Determinants of Intention to Use of Hospital Information Systems among Healthcare Professionals

Health information systems (HISs) are instrumental in improving the efficiency and effectiveness of hospital operations, from managing patient data to enhancing decision-making processes. This study, which holds significant implications for the healthcare industry, aimed to identify the factors that influence users’ intentions to use HISs. The research involved interviews with healthcare professionals licensed to use the HIS of a public hospital. The survey, conducted in 2020, received 113 responses. Statistical methods of descriptive analysis, correlation, and multiple linear regression were used. Two models were examined. The first model investigated the relationship between the dependent variable of the intention to use an HIS and the independent variables of perceived time savings and perceived privacy protection. The second model explored the impact of age and education as control variables in the connection between the intention to use an HIS and time savings and privacy protection. A significant moderate positive correlation was found between the intention to use an HIS and perceived time savings, while a significant weak positive correlation was detected between the intention to use an HIS and education. Regression analysis in the first model revealed a significant connection between the intention to use an HIS and perceived time savings. However, the perceived privacy protection variable did not show a significant relationship with the dependent variable. The second model showed statistical significance in the relationship between the intention to use an HIS and education, but not with the second control variable, age. It can be concluded that education strengthens the positive impact of perceived time savings on the intention to use a health information system (HIS), a finding that has immediate and practical implications for healthcare professionals and researchers in the field of health information systems and healthcare management, underlining the importance of this research in advancing the understanding and adoption of HISs in healthcare settings.

Digital capabilities of health workers to use electronic medical records: Digital maturity self-assessment in Indonesian hospitals

Digital skills influence the successful implementation of information systems and electronic medical records (EMR) in health care facilities. The Ministry of Health measures digital maturity levels to identify gaps in improving digital transformation, including the digital skill of health workers in hospitals in 2022. The digital maturity in all hospitals in Indonesia was measured quantitatively based on the 7 components of Hospital Information Systems Maturity Model (HISMM). The low response rate was anticipated with a number of efforts such as outreach, capacity building and a circular letter from the Ministry of Health. Internal and external consistency was conducted to ensure the quality of the self-assessment. Specifically, the level of digital skills was analysed by calculating the average score of one components of digital maturity, namely Human Resources, Skills and Use of Hospital Information Systems. There are 11 parameters for assessing the level of digital literacy, perceived usefulness and encouragement to use RME in hospitals. All parameters were assessed using a 5-point scale and were analysed quantitatively. The response rate was 31.9% (973 out of 3,052 hospitals) where overall average of digital maturity level was 2.6. Hospital type A, higher accreditation status and located in Jawa-Bali has relatively high maturity level compare to others. The digital skills of health workers in hospitals are relatively high, with the majority already using the hospital information system. This was align with the perception of the ease of use and usefulness of using a hospital information system. Even though there have been several digital health champions in the hospital, health workers have not been fully involved in the process of developing information systems in hospitals and not many hospitals have utilized HIS to assess the performance of health workers. It is clearly that health workers in hospitals are capable and quite aware of the use of digital technology. However, their role needs to be increased to support the development of appropriate digital technology.

Factors Influencing Clinicians' Use of Hospital Information Systems for Infection Prevention and Control: Cross-Sectional Study Based on the Extended DeLone and McLean Model.

Healthcare-associated infections have become a serious public health problem. Various types of information systems have begun to be applied in hospital infection prevention and control (IPC) practice. Clinicians are the key users of these systems, but few studies have assessed the use of infection prevention and control information systems (IPCISs) from their perspective. This study aimed to (1) apply the extended DeLone and McLean Information Systems Success model (D&M model) that incorporates IPC culture to examine how technical factors like information quality, system quality, and service quality, as well as organizational culture factors affect clinicians' use intention, satisfaction, and perceived net benefits, and (2) identify which factors are the most important for clinicians' use intention. A total of 12,317 clinicians from secondary and tertiary hospitals were surveyed online. Data were analyzed using partial least squares-structural equation modeling and the importance-performance matrix analysis. Among the technical factors, system quality (β=.089-.252; P<.001), information quality (β=.294-.102; P<.001), and service quality (β=.126-.411; P<.001) were significantly related to user satisfaction (R2=0.833), use intention (R2=0.821), and perceived net benefits (communication benefits [R2=0.676], decision-making benefits [R2=0.624], and organizational benefits [R2=0.656]). IPC culture had an effect on use intention (β=.059; P<.001), and it also indirectly affected perceived net benefits (β=.461-.474; P<.001). In the importance-performance matrix analysis, the attributes of service quality (providing user training) and information quality (readability) were present in the fourth quadrant, indicating their high importance and low performance. This study provides valuable insights into IPCIS usage among clinicians from the perspectives of technology and organization culture factors. It found that technical factors (system quality, information quality, and service quality) and hospital IPC culture have an impact on the successful use of IPCISs after evaluating the application of IPCISs based on the extended D&M model. Furthermore, service quality and information quality showed higher importance and lower performance for use intention. These findings provide empirical evidence and specific practical directions for further improving the construction of IPCISs.

