User Experience (UX) Design as a co-design methodology: lessons learned during the development of a web-based portal for cardiac rehabilitation.

Person-centred care advocates for co-design of all healthcare services and research interventions by the end-user. Co-design is widely used, but the methodological approaches, evaluation, and reporting of outcomes are often poorly defined. One methodology for co-design is the User Experience Design which provides guidance and theoretical frameworks to inform development and reporting measures. This article outlines the application of this approach in the development of a web-based cardiac rehabilitation program and reports on the very positive experiences of the patients involved in the process and how their input strategically influenced outcomes.

BatchBMD as an Efficient and Accurate Dual-Energy X-ray Absorptiometry Report Generator.

Dual-energy X-ray absorptiometry is the gold standard for evaluating Bone Mineral Density (BMD); however, a typical BMD report is generated in a time-inefficient manner and is prone to error. We developed a rule-based automated reporting system, BatchBMD, that accelerates DXA reporting while improving its accuracy over current systems. BatchBMD generates a structured report, customized to the specific clinical purpose. To compare BatchBMD to a Web-based Reporting (WBR) system for efficiency and accuracy, 500 examinations were randomly chosen from those performed at the Taipei Municipal Wanfang Hospital from January to March 2021. The final assessment included all 2326 examinations conducted from September 2020 to March 2021. The average reporting times were 6.7 and 10.8 min for BatchBMD and the WBR system, respectively, while accuracy was 99.4% and 98.2%, respectively. Most of the errors made by BatchBMD were digit errors in the appendicular skeletal muscle index. After correcting this, 100% accuracy across all 2326 examinations was validated. This automated and accurate BMD reporting system significantly reduces report production workload for radiologists and technicians while increasing productivity and quality. Additionally, the portable software, which employs a simple framework, can reduce deployment costs in clinical practice.

AutoProstate: Towards Automated Reporting of Prostate MRI for Prostate Cancer Assessment Using Deep Learning.

Simple SummaryInternational guidelines recommend multiparametric magnetic resonance imaging (mpMRI) of the prostate for use by radiologists to identify lesions containing clinically significant prostate cancer, prior to confirmatory biopsy. Automatic assessment of prostate mpMRI using artificial intelligence algorithms holds a currently unrealized potential to improve the diagnostic accuracy achievable by radiologists alone, improve the reporting consistency between radiologists, and enhance reporting quality. In this work, we introduce AutoProstate: a deep learning-powered framework for automatic MRI-based prostate cancer assessment. In particular, AutoProstate utilizes patient data and biparametric MRI to populate an automatic web-based report which includes segmentations of the whole prostate, prostatic zones, and candidate clinically significant prostate cancer lesions, and in addition, several derived characteristics with clinical value are presented. Notably, AutoProstate performed well in external validation using the PICTURE study dataset, suggesting value in prospective multicentre validation, with a view towards future deployment into the prostate cancer diagnostic pathway.Multiparametric magnetic resonance imaging (mpMRI) of the prostate is used by radiologists to identify, score, and stage abnormalities that may correspond to clinically significant prostate cancer (CSPCa). Automatic assessment of prostate mpMRI using artificial intelligence algorithms may facilitate a reduction in missed cancers and unnecessary biopsies, an increase in inter-observer agreement between radiologists, and an improvement in reporting quality. In this work, we introduce AutoProstate, a deep learning-powered framework for automatic MRI-based prostate cancer assessment. AutoProstate comprises of three modules: Zone-Segmenter, CSPCa-Segmenter, and Report-Generator. Zone-Segmenter segments the prostatic zones on T2-weighted imaging, CSPCa-Segmenter detects and segments CSPCa lesions using biparametric MRI, and Report-Generator generates an automatic web-based report containing four sections: Patient Details, Prostate Size and PSA Density, Clinically Significant Lesion Candidates, and Findings Summary. In our experiment, AutoProstate was trained using the publicly available PROSTATEx dataset, and externally validated using the PICTURE dataset. Moreover, the performance of AutoProstate was compared to the performance of an experienced radiologist who prospectively read PICTURE dataset cases. In comparison to the radiologist, AutoProstate showed statistically significant improvements in prostate volume and prostate-specific antigen density estimation. Furthermore, AutoProstate matched the CSPCa lesion detection sensitivity of the radiologist, which is paramount, but produced more false positive detections.

