The Impact of Regular Diet Recovery on Postoperative Rehabilitation After Elective Cesarean Section

Abstract Objective To determine the role of regular diet recovery after restoration of normal muscle strength of both lower extremities in promoting postoperative recovery in women undergoing elective cesarean section. Methods This was a prospective observational cohort study. Patients who underwent elective cesarean section at Xiangya Hospital, Central South University, from October 2022 to December 2022, were categorized into two groups based on the duration of postoperative fasting: the observation group resumed eating after regaining lower extremity muscle strength, while the control group adhered to traditional postoperative fasting guidelines, waiting 6 hours before eating. Primary outcomes included postoperative pain levels assessed by visual analog scale (VAS) pain scores and time to first flatus. Demographic characteristics, time to first lactation, hospital stay length, and patient satisfaction were also assessed. Statistical analysis was conducted using Student’s t test and Chi-squared test, with significance set at P < 0.05. Results Out of a total of 300 patients, 240 were included in the analysis, comprising 112 in the control group and 128 in the observation group. There were no significant differences in baseline demographic characteristics. The median values of the first flatus time and the first lactation time were 33.37 ± 1.22 vs. 18.06 ± 6.34 hours (P = 0.003) and 26.34 ± 8.21 vs. 7.05 ± 1.26 hours (P = 0.001) in the control and observation groups, respectively. The median hospital stay duration in the control and observation groups was 6.54 ± 0.53 vs. 4.84 ± 0.18 days (P = 0.000), respectively. Median postoperative VAS pain scores and patient satisfaction values were 8.57 ± 0.11 vs. 4.91 ± 0.27 (P = 0.000) and 9.36 ± 0.16 vs. 9.72 ± 0.08 (P = 0.005) in the control and observation groups, respectively. There were no statistically significant differences in other postoperative outcomes, such as intestinal obstruction, infection, and readmission within 42 days (P > 0.05). Conclusion Food intake after restoration of lower extremity muscle strength improves first flatus, relieves postoperative pain, shortens hospital stay, and enhances satisfaction after elective cesarean section, without adverse effects. It is crucial for postoperative rehabilitation and should be encouraged.

Clinical outcomes of three Level Interlaminar Endoscopic Decompression Surgery in the Treatment of Multilevel lumber Spinal Stenosis

This is the first case report to test the efficacy of three-level endoscopic spine surgery for the treatment of lumber spinal stenosis. The patient was a 50-year-old entrepreneur with pain on standing and walking and all activities that compress the canal such as lumber extension. Three-level endoscopic surgery was performed at L3-L4, L4-L5, and L5-S1. Certain clinical and surgical outcomes were selected such as the Visual Analogue Scale for pain, Oswestry Disability Index score for level of disability and functional limitation, pain on certain aggravating activities involving extension like walking and moving downstairs, spinal and core stability using prone segmental instability test and patient satisfaction values taken at baseline and after the endoscopic surgery at two levels i.e. immediately after surgery and after one month follow up. Multiple-level interlaminar endoscopic surgery was successful in terms of reducing pain, improving score on the Oswestry disability index, improving daily life activities, increasing spinal and core stability, and increasing patient satisfaction

Analysis of Infrastructure for Covid-19 Patient Satisfaction at RSDI Banjarbaru City

Patient satisfaction is a condition of fulfilling the wishes, expectations and needs of patients. Bases on data from March 2020 to March 2021, there was an increase in patients returning home at their own request by 3% and the lack of fulfillment of patient satisfaction values which were still below 90%. The aim of this study is to evaluate infrastructure and compliance of covid-19 patients. The study design used was cross sectional, whit a consecutive sampling and a sample size of 120 covid-19 patients. Data were analyzed using the spearman rank rest. The data collection was carried out from June 2021 to August 2021. The result of the study found that there was a relationship between physical facility and satisfaction with p value 0.001 and r 0.498. The better infrastructure provide, the higher definite satisfaction.

