Early prognostic stratification and identification of irreversibly shocked patients despite primary percutaneous coronary intervention.

Despite prognostic improvements in ST-elevation myocardial infarction (STEMI), patients presenting with cardiogenic shock (CS) have still high mortality. Which are the relevant early prognostic factors despite revascularization in this high-risk population is poorly investigated. We analyzed STEMI patients treated with primary percutaneous coronary intervention (PCI) and enrolled at the University Hospital of Trieste between 2012 and 2018. A decision tree based on data available at first medical contact (FMC) was built to stratify patients for 30-day mortality. Multivariate analysis was used to explore independent factors associated with 30-day mortality. Among 1222 STEMI patients consecutively enrolled, 7.5% presented with CS. CS compared with no-CS patients had worse 30-day mortality (33% vs 3%, P < 0.01). Considering data available at FMC, CS patients with a combination of age ≥76 years, anterior STEMI and an expected ischemia time > 3 h and 21 min were at the highest mortality risk, with a 30-day mortality of 85.7%. In CS, age (OR 1.246; 95% CI 1.045-1,141; P = 0.003), final TIMI flow 2-3 (OR 0.058; 95% CI 0.004-0.785; P = 0.032) and Ischemia Time (OR = 1.269; 95% CI 1.001-1.609; P = 0.049) were independently associated with 30-day mortality. In a contemporary real-world population presenting with CS due to STEMI, age is a relevant negative factor whereas an early and successful PCI is positively correlated with survival. However, a subgroup of elderly patients had severe prognosis despite revascularization. Whether pPCI may have an impact on survival in a very limited number of irreversibly critically ill patients remains uncertain and the identification of irreversibly shocked patients remains nowadays challenging.

658 Impairment of strain reserve extends to remote myocardium in the sub-acute phase of ST-elevation myocardial infarction

Abstract Aims In ST-elevation myocardial infarction (STEMI), subtle tissutal changes in remote myocardium predict long-term left ventricular (LV) remodelling and prognosis, independently of infarct size and microvascular obstruction. Whether there is a subclinical dysfunction of remote myocardium, detectable by longitudinal strain (LS) at echocardiography, and whether it varies in different locations of STEMI and with adenosine (ADO) challenge, is still unknown. Methods and results Fifty-three patients (age 65 ± 12.5 years, 44 male, 20 anterior and 33 non-anterior, P = 0.01) underwent rest/stress echocardiography at 7 ± 2 days after successfully treated STEMI, and at 6-months follow-up. Global LS (GLS), ischaemic and remote LS (iLS and rLS) were analysed in anterior and non-anterior STEMI. Both at rest and at follow-up, GLS was stratified by ejection fraction (EF) into three groups: EF &amp;lt; 40%, 40–49%, and ≥50%. Normal subjects, undergoing ADO stress echo, represented controls. Anterior STEMI showed lower GLS than controls (P &amp;lt; 0.001) and non-anterior STEMI (P &amp;lt; 0.001). ADO increased GLS in controls (P = 0.05), but neither in anterior nor in non-anterior STEMI, GLS changed during ADO stress, although significantly improved at follow-up (P &amp;lt; 0.001 and P = 0.002, respectively). In anterior STEMI, rLS was comparable to iLS at rest, during stress and at follow-up (P = ns), while in non-anterior STEMI rLS was higher than iLS throughout the study (P &amp;lt; 0.001). Patients with EF &amp;lt; 40% and 40–49% had similar values of GLS, iLS, and rLS, which were, both at rest and at follow-up, lower than those of patients with EF ≥ 50% (P &amp;lt; 0.001). Conclusions In the subacute phase, anterior STEMI shows the worst impairment of LS in both ischaemic and remote regions. Strain reserve to ADO is absent in remote myocardium, as well as in ischaemic zone, regardless of MI location. Global, ischaemic and remote LS may improve at follow-up.