Informing Cyber Threat Intelligence through Dark Web Situational Awareness: The AZSecure Hacker Assets Portal

To increase situational awareness, major cybersecurity platforms offer Cyber Threat Intelligence (CTI) about emerging cyber threats, key threat actors, and their modus operandi. However, this intelligence is often reactive, as it analyzes event log files after attacks have already occurred, lacking more active scrutiny of potential threats brewing in cyberspace before an attack has occurred. One intelligence source receiving significant attention is the Dark Web, where significant quantities of malicious hacking tools and other cyber assets are hosted. We present the AZSecure Hacker Assets Portal (HAP). The Dark Web-based HAP collects, analyzes, and reports on the major Dark Web data sources to offer unique perspective of hackers, their cybercriminal assets, and their intentions and motivations, ultimately contributing CTI insights to improve situational awareness. HAP currently supports 200+ users internationally from academic institutions such as UT San Antonio and National Taiwan University, law enforcement entities such as Calgary and Ontario Provincial Police, and industry organizations including General Electric and PayPal.

Clinical Course of Hepatitis B Viral Infection in Patients Undergoing Anti-Tumor Necrosis Factor α Therapy for Inflammatory Bowel Disease.

Background/AimsLittle is known about the clinical course of hepatitis B virus (HBV)-infected patients undergoing anti-tumor necrosis factor α (TNF-α) therapy for inflammatory bowel disease (IBD). We aimed to investigate the clinical course of HBV infection and IBD and to analyze liver dysfunction risks in patients undergoing anti-TNF-α therapy.MethodsThis retrospective multinational study involved multiple centers in Korea, China, Taiwan, and Japan. We enrolled IBD patients with chronic or resolved HBV infection, who received anti-TNF-α therapy. The patients’ medical records were reviewed, and data were collected using a web-based case report form.ResultsOverall, 191 patients (77 ulcerative colitis and 114 Crohn’s disease) were included, 28.3% of whom received prophylactic antivirals. During a median follow-up duration of 32.4 months, 7.3% of patients experienced liver dysfunction due to HBV reactivation. Among patients with chronic HBV infection, the proportion experiencing liver dysfunction was significantly higher in the non-prophylaxis group (26% vs 8%, p=0.02). Liver dysfunction occurred in one patient with resolved HBV infection. Antiviral prophylaxis was independently associated with an 84% reduction in liver dysfunction risk in patients with chronic HBV infection (odds ratio, 0.16; 95% confidence interval, 0.04 to 0.66; p=0.01). The clinical course of IBD was not associated with liver dysfunction or the administration of antiviral prophylaxis.ConclusionsLiver dysfunction due to HBV reactivation can occur in HBV-infected IBD patients treated with anti-TNF-α agents. Careful monitoring is needed in these patients, and antivirals should be administered, especially to those with chronic HBV infection.

Therapeutic plasma exchange for life-threatening pediatric disorders.

