Наукове обґрунтування необхідності використання психологічної платформи допоміжних репродуктивних технологій

The objective: to present scientific justification of need of use of psychological platform when carrying out various programs the auxiliary reproductive technologies. Materials and methods. 227 women with sterility by which auxiliary reproductive technologies were shown were surveyed. To all of them psychopathologic examination by criteria of the international classification of illnesses of the 10th revision is conducted. It was thus taped that 100 women suffered boundary alienations – the main group. The control group was made by 50 fertilny mentally healthy women. To all patients full clinical-laboratory examination according to the scheme accepted in clinics of reproductive was conducted. For clarification of the reason of emergence of boundary alienations the algorithm of inspection included clinical-psychopathologic interviewing and the standard psychological tests. Results. Development of boundary alienations is promoted by combination of several factors, such as not adaptive behavior which promoted intensifying of an internal strain, dysfunction of vegetative nervous system that was shown in vegetative disturbances. Long internally the strain led to development of high level of alarm, emission corticosteriodes and further to an immunoscarce state that promoted an exacerbation of chronic somatopathies. A srategiya «flight in illness» led the koping to development of the psychosomatic diseases which main goal there was treatment avoidance. Conclusion. The above is convincing scientific justification allocation of psychological platform of auxiliary reproductive technologies – as an independent scientific problem on which quality of the decision efficiency of treatment of sterility depends. Key seismic survey technologies for deep complex geological structures: A case study of the northern section of the Longmenshan Fault Fold Belt in the Sichuan Basin

The northern Sichuan Basin, spreading in the northern section of the Longmenshan Fault Fold Belt, is characterized by dramatic fluctuations in the surface landforms, development of abdominal faults, and low-quality seismic data, resulting in difficulties in clarifying relevant structures. The key target formation, the Mid-Permian Qixia Fm, is deeply buried with thin reservoirs and high heterogeneity, which brings great challenges to seismic prediction. Under such circumstances, researches have been conducted jointly in terms of seismic data acquisition, processing and interpretation, and finally some relevant seismic survey technologies were developed suitable for surface/underground complex structures. Through surface structural surveys, dynamic deep-well lithologic identification, single-point detector deployment and process optimization, acquisition parameters can be excited. In addition, by using an observation system with high-coverage, wide-azimuth and huge-displacement, quality of acquired seismic data can be enhanced dramatically. Seismic imaging technologies for complex structures have been developed to enhance the quality of images for deep formations. These technologies are dominated by microscopic logging-constrained tomography static correction, high-resolution processing with fidelity and amplitude preservation and all-around PSDM in an angular domain. By using high-resolution gravity, magnetic and electric data, details related to geological structures and faults can be identified. In combination with fine seismic data interpretation, structural details and fault features can be verified effectively. Based on forward modeling and fine seismic calibration of reservoirs in individual wells, suitable attributes can be identified for predictions related to the distribution of reservoirs. By using all these auxiliary technologies, a large-scale structural–lithologic composite trap with a total area of 1223 km2 has been discovered in the NW Sichuan Basin. The Shuangyushi–Jiangyou area as a whole distributes on structural highs. In the areas to the south of Shuangyushi, the Qixia Fm dolomite reservoirs of platform margins are continuously developed. In conclusion, these auxiliary technologies can effectively allow trap identification and thin reservoir prediction in complex structures in the study area. In addition to clarifying the exploration orientation and providing a necessarily technical supports for well development, these technologies help to accelerate the construction of demonstration projects for the exploration and development of deep marine carbonate formations.

Патологія ендометрія у жінок з нереалізованою репродуктивною функцією: діагностика і тактика лікування безплідності

The objective: rising of efficiency of diagnostics and sterility treatment at women with pathological changes an endometrium on the basis of improvement and introduction of algorithm of diagnostic and treatment-and-prophylactic actions. Materials and methods. Сomplex clinical-tool, laboratory and morphological examination of 120 patients with unrealized genesial function and various pathology an endometrium which were divided into four groups was conducted. Results. Use of the advanced differentiated approach to tactics of maintaining women with unrealized genesial function against various options of pathology the endometriya allows to increase for 10.0% efficiency of treatment of sterility (33.3% in comparison with 23.3%). Conclusion. The algorithm improved by us allows to increase efficiency of auxiliary genesial technologies at these women for 10.0% that grants to us the right to recommend it for wide use in practical health care. A combined stimulation technology for coalbed methane wells: Part 1. Theory and technology