Assessment of Patient Satisfaction Level to Dental Health Care Services in Indonesia

Objective: To know the level of patient satisfaction with dental healthcare services in Bantaeng, Indonesia. Material and Methods: This pilot pathfinder survey was conducted at Dental and Oral Health Services in Bantaeng and Pa'jukukang Sub-districts of Bantaeng Regency of South Sulawesi Indonesia. The Short-Form Patient Satisfaction Questionnaire (PSQ-18) scaled was used and it consists of 18 questions and seven subscales . The t-test and ANOVA were used with a significance level set at 5% (p<0.05). Results: The female participation (72.8%) was greater than the male (27.2). Regarding age, 74.6% were <40 years, 21.1% were between 40-60 years old and 4.4% were over 60 years old. On the subscale of technical quality, there was significant value in the age variable (p=0.45), education level (p=0.031), job (p=0.026), waiting time (p=0.46), distance to dentist=0.026), and location (p=0.007). There were significant values on the interpersonal aspect subscale that were present at age (p=0.016), education level (p=0.038), and occupation (p=0.007). The highest satisfaction score was found on the technical quality subscale (13.04 ± 1.75) and the lowest satisfaction score was seen on the general satisfaction subscale (4.11 ± 1.75). There was a significant difference between the value of patient satisfaction with age, education, and occupation . Conclusion: Patients from the District Bantaeng and Pa'jukukang Bantaeng regency of South Sulawesi were more satisfied with the service provided. The highest satisfaction score was found on the technical quality subscale and the lowest satisfaction score was found in the general satisfaction subscale.

Along with the increase in the number of human population and economic conditions that are increasingly advanced, the public awareness of health is increasing. In addition, currently access to services for the Social Security Administering Agency (BPJS) is getting wider, the hospital must be able to add capacity to existing facilities and improve the quality of services. This study aims to implement the Fuzzy Servqual method and Quality Function Deployment on the quality of BPJS Health services in H. Adam Malik Hospital and determine which attributes are priority improvements. Data processing based on the criteria of the Fuzzy-Servqual method by determining fuzzy sets, fuzzyfication and defuzzyfication. determine the Gap and determine the priority of improvement using the Quality Function Deployment method. The results of this study indicate that all 19 BPJS service attributes provided by applying the Fuzzy Servqual method still do not meet patient expectations, because the value of patient satisfaction with services is still lower than the expectation value. Attributes that must be prioritized to improve their quality, namely: Revelation Attribute of Q2 with a gap value -3.68, Revelation Attribute of Q7 with a gap value of -1.58., Revelation Attribute of Q12 with a gap value of -1.58. The technical response that became the main target was the Procurement of Excellent Training Services, the addition of AC Fan in the inpatient room, making improvements to the facilities and infrastructure.

The Analysis on Dental and Oral Health Services Based on “Rater” Service Quality Dimension to Patient Satisfaction at Natural Esthetic Clinic Makassar

The growth of Clinic of Dental Health Service is currently increasing rapidly in the City of Makassar South Sulawesi Province. Quality is a dynamic condition associated with products, services, human resources, processes, and environments that meet or exceed expectations, where the relationship between products and services or services provided to consumers can meet their expectations and satisfaction. The purpose of this research is to analyze the dimension of service quality that consist of reliability, assurance, tangible, empathy, and responsiveness, fequently abbreviated as RATER, to patient's satisfaction level in Natural Esthetic Clinic (NEC) Makassar. This research is a survey of cross sectional study approach with the number of samples as many as 74 visiting patients. The result shows that the total value of patient satisfaction (gap) obtained in the analysis is-0.44 on the average and belongs to Medium category, It means that the quality of services in NEC Makassar is ordinary classified.and has not yet met patient's satisfaction. The result of statistical analysis shows that the dimensions of reliability (0.000) assurance (0.004), tangibel (0.001), and responsivenes (0.000) have significant effect on patient satisfaction, except emphaty (0.060) which does not give significant difference to influence patient satisfaction level. The possible advices are the need for increasing attention and communication; giving a high sense of care; and establishing a clinical management information system (CMIS) to provide efficient, effective, and accurate services.

