Phenomenological Study: Experience of Voluntary Nurses in Providing Health Services During The COVID-19 Pandemic

Introduction: The COVID-19 pandemic has certainly caused a significant crisis in various groups and populations in the world, especially nurses. The roles and responsibilities that nurses have to play become a dilemma when they not only have to protect themselves, but also their colleagues and their families from this deadly virus infection. This study was to identify the experiences of nurses in providing nursing care to patients during COVID-19 pandemic. Methods: The study used a phenomenological design. Ten participants were selected by purposive sampling. The data were gathered by conducting in-depth interviews and analyzed by Colllaizi’s method. The data were collected in hospitals of the South Tapanuli area. Results: The data were gathered by conducting in-depth interviews and analyzed by Collaizi’s method. The result emerged five themes, namely: 1) Motivation of voluntary nurses to provide health services in the COVID-19 pandemic; 2) voluntary nurse psychological response; 3) problems faced by voluntary nurses; 4) the benefits of being a voluntary nurse during the COVID-19 pandemic; 5) the expectations of voluntary nurses. Conclusion: Voluntary nurses continue to carry out their duties in providing health services in the Covid-19 pandemic even though without any compensation received. The result of this research is recommended to the hospital and government giving the needs of voluntary nurses while providing health services during COVID-19 pandemic.

Comparison of musculoskeletal load using two devices for manual height adjustment of the hospital bed

Objectives. Patient manual handling in bed causes lower back pain (LBP) among Thai nurses. This study aims to develop an extension device for manual height adjustment of the hospital bed, and to evaluate its efficiency in preventing LBP among Thai nurses. Methods. Eleven participants were enrolled for electromyography measurement in eight right muscles. Subsequently, 56 volunteer nurses were recruited by convenience sampling for observational risk assessment by rapid entire body assessment (REBA) and satisfaction evaluation. The characteristics of the participants, percentage of maximum voluntary contraction (%MVC) of each muscle, REBA scores and satisfaction in using the device were analyzed by descriptive statistics, multivariate analysis of variance, paired t test and Wilcoxon signed-rank test, respectively. Results. %MVC values of the six muscles, i.e., biceps brachii, deltoideus, trapezius, latissimus dorsi, erector spinae and hamstring, were found to have a 4–18% decrease. The REBA scores decreased for both left and right when using the extension device, and all participants were more satisfied with the extension device than the hand crank of the hospital bed. Conclusions. The use of the extension device for manual height adjustment of the hospital bed developed from this study helps to prevent LBP among healthcare workers across settings.

From Kossuth’s Twin-Soul to the Nation’s Chief Nurse: the Legacy of Zsuzsanna Kossuth Meszlényi

In this study the personal life and professional career of Zsuzsanna Kossuth, the youngest sister of Lajos Kossuth, is discussed based on primary and secondary sources with special emphasis on her physical and emotional journey into exile, where she was surrounded by both Hungarian emigrés and American intellectuals. By the time she arrived in the United States in 1853, her personal life had been full of ups and downs: she had lost her husband and baby son within a year of each other, spent months in prison twice and had become sick, with only a cough initially, then pneumonia and finally “pulmonary affections.” In spite of the many setbacks she suffered, Kossuth also stands apart for the unusual reason that she had a career: in April, 1849, during the Hungarian War of Independence, she was appointed the Chief Nurse of camp hospitals. Although she has not become as famous as Florence Nightingale, viewed as the founder of modern nursing, Zsuzsanna Kossuth organized seventy-two camp hospitals and a network of volunteer nurses five years previous to the Crimean War. The year 2017 was dedicated in Hungary to her memory commemorating the bicentenary of her birth in particular and to the profession of nursing in general. Her legacy should be promoted globally.