As there are numerous coalbed methane (CBM) wells with low or zero gas production after traditional hydraulic fracturing in China, new technologies are needed for both the secondary stimulation of these wells and new wells’ stimulation. Based on the results of laboratory tests and theoretical analysis, this paper, the first of a two-part series, demonstrates the theory of a new combined stimulation technology for CBM wells. Firstly, gas contents in the coal seam, neighbouring shale and sandstone were measured and calculated. Then, the geomechanic, mineral composition and permeability of the tight sandstone were tested. Based on these tests and calculations, a combined stimulation technology is demonstrated. This new technology is to expand gas production from coal seams to coal measures; to expand the stimulation object from unitary coal seams to coal measures; and to change the stimulation method from the conventional hydraulic fracturing to fracture network stimulation. Results show that: (1) in addition to CBM, significant amounts of shale gas and tight sandstone gas can serve as complements of CBM; (2) the highest fracability formation is tight sandstone, followed by mudstone and coal. It is concluded that this combined stimulation technology can be demonstrated as conducting combined fracture network stimulation for coal measure gas reservoirs through multilayer and multi-cluster perforation, quadruplex hydraulic fracturing and other auxiliary technologies, e.g., ball-injection temporary plugging and limited entry fracturing. In part 2, the application of this combined stimulation technology to a CBM well in the Sunan Syncline is described.

UTERINE LEIOMYOMA AND PREGNANCY

Introduction. The uterine leiomyoma is a disease that is most often found among gynecological diseases, ranging from 30 to 50% in various sources, and leads to infertility. In 1-5% of women is the only cause of infertility, and with increasing age of patients is steadily increasing. As pregnancy occurs within thirty years, the problem becomes even more important.Aim. Identify current views on the effects of leiomyoma on reproductive function, and the effectiveness of infertility treatment through modern reproductive technologies.Research results. One of the methods of treatment of infertility is the use of auxiliary reproductive technologies, which, in the presence of organic pathology of the uterus, requires special treatment approaches. Therapeutic options for long-term therapy of uterine leiomyomas are limited, so the surgical method remains the main method of treatment, but this method does not exclude further tumor recurrence. Usually, hysterectomy is performed 6-8 times more than myomectomy and 15-20 times more than selective embolization of the uterine arteries, therefore, hysterectomy continues to be the main method of treatment that negatively affects the physical and psycho-emotional state, prevents pregnancy and is unacceptable for women planning pregnancy.Conclusions. 1. The presence of fibromatous nodes promotes the development of infertility and worsens the prediction of treatment with reproductive technologies.2. In preparation for the treatment of infertility with the methods of reproductive technologies, myomectomy is a standard treatment, after which it is possible, even a spontaneous pregnancy outbreak. However, data on the effectiveness of reproductive technologies in patients with myometectomy in history are controversial.3. In the absence of classical indications for surgical intervention, the surgical strategy for leiomyomas should be individual.

Комплексний підхід до лікування раннього токсикозу у вагітних після застосування допоміжних репродуктивних технологій

Obstetricians-gynecologists do not always pay sufficient attention to nausea and vomiting of pregnancy (NVP), sometimes its severity is underestimated and adequate treatment is not prescribed. The objective: to investigate the efficacy and safety of the complex therapy of NVP in pregnant women after ART, including a preparation of ginger extract with pyridoxine and psychotherapy. Materials and methods. Sixty pregnant women were examined after applying VRT, with mild manifestations of NVP. The main group consisted of 30 women, with Emegest (ginger and pyridoxine extract) and psychotherapy used in NVP therapy, and a comparison group of 30 women who only adhered to recommendations on changes in diet and life. The presence of the syndrome of autonomic dysfunction was established using the A.M.Veyn questionnaire. For self-assessment of the level of reactive and personal anxiety, testing was carried out according to the method of Ch.D. Spilberger, Yu.L. Khanin. The quality of life was assessed using the SF-36 questionnaire. Results. The use of recommended NVP therapy in pregnant women after ART makes it possible to improve the condition of women already for 2–3 days, and in 2 weeks of treatment it is essential to reduce or even eliminate the symptoms of NVP. Complex therapy reduces the severity of the syndrome of autonomic dysfunction and the level of personal and situational anxiety, which, against the background of improving the overall condition, positively affects the quality of life. The applied therapy had no side effects or complications, positively influenced the further course of pregnancy and the condition of the fetus and the newborn, in particular, the frequency of preeclampsia is decreased by half and the birth asphyxia by 1.5. Conclusion. The safety and efficacy of Emegest in the complex therapy of nausea and vomiting of pregnancy allows us to recommend its use in pregnant women after ART. Effectiveness of programs of auxiliary reproductive technologies in patients of late reproductive age

