Reliability and validity of a novel cognitive self-assessment tool for patients with cancer.

Subjective cognitive function is an important outcome measure in oncology. The Functional Assessment of Cancer Therapy-Brain (FACT-Br) is a quality of life (QoL) measure that includes indices of physical, emotional, social, and neurologic aspects of disease but does not measure cognitive function. This study seeks to validate a novel index of cognition derived from the FACT-Br. Patients with heterogeneous cancer diagnoses (N = 214) completed neuropsychological evaluation and self-report measures. Nine FACT-Br items regarding cognition were combined to form the FACT-Br-cognitive index (CI). Reliability was evaluated by exploratory factor analysis and internal consistency. Concurrent validity was assessed by correlating FACT-Br-CI with the Patient-Reported Outcomes Measurement Information System (PROMIS) Cognitive-8 scales. Discriminant validity was assessed by correlating FACT-Br-CI with other FACT-Br indices and the Beck Depression and Anxiety Inventories (BDI, BAI). Exploratory analyses evaluated the impact of cognitive performance and disease variables on FACT-Br-CI. The FACT-Br-CI consisted of a single factor that demonstrated high internal consistency (α = 0.867) and strong concurrent validity, correlating strongly with PROMIS Cognitive-8 scales (r = 0.675-0.782). The relationship between the FACT-Br-CI and other FACT subscales ranged from moderate to strong (r = 0.372-0.601), as did correlations with measures of depression (BDI, r = -0.621) and anxiety (BAI, r = -0.450). Modest correlations were observed with neuropsychological measures (rs = 0.249-0.300). The FACT-Br-CI is a reliable and valid measure of self-reported cognition. Studies that included the FACT-Br could be retrospectively analyzed to assess subjective cognitive outcomes, enriching the information from prior research. Integration of the FACT-Br-CI in routine clinical care may be an efficient method of monitoring cognition.

Change by challenge: A common genetic basis behind childhood cognitive development and cognitive training

The interplay of genetic and environmental factors behind cognitive development has preoccupied multiple fields of science and sparked heated debates over the decades. Here we tested the hypothesis that developmental genes rely heavily on cognitive challenges—as opposed to natural maturation. Starting with a polygenic score (cogPGS) that previously explained variation in cognitive performance in adults, we estimated its effect in 344 children and adolescents (mean age of 12 years old, ranging from 6 to 25) who showed changes in working memory (WM) in two distinct samples: (1) a developmental sample showing significant WM gains after 2 years of typical, age-related development, and (2) a training sample showing significant, experimentally-induced WM gains after 25 days of an intense WM training. We found that the same genetic factor, cogPGS, significantly explained the amount of WM gain in both samples. And there was no interaction of cogPGS with sample, suggesting that those genetic factors are neutral to whether the WM gains came from development or training. These results represent evidence that cognitive challenges are a central piece in the gene-environment interplay during cognitive development. We believe our study sheds new light on previous findings of interindividual differences in education (rich-get-richer and compensation effects), brain plasticity in children, and the heritability increase of intelligence across the lifespan.

Two variations and one similarity in memory functions deployed by mice and humans to support foraging.

Assessing variations in cognitive function between humans and animals is vital for understanding the idiosyncrasies of human cognition and for refining animal models of human brain function and disease. We determined memory functions deployed by mice and humans to support foraging with a search task acting as a test battery. Mice searched for food from the top of poles within an open arena. Poles were divided into groups based on visual cues and baited according to different schedules. White and black poles were baited in alternate trials. Striped poles were never baited. The requirement of the task was to find all baits in each trial. Mice's foraging efficiency, defined as the number of poles visited before all baits were retrieved, improved with practice. Mice learnt to avoid visiting unbaited poles across trials (long-term memory) and revisits to poles within each trial (working memory). Humans tested with a virtual reality version of the task outperformed mice in foraging efficiency, working memory, and exploitation of the temporal pattern of rewards across trials. Moreover, humans, but not mice, reduced the number of possible movement sequences used to search the set of poles. For these measures, interspecies differences were maintained throughout the 3 weeks of testing. By contrast, long-term memory for never-rewarded poles was similar in mice and humans after the first week of testing. These results indicate that human cognitive functions relying on archaic brain structures may be adequately modelled in mice. Conversely, modelling in mice fluid skills likely to have developed specifically in primates requires caution.