Assessing the Use of Hospital Information Systems (HIS) to Support Decision-Making: A Cross-Sectional Study in Public Hospitals in the Huíla Health Region of Southern Angola.

Hospital information systems could be relevant tools to inform hospital managers, support better management decisions in healthcare, and increase efficiency. Nonetheless, hospital managers’ effective use of these systems to support decision-making in Angola is unknown. Our study aimed to analyse the use of hospital information systems as a tool to support decision-making by hospital managers in Huíla, Angola. It was a descriptive, cross-sectional study inducted between July and September 2017 in seven hospitals in Huíla Province, Angola, specifically in the cities of Lubango and Matala. Thirty-six members of the hospital boards filled out a self-questionnaire that consisted of twenty questions based on the following issues: Characterisation of the interviewee’s profile; availability of information in the institution; and quality and usefulness of the available operational information. At least two thirds of the participants reported being unsatisfied or relatively satisfied with each assessed hospital information systems-specific feature. More than 50% have rarely or never used the health information system to support decision-making. Most managers do not use hospital information systems to support management-related decision-making in Angola. Improving the ability of hospital information systems to compute adequate indicators and training for hospital managers could be targets for future interventions to support better management-related decision-making in Angolan healthcare.

Use of Hospital Information System to Improve the Quality of Health Care from Clinical Staff Perspective

Background: The development of hospital information systems (HISs) has a significant effect on care processes. In this regard, the clinical staff’s perspective is very important in evaluating the success of these systems. The present study aimed to evaluate the clinical staff’s perspective at hospitals affiliated to Neyshabur University of Medical Sciences on the effectiveness of the HIS in improving their performance and strategies for increasing the system effectiveness. Materials and Methods: In the present cross-sectional study, 120 clinical staff who actively worked with the HIS were included. A two-part questionnaire was used for data gathering. Results: From the perspective of the research population, the effective score of HIS was %64.42 in improving their performance, and %81.85 in the case of developed HIS according to the suggested strategies. According to the research scale, the effectiveness of the system could be improved from good to excellent in the case of implementation of strategies, so that there was a statistically significant difference between the pre- and post-effectiveness (P&lt;0.001). Conclusion: Positive population perspective on the effectiveness of HIS in-performance improvement of the clinical staff indicated that there was a good context for the development and utilization of information technology in the hospitals. The clinical staff’s opinions and work needs as the primary group of patient care should be taken into consideration in order to increase the effectiveness of the HIS. [GMJ.2021;10:e1830]

Modelling the Acceptance and Use of Electronic Medical Records from Patients’ Point of View: Evidence from Saudi Arabia

