こう頭血管平滑筋しゅ（ｖａｓｃｕｌａｒ ｌｅｉｏｍｙｏｍａ）の一症例

Tumors of smooth muscle cell origin of the upper respiratory tract are extremely rare, especially of the larynx, and less than twenty cases have been reported in literature. Survey of literature also revealed that there was no case of laryngeal leiomyoma reported in Japan.A 47-year-old, otherwise healthy man visited our clinic with complaint of slight laryngeal discomfort. Indirect laryngoscopr showed a semiglobular tumor-like swelling of the right false cord. Direct laryngoscopy was performed under general anethesia and the tumor, lying in the submucotic region of the false cord, was enucleated totally. It was a firm, smooth and round tumor with a diameter of 8mm. Microscopic examination revealed that the tumor was a vascular leiomyoma most likely arising from the smooth muscle of the submucous small blood vessels of the right false cord. No cellular atypia or mitosis was observed. The patient is well without recurrence eleven months after operation.

Vascular leiomyosarcoma.The malignant counterpart of vascular leiomyoma

The clinical and pathological findings of six cases of leiomyosarcoma arising from blood vessels of different caliber are described. The term vascular leiomyosarcoma, having both a topographic and morphologic significance, is proposed for these tumors. The histologic pattern is characterized by a proliferation of atypical smooth muscle cells with a large number of intermingled blood vessels. Mitoses were counted per 10 high power field (hpf) and tumors were divided in three groups I, 10 to 20 mitoses, group II, 20 to 35 mitoses, and group III, more than 35 mitoses per 10 hpf. The mitotic index seems to be the most important pathological feature on which a prognostic evaluation for vascular leiomyosarcoma can be based. Tumors in group I had neither local recurrences nor metastases; the one tumor in group II had one local recurrence, but the patient is free of disease 6 years after surgical treatment; the three tumors in group III developed distant metastases and constitutional symptoms. Vascular leiomyoma, bizarre leiomyoma, and hemangiopericytoma are included in the differential diagnosis of vascular leimyosarcoma. The possibility that vascular leiomyosarcoma arising from small vessels represents the malignant counterpart of vascular leiomyoma is proposed.

Laryngeal Angiomyoma

A 53-year-old man complained of sudden dyspnea and laryngeal pain. Laryngeal examination revealed a large movable walnut-sized tumor in the supra glottic space of the larynx. The tumor was removed through the direct laryngoscopy under general anesthesia. Histological examination showed that the tumor was made up of a well circumscribed, circular mass of smooth muscle proliferating around the many vessels, that is the angiomyoma (vascular leiomyoma).

HEMANGIOPERICYTOMA OF THE UTERUS

Abstract A hemangiopericytoma of the uterus, which presented as a polypoid mass protruding into the vagina, was studied by light and electron microscopy. The appearance of the tumor was identical to that of hemangiopericytomas of other sites that have been reported, and the tumor cells did not ultrastructurally resemble endometrial stromal cells or uterine smooth-muscle cells. These findings confirm the existence of hemangiopericytoma of the uterus as an entity distinct from both stromal myosis and vascular leiomyoma, with which it has been confused. The treatment and prognosis of this rare tumor are also discussed—it should be considered a low-grade malignancy until proved otherwise.

Multiple congenital mesenchymal tumors. Multiple vascular leiomyomas in several organs of a newborn.

A case of multiple congenital mesenchymal tumors is described. The tumors involved the lungs, the small intestine, the colon, and the subcutaneous tissues. Twenty cases are reviewed, and comparisons are made. The features in the 20 cases are basically similar, and resemble one another; the tumors are multiple, congenital, and mesenchymal in origin. It is felt that the present case, perhaps as well as Bartlett's 4 cases, represents a different age of the tumors from those of the other cases, and shows a greater degree of differentiation.