Age-related differences in network structure and dynamic synchrony of cognitive control

Cognitive trajectories vary greatly across older individuals, and the neural mechanisms underlying these differences remain poorly understood. Here, we investigate the cognitive variability in older adults by linking the influence of white matter microstructure on the task-related organization of fast and effective communications between brain regions. Using diffusion tensor imaging and electroencephalography, we show that individual differences in white matter network organization are associated with network clustering and efficiency in the alpha and high-gamma bands, and that functional network dynamics partly explain individual differences in cognitive control performance in older adults. We show that older individuals with high versus low structural network clustering differ in task-related network dynamics and cognitive performance. These findings were corroborated by investigating magnetoencephalography networks in an independent dataset. This multimodal (fMRI and biological markers) brain connectivity framework of individual differences provides a holistic account of how differences in white matter microstructure underlie age-related variability in dynamic network organization and cognitive performance.

Association Between Midlife Obesity and Its Metabolic Consequences, Cerebrovascular Disease, and Cognitive Decline.

Chronic obesity is associated with several complications, including cognitive impairment and dementia. However, we have only piecemeal knowledge of the mechanisms linking obesity to central nervous system damage. Among candidate mechanisms are other elements of obesity-associated metabolic syndrome, such as hypertension, dyslipidemia, and diabetes, but also systemic inflammation. While there have been several neuroimaging studies linking adiposity to changes in brain morphometry, a comprehensive investigation of the relationship has so far not been done. To identify links between adiposity and cognitive dysfunction. This observational cohort study (UK Biobank), with an 8-year follow-up, included more than 20 000 participants from the general community, with a mean age of 63 years. Only participants with data available on both baseline and follow-up timepoints were included. The main outcome measures were cognitive performance and mediator variables: hypertension, diabetes, systemic inflammation, dyslipidemia, gray matter measures, and cerebrovascular disease (volume of white matter hyperintensities on magnetic resonance imaging). Using structural equation modeling, we found that body mass index, waist-to-hip ratio, and body fat percentage were positively related to higher plasma C-reactive protein, dyslipidemia, hypertension, and diabetes. In turn, hypertension and diabetes were related to cerebrovascular disease. Finally, cerebrovascular disease was associated with lower cortical thickness and volume and higher subcortical volumes, but also cognitive deficits (largest significant pcorrected = 0.02). We show that adiposity is related to poor cognition, with metabolic consequences of obesity and cerebrovascular disease as potential mediators. The outcomes have clinical implications, supporting a role for the management of adiposity in the prevention of late-life dementia and cognitive decline.

Intraindividual variability in gait and cognitive performance is distinct from central-tendency measures and associated with clinical outcomes in aging. Knowledge concerning intraindividual variability in neural activity, however, has been relatively scarce, and no research to date has reported on such variability during active walking. The current study addressed this major gap in knowledge. Participants were community-residing older adults (n = 394; mean age = 76.29 ± 6.65 years; %female = 55). Functional near-infrared spectroscopy (fNIRS) was used to measure oxygenated hemoglobin (HbO2) in the prefrontal cortex under three experimental conditions: single-task-walk, single-task-alpha (cognitive task), and dual-task-walk, which required the participants to perform the two single tasks simultaneously. Intraindividual variability in neural activity was operationalized using the standard deviation of fNIRS-derived HbO2 observations assessed during a 30-s interval in each experimental condition. The increase in intraindividual variability in neural activity in the dual-task-walk condition compared to both single-task conditions was associated with the presence of cognitive impairments and being a male. Furthermore, measures of intraindividual variability in neural activity and gait performance were positively correlated only under the dual-task-walk condition. Intraindividual variability in the neural activity of gait may be a novel marker for age-related impairments in mobility and cognitive function. A framework for identification of a resting-bold connectome associated with cognitive reserve