546 Antegrade in-stent CTO recanalization

Abstract Methods and results A 64-year-old man with prior PCI and stent of proximal LAD due to an anterior ST-elevation myocardial infarction (STEMI) presented with exertional angina (CCS III), despite optimal medical therapy (OMT). The echocardiogram showed a dilatated left ventricle with anterior and apical akinesia and a severely reduced left ventricle ejection fraction. Coronarography was performed and a chronic total occlusion was found at the proximal edge of the stent previously implanted in the proximal LAD, with a thin tapered entry (J-CTO score 1). Moderate angiographic disease was present in the circumflex (LCX) and in the right coronary artery (RCA). Interventional collaterals were absent. Dobutamine stress echocardiogram was performed to unmask myocardial viability. Indeed, during intravenous Dobutamine administration, we registered an increase in the left ventricle function, whereas only apex remained still akinetic. Accordingly, the patient underwent LAD CTO PCI using a 7 Fr EBU 4.0 guiding catheter, via right femoral artery access. The RCA ostium was engaged with a 6 Fr Judkins right 4.0 guiding catheter, via right radial artery access. Antegrade wire escalation technique was attempted. Due to scarce support, a 7 Fr Guidion guiding catheter extension and a Corsair microcatheter were placed in the proximal LAD. Antegrade crossing was very difficult due to intrastent high plaque burden. The occlusion was crossed with an Asahi Conquest Pro 9 guidewire. Subsequently, an Asahi Gaia third guidewire was advanced through the intrastent segment and then in the distal part of LAD. The advance of microcatheter was challenging but successfully achieved taking advantage of the low profile, high torqueability and trackability of the Asahi Corsair Pro microcatheter. Microcatheter tip injection confirmed the correct position in the vessel’s true lumen. An Asahi Grand Slam guidewire was placed in the distal LAD to provide extra support for delivery of interventional devices. The lesion was pre-dilated with progressively larger balloon, starting from a 1.1 mm diameter semi-compliant over-the-wire balloon (OTW). Two stents were implanted with a minimal overlap at the distal edge of the proximal stent (Resolute Onyx 3.0 × 38 mm and 2.5 × 24 mm). The result was improved with stents high-pressure post-dilatation and with selective intracoronary adenosine and nitroglycerin administration with final Thrombolysis in Myocardial Infarction (TIMI) grade 3 flow. The total amount of contrast media used was 210 ml. The total procedure time was with 125 min with 45 min of fluoroscopy. No complications occurred. Conclusions CTO PCI still represents one of the most challenging subsets of coronary interventions despite the improvement in technology and techniques. Although data regarding percutaneous PCI CTO are still inconsistent, successful CTO recanalization has been associated with relief of angina and ischemia-related dyspnoea (Werner at al., 2018). In stable patients CTO PCI has been associated with a lower risk of death, stroke, and coronary artery bypass grafting and less recurrent angina pectoris in some registry studies (Christakopoulos et al., 2015). Additionally, CTO PCI increased left ventricle function in a subgroup of patients with LAD CTO (Henriques et al., 2016). Conversely, randomized multicentre failed to demonstrate a superiority of CTO PCI medical to OMT in terms of major adverse cardiac events (MACE) and all-cause mortality.

593 Spontaneous coronary artery dissection in young

Abstract Aims We present the case of a 25 years-old patient, presented to the Emergency Department for chest pain and epigastralgia complicated by vomiting. Symptoms started about 30 h before, afterwards an effort. The patient did not have any known cardiovascular risk factors (except for sporadic consumption of cannabis) and family medical history was negative for premature cardiovascular disease or sudden cardiac death. His past medical history was unremarkable. Methods and results Clinical examination was normal. ECG was suggestive for subacute anterior ST-elevation myocardial infarction (QS in V1–V4 leads with ST segment elevation and biphasic T-waves in V4–V6) and was accompanied by significant elevation of myocardial necrosis biomarkers (hsTnT 3416 ng/l, n.v. &amp;lt;15). Echocardiography revealed a severely reduced left ventricular ejection fraction (LVEF = 30%) with regional wall motion abnormalities (apical and septal akinesia, and anterior mid-ventricular segment hypokinesia). Coronary angiography was suggestive for a dissection of the proximal-to-mid left anterior descending artery. Dissection flap, subocclusion of the first septal branch, and intramural thrombosis were noted. Direct stenting with drug-eluting stent was successful, with thrombolysis in myocardial infarction (TIMI) flow grade 3. The patient was started on dual antiplatelet treatment (aspirin and ticagrelor), ACE-inhibitors, beta blockers, and spironolactone. Further testing ruled out other conditions that may be associated with spontaneous coronary artery dissection (autoimmune diseases, including vasculitis, connective tissue disease, coagulative disorders). The clinical course was complicated by pericarditis and the development of LV apical thrombus. So, patient was started on anticoagulation (warfarin) with subsequently downgrading from ticagrelor to clopidogrel. Echocardiographic follow-up documented a progressive improvement of LVEF (50%), and global longitudinal strain (GLS −15%) with persistence of wall motion abnormalities in the apical and septal segments. Partial resolution of intracavitary thrombi was documented. CMR revealed LV apical aneurysm. Moreover, the use of dedicated sequences showed transmural late gadolinium enhancement in akinetic segments, and fatty metaplasia of the apical portion of the anterior wall and anterior septal wall. Finally, early gadolinium enhancement imaging, confirmed the persistence of LV stratified apical thrombus. Conclusions Spontaneous coronary artery dissection (SCAD) is an underdiagnosed and frequent cause of acute coronary syndrome (ACS) in young-to-middle aged patients, particularly women; therefore, even in young people, a chest pain episode suggestive for an ischaemic cause should never be underestimated. Cardiac magnetic resonance is the most accurate method to evaluate the sequelae of myocardial infarction (including LV size and function, presence of fatty metaplasia, extent and transmurality of ischaemic scar, presence of thrombi), to aid in differential diagnosis and to define prognosis and guide the therapeutic management.

