Objective:
To identify possible factors associated with chronicity of motion sickness in a population that suffered a mTBI.
Background:
Dizziness is a very common symptom following a mild traumatic brain injury (mTBI). The pathophysiology of post-traumatic dizziness/vertigo is quite variable, may be the result of damage to the peripheral vestibular system or the central nervous system. While most peripheral vestibular disorders can be easily identified by testing and examination, those without inner-ear causes result in persisting complaints of dizziness and motion sickness that are more difficult to understand and manage. In our study there is a subgroup of patients who suffered a brain concussion (with no obvious head injury) in which dizziness persists after months.
Design/Methods:
we analyzed medical records from 2006 to 2015 of patients experiencing dizziness caused by a mTBI. Patients with peripheral causes: BPPV, vestibular hypofunction, labyrinthine concussion, etc, were excluded. All patients studied had normal neuroimaging and were evaluated by an experienced neuroothologist.
Results:
454 patients with mTBI and dizziness or lightheadedness were found, of which 98 patients met the inclusion criteria. 63% were women (n = 62). Patients were divided into 2 groups according to time of subjective recovery of symptoms: Those who resolved their dizziness with DHI (Dizziness Handicap Inventory) less than 14 points or similar values prior to TBI (W45:M30). Those with persisting symptomatology at the time of final evaluation (3 months) (W17:M6).
Conclusions:
We found a significant relationship between the chronicity of symptoms in patients where the injury was related to the workplace and that of patients who had a high initial score of the emotional subscale of the DHI. We did not observe any relationship with the overall scale score of DHI nor with the presence of migraine. Disclosure: Dr. Bisonni has nothing to disclose. Dr. Videla has nothing to disclose.