The concept of cognitive reserve proposes that specific life experiences result in more flexible or resilient cognitive processing allowing some people to cope better with age- or disease-related brain changes than others. Imaging studies seeking to understand the neural implementation of cognitive reserve have most often used task-related fMRI studies. Using that approach, we recently described a task-invariant cognitive-reserve network whose expression correlated with IQ and that moderated between cortical thickness and cognitive performance. Here we sought to identify a pattern of resting BOLD connectivity related to cognitive reserve. We identified a connectome pattern whose connectivity correlated with IQ in both the derivation sample and a separate replication sample. The majority of the edges showing positive relationships with IQ implicate frontal regions. In the derivation sample, connectivity either moderated the relationship between mean cortical thickness and a set of cognitive outcomes or accounted for unique variance in cognitive performance after accounting for cortical thickness. In a replication sample we found that expression of this connectome correlated significantly with the primary endpoint of IQ, and also accounted for unique variance in cognitive performance beyond cortical thickness. Our findings represent an intermediate level of replication and are unlikely to have arisen purely by type-I error. This connectivity pattern therefore meets some of our theoretical criteria for a cognitive reserve-related network and provides insight into the neural implementation of cognitive reserve. Further, expression of this connectome could potentially be used as a direct measure of cognitive reserve, and as an outcome measure for intervention studies that seek to influence cognitive reserve. Future validation of and re-derivation of the pattern in expanded data sets by our and other groups will lead to further improved estimates of cognitive reserve in resting functional connectivity.

Effect of cognitive reserve on structural and functional MRI measures in healthy subjects: a multiparametric assessment.

Cognitive reserve (CR) contributes to inter-individual variability of cognitive performance and to preserve cognitive functioning facing aging and brain damage. However, brain anatomical and functional substrates of CR still need to be fully explored in young healthy subjects (HS). By evaluating a relatively large cohort of young HS, we investigated the associations between CR and structural and functional magnetic resonance imaging (MRI) measures in early adulthood. A global Cognitive Reserve Index (CRI), combining intelligence quotient, leisure activities and education, was measured from 77 HS and its brain anatomical and functional substrates were evaluated through a multiparametric MRI approach. Substrates of the three subdomains (cognitive/social/physical) of leisure activities were also explored. Higher global and subdomain CRIs were associated with higher gray matter volume of brain regions involved in motor and cognitive functions, such as the right (R) supplementary motor area, left (L) middle frontal gyrus and L cerebellum. No correlation with measures of white matter (WM) integrity was found. Higher global and subdomains CRIs were associated with lower resting-state functional connectivity (RS FC) of L postcentral gyrus and R insula in sensorimotor network, L postcentral gyrus in salience network and R cerebellum in the executive-control network. Moreover, several CRIs were also associated with higher RS FC of R cuneus in default-mode network. CR modulates structure and function of several brain motor and cognitive networks responsible for complex cognitive functioning already in young HS. CR could promote optimization of the recruitment of brain networks.

Development, validation, and feasibility of a school-based short duration integrated classroom yoga module: A pilot study design.

BACKGROUND:The practice of yoga is proven to have physical, cognitive and emotional benefits for school children. Despite this many schools do not include yoga in their daily schedule. The reasons cited are lack of time and resources. To overcome these problems the present study aimed to develop and validate a short duration Integrated classroom yoga module. The design guidelines were that it should be possible to practice in the classroom environment and that it could be led by the class teacher. In this way the module would overcome the problem of both time and resource.MATERIALS AND METHODS:The study had two main phases. In the first phase, selected ICYM practices based on the literature review were validated by 21 subject matter experts using Lawhse's content validity ratio (CVR) formula. In the second phase, a pilot study using a paired sample prepost measurement design was carried out on 49 high school children. The study was conducted in June 2019. The intervention period was 1 month, and the test variables were physical fitness, cognitive performance, self-esteem, emotional well-being, and personality characteristic. Paired sample t-test was the analysis tool and the software used was the Statistical Package for the Social Science version 26.RESULTS:In the Lawshe's CVR analysis, 17 out of the 24 practices tested were rated by experts as essential as was the overall module (CVR score ≥0.429). In the pilot study, there were significant differences in the postmean scores compared to premean scores, for all the 4 EUROFIT physical fitness testing battery tests (P < 0.02), all the three scores of the Stroop color-word naming task (P < 0.001) and the Rosenberg self-esteem scale (P < 0.008).CONCLUSION:ICYM was validated and found feasible by the present study. It was found to have a statistically significant impact on physical fitness, cognitive performance, and self-esteem variables. However, a randomized control trial with a longer intervention period is needed to strengthen the present study.