Clinical Profile and Angiographic Pattern of Coronary Artery Disease in Young Patients with Acute Coronary Syndrome

Aim of this study was to determine the clinical profile and angiographic pattern in young patients (≤35 years) who presented with Acute coronary syndrome (ACS) to cardiac catheterization lab of tertiary care. We prospectively recruited all young patients (≤35 years) who presented to our center with ACS from August 2020 to December 2020 and underwent coronary angiography. The primary endpoint was clinical profile including demographics, co-morbidities and angiographic findings. The secondary endpoint was in-hospital and three months mortality. A total of 1742 patients with ACS were presented to our hospital. Out of them 108 (6.2%) were ≤35 years of age. There were 86% Male, 76% fall in age group of 31–35 years. 65% were overweight. 83% were active smoker and 15% were tobacco chewer. 28% were hypertensive, 12% were diabetic and 8% were dyslipidemic. ST elevation myocardial infarction (STEMI) was the most common presentation (91%) with Anterior STEMI was the most common location (70%). Most had single vessel disease (62%) with left anterior descending (LAD) artery being the most common culprit vessel (70%). Proximal LAD was the most common site (62%) with type B lesion being the most common pattern of involvement (44%). In-hospital and at 3 months mortality was 1.9% and 4% respectively. Our findings suggest that young males were most common presenter with ACS, being overweight and smoking were the most common risk factors. Proximal LAD involvement with type B lesion causing anterior STEMI was the most common angiographic finding.

Activation of PPCI team in the octogenarian and nonagenarians population: real-world single-centre experience.

ObjectiveAdvancement in healthcare provision has led to increasing octogenarian ST elevation myocardial infarction (STEMI) presentation to hospital for early revascularisation therapies. Limited literature to date exists to suggest octogenarian STEMI population; with majority of trials excluding these age group patients. Due to an ageing population, we expect increasing rates of STEMI in the octogenarian and nonagenarian population in the future. This study seeks to identify the outcomes of patients over the age of 80 presenting with STEMI and determine the factors associated with better or worse outcome.Patients and methodsThis study is a single-centre retrospective observational study involving patients’ age 80 or older presenting with STEMI between January 2014 and December 2019. Patient data were collected by chart review and analysis of the local STEMI database. Standard Bayesian statistics were employed for analysis.Results1301 patients presented with STEMI during this period. 159/1301 (12.2%) were 80 years or older that fulfilled STEMI criteria, 35/159 (22.1%) were medically managed. 107/124 (86.29%) had angiographic evidence of acute total or partial thrombotic occlusion, and 97/107 were treated with primary percutaneous coronary intervention (PPCI). The activation ECG most commonly exhibited an anterior STEMI, while inferior STEMI ECGs had the strongest positive predictive value. PPCI group had a 30-day mortality rate of 20% (p=0.07) and 1-year mortality was 22.4%. Highest mortality was observed with cardiogenic shock, low ejection fraction, higher high sensitivity cardiac troponin T and creatinine at presentation. Conservatively managed patients had significant higher mortality rate (48% vs 22.4%, p=0.005) at 1 year.ConclusionPatients over the age of 80 who present with STEMI and undergo PPCI have a significantly lower mortality rate at 1 year. These patients have a 77.6% survival at 1 year, with 92.4% likelihood of discharge to home (without need for long-term nursing home care). Cardiogenic shock in this group was associated with a 1-year mortality of 87.5%. Despite the advanced age, we suggest favourable outcomes described in the absence of patients presenting with cardiogenic shock.

Predictors of Mortality and Long-Term Outcome in Patients with Anterior STEMI: Results from a Single Center Study.