Peningkatan Kepuasan Pasien BPJS Melalui Peningkatan Empati Perawat

Kepuasan pasien merupakan elemen penting dalam menyediakan pelayanan yang lebih baik. Kepuasan pasien dapat dicapai dengan meningkatkan perilaku caring perawat. Tingkat empati perawat yang tinggi dapat menjadi prediktor sikap caring yang dimiliki oleh perawat. Penelitian ini menggunakan desain penelitian kuantitatif asosiatif analitik dengan menggunakan pendekatan cross-sectional. Penelitian dilaksanakan di Rumah Sakit Islam Kendal. Sampel penelitian ini adalah pasien rawat inap kelas 3 yang menjadi peserta BPJS sebanyak 166 responden, yang didapatkan melalui Proportionate Stratified Random Sampling. Hasil penelitian menunjukkan bahwa seluruh responden memiliki baseline data empati perawat yang sama. Hal ini menunjukkan bahwa perawat selalu menunjukkan sikap empati kepada semua pasien yang dikelola. Semua variabel kualitas pelayanan keperawatan memiliki hubungan yang kuat dengan arah hubungan searah, yang berarti bahwa semakin tinggi nilai kualitas pelayanan keperawatan maka akan semakin tinggi pula nilai kepuasan pasien. Empati perawat yang dilakukan pada saat melaksanakan asuhan keperawatan pada pasien merupakan prediktor utama atau penentu peningkatan kepuasan pasien Peserta BPJS Di Rawat Inap RSI Muhammadiyah Kendal.

Since since BPJS and other government programs, currently based on assessment, the quality of health services, especially inpatient care, is more likely to decrease if the value of patient satisfaction. This study aims to analyze the quality of service with patient satisfaction through seven dimensions of health quality consisting of Guarantees, Empathy, Reliability, Responsiveness, Physical Views, Medical Services and Professionalism. Dimension is used to analyze the quality of service with satisfaction of inpatient BPJS class III in RSU Haji Surabaya. The method used in this study using descriptive qualitative methods, data sources used are primary data and secondary data. In the study the authors also used research techniques in the form of observation / observation, interviews and documentation in order to obtain primary data to find out how the quality of service with patient satisfaction BPJS class III in RSU Haji Surabaya. Based on the result of the analysis and the discussion that has been done, the quality of service with the satisfaction of the inpatients of BPJS class III hospital in RSU Haji Surabaya, according to the dimension of guarantee, empathy, reliability, responsiveness, medical service and professionalism of inpatient grade III service at RSU Haji Surabaya said good, so that patients feel satisfied, while the physical display dimensions are still said to be quite good and patients feel quite satisfied. This is evidenced by the room is quite hot and the bathroom can be said not too clean. Commentary on: Botulinum Toxin for the Treatment of Excessive Gingival Display: A Systematic Review.

I would like to commend Nasr et al for their study “Botulinum Toxin for the Treatment of Gingival Display: A Systematic Review.”1 It is well designed and provides an outstanding review of the articles reporting on this novel, safe, and cosmetically effective treatment alternative for this condition, which at times, affects the self-esteem of many of those presenting with it. Ten years after my initial publication in 2005,2 17 subsequent articles by other authors on this subject have followed, including three additional articles that I wrote.3-5 The off-label use of botulinum toxins has disseminated globally, and many health practitioners are incorporating them into the aesthetic alternatives offered to our patients. Four articles selected for inclusion in this study were Polo,3 Mazzuco and Hexsel,6 Sucupira and Abramovitz,7 and Suber et al.8 As well pointed out by Nasr and his group, there are some differences among the authors of these articles in the techniques utilized and doses employed in injecting the targeted muscles. The question about the possible variability of effectiveness dependent on the use of existing botulinum toxins may also be a concern. As they point out, complications arising from this minimally invasive treatment modality are “most often due to poor injection techniques or inappropriate amounts of botulinum toxin resulting from limited experience.” Hence, this well-designed study was necessary, and its presentation is timely. It should be mentioned that, as reported, the average improvement within these four groups also varied significantly: from 71.93% to 98%. Patient satisfaction results based on postinjection surveys were reported by three of these four authors (Polo,3 Sucupira and Abramovitz,7 Suber et al8), and their results ranged from 4.66 to 4.88 on a 5-point scale. The effect of lesion size on clinical outcomes after microfracture: a 2 to 8 year follow-up study