Background: The employment of IT in healthcare reflects a need to meet the increasing demands of patients and to upgrade the quality and productivity of the provided services. However, the literature demonstrates many failures in systems and IT technology implementation. One of the reasons behind these failures is users&amp;rsquo; resistance. The ministry of health (MOH) in Saudi Arabia (SA) has prioritized the embracing of new technologies that could improve the healthcare services, such as electronic medical record (EMR) systems. On the other hand, studies that address the acceptance and use of hospital information systems (HIS) and EMRs in SA from patients&amp;rsquo; standpoint are scarce. The aim of this study was to explore patients&amp;rsquo; acceptance of an EMR system by proposing a model adapted from the technology acceptance model (TAM) and the unified theory of acceptance and use of technology (UTAUT).&#x0D; &#x0D; Methods: An online questionnaire was used where questions were based on the proposed model constructs (facilitating conditions (FC), perceived ease of use (PEOU), perceived usefulness (PU), attitude (ATT), and behavioral intention (BI)). 116 patients responded to the questionnaire. The model was then tested to identify the factors that affect patients&amp;rsquo; intention to use the online services of the EMR.&#x0D; &#x0D; Results: The analysis of the research model suggested that (1) FCs influenced the degree of system&amp;rsquo;s PEOU. (2) PEOU affected the EMR&amp;rsquo;s PU. (3) PU affected patients&amp;rsquo; ATT toward using the EMR. (4) A positive effect is found between ATT and BI to use EMR.&#x0D; &#x0D; Conclusion: The study has implications for healthcare IT acceptance research and efforts in SA, as to date patients&amp;rsquo; perspectives have received little attention. The research also makes a theoretical contribution as it validated the proposed model.

The antecedents of effective use of hospital information systems in the chinese context: A mixed-method approach

Hospital information system (HIS) implementation has become an important part of health informatics construction in China. However, effective use of HIS in the Chinese context has scarcely been discussed in the academic literature. Drawing upon Burton-Jones and Grange's (2013) effective use theory, this paper investigates the antecedents of medical workers’ effective use of HIS and the relationships between the antecedents in the Chinese context from a post-acceptance perspective. It employs a mixed-method approach in which a qualitative study was conducted to identify the antecedents and to develop the hypotheses, and a quantitative study was conducted to verify the hypotheses. The qualitative data (from in-depth interviews) and quantitative data (from a questionnaire survey) were collected from five public hospitals in north-eastern China. The meta-inferences from this study show that: (i) effective use of HIS is jointly determined by self-efficacy, management support, information quality and system quality; (ii) management support strengthens the effect of self-efficacy on effective use of HIS for female workers but not for male workers; and (iii) self-efficacy has a significant positive effect on female workers’ effective use of HIS but is not significant for male workers, while information quality has a significant positive effect on male workers’ effective use of HIS but is not significant for female workers. This paper extends effective use theory and enriches HIS research with practical implications for HIS implementation in China and other developing countries.

Hospital staff acceptance toward management information systems in Indonesia

BackgroundThe use of hospital information systems in Indonesian hospitals have not met the proper level of acceptance. This study aims to assess the intention to use information systems among hospital staff. MethodsThis study used an online survey that was disseminated for 2 weeks in July 2019. The questions were developed based on the Technology Acceptance Model with the addition of Perceived Risk. The survey yielded 449 total responses, with only 400 determined to be valid. A total of three factors, namely: Perceived Usefulness, Perceived Ease of Use, and Perceived Risk were measured and then analyzed for its influence on Intention to Use, and to each other. ResultsThe finding shows that only Perceived Usefulness has a significant influence on Intention to Use (β = 0,57). Furthermore, higher Perceived Ease of Use was associated with higher Perceived Usefulness (β = 0,68). Accordingly, higher Perceived Ease of Use also associated with significantly lower Perceived Risk (β = -0,49). The finding indicates that hospital employees do not yet have a strong perception of risk toward using hospital information systems. Conclusionsemployees’ perception of the benefits of using hospital information systems greatly determines their intention to use hospital information systems.

Identifikasi Dampak Penggunaan Sistem Informasi Rumah Sakit (SIRS) Terhadap Pelayanan Kesehatan Menggunakan Hot-Fit Model 2006

Hospital Information System (SIRS) is an integrated information system that aims to handle all hospital management processes, ranging from diagnostic services, medical data measures, medical records, pharmaceuticals, pharmaceutical warehouses, billing, personnel databases, payroll, and accounting to the control management. Based on Indonesian Constitution No. 44 Article 52 year 2009, paragraph 1 states that "Indonesian hospitals are required to record and report all activities that occur within a hospital". On the Indonesia ministry regulation, PERMENKES No. 1171 year of 2011, Article 1 paragraph 1 states that "Each hospital is required to apply SIRS". Therefore Dr. Samratulangi Tondano District Hospital of Minahasa Regency in the North Sulawesi uses SIRS to improve employee performance in providing health services to the community. The paper discussed how to identify the impact of the use of Hospital Information Systems (SIRS) on health services. The purpose of this study is to look at the impact of the use of the SIRS system based on four indicators contained in the 2006 Human Organizational Technology (HOT-Fit) method in addition to the DeLone and McLean Success 2003 methods. By taking 150 respondents of SIRS users randomly, data were collected and analyzed using SPSS and AMOS software.The four hypotheses derived from three components namely "Technology", "Human" and "Organization" have a positive impact and provide a net benefit to the use of the system. In short, the majority of users SIRS only focuses on functions for registration and administration rather than clinical functions. The availability of IT units and IT personnel influences the use of SIRS as well.