Therapeutic plasma exchange (TPE) is acknowledged to be an effective treatment in life-threatening pediatric disorders. Apheresis for pediatric diseases has been poorly investigated, and most studies to date featured small numbers of patients and lacked control groups. The objective of the present study was to evaluate the tolerance of TPE in pediatric patients. A retrospective cohort study via a web-based electronic case report form including pediatric patients referred for TPE between January 2005 and December 2014. A total of 78 patients (median [range] age: 9.8 [0.53-17.93]) and 731 TPE procedures were analyzed. The indications were antibody-mediated rejection (n=33; 42%) and desensitization therapy (n=5; 6%) after solid organ or hematopoietic stem cell transplantation, thrombotic microangiopathy (n=17; 22%), pediatric inflammatory diseases (n=16; 21%), kidney diseases (n=6; 8%), and hyperviscosity syndrome (n=1; 1%). On average, each patient underwent six procedures during the first session [range: 1-19]. In the 2 weeks following the start of a session, 72 patients (92%) presented a total of 311 adverse events (AEs) potentially related to TPE. The risk of AEs was not related to the indication for TPE, the intensity of care, venous access, plasma substitute use, or body weight. None of the deaths was related to the TPE. We studied one of the largest retrospective pediatric cohorts described to date. Our experience of TPE children's TPE feasibility concerned specific, life-threatening conditions and otherwise treatment-refractory diseases.

Clinical Features of COVID-19 on Patients With Neuromyelitis Optica Spectrum Disorders.

Background and ObjectivesTo describe the clinical features and disease outcomes of coronavirus disease 2019 (COVID-19) in patients with neuromyelitis optica spectrum disorder (NMOSD).MethodsThe Neuroimmunology Brazilian Study Group has set up the report of severe acute respiratory syndrome (SARS-CoV2) cases in patients with NMOSD (pwNMOSD) using a designed web-based case report form. All neuroimmunology outpatient centers and individual neurologists were invited to register their patients across the country. Data collected between March 19 and July 25, 2020, were uploaded at the REDONE.br platform. Inclusion criteria were as follows: (1) NMOSD diagnosis according to the 2015 International Panel Criteria and (2) confirmed SARS-CoV2 infection (reverse transcription-polymerase chain reaction or serology) or clinical suspicion of COVID-19, diagnosed according to Center for Disease Control / Council of State and Territorial Epidemiologists (CDC/CSTE) case definition. Demographic and NMOSD-related clinical data, comorbidities, disease-modifying therapy (DMT), COVID-19 clinical features, and severity were described.ResultsAmong the 2,061 pwNMOSD followed up by Brazilian neurologists involved on the registry of COVID-19 in pwNMOSD at the REDONE.br platform, 34 patients (29 women) aged 37 years (range 8–77), with disease onset at 31 years (range 4–69) and disease duration of 6 years (range 0.2–20.5), developed COVID-19 (18 confirmed and 16 probable cases). Most patients exhibited mild disease, being treated at home (77%); 4 patients required admission at intensive care units (severe cases); and 1 patient died. Five of 34 (15%) presented neurologic manifestations (relapse or pseudoexacerbation) during or after SARS-CoV2 infection.DiscussionMost NMOSD patients with COVID-19 presented mild disease forms. However, pwNMOSD had much higher odds of hospitalization and intensive care unit admission comparing with the general Brazilian population. The frequency of death was not clearly different. NMOSD disability, DMT type, and comorbidities were not associated with COVID-19 outcome. SARS-CoV2 infection was demonstrated as a risk factor for NMOSD relapses. Collaborative studies using shared NMOSD data are needed to suitably define factors related to COVID-19 severity and neurologic manifestations.

The Effect of Regional Income, Leverage, Regional Property on Internet Financial Reporting Disclosure

This study aims to analyze the effect of local revenue, leverage, local government wealth on the disclosure of web-based local government financial statements. The population and sample of this study are the regional government financial reports of Central Java Province which consist of 29 districts and 6 cities for the 2015-2018 fiscal year which have been audited by the BPK. The analysis method uses a multiple regression panel data model with the Eviews 9 application. The results of this study indicate that local revenue and general allocation funds have a significant positive effect on the disclosure of web-based local government financial reports. However, the wealth of the local government in turn has a negative effect on the disclosure of web-based local government financial reports. Meanwhile, leverage has no effect on the disclosure of web-based local government financial statements. The results of this study can be used as a consideration for the government informing regulations, rewards, and sanctions in order to increase the transparency of digital financial reports.