Effects of Different Doses of Caffeinated Coffee on Muscular Endurance, Cognitive Performance, and Cardiac Autonomic Modulation in Caffeine Naive Female Athletes

Caffeine is widely consumed among elite athletes for its well-known ergogenic properties, and its ability to increase exercise performance. However, studies to date have predominantly focused on the anhydrous form of caffeine in male participants. The aim of the study was to investigate the effect of caffeinated coffee ingestion on lower-upper body muscular endurance, cognitive performance, and heart rate variability (HRV) in female athletes. A total of 17 participants (mean ± standard deviation (SD): age = 23 ± 2 years, body mass = 64 ± 4 kg, height = 168 ± 3 cm) in a randomized cross-over design completed three testing sessions, following the ingestion of 3 mg/kg/bm of caffeine (3COF), 6 mg/kg/bm of caffeine (6COF) provided from coffee or decaffeinated coffee (PLA) in 600 mL of hot water. The testing results included: (1) repetition number for muscular endurance performance; (2): reaction time and response accuracy for cognitive performance; (3): HRV parameters, such as standard deviation of normal-to-normal (NN) intervals (SDNN), standard deviation of successive differences (SDSD), root mean square of successive differences (RMSSD), total power (TP), the ratio of low- and high-frequency powers (LF/HF), high-frequency power (HF), normalized HF (HFnu), low-frequency power (LF), and normalized LF (LFnu). A one-way repeated measures ANOVA revealed that 3COF (p = 0.024) and 6COF (p = 0.036) improved lower body muscular endurance in the first set as well as cognitive performance (p = 0.025, p = 0.035 in the post-test, respectively) compared to PLA. However, no differences were detected between trials for upper body muscular endurance (p = 0.07). Lastly, all HRV parameters did not change between trials (p > 0.05). In conclusion, ingesting caffeinated coffee improved lower body muscular endurance and cognitive performance, while not adversely affecting cardiac autonomic function.

Cognitive Reserve and Well-Being Among Centenarians

The purpose of this study was to assess cognitive reserve among centenarians and to evaluate whether reserve variables explain significantly more variance in cognitive functioning than education alone. Centenarians from the Georgia Centenarians Study were included. Results indicate that education, activity, social engagement, and engaged lifestyle significantly related to cognitive functioning. After adding cognitive reserve variables, the effect of education on cognitive functioning diminished. The overall model fit well, Χ2 (df=6) = 12.35, p = .06; CFI = .97, RMSEA = .067. Education indirectly related to cognitive functioning through occupational responsibility and engaged lifestyle. Social engagement directly related to cognitive functioning but also indirectly through activity levels. The four direct predictors (i.e., education, social engagement, activity, and engaged lifestyle) explained 33 percent of the variance in cognitive functioning. The results provide important information for cognitive reserve theories with implications for physical health and interventions that build cognitive reserves.

Higher Awareness of Positive and Negative Age-Related Changes Relate to Lower Objective Cognitive Scores

Existing evidence suggests that individuals’ subjective experience of cognitive decline may be a risk state for dementia. However, whether self-awareness of positive changes confer cognitive protection is unknown. We examined the extent to which awareness of positive (AARC gains) and negative (AARC losses) age-related changes explains variability in objective cognitive performance in a sample of 6,231 UK residents (Mean age= 66.1 years, 75.9% women) without cognitive impairment. We tested a structural equation model with AARC gains and losses as predictors of cognitive performance and depressive symptoms as a mediator of the association of AARC losses with cognitive performance. The model fit the data well. The correlation between AARC gains and losses was negligible, yet higher levels of both AARC gains and losses predicted poorer cognitive scores. Hence, higher AARC gains did not confer cognitive protection. This unexpected pattern of results underscores the complexity of mapping individuals’ awareness onto objective outcomes.

Variability in working memory performance on mobile devices is sensitive to mild cognitive impairment: Results from the Einstein Aging Study