Human Rights Perspectives on Voluntary Health Workers Nurses Who Work at Government Agencies

Nurses as professionals whose existence is recognized by the State have rights that should be fulfilled and protected as professionals and general citizens. The rights that have been guaranteed by various laws and regulations are supposed to remain insufficient to comprehensively accommodate voluntary health worker nurses spreading over the country. Various Indonesian laws and regulations on human rights have guaranteed the rights to work and decent life as well as the rights to recognition, legal certainty, and legal protection. This study aimed to know about the rights to obtain work and decent life viewed from the human rights perspective besides knowing about the guarantee of recognition, legal certainty, and legal protection to voluntary health worker nurses at Bima City, West Nusa Tenggara Province. This study was conducted at Bima City’s General Hospital and Paruga Health Center (Puskesmas). It applied a socio-legal approach and explicative-analytical specification. The data-gathering technique was carried out through interviews with informants, namely the Head of General and Personnel Affairs of Health Office of Bima City, the Head of General and Personnel Affairs of Personnel and Human Resources Office of Bima City, the Head of General and Personnel Affairs of Bima City’s General Hospital and Puskesmas Paruga beside twenty voluntary health worker nurses. The results of this study showed that there was no written legal regulation regarding the procedure of recruiting voluntary health worker nurses in the form of regional regulation nor the Mayor’s Decree that could be used as a legal basis for recruiting voluntary health worker nurses. This led to implications of the fulfillment of the rights to the voluntary health worker nurses, especially in the matters of work and decent life beside guaranteed legal recognition, certainty, and protection.

International Nursing Leadership: The Vietnam Nurse Practice Improvement Project

Leadership occurs between nurses who advocate for partnerships in various health care settings globally. The Vietnam Nurse Project/Vietnam Nurse Practice Improvement Project not only has impacted a group of volunteer nurses in United States, but has contributed to the change of nursing practice to improve patient care in Vietnam. Experiences relating to the differences in the organization of health care leadership between the United States and Vietnam are discussed. This article addresses the impact of the Vietnam Nurse Project/Vietnam Nurse Practice Improvement Project to transform nursing practice in Vietnam and the leadership vision of the nurses in the United States.

Determination of the relationship of the spiritual orientation of nurses with compassion fatigue, burnout, and compassion satisfaction.

This study aimed to determine the relationship of the spiritual orientation of nurses with compassion fatigue, burnout, and compassion satisfaction of nurses working in two large hospitals. This was a descriptive relational study conducted in Atatürk University Research Hospital between April and September 2016. Data were collected from 379 volunteer nurses using the convenience sampling method. A questionnaire, Professional Quality of Life Scale (ProQOL) R-IV and Spiritual Orientation Inventory (SOI) were used to collect the research data. It was found that 55.7% of nurses were aged 18 to 27 years, 74.1% of nurses were females. The professional satisfaction of the nurses was high, with low rates of burnout and compassion fatigue. The spiritual orientation of the nurses was high according to the average SOI total score (98.83 ± 15.04). The mean score for compassion fatigue was 21.50 ± 8.69, and the mean score for compassion satisfaction was 34.10 ± 9.91. The mean score for burnout was 19.83 ± 6.39. Nurses' compassion fatigue, burnout and compassion satisfaction should be evaluated regularly. The results revealed no correlation between spiritual orientation and these parameters. Studies investigating compassion fatigue among nurses should question the time nurses spend on themselves and leisure time.

1914-1917 : Paris et l’ American Ambulance . La philanthropie américaine à l’œuvre

This paper firstly exposes the different types of Military Ambulances (temporary wartime hospitals) which the Americans in France were able to relate to and identify with in 1914. It also focuses on the philanthropic funding and management of these institutions. By 1914 the United States had decided to remain neutral in World War I. Although this decision was embarrassing for the Parisian American Community, the latter behaved in a sovereign way and acted according to their philanthropic habitus to reproduce with improved techniques what had been set up by previous generations during the American Civil War (1861-1865) and the French Paris siege (1870-1871). The American Ambulance Service, located in the premises of the Pasteur High School in Neuilly, was a success. It was led and managed by the experienced officers of the American Hospital who were made up of the elite in the community. Medical services were not sufficient. The American Ambulance Service was also needed to organize the motorized transportation of the (French and British) wounded, which meant collaborating with ambulance drivers and American drivers working for American universities (American Ambulance Field Service) ; Chief Officers, Managers, General Practitioners, Surgeons and volunteer nurses were all American, the technical staff was American, the funding (including what came from lower classes) was American. Efficiency was guaranteed. Between 1914 and 1917, the American Hospital, the American Ambulance and the American Ambulance Field Service were unique opportunities to observe American philanthropy abroad deploying in total liberty.