Purpose: Microfracture is often the first choice treatment for chondral defects in the knee, but some studies have suggested outcomes are influenced by lesion size at the time of the index surgery. Some authors have suggested that microfracture is suitable only for lesions up to 200 mm2, and other authors have placed an upper size limit of 400 mm2 for microfracture. The purpose of this study was to determine if there was a relationship between the size of the cartilage lesion treated with the appropriate microfracture surgical technique and rehabilitation protocols and patient-centered outcomes. We hypothesized that at a minimum of two years after microfracture patient-reported function, activity level, and satisfaction with outcomes would be similar regardless of the chondral lesion size at the time of index surgery. Methods: An ethics board-approved clinical data registry was queried for knees with contained focal chondral lesions and treated with microfracture by a single surgeon. Six hundred ninety-two (692) knees met the inclusion criteria. The lesions were on the femoral condyles, the tibial plateaus, or in the patellofemoral joint. The size of the chondral lesion was carefully measured and documented at the index surgery. Patients were assigned to one of four groups based on lesion size: Group I <100 mm2, Group II 100-300 mm2, Group III 301-400 mm2, and Group IV >400mm2. Previously published microfracture technique and rehabilitation protocols were used. Average follow-up was four (4) years with a range of 2 to 8 years. The patient-centered outcomes measures used for this study were the Lysholm function score, the Tegner activity level scale, and patient satisfaction with outcomes where 0 = totally dissatisfied and 10 = totally satisfied. Results: At the latest follow-up, data were available as follows: Group I contained 123 patients, Group II had 138, Group III included 161, and Group IV contained 138 patients. Preoperatively, Groups I-III had an average Lysholm score of 62, and Group IV had an average Lysholm score of 50. At an average 4 years after the index microfracture procedure (range, 2 to 8 years) with 80% (560 of 692) follow-up, there were no significant differences between groups for Lysholm, Tegner, or patient satisfaction values (Table I). These findings were the same regardless of lesion location (femur, tibia, or patellofemoral joint). Conclusions: In this large group of patients with an average 4-year follow-up, lesion size at the time of index microfracture had no effect on the patient-reported function (Lysholm), activity level (Tegner) or patient satisfaction with outcomes. These results were consistent regardless of lesion location. These findings confirm our hypothesis that patient-centered outcomes are the same for a contained chondral lesion of the knee regardless of lesion size. Spinal Versus General Anesthesia for Day-Case Laparoscopic Cholecystectomy: A Prospective Randomized Study

The aim of this study was to compare the surgical outcome of day-case laparoscopic cholecystectomy (DCLC) performed with the patient under spinal anesthesia with that performed with the patients under general anesthesia in the management of symptomatic uncomplicated gallstone disease. One hundred eighty patients were prospectively randomized to either the spinal anesthesia DCLC group (SA-DCLC group) or the general anesthesia DCLC group (GA-DCLC group). Intraoperative events related to spinal anesthesia, postoperative complications, and pain scores were recorded. The incidences of both overnight stay and readmissions were also recorded. Patient satisfaction values as to the anesthetic technique and same-day discharge were assessed by direct questionnaire at the end of the first postoperative week. In both groups, all procedures were completed laparoscopically. In the SA-DCLC group, there were 4 (4.4%) anesthetic conversions due to intolerable right shoulder pain, and those 4 patients were excluded from further analysis. In the SA-DCLC group, all patients were discharged on the same day. Overnight stay was required in 8 patients (8.9%) in the GA-DCLC group (P<.001). The cause of overnight stay was nausea and vomiting in 4 patients (4.4%), inadequate pain control in 3 patients (3.3%), and unexplained hypotension in 1 patient (1.1%). Readmission was required in 1 patient (1.1%) in the GA-DCLC group. The difference in patient satisfaction scores with regard to both anesthetic technique and same-day discharge was not statistically significant between the two groups studied. DCLC performed with the patient under spinal anesthesia is feasible and safe and is associated with less postoperative pain and lower incidence of postoperative nausea and vomiting and therefore a lower incidence of overnight stay compared with that performed with the patient under general anesthesia.