AbstractBackgroundMost prior research on distinguishing cognitive impairment from healthy controls focuses on measuring mean‐level of cognitive performance on one or relatively few assessments, ignoring potentially valuable information about variability. Within‐person variability in cognitive performance is emerging as a promising indicator of mild cognitive impairment (MCI) and dementia. The limited research on this topic, however, typically examines speeded responses and variability transpiring across moments during a single test administration. The current study extends prior work by using ecological momentary assessments (EMA) to examine cognitive status differences in variability across hours and days in three dimensions of working memory (WM): object‐location binding, object‐feature binding, and spatial WM.MethodIn a sample of 243 systematically recruited, community‐dwelling adults age 70+ from the Einstein Aging Study (Mean age=77.46 years, SD=5.13), we identified 68 individuals meeting Jak‐Bondi criteria for MCI and 175 cognitively normal controls (Table 1). All participants performed object‐location binding, object‐feature binding, and spatial WM tasks on a smartphone up to four times daily for 14 days, totaling up to 56 assessments per person. We employed heterogeneous variance models using log‐linear prediction of residual variance to simultaneously assess MCI differences in mean performance and within‐person variability. Models adjusted for linear trends across assessments and study days, as well as age, education, gender, and ethnicity (Table 2; Figure 1).ResultsFor mean performance, individuals with MCI exhibited worse performance than healthy controls in measures of object‐location binding (Est.=0.14, SE=0.02, p&lt;.001), object‐feature binding (Est.=‐0.27, SE=0.04, p&lt;.001), and spatial WM (Est.=0.90, SE=0.20, p&lt;.001). For variability across assessments, individuals with MCI exhibited greater within‐person variability than controls in object‐location binding (Est.=0.89, SE=0.03, p&lt;.001) and object‐feature binding (Est.=0.37, SE=0.03, p&lt;.001) performance. Variability in spatial WM performance did not vary as a function of cognitive status (Est.=0.02, SE=0.03, p=.63).ConclusionResults suggest both mean and variability in performance are sensitive to MCI and exhibit dissociative patterns by WM dimension. Variability in object‐location and object‐feature binding performance may provide specific detection of MCI. The 14‐day smartphone‐based EMA offers novel opportunity to leverage performance variability across frequent assessments for identifying early clinical manifestations of cognitive impairment.

Higher perceived age‐related gains and losses relate to lower objective cognitive scores

AbstractBackgroundAwareness of age‐related changes (AARC), including perceived losses (e.g., memory decline) and gains (e.g., wisdom), comprise an important component of the aging process. Indeed, subjective perceptions of aging and health can reveal critical information not captured by objective measures. In particular, growing biomarker evidence suggests that subjective cognitive decline (SCD) may be a risk state for dementia. However, whether perceived age‐related gains confer cognitive protection is not known. We examine the extent to which perceived age‐related gains and losses explain variability in cognitive performance across three stages of older adulthood.MethodWe used cross‐sectional data from the PROTECT cohort (assessment in 2019); 6,377 UK residents (age range= 51‐95 years, mean age= 66.1 years, 75.9% women) without dementia completed measures of depression, AARC gains and losses, and undertook cognitive tasks. Structural equation modeling (SEM) was used to test whether AARC gains and losses predict scores on objective cognitive tasks. In the model, we included depressive symptoms as a mediator of the associations of AARC losses with scores on cognitive tasks. We also controlled for potential confounders. We tested the same model among three sub‐samples of individuals aged 50 to 60 (N= 1,628), 61 to 70 (N= 3,076), and 71 and over (N= 1,673).ResultThe model fit the data well among all three groups. The correlation between AARC gains and losses was negligible, yet higher levels of both predicted poorer cognitive scores among all groups. Associations of gains and losses with verbal reasoning were small to moderate, whereas associations with remaining cognitive tasks were of negligible size. Depressive symptoms mediated the effect of AARC losses on verbal reasoning in the two older groups.ConclusionAs hypothesized, individuals with higher perceived losses demonstrated poorer cognition. In contrast, higher perceived gains did not confer protection; rather, perceived gains were also associated with lower cognitive performance. This unexpected pattern of results, seen across all age groups, underscores the complexity of mapping perceived experiences onto objective outcomes, and emphasizes the importance of considering subjective aging more broadly when refining the clinical construct of SCD as a reliable marker of cognitive decline.

Structure-function coupling in the human connectome: A machine learning approach

While the function of most biological systems is tightly constrained by their structure, current evidence suggests that coupling between the structure and function of brain networks is relatively modest. We aimed to investigate whether the modest coupling between connectome structure and function is a fundamental property of nervous systems or a limitation of current brain network models. We developed a new deep learning framework to predict an individual's brain function from their structural connectome, achieving prediction accuracies that substantially exceeded state-of-the-art biophysical models (group: R=0.9±0.1, individual: R=0.55±0.1). Crucially, brain function predicted from an individual's structural connectome explained significant inter-individual variation in cognitive performance. Our results suggest that structure-function coupling in human brain networks is substantially tighter than previously suggested. We establish the margin by which current brain network models can be improved and demonstrate how deep learning can facilitate investigation of relations between brain function and behavior.

Grip Strength, Neurocognition, and Social Functioning in People WithType-2 Diabetes Mellitus, Major Depressive Disorder, Bipolar Disorder, and Schizophrenia.