The Local Health Care System of the Province of Ilocos Sur

Health is fundamental to life. It is in this premise that efforts are being undertaken to improve the health system of every country. The focus of the study was on the different components of the Local Health Care System and the quality of health services of selected Rural Health Units and District Hospitals in the province of Ilocos Sur. The study revealed that the Administrative Capability along leadership and good governance is “moderate”. Along the capability of health personnel in terms of ratio to total population, only two municipalities have the ideal number of doctors and nurses while only five municipalities have an ideal number of midwives. There is also a shortage of human resources for health, particularly doctors, in almost all the district hospitals. However, the limited number of nurses and midwives is compensated by the presence of contractual and volunteer nurses and midwives. In general, the clients expressed their satisfaction on the services provided. Suggestions were made largely on availability of free medicines, laboratory services, reduction of waiting time, and improvement of facilities. Efforts should be undertaken to ensure that programs and policies are implemented and objectives are reached through improved coordination across local health systems. Nurses Workload Comparison Between Voluntary Nurses and Government Employee Nurses

Introduction: Padangsidimpuan General Hospital is a hospital owned by the Padangsidimpuan City government which employs Voluntary Nurses and government Employee Nurses. The purpose of this study is to determine the workload comparison of Voluntary Nurses and Government Employee Nurses. Methods: Research was conducted using quantitatif and qualitative methods using observation techniques work sampling, and interview guidelines. Partisipant has been observed were 15 Voluntary Nurses and 15 Government Employee nurses who are on duty at that time by using accidental sampling technique. Samples taken were all activities carried out by Voluntary Nurses and Government Employee Nurses, such as direct nursing activities, indirect nursing activities, personal activities, and non-productive activities. Results: The results of observations and interviews described there is difference between workload of voluntary nurses with government employee nurses. After analysed using Collaizi’s Methode and The mannwhitney test on the workload of nurses Voluntary Nurses and Government Employee Nurses found a significant difference with the value of p = 0.0001 (p <0.05) which means there is a difference in workload between Voluntary Nurses and Government Employee Nurses. With the workload on Voluntary nurses amounted to 74.8%, while the Government Employee nurses amounted to 69.6%. The results of the study suggest that the management of Padangsidimpuan General Hospital to manage nursing staff optimally to achieve quality nursing services. Conclusions: There is difference in workload between Voluntary nurses and government employee nurses.

Developing medical oncology in Africa.

BackgroundKenya, like most other developing countries, is undergoing an epidemiologic shift of disease patterns characterized by an increasing prevalence of cancer and other non-communicable diseases straining health care resources which were mainly intended for communicable diseases. We describe the development of sustainable cancer prevention and control programs at Moi University and Moi Teaching and Referral Hospital in Western Kenya.MethodsThe cancer prevention and control program at Moi Teaching and Referral Hospital was started by volunteer nurses and clinicians in 2005 in response to a high prevalence of AIDS-related Kaposi sarcoma. Chemotherapy was donated by a local drug store until 2007 when Eli Lilly pharmaceuticals from Indianapolis in the USA started helping the program through the Academic Model Providing Access to Healthcare (AMPATH). Due to good response rates of patients with AIDS-related Kaposi sarcoma, and lobbying by volunteers, the service became a formal department of Moi Teaching and Referral hospital in 2008.ResultsThe department has now grown to become the second largest public cancer centre in the country registering about 9000 patient visits per year. In addition, staff have now specialized in various areas such as medical oncology, palliative care, surgical oncology, nursing and gynaecology oncology.ConclusionsThe development of a medical oncology program requires a multi-disciplinary team focused on integration within existing programs and expansion of collaborative networks in order to provide the best care to patients.