Anterior ST segment elevation myocardial infarction (A-STEMI) has the worst prognosis among all infarct sites due to larger infarct size and the higher cardiac enzyme release. We retrospectively analyzed 584 A-STEMI undergoing urgent coronary angiography from October 2008 to April 2019. The median follow-up time was 1774 days with a minimum of a 1-year follow-up for 498 patients. In-hospital mortality was 8.6%, while long-term, all-cause mortality and 1-year mortality were 18.8% and 6.8%, respectively. The main predictors for in-hospital mortality were ejection fraction (LV-EF), baseline estimated glomerular filtration rate (eGFR), female gender and cardiogenic shock (CS) at admission, while long-term predictors of mortality were age, coronary artery disease (CAD) extension and LV-EF. Patients presenting with CS (6.5%) showed a higher mortality rate (in-hospital 68.4%, long term 41.7%). Among 245 patients (42%) with multivessel disease (MVD), complete revascularization (CR) during the index procedure was performed in 42.8% of patients and more often in patients with CS at admission (19.1% vs. 6.1%, p = 0.008). Short- and long-term mortality were not significantly influenced by the revascularization strategy (CR/culprit only). Our study confirmed the extreme fragility of A-STEMI patients, especially in case of CS at admission. LV-EF is a powerful predictor of a poor outcome. In MVD, CR during p-PCI did not show any advantage for either long- or short-term mortality compared to the culprit-only strategy.

Ventricular Double Rupture: A Rare Combination of Ventricular Septal Rupture and Concealed Post-Infarction Free Wall Rupture

A 72-year-old woman with well-controlled hypertension presented to our hospital due to worsening symptoms of heart failure. She had a history of acute anterior wall ST-elevation myocardial infarction (MI) diagnosed 2 weeks previously, when she received clinically successful fibrinolytic therapy at another regional hospital 4 hours after symptom onset. Four (4) days after hospital discharge, she gradually developed dyspnoea on exertion and orthopnoea. No recurrent chest pain was reported. On examination, her blood pressure was 120/65 mmHg with a regular pulse rate at 90/minute.

Evolution of single-lead ECG for STEMI detection using a deep learning approach

BackgroundWhile ST-Elevation Myocardial Infarction (STEMI) door-to-balloon times are often below 90 min, symptom to door times remain long at 2.5-h, due at least in part to a delay in diagnosis. ObjectivesTo develop and validate a machine learning-guided algorithm which uses a single‑lead electrocardiogram (ECG) for STEMI detection to speed diagnosis. MethodsData was extracted from the Latin America Telemedicine Infarct Network (LATIN), a population-based Acute Myocardial Infarction (AMI) program that provides care to patients in Brazil, Colombia, Mexico, and Argentina through telemedicine. Sample: the first dataset was comprised of 8511 ECGs that were used for various machine learning experiments to test our Deep Learning approach for STEMI diagnosis. The second dataset of 2542 confirmed STEMI diagnosis EKG records, including specific ischemic heart wall information (anterior, inferior, and lateral), was derived from the previous dataset to test the STEMI localization model. Preprocessing: Detection of QRS complexes by wavelet system, segmentation of each EKG record into individual heartbeats with fixed window of 0.4 s to the left and 0.9 s to the right of main. Training & Testing: 90% and 10% of the total dataset, respectively, were used for both models. Classification: two 1-D convolutional neural networks were implemented, two classes were considered for first models (STEMI/Not-STEMI) and three classes for the second model (Anterior/Inferior/Lateral) each corresponding to the heart wall affected. These individual probabilities were aggregated to generate the final label for each model. ResultsThe single‑lead ECG strategy was able to provide an accuracy of 90.5% for STEMI detection with Lead V2, which also yielded the best results overall among individual leads. STEMI Localization model provided promising results for anterior and inferior wall STEMIs but remained suboptimal for Lateral STEMI. ConclusionsAn Artificial Intelligence-enhanced single‑lead ECG is a promising screening tool. This technology provides an autonomous and accurate STEMI diagnostic alternative that can be incorporated into wearable devices, potentially providing patients reliable means to seek treatment early and offers the potential to thereby improve STEMI outcomes in the long run.