Background: Frailty is a common syndrome among older adults and patients with several comorbidities. Grip strength (GS) is a representative parameter of frailty because it is a valid indicator of current and long-term physical conditions in the general population and patients with severe mental illnesses (SMIs). Physical and cognitive capacities of people with SMIs are usually impaired; however, their relationship with frailty or social functioning have not been studied to date. The current study aimed to determine if GS is a valid predictor of changes in cognitive performance and social functioning in patients with type-2 diabetes mellitus and SMIs. Methods: Assessments of social functioning, cognitive performance, and GS (measured with an electronic dynamometer) were conducted in 30 outpatients with type 2 diabetes mellitus, 35 with major depressive disorder, 42 with bipolar disorder, 30 with schizophrenia, and 28 healthy controls, twice during 1-year, follow-up period. Descriptive analyses were conducted using a one-way analysis of variance for continuous variables and the chi-squared test for categorical variables. Differences between groups for the motor, cognitive, and social variables at T1 and T2 were assessed using a one-way analysis of covariance, with sex and age as co-variates (p < 0.01). To test the predictive capacity of GS at baseline to explain the variance in cognitive performance and social functioning at T2, a linear regression analysis was performed (p < 0.05). Results: Predictive relationships were found among GS when implicated with clinical, cognitive, and social variables. These relationships explained changes in cognitive performance after one year of follow-up; the variability percentage was 67.7%, in patients with type-2 diabetes mellitus and 89.1% in patients with schizophrenia. Baseline GS along with other variables, also predicted changes in social functioning in major depressive disorder, bipolar disorder, and schizophrenia, with variability percentages of 67.3, 36, and 59%, respectively. Conclusion: GS combined with other variables significantly predicted changes in cognitive performance and social functioning in people with SMIs or type-2 diabetes mellitus. Interventions aimed to improve the overall physical conditions of patients who have poor GS could be a therapeutic option that confers positive effects on cognitive performance and social functioning.

Effects of cocoa-rich chocolate on cognitive performance in postmenopausal women. A randomised clinical trial

ABSTRACT Objectives The aim of this research was to evaluate the effects of adding 10 g of cocoa-rich chocolate (99%) to the habitual diet on cognitive performance in postmenopausal women. Methods Following a randomised controlled parallel clinical trial, a total of 140 postmenopausal women aged 50–64 were recruited. The intervention group (n = 73) consumed daily 10 g of chocolate (99% cocoa) in addition to their usual food intake for 6 months, whereas the control group (n = 67) did not receive any intervention. Attention and executive functions, verbal memory, working memory, phonological fluency, category fluency and clinical variables were assessed at baseline and 6 months. Results Trail Making Test B execution time showed a decreased of −12.08 s (95% CI: −23.99, −0.18; p = 0.047) in the intervention group compared to the control group, after adjusting for age, educational level, time elapsed from the beginning of menopause and daily energy consumption (Cohen’s d = −0.343). Attention, immediate or delayed verbal memory, phonological or category fluency, and working memory remained unchanged. Conclusions The consumption of cocoa-rich (99%) chocolate in addition to the habitual diet could be related to a slight improvement in cognitive performance regarding cognitive flexibility and processing speed in postmenopausal women, with no changes in the rest of the cognitive performance variables evaluated.Trial registration: This clinical trial has been registered at clinicaltrials.gov as NCT03492983.

Assessment of Neighborhood Poverty, Cognitive Function, and Prefrontal and Hippocampal Volumes in Children