Crashing project and design of daily report system: A case study of IT security company

Delayed projects not only lead to over budgetbut could result in customer unsatisfaction and lower company reputation. Therefore, need to optimize project time to shortened project duration and controlling project through real time daily report between technician and management team. During the pandemic in 2020 there is a health protocol that requires maintaining distance, so it is necessary to establish a web-based interactive daily report. The aims of this study are to optimize project duration and improve daily report system between technician and management team. To optimize project duration use mitigation then continue with Critical Path, Project Evaluation Review Technique, and Crashing Project. The design of web-based daily report is approach with Fishbone Diagram, Quality Function Deployment then The FAST diagram process. The result of this research shows that the project duration of IT security system installation at PT XYZ can be shortened from 86 days to 43 days and the systems of daily report are proposed with web-based system that were designed with function analysis system technique The FAST diagram process.

Implementation of Extreme Programming Method in Web Based Digital Report Value Information System Design

The process of data collection and processing of student scores that is carried out continuously from year to year is felt to be unable to produce fast, precise and accurate information, because it takes a long time. Computerized information system processing student grades is expected to be able to ease the work of homeroom teachers, so that they can produce valid information in a short time and can be accessed anywhere. With good processing and management, value data processing will be easier, faster, more accurate. The purpose of this research is to build a web-based digital report card information system as a solution offered to help solve problems faced by schools. The system development method used is Extreme Programming (XP) which has several stages, namely planning, designing, coding, testing and Software Increment. The results of this study are the creation of an information system that can provide several advantages as well as efficiency and effectiveness in processing information and managing value data up to the printing of student report cards.

Analysis And Design Of Information System Report Of Medical Check Up In Chio Medical Center Clinic Web-Based

Chio Medical Center is one of the services in the health sector that is engaged in special medical check-ups for prospective employees who will work and permanent employees in a company. Currently, the system that processes medical check-up report data has not been computerized properly, so that data or files are stored conventionally and it is difficult to find the required data. The research method used in this study is the System Development Life Cycle (SDLC) method and system development using the Waterfall model which starts from analysis and observation of the system running in the company. The results of the observations show that the system that runs at the Chio Medical Center clinic has not been computerized properly, namely in recording examination reports using excel and word files so that they have not been stored in a structured database. This system is designed using the Dreamweaver application as a web-based editor tool and uses the PHP and MySQL programming languages ​​as the database. The result of this research is a web-based medical check-up report information system which is expected to assist employees in inputting data and improve the process of providing information services to clients so that it is faster and more accurate.

PEPPRO: quality control and processing of nascent RNA profiling data

Nascent RNA profiling is growing in popularity; however, there is no standard analysis pipeline to uniformly process the data and assess quality. Here, we introduce PEPPRO, a comprehensive, scalable workflow for GRO-seq, PRO-seq, and ChRO-seq data. PEPPRO produces uniformly processed output files for downstream analysis and assesses adapter abundance, RNA integrity, library complexity, nascent RNA purity, and run-on efficiency. PEPPRO is restartable and fault-tolerant, records copious logs, and provides a web-based project report. PEPPRO can be run locally or using a cluster, providing a portable first step for genomic nascent RNA analysis.

The outcome of a national MS-Covid-19 study: What the Turkish MS cohort reveals?

