, p<0.05), and 12.6% versus 6% had elevated very-low-density lipoprotein (VLDL) and triglyceride (TG, p<0.05), 8% patients had elevated FBS (more than 100 mg per deciliter) versus 2.6% (p<0.05), and metabolic syndrome in 65 versus 2% (p<0.05). These changes were related to the severity of psoriasis.Conclusions: Pediatric psoriatic patients in our population may have an atherogenic lipid profile with an increased prevalence of risk factors for cardiovascular diseases, especially those with moderate to severe psoriasis.

Lipid Accumulation Product as an Index for Visceral Obesity and Cardiovascular Risk among a Sample of Obese Egyptian Women

Background:Lipid accumulation product (LAP) is one of the indices that can demonstrate cardiovascularrisk factors according to international studies. Aim: To evaluate relationship between LAP, visceral obesity and different cardio vascular risk factors among sample of obese Egyptian Women. Methods: A retrospective cross-sectional observational study included 350 obese Egyptian women; aged 25- 55 years. They were subjected to blood pressure and anthropometric assessment (weight, height and waist circumference), abdominal ultrasound (visceral fat) and laboratory tests (fasting blood sugar and lipid profile).Results: The majority of the participated women had wide WC (88.9%) and increased visceral fat (80.6%). Hyperglycemia was present among (47.4%), hypertension among 42.6%, high triglycerides among 26.6% and low HDL among 44.3%. The odds of obtaining LAP &gt;80.74 cm.mmol/L (4th quartile) is 2.7 times higher in individuals with hypertension(BP &gt; 130/85),and 6.79 times higher in individuals with visceral fat ≥ 5 cm. Depending on the visceral fat as a standard for classification of visceral obesity (&gt;5 cm for women), revealed that the AUCs of LAP was 0.752 (95% CI 0.697–0.806) in women. The cutoff point of LAP;for prediction of visceral obesity among Egyptian women, was 61.69; using ROC analysis; with sensitivity 68% and specificity 75% and PPV/NPV 1.05 and accuracy 71.5% .Conclusion: LAP index had significant correlations with visceral obesity and hypertension. The cutoff point of LAP 61.69 can be used to predict visceral obesity among Egyptian women.

The Prevalence of Metabolic Syndrome of Patients on Treatment with Haloperidol and Risperidone or Olanzapine

Background: The background of the work stems from the observation due to the prevalence of antipsychotic drugs in causing metabolic disorder. The metabolic disorder is one of the major concern of antipsychotic drugs for schizophrenic patients. The present study is mainly aimed to establish the role of haloperidol and risperidone or olanzapine in causing metabolic syndrome of schizophrenic patients.&#x0D; Methods: Sixty-four schizophrenic patients were divided into two groups. First and second group received haloperidol and risperidone or olanzapine respectively for three months. Body mass index, random blood sugar and lipid profile were investigated before and after the treatment.&#x0D; Results: Both groups of schizophrenic patients have shown a significant increase in the BMI, random blood sugar and lipid profile indicating a propensity to cause metabolic disorder.&#x0D; Conclusion: From the present study it is concluded that there is a high prevalence of metabolic disorder those who received antipsychotic drugs like haloperidol and risperidone or olanzapine.

Electrophysiological estimation of subclinical neuropathy and its types in diabetes mellitus patients

Diabetes mellitus is the leading cause for peripheral neuropathy in India. It is a devastating complication which leads to amputation of limbs affecting the physical and social quality of life. Subclinical peripheral neuropathy is one which doesn’t have any symptoms or signs, but positive electrophysiological findings.: 1): To estimate the prevalence of subclinical peripheral neuropathy and its types among the diabetes mellitus patients. 2): To determine the various risk factors influencing them. 51 patients with diabetes mellitus without symptoms and signs of peripheral neuropathy were selected based on inclusion criteria and after assessing the screening scores. Necessary blood investigations was done and nerve conduction test was carried out in both upper and lower limbs for motor and sensory study.22(43%) out of the 51 diabetic patients had subclinical peripheral neuropathy. Lower limb(29.5%) predmominates than upper limb, sensory(23.5%) more common than motor neuropathy. Axonal neuropathy was the predominant (29%). Statistical significance was observed between the duration of diabetes mellitus, high triglyceride levels, high blood sugar profile and high HbA1C values with the presence of subclinical peripheral neuropathy.Nerve conduction study is the best for diagnosing subclinical neuropathy in diabetes mellitus patients. By early diagnosis and control of diabetes we can prevent dreadful complications.

Evaluation of uric acid disorders and associated factors in essential hypertensive patients at Wolkite University specialized hospital, Southern Ethiopia.