The association between poverty and unfavorable cognitive outcomes is robust, but most research has focused on individual household socioeconomic status (SES). There is increasing evidence that neighborhood context explains unique variance not accounted for by household SES. To evaluate whether neighborhood poverty (NP) is associated with cognitive function and prefrontal and hippocampal brain structure in ways that are dissociable from household SES. This cross-sectional study used a baseline sample of the ongoing longitudinal Adolescent Brain Cognitive Development (ABCD) Study. The ABCD Study will follow participants for assessments each year for 10 years. Data were collected at 21 US sites, mostly within urban and suburban areas, between September 2019 and October 2018. School-based recruitment was used to create a participant sample reflecting the US population. Data analysis was conducted from March to June 2019. NP and household SES were included as factors potentially associated with National Institutes of Health Toolbox Cognitive Battery subtests and hippocampal and prefrontal (dorsolateral prefrontal cortex [DLPFC], dorsomedial PFC [DMPFC], superior frontal gyrus [SFG]) volumes. Independent variables were first considered individually and then together in mixed-effects models with age, sex, and intracranial volume as covariates. Structural equation modeling (SEM) was used to assess shared variance in NP to brain structure and cognitive task associations. The tested hypotheses were formulated after data collection. A total of 11 875 children aged 9 and 10 years (5678 [47.8%] girls) were analyzed. Greater NP was associated with lower scores across all cognitive domains (eg, total composite: β = -0.18; 95% CI, -0.21 to -0.15; P < .001) and with decreased brain volume in the DLPFC (eg, right DLPFC: β = -0.09; 95% CI, -0.12 to -0.07; P < .001), DMPFC (eg, right DMPC: β = -0.07; 95% CI, -0.09 to -0.05; P < .001), SFG (eg, right SFG: β = -0.05; 95% CI, -0.08 to -0.03; P < .001), and right hippocampus (β = -0.04; 95% CI, -0.06 to -0.01; P = .01), even when accounting for household income. Greater household income was associated with higher scores across all cognitive domains (eg, total composite: β = 0.30; 95% CI, 0.28 to 0.33; P < .001) and larger volume in all prefrontal and hippocampal brain regions (eg, right hippocampus: β = 0.04; 95% CI, 0.02 to 0.07; P < .001) even when accounting for NP. The SEM model was a good fit across all cognitive domains, with prefrontal regions being associated with NP relations to language (picture vocabulary: estimate [SE], -0.03 [0.01]; P < .001; oral reading: estimate [SE], -0.02 [0.01]; P < .001), episodic memory (picture sequence: estimate [SE], -0.02 [0.01]; P = .008), and working memory (dimensional card sort: estimate [SE], -0.02 [0.01]; P = .001; flanker inhibitory control: estimate [SE], -0.01 [0.01]; P = .01; list sorting: estimate [SE], -0.03 [0.01]; P < .001) and hippocampal regions being associated with NP associations with language (picture vocabulary: estimate [SE], -0.01 [0.004]; P < .001) and episodic memory (picture sequence: estimate [SE], -0.01 [0.004]; P < 0.001). In this study, NP accounted for unique variance in cognitive function and prefrontal and right hippocampal brain volume. These findings demonstrate the importance of including broader environmental influences when conceptualizing early life adversity.

Establishing two principal dimensions of cognitive variation in logopenic progressive aphasia.

Logopenic progressive aphasia is a neurodegenerative syndrome characterized by sentence repetition and naming difficulties arising from left-lateralized temporoparietal atrophy. Clinical descriptions of logopenic progressive aphasia largely concentrate on profiling language deficits, however, accumulating evidence points to the presence of cognitive deficits even on tasks with minimal language demands. Although non-linguistic cognitive deficits in logopenic progressive aphasia are thought to scale with disease severity, patients at discrete stages of language dysfunction display overlapping cognitive profiles, suggesting individual-level variation in cognitive performance, independent of primary language dysfunction. To address this issue, we used principal component analysis to decompose the individual-level variation in cognitive performance in 43 well-characterized logopenic progressive aphasia patients who underwent multi-domain neuropsychological assessments and structural neuroimaging. The principal component analysis solution revealed the presence of two, statistically independent factors, providing stable and clinically intuitive explanations for the majority of variance in cognitive performance in the syndrome. Factor 1 reflected ‘speech production and verbal memory’ deficits which typify logopenic progressive aphasia. Systematic variations were also confirmed on a second, orthogonal factor mainly comprising visuospatial and executive processes. Adopting a case-comparison approach, we further demonstrate that pairs of patients with comparable Factor 1 scores, regardless of their severity, diverge considerably on visuo-executive test performance, underscoring the inter-individual variability in cognitive profiles in comparably ‘logopenic’ patients. Whole-brain voxel-based morphometry analyses revealed that speech production and verbal memory factor scores correlated with left middle frontal gyrus, while visuospatial and executive factor scores were associated with grey matter intensity of right-lateralized temporoparietal, middle frontal regions and their underlying white matter connectivity. Importantly, logopenic progressive aphasia patients with poorer visuospatial and executive factor scores demonstrated greater right-lateralized temporoparietal and frontal atrophy. Our findings demonstrate the inherent variation in cognitive performance at an individual- and group-level in logopenic progressive aphasia, suggesting the presence of a genuine co-occurring cognitive impairment that is statistically independent of language function and disease severity.