Improving Satisfaction in Patients Receiving Mental Health Care: A Case Study

Patient satisfaction is increasingly becoming an important component of quality for behavioral health care systems. The following report describes Group Health Cooperative's Behavioral Health Services department experiences over a 5-year period in moving from uncertainty about the value of patient satisfaction and the ability to positively impact patient ratings to achieving a significant improvement in patient ratings of satisfaction with mental health care. In this process, the Behavioral Health Department developed a deeper understanding of patient requirements and improvement strategies which could impact these requirements. A description of the results achieved along with the role of quality improvement processes in understanding and improving patient satisfaction in mental health care is presented.

A patient satisfaction survey for haemophilia treatment centres

The importance of patient satisfaction has continued to grow such that patient satisfaction is now viewed as a vital component of health-care delivery. This is evidenced by the expanding body of research in the area and the use of measures of patient satisfaction as indicators of health-care quality. The value of patient satisfaction is particularly apparent in the setting of chronic disease where medical care utilization is high, compliance with therapy is critical and the patient-provider relationship is often long-term. Although several validated tools exist to quantify general measures of patient satisfaction, there is a recognized need for disease-specific instruments. Not only are there issues that are unique to haemophilia, but many patients receive care via a specialized comprehensive clinic model. The authors were unaware of an instrument that could adequately address patient satisfaction issues specific to haemophilia; thus, they undertook to develop one. The patient satisfaction survey presented here contains fixed-choice, Likert-scale and open-ended questions adapted from previously validated questionnaires. Assessment of face validity and internal consistency indicate that the survey is measuring one underlying construct - patient satisfaction. Information acquired through this survey will provide a quantitative assessment of patient satisfaction within a clinic population of persons with bleeding disorders and could be used to guide decisions regarding provision of health-care services.

Influence of the Doctor??s Gender in the Satisfaction of the Users

This study was carried out in the framework of a wider research project concerning the degree of patient satisfaction with the various types of primary health care. We have studied the relationship among the gender of the doctor, the gender of the patient and the type of primary health care center involved. In 1 type of primary care center (health centers) the medical staff work as a team, whereas in the other (ambulatory care services), the doctor works alone. The survey was conducted among 86 doctors and 860 patients from urban areas in Andalusia, Spain. The degree of patient satisfaction was tested on Likert scales. Both male and female patients attended by female doctors were more satisfied than those attended by male doctors (P < 0.005). Both male and female patients were attended in equal proportions by both male and female doctors (P > 0.20). Overall patient satisfaction values were not affected by the patient's gender (P > 0.40). In comparing overall satisfaction among patients according with the doctor's gender and type of primary health care center, there was a greater degree of satisfaction with female doctors working in health centers (P < 0.01) and no difference existed in ambulatory care services in this area.

This study concerns patients' satisfaction with care in a surgical hospital. The study was done in May and in August-September 1989. Questionnaires were given to 143 patients on five wards, 70% were returned. On the whole, the patients were satisfied, which is in consistence with previous research results. The patients' expectancies concerning results of care were usually fulfilled, they were satisfied with information and nursing. However, some of them wanted more information about medical tests and interventions and some criticism concerning environment and facilities was expressed. The author also discusses difficulties in studying patient satisfaction and the value of patient satisfaction as a criterion on quality of care.

A social worker's use of a patient attitude survey in the administration of a small health clinic.

This paper has been a report of a patient attitude survey with 100 randomly selected parents and/or adult patients. It demonstrates that most consumers of our service were pleased with the humaneness and competence of the professional staff. It identified areas of weakness which needed improvement. Constructive action to correct these weaknesses has been initiated. Therefore, this process has demonstrated the value of client feedback for agency operation. It is important to caution the reader that because of the vulnerability of the patients they learn to be accommodating even if unintentional agency practices embarrass and degrade them. The value of patient satisfaction as a measure of the humanism provided by an agency depends on the population being served--their expectations, fears, and hierarchy of concerns.