Effect of internal & external motivation and Burnout levels of nurses on work performances


 
 
 
 The crucial role of delivering health care services belongs to the health care providers. Among them, nurses have an important place both in regards to their numbers and the burden they take on. Therefore, the purpose of this research is to understand the relations between their work performances and their burnout level in conjunction with their internal& external motivation. The research is a cross-sectional observational study. Surveys were distributed to 600 nurses across the Asian part of ?stanbul among 4 hospitals. The study included the volunteer nurses who were actively working in the institutions, who were given permission to participate in the study, except for the nurses who were on leave/had a report during the data collection period. While collecting data, burnout, motivation and workers’ perceived performance scales were used. 458 nurses completed the surveys out of 600 distributed, of which 439 were deemed to be appropriate to be included in the analysis. All the data were analyzed at SPSS 22.0 version, with the mean, standard deviation, range and such descriptive statistics being used with p<0.05 significance level. Analyzes demonstrated a statistically significant negative correlation between burnout and perceived performance (p<0.01, r=-,422). On the other hand, the analyzes also demonstrated a positive but statistically significant and low-level correlation between motivation (internal and external) and perceived performance (p<0.01, r=,173; 226). Burnout caused by independent variables has a statistically significant and negative correlation between both internal and external motivation (p<0.01). Perceived work performance of nurses, one of the most important factors in delivering health care services, can be elevated by decreasing burnout level and by increasing their internal/external motivation. So, it is important for administrations to take appropriate measures to lower the burnout level and increase the motivation of nurses.
 
 
 

Legal Protection for Nurses in Non Civil Servants in Health Centers in Langgudu Sub-District Bima Regency After the Enactment of Law Number 38 the Year 2014 Concerning Nursing

Local Government Clinic in Langgudu Subdistrict, Bima Regency, is one of the Government institutions that employ health and non-health workers. One of the health workers themselves is nurses. Nurses who practiced at the clinic have recruited both nurses who have the status of civil servants, impermanent officers, and voluntary nurses. The voluntary nurses who were recruited and appointed by the head of the clinic. On the other hand, the recruitment in the local government should comply with the legislation of the state civil apparatus. As the recruitment is not by the legislation of the state civil apparatus, it will have an impact on the legal protection for the nurses in a local government clinic. This research was done at a clinic in Langgudu subdistrict of Bima Regency using sociology juridical research method. The specification of the research was descriptive analytic using primary and secondary data. Data collection techniques used in the research was face to face interview, question draft distribution, and interview by phone. The Information or resource of this research was the head of the health official, heads of clinics in Langgudu subdistrict and the chairman of Indonesian National Nurses Organization of Bima Regency. Then, The respondents were the nurses who work at the clinic in Langgudu Subdistrict, either nurse who have the status of civil servant, local honorary workers or the voluntary nurses. The research result found that the regulation of employee appointment to work in a government institution was set in regent’s regulation number 15A about appointment guideline, placement, Dismissal, and honorary workers discipline of local government. In the regulation, the person who has authority in appointing the employees is the regent as the builder of the officers. In implementing of the regulation the head of the Clinic appointsnurse workers by himself to employ at the clinic. Thus, such things have already broken authority as leader of the institution in the sphere of local government as it is not by the rules of the Regent of Bima. Therefore, such appointments an obstacle in giving protection for voluntary Nurses because the process of appointment is not appropriate and breaking authority. As a head of clinics is not given authority and should not recruit or appoint the workers to employ at government institutions. The appointment of the workers shows that understanding of heads of clinics about management of the worker appointment at the clinics still lack, in addition to the absence of legislation regulations managing the volunteers so that it is an obstacle in providing legal protection for voluntary nurses who work in a clinic.