Abstract 10152: ST-Elevation Myocardial Infarction Induced by Acute Occlusion of an Ostial Left Intermammary Artery Graft Occurring 13 Years After Coronary Artery Bypass Graft Surgery

The left intermammary artery (LIMA) graft is commonly used in coronary artery bypass grafting (CABG) to left anterior descending (LAD) because of its superior long-term patency. Acute occlusion of the LIMA causing ST-elevation myocardial infarction (STEMI) in the late postoperative period is rare, and its pathological mechanism is unclear. We present a case of acute occlusion of the LIMA in a 87-year-old woman who underwent CABG surgery 13 years previously. She was transferred to our hospital because of acute chest pain. Electrocardiography showed ST-elevation in anterior leads. Emergency coronary angiography revealed total occlusion of proximal LAD segment and the ostial LIMA (Figure). Therefore, we judged that the LIMA occlusion was causing anterior STEMI, and performed emergency percutaneous intervention. We successfully implanted a drug eluting stent at the culprit lesion and final angiography showed good reperfusion. Although the LIMA graft is expected long-term patency, early recognition of the possibility of LIMA occlusion might be necessary in patients with AMI after CABG surgery.

Abstract 9332: A Case of Fulminant Vasculitis Complicated by ST-Elevation Myocardial Infarction and Stroke

Introduction: Acute coronary syndrome is a rare complication of vasculitis. We present a case of fulminant medium-vessel vasculitis complicated by ST-elevation myocardial infarction (STEMI) and stroke. Case report: A 30-year-old woman with a history of painful skin lesions presented with acute headache, neck pain, and right arm weakness with paresthesia. CT angiography revealed high-grade stenosis of the internal carotids. Subsequently, she reported acute chest pain and dyspnea. ECG revealed anterior STEMI (Figure 1). She underwent emergent coronary angiography that revealed an acute occlusive thrombus of the proximal LAD that was successfully treated with aspiration thrombectomy and drug-eluting stent deployment (Figure 1). The patient was transferred to our institution with concern for fulminant vasculitis complicated by STEMI and stroke. Upon arrival to our hospital, CT and MRI revealed severe occlusions of the bilateral internal carotid arteries and a left middle cerebral artery infarct (Figure 1). Echocardiogram demonstrated severe left ventricular systolic dysfunction with regional wall motion abnormalities. Additional imaging demonstrated inflammation of the thoracic aorta, common iliac, and renal arteries, as well as occlusions of the inferior mesenteric and infratibial arteries. An extensive serologic evaluation for rheumatologic disease was negative. The patient was treated with IV corticosteroids and cyclophosphamide for fulminant medium-vessel vasculitis. Discussion: This report provides a rare opportunity to consider the management of fulminant vasculitis, most likely polyarteritis nodosa, presenting as STEMI and acute stroke. The patient’s presentation, imaging, and laboratory findings were inconsistent with alternative diagnoses such as Tayakasu’s arteritis, Behçet’s disease, or a systemic rheumatologic condition. The patient improved with immunosuppression and follows with cardiology for chronic heart failure.

Aim: Left ventricular (LV) apical thrombus development is one of the important advers event patients with anterior ST elevation myocardial infarction (STEMI) and accompanies with LV apical aneurysm and systolic dysfunction. SYNTAX score is an important determinant of cardiovascular morbidity and mortality in patients with coronary artery disease. We aimed that the relationship between LV apical thrombus and SYNTAX score and other clinical risk factors patients with anterior STEMI Methods: A total of 205 consecutive anterior STEMI patients undergoing primary PCI (35 female/ 170 male mean age: 55.8 ± 9.6 years) were included in our study. We assessed the severity and complexity of coronary artery disease by using SYNTAX score. The patients were divided into two groups; patients with apical thrombus (n=19) and patients without apical thrombus(n=186). Results: We found that systolic and diastolic blood pressure were lower patients with apical thrombus groups. (p&amp;lt;0.001 for both). Syntax score was higher and LV ejection fraction (LVEF) is lower in patients with apical thrombus (p&amp;lt;0.001 for both). We also showed that LV apical thrombus formation was independently associated with lower systolic blood pressure (p=0.006), SYNYAX score (p=0.003), and depressed LVEF (p=0.001). Conclusion: In this study, impaired LVEF, lower systolic blood pressure and SYNTAX score have been found to be predictors of LV thrombus generation. Patients presenting with anterior myocardial infarction who have high SYNTAX scores (&amp;gt; 18.25) at PPCI should be followed up closely.

