Abstract

Objective To investigate the effects of hospital-community-family continuing nursing in patients with type 2 diabetes mellitus on the control of blood glucose and diet. Methods A total of 118 patients with type 2 diabetes in the endocrinology department of Jiangsu Province Hospital of TCM from March 2015 to March 2016 were selected using convenience sampling methods and divided into control group and experimental group using random number table, with 59 cases in each group. Patients in the control group were given endocrinology specialist nursing and routine discharge guidance, while patients in the experimental group were given endocrinology specialist nursing and hospital-community-family continuing nursing discharge guidance. The control effects of blood glucose and diet of patients were compared between the two groups. Results After the intervention for six months, the control level of blood glucose of patients in the experimental group was significantly better than that in the control group (t=3.98, P<0.05) . The food intake compliance of the patients in the experimental group were better than that in the control group, with a statistically significant difference between the two groups (P<0.05) . Conclusions The implementation of hospital-community-family continuing nursing in the patients with type 2 diabetes mellitus is beneficial to the blood glucose control and improvement of diet of patients.