Impact of training giving to peadiatric nurses by interactive workshops against to problems during lactation period

Aim: The study has been conducted in order to determine the impact of training which giving to peadiatric nurses by interactive workshops against to problems during lactation period.Methods: This study, which is an experimental type in which pre-test/ post-test measurements were taken, was carried out in a state hospital in Samsun in October 2014. Before the training, the nurses accepted the presentation invitation. Evaluation of participants’ knowledge on the subject was performed via a Multiple Choice Questionnaire (MCQ) prepared by the researchers in relation with the literature. The study was conducted by 33 volunteer nurses working in the pediatric departments of a state hospital in the conference hall. MCQ, consisting of 24 questions, was answered by the participants before and after the interactive workshop. Analyses were performed using commercial software (IBM SPSS Statistics 20, SPSS Inc. An IBM Corp., Armonk, NY)Findings: Nurses (n= 33) who work in pediatric units participated in the study. The average age of nurses who participated to study is 36.30 ± 6.47 (min=21 max=48). There was a significant improvement in mean test score after the lecture when compared with pre-lecture score (Mean =23.66, SD=0.54 vs. Mean=12.24, SD=4.11, z= -5.021 p = 0.000)Conclusions: Lecture based interactive workshop on the problems faced during lactation period helps to improve nurses’ knowledge. It also helps in overcoming deficiencies in nurses training.

Dual Defibrillation is Highly Variable: An Analysis of Pulse Interval Delivered in Dual Defibrillation

Background: Dual defibrillation (DD) is a technique where two external defibrillators are applied with two different pad configurations and discharged to treat refractory ventricular fibrillation (RVF). Although commonly called dual sequential defibrillation (DSD), if the delivered electrical pulses overlap with no pulse interval, the shocks are actually dual simultaneous defibrillation (DSiD). Manual DD technique is not standardized and the effect that the method of activation has on the delivered pulse interval has never been studied. Objectives: This study measured the timing of four methods of DD and the resulting inter-shock intervals, frequency with which they were either DSiD or DSD, and frequency which the true DSDs delivered any previously reported optimum pulse interval. Methods: This was a single-blinded prospective evaluation of a convenience sample of volunteer physicians, nurses, and paramedics each performing DD in our simulation center on two types of defibrillators using four techniques: single operator-simultaneous with 2 hands (SOSI), two operators-simultaneous (TOSI), single operator-sequential with 1 hand (SOSE1), and single operator-sequential with 2 hands (SOSE2). Results: The four DD methods generated a variable set of pulse intervals depending on the technique and defibrillator employed. The pulse intervals ranged from 0 msec (i.e., overlapping waveforms or DSiD) to 1800 msec. Of all DD attempts, 85.9% met the definition of DSD, 14.1% were DSiD, and 49.4% delivered any one of the optimum pulse intervals previously described in the literature. SOSI and TOSI techniques resulted in DSD between 47.2 and 87.6% of the time, depending on the technique and defibrillator. Shocks delivered sequentially on purpose (SOSE2 and SOSE1) were always DSD but with widely variable pulse intervals. SOSI resulted in the shortest pulse intervals, SOSE1 resulted in the longest, and TOSI and SOSE2 were the least skewed. Conclusion: DD using the various methods currently employed produces a highly variable set of pulse intervals even within a single method. It is difficult to reach a conclusion about the efficacy of DD unless the delivered pulse interval is measured or the method of activation reproducibly produces a precise pulse interval.

‘The lure of war’: reconsidering the motivations of nurses to volunteer in the First World War

ABSTRACTThis article argues that the motivations for British women to volunteer for the First World War were more nuanced and complicated than the formulaic binaries of patriotism versus pacifism. It reads the war-time memoirs of two women in military medical care, May Sinclair’s A Journal of Impressions in Belgium and Olive Dent’s A Volunteer Nurse on the Western Front to demonstrate how understandings of gender roles and nationalist affiliations rendered complexity to the reasons why certain women volunteered for war-work. These two women volunteered very early in the war and published their life-writing during the war (1915 and 1917 respectively). Consequently, they did not have the advantage of hindsight, and their writings were the product of the immediate pressures of the war environment. By reading the memoirs of these women and unpacking their overt motives to volunteer, this article reveals the nuances in the reasons women volunteered to engage in military medical work during the First World War.