Extensive anterior myocardial infarction of an older non diabetic patient has better prognosis compared to a younger patient: a case report

ST elevation myocardial infarction (STEMI) incidence rates has been decreased inversely to non-STEMI (121 to 77, 126 to 132 per 100.000 case respectively). Diabetes as a risk factors of STEMI is found in 20% patients. The increment of fibrinogen level in diabetic patient may induce compact clots resistance to fibrinolysis that lead to hypercoagulable state problem. Percutaneous coronary intervention (PCI), a non-surgical invasive procedure, can be done to relieve the obstruction. We reported two case reports, patients with anterior extensive STEMI who had underwent primary PCI. Both patients were found a total occlusion at proximal segment of LAD. First case was presented type 2 diabetes mellitus as comorbid while the second case was without diabetes. High burden thrombus was found on the diabetic patient which had poor prognosis, but on the non diabetic patient was found only less thrombus which was more stable condition. There is a strong correlation between type 2 diabetes mellitus and cardiovascular disease (CVD). Glucose control is important for prevention of CVD. Dietary changes that are characterized by increased use of natural sources diets such as whole grains, vegetables, and fruit-based foods and increased physical activity is remarkably strong factors for diabetes prevention.

Glycaemic status and reperfusion injury in patients with ST-segment elevation myocardial infarction

Abstract Background Failed myocardial tissue reperfusion due to microvascular injury despite successful culprit lesion percutaneous coronary intervention (PCI) is associated with poor clinical outcome in patients with ST-elevation myocardial infarction (STEMI). A possible influence of dysglycaemia on myocardial reperfusion injury is unclear. Objectives To investigate the association between glycaemic status and microvascular injury determined by magnetic resonance imaging in STEMI patients. Methods This prospective observational cohort study included 260 consecutive STEMI patients undergoing primary PCI between 2016 and 2019. Peripheral venous blood samples for glucose and HbA1c measurements were drawn on admission. Primary microvascular injury endpoint was defined as presence of intramyocardial haemorrhage (IMH) assessed by cardiac magnetic resonance T2* mapping at 4 (interquartile range [IQR]:2–5) days after PCI. Results HbA1c (odds ratio [OR]: 1.73 [95% CI: 1.24–2.40]; p=0.001), pre-diagnosis of diabetes (OR: 2.63 [95% CI: 1.18–5.90]; p=0.02) and glucose concentration (OR: 1.01 [95% CI: 1.00–1.01]; p=0.01) significantly predicted IMH, which was present in 90 (35%) patients. Of these three parameters, only HbA1c remained significantly associated with IMH (OR: 2.12 [95% CI: 1.12–3.99]; p=0.02) after adjusting for total ischemic time, culprit lesion location, pre- and post-interventional TIMI flow and peak biomarker concentrations (troponin, N-terminal pro-B-type natriuretic peptide and C-reactive protein). The rate of IMH was 24% in patients with HbA1c &amp;lt;5.7%, 43% in patients with HbA1c ≥5.7 to 6.4% and 59% in patients with HbA1c ≥6.5% (p&amp;lt;0.001). Conclusions In STEMI patients undergoing primary PCI, admission HbA1c was independently associated with reperfusion injury as determined by IMH. These findings suggest that IMH could represent the underlying pathophysiological link between dysglycaemia and adverse outcomes following STEMI. ECHOCARDIOGRAPHIC ASSESSMENT OF RIGHT VENTRICULAR FUNCTION AFTER SUCCESSFUL REVASCULARIZATION FOR ACUTE ANTERIOR MYOCARDIAL INFARCTION WITHOUT RIGHT VENTRICULAR INFARCTION

Background: Right ventricular (RV) affection in acute left ventricular (LV) myocardial infarction (MI) is frequently underestimated in the clinical setting as the diagnostic limitations of the electrocardiogram and echocardiography. Objective: To assess RV function in patients presented with first acute anterior ST elevation myocardial infarction (STEMI) who underwent successful primary percutaneous coronary intervention (PCI) and factors affecting it. Patients and Methods: Forty consecutive patients with anterior STEMI who underwent successful primary PCI in the Cathetrization (cath) labs of Al-Azhar University Hospitals, Cairo, Egypt from March 2019 to April 2020 for first acute anterior STEMI without RV were enrolled in the study. Presence of a coexisting clinical condition that might affect RV function, patients with RV infarction or those having significant stenosis (>50%) affecting RV branch or right coronary artery proximal to RV branch were excluded. Echocardiography was performed during the hospital stay to assess the LV and RV systolic and diastolic functions with special focus on tricuspid annular plane systolic excursion, mid RV end-diastolic dimension, right atrial area, RV fractional area change, and tissue Doppler-derived myocardial performance index. Results: RV dysfunction, according to our definition in the first anterior MI, occurred in (55%) of the study population. Independent predictors for abnormal RV function were left circumflex artery mid or proximal affection, eventful procedure, occurrence of no reflow, glucose level, LV end-systolic dimension, LV end-diastolic dimension, and LV ejection fraction. Conclusion: RV dysfunction is detected in anterior myocardial infarction after successful revascularization. Several echocardiographic parameters may be possible measures for RV dysfunction including RVFAC, TAPSI, MPI and S’.