BackgroundEssential hypertension is a condition characterized by a rise in blood pressure of undetermined cause, includes 90% of all hypertensive cases and is a highly important public health challenge with major modifiable cause of morbidity and mortality. Uric acids disorders in particular hyperuricemia are significant problems in essential hypertensive patients and can cause substantial morbidity and mortality. Determination of uric acid disorders may play a major role in the management and early aversion of complications in hypertensive patient. Therefore, this study aimed to determine uric acid disorders and associated factors among essential hypertensive adults in the outpatient department at Wolkite University specialized Hospital, Southern Ethiopia from November 1 to February 30, 2021.Methods and materialsAn institional based cross sectional study was conducted on 270 essential hypertensive adults on follow-up in outpatient department from November 1 to February 30, 2021. Structured questionnaires through face to face interviews and participants’ medical records were used to collect information on determinants related with uric acid disorders. The blood specimen was collected and level of serum uric acid, blood sugar and lipid profile was measured using standard principles and procedures with an ABX Pentra 400 automated chemistry analyzer. Bivariate and multivariate logistic regression analyses were done to identify factors associated with hyperuricemia. The p-value was set at <0.05 with a 95% confidence interval of the adjusted odds ratio.ResultsA total of 270 adult essential hypertensive patients were participated in the study, among those 196(27.4%) of study participants were hyperuricemic with 95%CI (21.9, 33.3). Being alcoholic [(AOR: 15.68, 95% CI: (5.93, 21.41)], taking antihypertensive medication [(AOR: 11.56, 95% CI: (3.94, 23.80)], BMI > = 30 [(AOR: 4.89, 95% CI: (1.46, 25.5)] and being centrally obese [(AOR: 6.87, 95% CI: (2.53, 18.63)] were factors significantly associated with hyperuricemia.ConclusionIn this study, the high burden of hyperuricemia (27.4%) was observed in essential hypertensive patients with follow-up in outpatient department. Taking alcohol and antihypertensive medication, being overweight and centrally obese were identified factors of uric acid disorders. The finding of this study should be taken into consideration to implement preventive interventions on identified predictors in hypertensive patients. Taking fruit and vegetable, and promoting physical exercise and determinations of serum uric acid level in adult essential hypertensive patients was recommended to minimize the emergence of hyperuricemia.

Cardiovascular Diseases Risk Prediction Using the Framingham Risk Score

Introduction: Cardiovascular diseases (CVD) are the world’s most common cause of death. The Framingham risk score (FRS) is the most commonly used method for CVD risk assessment. Low-risk individuals have 10% or less coronary heart diseases (CHD) risk at 10 years, 10-20% have intermediate risk and 20% or more have high risk. Aim of Work: To estimate the risk of developing cardiovascular diseases over next ten years among the administrativestaff at Zagazig University, Egypt. Material and Methods: A structured questionnaire was used to get data about socio demographic characteristics, anthropometric measurements was done, laboratory investigations to measurefasting and post brandial blood sugar and lipid profile. The Framingham risk score (FRS) was calculated by using information on age, gender, smoking ,diabetes mellitus (DM), systolic blood pressure , treatment for hypertension, and total blood cholesterol and high-density lipoprotein levels for everysubject. Results: About 55% of the studied sample were males and 44.8%were females, the overall prevalence of systolic hypertension was 45.7% ,36.2% had a positive family history of CVD , 25% were physically active( the majority of them were males),21.5% were diabetic, 63.7% had highblood cholesterol,55.1% had central obesity,68.1% were overweight orobese. FRS mean for males was higher as compared to females (14.3±7.1and11.9±5.8 respectively). Participants with high CVR risk were mainlyinactive, smokers, with positive family history, diabetics, hypertensive,with high blood cholesterol level, high density lipoprotein and with centralobesity .A statistically significant positive correlation was observed betweenFRS and age, cholesterol, systolic blood pressure, blood glucose ,weight ,waist circumference and body mass index(BMI) Conclusion: About 42%of our studied subjects were at low anticipated 10-year CVD risk, 30% were at moderate risk, and 28% of them were at high risk .The most significantrisk factors were male gender ,smoking, physical inactivity , hypertension,diabetes mellitus, obesity, abdominal obesity , high levels of total cholesteroland low HDL-cholesterol level in addition to positive family history . CVDis a preventable public health issue; most of its risk factors are modifiable.

Sociodemographic correlates of abnormal blood profile in tribal districts of Eastern India.