BackgroundThe pandemic of the new type of corona virus infection 2019 [Covid-19] also affect people with Multiple Sclerosis (pwMS). Currently, the accumulating information on the effects of the infection regarding the demographic and clinical characteristics of the disease, as well as outcomes within different DMTs¸ enable us to have better practices on the management of the Covid-19 infection in pwMS. ObjectiveTo investigate the incidence of coronavirus disease 2019 (Covid-19) and to reveal the relationship between the demographic-clinical and therapeutic features and the outcome of Covid-19 infection in a multi-center national cohort of pwMS. MethodsThe Turkish Neurological Society-MS Study Group in association with the Italian MuSC-19 Study Group initiated this study. A web-based electronic Case Report Form (eCRF) of Study-MuSC-19 were used to collect the data. The demographic data and MS histories of the patients were obtained from the file tracking forms of the relevant clinics. Results309 MS patients with confirmed Covid-19 infection were included in this study. Two hundred nineteen (219) were females (70.9%). The mean age was 36.9, ranging from 18 to 66, 194 of them (62.8%) were under 40. The clinical phenotype was relapsing-remitting in 277 (89.6%) and progressive in 32 (10.4%). Disease duration ranged from 0.2 years to 31.4 years. The median EDSS was 1.5, ranging from 0 to 8.5. The EDSS score was<= 1 in 134 (43%) of the patients. 91.6% of the patients were on a DMT, Fingolimod was the most frequently used drug (22.0%), followed by Interferon (20.1%). The comorbidity rate is 11.7%. We were not able to detect any significant association of DMTs with Covid-19 severity. ConclusionThe Turkish MS-Covid-19 cohort had confirmed that pwMS are not at risk of having a more severe COVID-19 outcome irrespective of the DMT that they are treated. In addition, due to being a younger population with less comorbidities most had a mild disease further highlight that the only associated risk factors for having a moderate to severe COVID-19 course are similar with the general population such as having comorbid conditions and being older.

For this reason, an information system is designed and built to manage web-based financial reports to make it easier to present reports more efficiently—design and Build of Financial Information System PT. Total Ocean Crossing begins with collecting data, and then from that data, functional and non-functional analysis can be carried out. After that, do the design, prepare black box testing test scenarios, determine the schedule of activities, implement the system, and finally do system testing—financial Information System PT. Total Lintas Samudera has two user levels, namely admin and leadership. The test results found that the test was by the system design, and everything could work as expected.

Investigating Web-Based Sustainability Reporting in Italian Public Universities in the Era of Covid-19

Universities are increasingly called to demonstrate to stakeholders their active commitment to responding to social and environmental issues through comprehensive reporting practices. Such expectations were further enhanced by the recent pandemic outbreak unleashed by coronavirus disease 2019 (COVID-19), which has bolstered social pressures on universities to demonstrate their active engagement in supporting health systems in fighting against the virus as well as in ensuring adequate e-learning paths for students. Therefore, this study aims to investigate to what extent Italian public universities disclose sustainability information on their websites. The study is based on a well-established framework adapted from the literature. The category “Sustainability and COVID-19” has been added to also examine the extent to which universities disclose information regarding policies and strategies adopted to cope with the ongoing pandemic crisis. Results evidence that most of the sampled universities devote a specific section of their websites to disclose information on sustainability issues. Results also highlight a strong commitment to the disclosure of social issues. Particular attention is devoted to disclosing information about “Social Performance”; “Sustainability and COVID-19” and “Society Issues”. The results may be beneficial to both policymakers and university governors in gaining awareness about the potential of websites in engendering insights into the social issues that involve a university and a wider range of stakeholders.