The Role of Cardiac Magnetic Resonance in Assessment of Myocardial Contractile Function After Exercise Training Based Cardiac Rehabilitation Program in Anterior ST-Elevated Myocardial Infarction Patients Treated with Primary Percutaneous Coronary Intervention

Abstract Back ground Regular exercise training program has been shown to reduce mortality, improve functional capacity and control risk factors in myocardial infarcted patients. Wall motion score index (WMSI) is a strong independent mortality predicator in patient with previous MI AIM The main objective of this study was to investigate the impact of exercise training on regional left ventricular systolic function, in patients post anterior myocardial infarction. using cardiac magnetic resonance imaging Methods We recruited twenty-six adult patients on optimal medical therapy one month after an anterior myocardial infarction underwent successful percutaneous coronary intervention (PCI) were assigned to a three-month exercise training program group or to a control group, (control group: n = 10, training group: n = 16). Symptom limited treadmill exercise (Modified Bruce Protocol) test was done to exercise training group before and repeated after finishing the CR program. Cardiac Magnetic Resonance (CMR) was performed for all patients 4 weeks after PCI and was repeated after completion of the study period that was lasted for 12 weeks. Results A total number of patients, included 26 (100%) males, assigned to control group n:10 without an exercise mean age 50± 8 years, weigh 84 ±17 kg, height 175±7.7 cm and EF was 36.62 ±14.23 and to training group n:16 with 3-months exercise training program, the mean age, weight, height and EF were 50.19±8.68 years, 82.69±16 kg, 172.81±7.74 cm, 36.62±14.32% respectively While there was significant improvement in EF (P &amp;lt; 0.0008), WMSI (P &amp;lt; 0.00003) without significant change in LVESV and LVEDV in exercise training group, there was no significant change in EF, WMSI, LVESV and LVEDV in control group. Conclusion CMR revealed that exercise training in post-myocardial infarction patients could have beneficial effects on LV global and regional function without adversely affecting LV remodeling or causing serious cardiac complications with significant improvement in exercise capacity after 3 months of exercise training program.

The de Winter Pattern as Pre-Anterior ST-Elevation-Myocardial-Infarction. “An Evolution Sequence”: A Case Report

Introduction: The de Winter pattern (dWP) was first described by de Winter and colleagues in 2008 as static pattern associated with anterior myocardial infarction. A recent study showed the evolution sequence of this pattern into typical ST-elevation myocardial infarction (STEMI). This case discussed dWP who present as pre-anterior STEMI.&#x0D; Case Illustration: A-56-year old Male arrived in the emergency room complained chest pain about 3 hours. The patient also complained of diaphoresis, nausea, and fatigue. The patient has a previous history of hypertension. The vital signs were stable with an unremarkable physical examination. The initial electrocardiogram (ECG) revealed sinus rhythm with j-point depression followed by prominent T wave in precordial leads, slight ST-segment elevation in aVR, and loss of precordial R-wave progression. The initial-troponin-T was 31 pg/mL. Follow-up 1-hour after initial ECG showed typical ST-segment-elevation in V1-V4. The patient undergoing thrombolytic, followed by angiography that showed subtotal occlusion in the proximal left anterior descending (LAD) artery, occlusion in the proximal circumflex artery and stenosis in proximal right coronary artery, echocardiography revealed regional wall motion abnormality in the septal and anterior segments and preserved ejection fraction 58%, the patient was discharged after 8-days treated in intensive cardiac care unit.&#x0D; Conclusion: dWP has been shown as static and dynamic pattern in some conditions and associated with acute LAD occlusion. In this case, we showed dWP as early anterior STEMI, recognition of this pattern lead to early reperfusion and better myocardial salvage as anterior STEMI has a poor outcome.