The aim of the study was to study the effectiveness of assisted reproductive technologies (ART) programs in patients of different age groups, taking into account clinical and laboratory data. Materials and methods. A prospective cohort study included 188 patients with infertility of various genesis who were stratified according to age: group 1 (n=87) - patients of late reproductive age - LRA (>35 years), group 2 (n=101) - patients of early reproductive age (≤35 years). Results. In patients with ART compared with patients with LRA, the chances of pregnancy were reduced by 2.2 times (odds ratio - OR 2.2, 95% confidence interval - CI 1.1-4.3), the chances of live birth were 2 times (OR 2.0; 95% CI 1.0-3.9). The only factor affecting the onset of pregnancy in addition to age was the number of embryos received. The threshold age at which and above which the chances of pregnancy and live birth decreased as much as possible were the age of 37 years: the OR of pregnancy 2.6 (95% CI 1.4-5.1, AUC 61.3%), OR live birth 2.6 (95% CI 1.3-5.1, AUC 60.8%). Conclusions. In patients of LRA there is a twofold decrease in the effectiveness of ART programs due to a significant decrease in the number of oocytes obtained, mature oocytes, and, accordingly, embryos.

НАПРЯМКИ ВДОСКОНАЛЕННЯ ПРАВОВОГО РЕГУЛЮВАННЯ СУРОГАТНОГО МАТЕРИНСТВА В УКРАЇНІ

The article is devoted complex research of problem of defence of rights for children, bear as a result of application of auxiliary genesial technology – substitute maternity. In research the basic conceptual going is described near determination of origin of bear by a substitute a mother child, the problem of absence of the legislative adjusting of legal status of embryo of man is set, conceived in-vitro and the ways of its decision are offered. Attention is accented on the ambiguousness of the legal adjusting of the phenomenon of substitute maternity in the world. In research the possible risks of unhonesty of medical establishments which give services in auxiliary genesial technologies, and consequences of errors of doctors-reproduktologists, are analysed as subsequent limitation of rights for a child by a foreign country on the example of case of «Paradizo and Kampanelli v. Italy». Pointlessness of the legislative fixing of term, during which a substitute mother must give a consent to registration of persons the parents of child, is set, taking into account possible manipulations from the side of substitute mother and potential loss by the new-born child of parents in the case of death of substitute mother to signing of the proper consent. In research the role of the Ukrainian national courts is analysed in establishment of legal fact of domestic relations between parents and new-born child for confession of state power of birth certificates, given out the Ukrainian organs of state civil registration foreign organs. Concentrated attention on absence of permanent practice of national courts in relation to determination of type of judicial realization as in certain cases a court specifies on the substitution of concepts «establishment of legal fact of domestic relations» and «confession of paternity declarants». As a result of research a conclusion is done about the necessity of development of international convention, which will fasten the standards of defence of rights for children, bear as a result of application of auxiliary genesial technologies, and also outlined tasks which appear before a national legislator and Ukrainian courts, to provide the high-quality legal adjusting of substitute maternity, taking into account rights for a child above all things.

Influence of DNA fragmentation in the sperm on the development of embryos in vitro fertilization program

The effect of the level of DNA fragmentation in the sperm on the development of early embryos in IVF programs for men with low fertility was investigated. A significant negative association between the level of DNA fragmentation in the sperm and the total blastocyst formation rates and a percent of blastocysts with high quality morphology in patients with impaired fertility is proved (rs = -0,59; rs= -0,56). Evaluation of semen quality in men with low fertility is a necessary step before the use of assisted reproductive technologies. The connection between the levels of sperm DNA fragmentation and the peculiarities of the early development of embryos obtained in programs of extracorporal fertilization has been proved. Assessing the quality of sperm in men with reduced fertility is a necessary measure before using auxiliary reproductive technologies. According to literature, the morphology of the blastocyst does not always depend on chromosomal changes in the embryos. Consequently, it is promising to carry out pre-implantation genetic diagnostics of embryos obtained in programs of extracorporal fertilization in patients with high levels of DNA fragmentation in spermatozoa to study the effect of this violation on the karyotype of embryos.