"¡Viva La Clinica!": The United Farm Workers' Fight for Medical Care.

Farm work is among the most dangerous and unhealthy occupations in the United States, but efforts to address farm workers' health needs have been sporadic and inadequate. In the 1960s and 1970s, the United Farm Workers of America (UFW) famously organized workers in California's grape and lettuce fields and won national and international recognition through its boycott tactics. The UFW also opened several medical clinics, staffed by volunteer nurses and physicians, and created the Robert F. Kennedy Medical Plan, a medical insurance program, for its members. Both efforts were initially successful, but foundered in the face of many obstacles, including the reluctance of the UFW leadership to organize undocumented farm workers. However, the UFW's medical work laid the foundation for continuing efforts on behalf of farm workers' and undocumented people's right to health care.

Safeguarding older adults with dementia, depression, and delirium in a temporary disaster shelter.

Natural disasters impact people of every age in the communities where they occur, with older adults being a vulnerable subset of the population. Most disaster shelter volunteer nurses are experienced in addressing common health needs of older adult clients such as diabetes, hypertension, and pulmonary disease. These nurses also have the requisite training to respond to more acute medical events, including the symptoms of a heart attack or stroke. They provide care and comfort to those suffering from the distress, anxiety, and fear caused by disasters. However, they may be less adept at triaging and caring for older adults with mental health conditions such as delirium, depression, or dementia. The trauma associated with a disaster and relocation will challenge cognitive abilities in those with dementia, may exacerbate existing depression, or lead to the onset of delirium, which is a medical emergency. Older adults experiencing these conditions are at risk for harm and deterioration with serious short and long-term consequences. Since disaster shelter volunteer health care staff may not be well-versed in distinguishing between dementia, depression, or delirium, behavior observation, and safety considerations are critical determinants of whether it is possible to support the older adult in the shelter environment or it is necessary to transition to a higher level of care.

Mobile Decision Support Tool for Emergency Departments and Mass Casualty Incidents (EDIT): Initial Study.

BackgroundChemical exposures pose a significant threat to life. A rapid assessment by first responders and emergency nurses is required to reduce death and disability. Currently, no informatics tools exist to process victims of chemical exposures efficiently. The surge of patients into a hospital emergency department during a mass casualty incident creates additional stress on an already overburdened system, potentially placing patients at risk and challenging staff to process patients for appropriate care and treatment efficacy. Traditional emergency department triage models are oversimplified during highly stressed mass casualty incident scenarios in which there is little margin for error. Emerging mobile technology could alleviate the burden placed on nurses by allowing the freedom to move about the emergency department and stay connected to a decision support system.ObjectiveThis study aims to present and evaluate a new mobile tool for assisting emergency department personnel in patient management and triage during a chemical mass casualty incident.MethodsOver 500 volunteer nurses, students, and first responders were recruited for a study involving a simulated chemical mass casualty incident. During the exercise, a mobile application was used to collect patient data through a kiosk system. Nurses also received tablets where they could review patient information and choose recommendations from a decision support system. Data collected was analyzed on the efficiency of the app to obtain patient data and on nurse agreement with the decision support system.ResultsOf the 296 participants, 96.3% (288/296) of the patients completed the kiosk system with an average time of 3 minutes, 22 seconds. Average time to complete the entire triage process was 5 minutes, 34 seconds. Analysis of the data also showed strong agreement among nurses regarding the app’s decision support system. Overall, nurses agreed with the system 91.6% (262/286) of the time when it came to choose an exposure level and 84.3% (241/286) of the time when selecting an action.ConclusionsThe app reliably demonstrated the ability to collect patient data through a self-service kiosk system thus reducing the burden on hospital resources. Also, the mobile technology allowed nurses the freedom to triage patients on the go while staying connected to a decision support system in which they felt would give reliable recommendations.