ES15.03 Effective and Interpretable Reporting of Genotyping Results

Genomic advances over the past several decades have transformed lung cancer care and improved patient outcomes. Clinicians can leverage information about driver mutations or biologically relevant markers to select therapies that are more effective and less toxic than conventionally chemotherapy. Despite the promise of precision medicine, however, there are major gaps in lung cancer care delivery. A major problem in lung cancer is significant underutilization of indicated genomic testing and targeted therapy. Data suggest that up to half of all patients who could benefit from genomic care may not be getting the care that they need. Myriad factors contribute to genomic care gaps. A significant factor in poor care delivery relates to the complexity of genomic data interpretation. Many physicians report low levels of genomic confidence, find genomic results difficult to interpret and explain, and have trouble keeping up with rapidly emerging evidence. In addition, although the vast majority of patients want to genomic information returned to them, they frequently do not remember or misunderstand their test results. Moreover, many health care systems are poorly configured to deal with genomic sequencing data. Specifically, data are often housed in electronic medical records in a relatively inaccessible unstructured format that limits options for clinical decision support. Given these significant care gaps, there is an urgent need to develop point-of-care tools to improve the interpretability of genomic reports. In this session, we will provide an overview of interventions that have been created to improve genomic reporting for providers and patients. We will provide an overview of a study in which we evaluated interactive, web-based clinician-directed genomic reports. Report use improved comprehension of key genomic concepts. We will also highlight work on the HOPE-Genomics intervention. HOPE-Genomics is a “patient-friendly” web-based tool that 1) educates patients about genomics, 2) enables direct-to-patient return-of-results, and 3) facilitates sharing of genomic results within families. This intervention is highly acceptable to patients, family members and physicians. We will provide an overview of the HOPE-Genomics tool and outline an upcoming randomized trial to evaluate tool efficacy. Finally, we will highlight the need for additional innovation in point-of-care education for patients and providers and stress the importance of open-source development and the sharing of best practices. It is imperative that we not only create tests and therapies to improve lung cancer outcomes, but we have the tools needed to realize the effective translation of genomic technologies in routine clinical care.

The patient experience of peripheral intravenous therapy: development of a patient survey, initial findings, and next steps

Peripheral intravenous cannulation (PIVC) is one of the most commonly performed invasive procedures in healthcare and can be a stressful experience for patients. Co-creating a patient journey map of intravenous therapy (IVT) together with patients highlighted the need to better understand patient experiences of IVT and informed the development of a patient-reported experience measure of intravenous therapy (IVT). The British Columbia (BC) Lower Mainland IVT Working Group, the BC Office of Patient-Centred Measurement and the provincial supplier of IVT products, hypothesized patient feedback about their IVT experiences would garner new insights to improve both patient experiences and outcomes related to IVT. Leveraging BC's province-wide, coordinated, scientifically rigorous patient-centred measurement program (BCPCM), a module of eight questions were developed, tested and fielded with the 2018 BC Emergency Department patient survey (n=14 076). Weekly monitoring of patient responses, through the BCPCM's web-based Dynamic Analysis and Reporting Tool (the DART), showed key themes and opportunities for improvement, leading to a test of change that introduced a patient information card (Why do I need an IV, What will happen when I get an IV, Tell a nurse if the following happens). This paper outlines the development of the IVT patient experience survey, and presents initial findings and the next steps to take action on the results. Additional data collection is now underway to solicit patient feedback of IVT across BC in outpatient cancer care and acute care hospital settings.

&lt;i&gt;PhyloVision&lt;/i&gt;: Interactive Software for Integrated Analysis of Single-Cell Transcriptomic and Phylogenetic Data

Recent advances in CRISPR-Cas9 engineering and single-cell assays have enabled the simultaneous measurement of single-cell transcriptomic and phylogenetic profiles. However, there are few computational tools enabling users to integrate and derive insight from a joint analysis of these two modalities. Here, we describe PhyloVision: an open source software for interactively exploring data from both modalities and for identifying and interpreting heritable gene modules whose concerted expression are associated with phylogenetic relationships. PhyloVisionprovides a feature-rich, interactive, and shareable web-based report for investigating these modules, while also supporting several other data and meta-data exploration capabilities. We demonstrate the utility of PhyloVisionusing a published dataset of metastatic lung adenocarcinoma cells, whose phylogeny was resolved using a CRISPR/Cas9-based lineage-tracing system. Together, we anticipate that PhyloVisionand the methods it implements will be a useful resource for scalable and intuitive data exploration for any assay that simultaneously measures cell state and lineage.