The effect of the localisation of an underlying ST-elevation myocardial infarction on the VF-waveform: A multi-centre cardiac arrest study

IntroductionIn cardiac arrest, ventricular fibrillation (VF) waveform characteristics such as amplitude spectrum area (AMSA) are studied to identify an underlying myocardial infarction (MI). Observational studies report lower AMSA-values in patients with than without underlying MI. Moreover, experimental studies with 12-lead ECG-recordings show lowest VF-characteristics when the MI-localisation matches the ECG-recording direction. However, out-of-hospital cardiac arrest (OHCA)-studies with defibrillator-derived VF-recordings are lacking. MethodsMulti-centre (Amsterdam/Nijmegen, the Netherlands) cohort-study on the association between AMSA, ST-elevation MI (STEMI) and its localisation. AMSA was calculated from defibrillator pad-ECG recordings (proxy for lead II, inferior vantage point); STEMI-localisation was determined using ECG/angiography/autopsy findings. ResultsWe studied AMSA-values in 754 OHCA-patients. There were statistically significant differences between no STEMI, anterior STEMI and inferior STEMI (Nijmegen: no STEMI 13.0mVHz [7.9–18.6], anterior STEMI 7.5mVHz [5.6–13.8], inferior STEMI 7.5mVHz [5.4–11.8], p = 0.006. Amsterdam: 11.7mVHz [5.0–21.9], 9.6mVHz [4.6–17.2], and 6.9mVHz [3.2–16.0], respectively, p = 0.001). Univariate analyses showed significantly lower AMSA-values in inferior STEMI vs. no STEMI; there was no significant difference between anterior and no STEMI. After correction for confounders, adjusted absolute AMSA-values were numerically lowest for inferior STEMI in both cohorts, and the relative differences in AMSA between inferior and no STEMI was 1.4–1.7 times larger than between anterior and no STEMI. ConclusionThis multi-centre VF-waveform OHCA-study showed significantly lower AMSA in case of underlying STEMI, with a more pronounced difference for inferior than for anterior STEMI. Confirmative studies on the impact of STEMI-localisation on the VF-waveform are warranted, and might contribute to earlier diagnosis of STEMI during VF.

Vaccine-induced immune thrombocytopenia and thrombosis associated anterior ST-elevation myocardial infarction.

A 40-year-old male, with no cardiac risk factors, presented with sudden onset, severe, central chest pain 7 days after receiving the first Oxford-Astra Zeneca SARS-CoV-2 vaccine dose. This was associated with anterior ST-segment elevation on electrocardiogram (Panel A), marked troponin elevation (peak 2726 ng/L; normal < 14ng/L), significant thrombocytopenia with a platelet count of 7 × 109/L, a D-dimer of 80 000µg/L (FEU) and anti-PF4 seropositivity. Echocardiography revealed akinesia of the mid-to-apical antero-septum and apex (ejection fraction ∼45%). Refractory thrombocytopenia precluded coronary angiography and primary angioplasty. Computed tomography coronary angiography demonstrated extensive layered thrombus from the ostium of the left anterior descending (LAD) artery with moderate proximal-LAD stenosis and complete mid-LAD occlusion (Panel B), suggesting the diagnosis of coronary thrombosis due to vaccine-induced immune thrombocytopenia and thrombosis (VITT). Spatio-temporal hybrid neural networks reduce erroneous human "judgement calls" in the diagnosis of Takotsubo syndrome.

BackgroundWe investigate whether deep learning (DL) neural networks can reduce erroneous human “judgment calls” on bedside echocardiograms and help distinguish Takotsubo syndrome (TTS) from anterior wall ST segment elevation myocardial infarction (STEMI).MethodsWe developed a single-channel (DCNN[2D SCI]), a multi-channel (DCNN[2D MCI]), and a 3-dimensional (DCNN[2D+t]) deep convolution neural network, and a recurrent neural network (RNN) based on 17,280 still-frame images and 540 videos from 2-dimensional echocardiograms in 10 years (1 January 2008 to 1 January 2018) retrospective cohort in University of Iowa (UI) and eight other medical centers. Echocardiograms from 450 UI patients were randomly divided into training and testing sets for internal training, testing, and model construction. Echocardiograms of 90 patients from the other medical centers were used for external validation to evaluate the model generalizability. A total of 49 board-certified human readers performed human-side classification on the same echocardiography dataset to compare the diagnostic performance and help data visualization.FindingsThe DCNN (2D SCI), DCNN (2D MCI), DCNN(2D+t), and RNN models established based on UI dataset for TTS versus STEMI prediction showed mean diagnostic accuracy 73%, 75%, 80%, and 75% respectively, and mean diagnostic accuracy of 74%, 74%, 77%, and 73%, respectively, on the external validation.