Reengineering of the management processes of information support of heat supplying organizations of heating systems of a city

Changes in management of the heat-supplying organizations of the modern city determined by development of technologies in this sphere are considered in the article. Three groups of technologies are allocated, the priority of the auxiliary technologies considered is given. The author points out that there is the need of the information support of management of the heating systems management in the city, which need fundamental reconsideration and radical redesign. The additional argument of the need of reengineering of the heat supplying organizations management is the consideration of the results of their activity, connected with considerable thermal losses. The key provisions of the offered measures for reengineering, based on the creation of processes of information support of management "from scratch" are described. Negative changes of the performance of the heating systems of concrete types were estimated q, and thanks to that the new integrated approach to the choice of methods and technical means of diagnostics of engineering systems and analytical dependences of determination of terms of repair were offered. Obtaining of qualitatively new information through reengineering measures for making managerial decisions is considered. Scenarios of prospects of energy industry, which have been differentiated as the most realistic or connected with the risk of increase in the stubborn problems, minimized by reengineering, were analyzed.

Outcome of art programs for patients with habitable inequality of pregnancy in anamnesis

The aim of the study was to assess the effectiveness of auxiliary reproductive technologies (ART) programs in patients with a habitual miscarriage (HM) in an anamnesis. Materials and methods. A prospective cohort study included 200 patients with infertility, 100 of whom had a history of HM (2 or more pregnancy loss before 22 weeks of gestation) - group 1, and 100 patients had tubular peritoneal infertility without loss of pregnancy in the history (group 2 ). Results. Patients with ART programs with a history of HM are characterized by a higher age and, as a consequence, a lower level of AMH, a greater BMI, a higher level of androgens, and poorer embryological indicators (fewer mature oocytes, more oocytes with dysmorphisms, and fewer blastocysts of excellent quality). The frequency of biochemical and clinical pregnancy did not differ in comparison groups. The frequency of spontaneous abortions was observed in 12% in group 1 and in 3% in group 2 (p=0.0156). The incidence of live birth was 17% in group 1 and 24% in group 2 (p=0.2201). Conclusions. The effectiveness of ART programs is lower in patients with habitual miscarriage: the odds of live birth are 1.4 times lower due to the obtaining of a smaller number of blastocysts of excellent quality, and the chances of spontaneous miscarriage are 4.4 times higher than in patients without loss of pregnancy in the anamnesis.

The article presents modern literature data on the stimulation of ovulation in programs of assisted reproductive technologies in women over 40, the advantages and disadvantages of conventional ovarian induction schemes, the clinical and economic effectiveness of the use of coryphollitropine-a for folliculogenesis.

Оптимізація підходів до розродження жінок, які завагітніли після застосування лікувальних програм допоміжних репродуктивних технологій

The objective: to analyze the approaches to delivery of women with pregnancy after the application of curative programs of assisted reproductive technologies (ART). Patients and methods. In the process of the study, the approaches to the choice of the method of delivery of women after the application of ART were studied and analyzed. To achieve this goal, women were divided into two groups: the 1-st group – women after ART (2005-2009), whose delivery was performed in a conventional way, their number was 132 women; the 2-nd group – women after ART (2010-2014), whose delivery was performed according to the developed algorithm, their number was 162 women giving birth. Results. The comparative characteristics of the two groups established that the delivery of women after the use of assisted reproductive technologies (ART), which gave birth through the natural birth canal, in 70% ended with physiological labor. This trend indicates that the delivery of women after ART through the natural birth canal is one of the concepts for a significant reduction in the percentage of cesarean section (CS) in the hospital. Conclusion. To determine the optimal method of delivery in patients after ART, first of all, it is necessary to take into account the etiologic factor of infertility and the features of the course of pregnancy, and not the presence of an ART program in the history, as indications for the planned CS. Профілактика невдалих спроб застосування допоміжних репродуктивних технологій