Health Professionals’ Attitudes towards Digital Transformation through the Use of Hospital Information Systems: The Case of a General Oncology Hospital of Attica

Purpose: The study of the attitude of Health Professionals of the General Oncology Hospital of Attica towards the integrated Hospital Information Systems (HISs) as well as of the socio-demographic factors that shape it. Material-Method: A stratified sampling quantitative survey was conducted using the occupational category of the employees as the stratification variable. Data were collected by employing the standard Nurses Attitudes towards Computerization (NATC) self-report questionnaire. Statistical analysis was conducted with the use of the statistical data processing SPSS-v25 software. Results: The staff of the General Oncology Hospital under study have a positive attitude towards the use of Hospital Information Systems (Mean = 3.73, SD. = 0.54). Statistically significant differences between occupational categories were found only for the dimension of “Providing quality care to the patient” (p = 0.007) and more specifically among nurses and doctors. Regarding the effect of demographic factors on the five dimensions that form the attitude of health professionals towards the HIS, only “gender” (p = 0.009) and “occupation categories” (p = 0.007) seem to influence this attitude and specifically the dimension “Providing quality care to the patient”, whereas “previous experience in use of computers at work” affects the dimension “Reduction in jobs” (p = 0.009). Conclusions: The statistical analysis of the data showed that the employees of the General Oncology Hospital under study have a positive attitude towards the use of the integrated Hospital Informatics Systems (HISs). Males tend to have the most positive attitude while employees who already make use of personal computers in their work, tend to disagree with the view that HIS will lead to a job reduction.

Safe use of hospital information systems: an evaluation model based on a sociotechnical perspective

ABSTRACTThere is growing evidence that shows Hospital Information Systems (HIS) can pose risks to patient safety when they are poorly designed, implemented, or adopted. Most of the preventive solutions have focused on improving the software design, whereas, this study proposes a model for the safe use of HISs based on a sociotechnical perspective. A questionnaire survey was conducted to test the proposed model, in which a total of 364 medical doctors from three Malaysian government hospitals implementing the Total Hospital Information System (THIS) participated. Psychometric testing and factor analysis of the questionnaire instrument established its construct validity and reliability. Structural Equation Modelling (SEM) analysis showed knowledge, system quality, teamwork, and task stressor were the antecedents of safe use of a HIS that predicted the HIS use. In addition, HIS use is a predictor of the HIS use outcomes. Furthermore, HIS use is the mediator between antecedents of safe use of a HIS and the HIS use outcomes. The proposed model has portrayed predictive capability, implying that the model could effectively explain the safe use of a HIS and its outcomes. Hence, the model can provide a basis for evaluating the safe use of a HIS from a sociotechnical standpoint.

Healthcare practitioner behaviours that influence unsafe use of hospital information systems.

This study aims to investigate healthcare practitioner behaviour in adopting Health Information Systems which could affect patients' safety and quality of health. A qualitative study was conducted based on a semi-structured interview protocol on 31 medical doctors in three Malaysian government hospitals implementing the Total Hospital Information Systems. The period of study was between March and May 2015. A thematic qualitative analysis was performed on the resultant data to categorize them into relevant themes. Four themes emerged as healthcare practitioners' behaviours that influence the unsafe use of Hospital Information Systems. The themes include (1) carelessness, (2) workarounds, (3) noncompliance to procedure, and (4) copy and paste habit. By addressing these behaviours, the hospital management could further improve patient safety and the quality of patient care.

