Abstract

Objective To investigate the efficacy of saxagliptin combined with insulin on type 2 diabetes mellitus (T2DM) and its effects on blood glucose fluctuation. Methods A total of 208 patients with T2DM in the People’s Hospital of Zhoukou from August 2016 to August 2018 were included in the study, and all the patients were randomly divided into experimental group and control group according to the random number table method, with 104 cases in each group. Control group was given glimepiride combined with insulin, and experimental group was given saxagliptin combined with insulin. Blood glucose test levels (fasting blood glucose, postprandial 2 h blood glucose), glycosylated hemoglobin (HbA1c) level, blood glucose fluctuations including standard deviation of blood glucose (SDBG), mean of daily differences (MODD), mean amplitude of glycemic excursions (MAGE) and the incidence rate and cumulative time of hypoglycemia events were compared between the two groups before treatment and after 12 weeks of treatment. Results After 12 weeks of treatment, the fasting blood glucose, postprandial 2 h blood glucose and HbA1c in the two groups were decreased compared with those before treatment (P all<0.05), and the postprandial 2 h blood glucose and HbA1c in experimental group were significantly lower than those in control group (P all<0.05). The SDBG, MAGE and MODD in the two groups were decreased compared with those before treatment (P all<0.05), and the indexes in experimental group were significantly lower than those in control group (P all<0.05). The incidence and cumulative time of hypoglycemia events in experimental group were smaller than those in control group (P all<0.05). Conclusions Saxagliptin combined with insulin for T2DM patients can effectively reduce blood glucose, improve islet cell function, and help to control blood glucose fluctuation, without increase in the occurrence of hypoglycemia events.