Objective:To assess the correlates of sociodemographic profile with abnormal blood profile in tribal districts of Odisha.Design:This was a cross-sectional study. The abnormal blood profile cut-offs were: Random blood sugar >200 mg/dl, Triglycerides >150 mg/dl, and Cholesterol >200 mg/dl.Setting:The study was carried out in Daringbadi and Tangi (Choudwar) blocks from Kandhamal and Cuttack district, respectively, in the eastern state of Odisha in India.Participants:Data of 2,000 households selected consecutively from 30 villages was collected for persons aged more than 6 years from either block on sociodemographic aspects and food consumption by households. Anthropometric measurements were taken for persons aged 11 years and above. Approximately 10%, that is, 400 households were chosen for blood sample collection for estimation of random blood sugar (RBS) and lipid profile [Serum Triglycerides (TG) and cholesterol] in persons aged 11 years and above.Results:The blood reports revealed that out of the total samples collected, 9.2% had elevated RBS, 20.8% had elevated TG, and 8.8% had elevated cholesterol levels overall. The prevalence of elevated RBS, TG, and Cholesterol was 2.3%, 15%, and 5.3%, respectively, in Daringbadi, while in Tangi-Choudwar the elevated markers were 17%, 27.2%, and 12.8% for RBS, TG, and Cholesterol, respectively. Our study found that compared to the Daringbadi block, a significantly higher (P < 0.05) proportion of people from the Tangi-Choudwar block suffered from hyperglycemia (OR = 0.11; 95% CI: 0.06, 0.20), hypertriglyceridemia (OR = 0.47; 95% CI: 0.34, 0.64), and hypercholesterolemia (OR = 0.38; 95% CI: 0.24, 0.59). Hence, it was observed that the population of Daringbadi was 89%, 53%, and 62% less likely to have elevated random blood sugar level, serum triglycerides, and serum cholesterol, respectively, than the population of Tangi-Choudwar block. Overall, the population of Tangi-Choudwar was found to be more predisposed to an abnormal blood profile which might be indicate a less healthy lifestyle and diet in this block as compared to Daringbadi block.Conclusions:This study found that the sociodemographic factors influencing lifestyle and diet patterns of a population have an impact on the health of a population. We found that the tribal block which was more developed and had more intake of junk food and sedentary lifestyle similar to urban areas also had a greater proportion of population with an abnormal blood profile as compared to the less-developed tribal block. Hence, health promotion for a healthy lifestyle and diet is needed as a part of national health policy to implement primordial prevention and to prevent the emergence of risk factors from an early age. The frontline health workers and family physicians can play an important role in promoting a healthy lifestyle.

The interplay between fasting, gut microbiota, and lipid profile.

This study aimed to investigate the consequence of Ramadan fasting on gut bacterium (Bacteroides and Firmicutes), serum concentration for butyrate, and lipid profile. Thirty healthy subjects were enlisted and investigated two times (before and at the end of Ramadan). Fasting blood samples were obtained for measuring fasting blood sugar (FBS) and lipid profile and serum butyrate concentration. Anthropometrics variables were measured before and after Ramadan for all 30 subjects. Quantitative reverse transcription polymerase chain reaction (RT-PCR) analysis, targeting the genome of Bacteroides and Firmicutes was performed to determine its presence in the stool samples. Food intake was assessed by a 3-day food record before and after Ramadan. Statistical analysis was performed by SPSS ver.13 and Minitab ver.17. P < 0.05 considered the level of significance. The study results showed that serum levels of butyrate significantly increase during the month from 0.23 ± 0.02 mM to 0.46 ± 0.03 mM (P < 0.05). The gut Bacteroides and Firmicutes increased by 21 and 13 percent after Ramadan compared to before (P < 0.05). The increment in Bacteroides occurred in both sexes, but Firmicutes significantly increased only in women. Food intake was decreased during Ramadan. Ramadan fasting caused significant reduction in BMI from 25.72 ± 0.58 kg/m2to 25.25 ± 0.55 kg/m2 (P < 0.05). Serum levels of LDL, HDL, LDL/HDL ratio, and total cholesterol significantly decreased during Ramadan (P < 0.05). However, the decrease in FBS and TG level were not statistically significant (P > 0.05). It can be stated that the promotion of Bacteroides and Firmicutes in the gut might play a crucial role in health promotion. However, more research is needed to achieve a definite conclusion.

The relationship between maternal serum Interleukin-6 and CRP levels at first trimester of pregnancy and gestational diabetes occurrence

Introduction: Diabetes is the most common medical complication of pregnancy. Identifying the new risk factor can help to early diagnosis of the disorder. CRP and Interleukin-6 are two inflammatory markers which can anticipate some pregnancy adverse complications. This study was performed with aim to assess the relationships between maternal serum CRP and Interleukin-6 (IL-6) at first trimester of pregnancy and gestational diabetes Mellitus (GDM) occurrence. Methods: This descriptive-analytical cross-sectional study was performed in 2015-2016 on 500 pregnant women with gestational age < 14 weeks in Nilou laborator, Tehran. Serum levels of CRP, IL-6, fasting blood sugar (FBS) and lipid profile were measured. GDM screening was performed in 24-28 weeks of gestation. Data were analyzed by SPSS statistical software (version 21) and Kolmogorov-Smirnov, Chi-square, Mann-Whitney tests and Logistic regression model. P<0.05 was considered statistically significant. Results: GDM was found in 78 women (15.6%). Median CRP levels in GDM and non-GDM groups (4.8 vs. 5.22 mg/ml) (P=0.089) and median IL-6 (1.45 vs. 1.5 pg/ml) (P=0.574) was different, but the difference was not statistically significant. After adjusting the confounding variables in the logistic regression model, there were no statistically significant relations between serum level of CRP (OR=1.01, CI95%=0.96-1.07) (P=0.50) and IL-6 (OR=1.01, CI95%=0.89-1.46) (P=0.88) with GDM occurrence. Conclusion: CRP and IL-6 levels are not significantly associated with the occurrence of GDM.