Anglicization of hospital information systems: Managing diversity alongside particularity

ContextDespite widespread efforts to improve the quality and safety of healthcare through use of hospital information systems (HIS), many healthcare organizations face challenges in implementation and effective use of these applications, in particular when systems have been developed internationally (mainly in the US). Suppliers of these technologies also find it challenging to produce systems that work effectively across a range of geographical, cultural and institutional boundaries. In this paper, we seek to understand the strategies used by suppliers and adopters of HIS to overcome the challenges involved in the development and adoption of generic overseas systems. MethodsWe conducted a qualitative study, by interviewing 176 individuals (eight organizations), observing two user groups, and running a supplier focus group. We used inductive thematic analysis to assess emerging strategies in developing and implementing overseas packaged HIS in English settings. FindingsThe health sector in England has entered a period of potentially transformative change with many international HIS suppliers entering the market. This has provoked call for the ‘Anglicization’ of generic systems. This endeavor, has resulted in emergence of more or less aligned supplier and user strategies to overcome the difficulties in the process. This includes a continuous process of identification and classification of requests (by suppliers), and unification and voicing of needs (by adopters). ConclusionsThe complexity of health service provision, drives calls for customization of technologies in this sector. Consequent tensions between ‘standardization’ and ‘localization’ are requiring suppliers of generic solutions to develop more sophisticated strategies as they pursue international growth of their market.

Barriers for the Using of Information Systems in Hospitals: A Qualitative Study

Background: Information technology is one of the most important issues in health systems. It has been developed in health sectors and is widely used by hospital managers. Identifying barriers in the use of hospital information systems can be the first step for better information management and better implementation of quality improvement and clinical governance plans. Objectives: The aim of this study is to investigate the barriers for the using of information systems in hospitals. Methods: This is a qualitative study, which used content analysis for analyzing to achieve study objectives. Sampling was done using purposive and heterogeneous methods and data collection continued until data saturation. The researcher was referred to hospital managers to identify individuals who had the most information regarding health information systems to identify barriers of using information system. Results: Results of this study showed that challenges and barriers of information management and use of information systems are related to each other. Based on the results of this study the most important challenges of using information systems are knowledge factors. Conclusions: Designing a comprehensive plan can eliminate barriers of using information systems because barriers and challenges are integrated and related to each other.

Sociotechnical factors influencing unsafe use of hospital information systems: A qualitative study in Malaysian government hospitals.

The objective of this study is to identify factors influencing unsafe use of hospital information systems in Malaysian government hospitals. Semi-structured interviews with 31 medical doctors in three Malaysian government hospitals implementing total hospital information systems were conducted between March and May 2015. A thematic qualitative analysis was performed on the resultant data to deduce the relevant themes. A total of five themes emerged as the factors influencing unsafe use of a hospital information system: (1) knowledge, (2) system quality, (3) task stressor, (4) organization resources, and (5) teamwork. These qualitative findings highlight that factors influencing unsafe use of a hospital information system originate from multidimensional sociotechnical aspects. Unsafe use of a hospital information system could possibly lead to the incidence of errors and thus raises safety risks to the patients. Hence, multiple interventions (e.g. technology systems and teamwork) are required in shaping high-quality hospital information system use.

Factors Affecting Successful Implementation of Hospital Information Systems.

Background:Today, the use of information systems in health environments, like any other fields, is necessary and organizational managers are convinced to use these systems. However, managers’ satisfaction is not the only factor in successfully implementing these systems and failed information technology projects (IT) are reported despite the consent of the directors. Therefore, this study aims to determine the factors affecting the successful implementation of a hospital information system.Methods:The study was carried out as a descriptive method in 20 clinical hospitals that the hospital information system (HIS) was conducted in them. The clinical and paraclinical users of mentioned hospitals are the study group. 400 people were chosen as samples in scientific method and the data was collected using a questionnaire consisted of three main human, managerial and organizational, and technological factors, by questionnaire and interview. Then the data was scored in Likert scale (score of 1 to 5) and were analyzed using the SPSS software.Results:About 75 percent of the population were female, with average work experience of 10 years and the mean age was 30 years. The human factors affecting the success of hospital information system implementation achieved the mean score of 3.5, both organizational and managerial factors 2.9 and technological factors the mean of 3.Conclusion:Human factors including computer skills, perceiving usefulness and perceiving the ease of a hospital information system use are more effective on the acceptance and successful implementation of hospital information systems; then the technological factors play a greater role. It is recommended that for the successful implementation of hospital information systems, most of these factors to be considered