The objective: depression of frequency of unsuccessful attempts of auxiliary genesial technologies on the basis of studying of new aspects of pathogenesis of female sterility, and also improvement and introduction of algorithm of prognostic and treatment-and-prophylactic actions. Patients and methods. Аt 1 stage (forecasting of results of ART) 97 married couples (1 group) which passed sterility treatment were surveyed. At the 2nd stage (prophylaxis of unsuccessful attempts of ART) 108 patients of genesial age who had in the anamnesis of 2 and more attempts of ART were surveyed. Results. Unsuccessful attempts ART arise at various reasons of sterility: a tubal and peritoneal factor – 48.5%; endocrine disturbances – 44.3%; a man’s factor – 29.9%; an external genital endometriosis of the I–II st – 21,7%, and also a combination of above-mentioned reasons of sterility – 60.8%. At female factors of sterility depression of an ovarialny reserve and unavailability of functional state an endometrium which take place are the main reasons of unsuccessful attempts of auxiliary genesial technologies as is independent, and in a combination. Conclusion. For rising of efficiency of ART it is necessary to use, first of all, effective forecasting taking into account functional, laboratory and morphological methods of researches. Further, it is necessary to develop the individual program of specific preparation taking into account all preliminary results. Прогнозування невдалих спроб застосування допоміжних репродуктивних технологій

The objective: development of algorithm of forecasting of unsuccessful attempts of auxiliary genesial technologies on the basis of the mathematical analysis of the main reasons and risk factors. Patients and methods. 97 married couples which passed sterility treatment on the basis of clinic of auxiliary genesial technologies were surveyed. The complex of the conducted researches included clinical, endocrinologic, immunohistochemical, ehografical and statistical methods. Results. For forecasting of unsuccessful attempts of auxiliary genesial technologies it is necessary to use the constructed mathematical model which includes 4 most significant changes (as significance value depression): optical density of an expression of LIF in superficial and glandular epithelium the endometrium, received in the period of assumed «implantation windows» in cycle before carrying out an extracorporal fertilization, the maintenance of VEGF in cervical slime in day of transvaginal puncture, S/D and IR of spiral arteries in day of introduction of triger of an ovulation. Conclusion. Use of offered algorithm allows to increase forecasting effectiveness of unsuccessful attempts of auxiliary genesial technologies. Оптимізація лікування безплідності у жінок з доброякісною патологією грудних залоз

The objective: rising of efficiency of treatment of sterility at women with pathology of mammary glands on the basis of studying the clinical-ehografical, endocrinologic and hemodynamic features and improvement of algorithm of auxiliary genesial technologies. Patients and methods. Were included in research of 90 women who were on sterility treatment by various techniques including auxiliary genesial technologies. The main methods of research were clinical, endocrinologic, ehografical and immunologic. Results.Сlinical efficiency of treatment of sterility at women with pathology of mammary glands when using the short protocol makes 33.3%, and long – 50.0%. The main complications of pregnancy is its spontaneous discontinuing in 1 trimester – 20.0% and 6.7%, and also premature births – 20.0% and 6.7% respectively. Conclusion. Use of the algorithm improved by us allows to increase efficiency of treatment of sterility, and такеж to provide low level of perinatal pathology. Practical introduction of the received results allows to improve sterility treatment without deterioration of pathology of mammary glands. Flow Capacity Analysis For Mpl Katowice Pyrzowice Terminals With The Indication Of Zones Requiring Architectonic Changes

Abstract In order to improve the quality of passenger service at an airport terminal it is necessary to continuously monitor the level of the services rendered. Devising a strategy for the process of modernization or extension of an airport, with a view to maintaining or elevating the standards offered, requires a skilled determination of the investment commencement time. The rationale behind such investments is a deliberate prognosis of changes in the numbers of passengers visiting the airport in order to avoid exceeding maximum capacity at peak times. The launch of construction works while maintaining continuity of operations at the airport is connected with a conscious utilization of any available spatial reserves within the existing structures. Analyses of flows of users receiving services in various zones of a building, in consideration of the limit values set for the appropriate standards, are some of the tools that facilitate their proper design. Design that includes the statistical trends of changes in the amount of users receiving service requires a responsible and conscious decision making around the shaping of relations between spatial arrangements and auxiliary technologies. The paper presents different methods of working with large quantities of information which the designer must analyze and comprehend. The aim is to make sound investment and design decisions that would precede and prevent the loss of efficiency and performance in relation to the satisfaction of users.
