Academic Practice Partnerships: Improving Healthcare Access and Decreasing System Burden Utilizing Interprofessional Competencies

Collaboration between academic centers and healthcare systems/practices has demonstrated improved outcomes, fostered innovation, and increased nursing research. Although evidence for their importance has been established, all too often these academic-practice partnerships (APP) encounter hurdles that limit success. Lack of time, poor communication, and unclear leadership endorsement are often barriers to effective APPs. Utilizing the challenges of the COVID-19 pandemic, a successful APP was developed based on the Interprofessional Education Collaborative (IPEC) competencies. The IPEC framework addressed needs related to the COVID-19 pandemic and became the foundation for many future endeavors between a large children’s healthcare system and a university school of nursing. This APP demonstrated the appropriate capacity and organizational structure to provide simultaneous benefits to multiple parties while improving systemwide healthcare outcomes. This article outlines the development and implementation processes, subsequent evaluation, and implications for practice related to this highly successful partnership.

Ethical climate, moral resilience, and ethical competence of head nurses.

The ethical competence of head nurses plays a pivotal role in nursing ethics. Ethical climate is a prerequisite for ethical competence, and moral resilience can positively influence an individual's ethical competence. However, few studies have focused on the relationship between ethical climate, moral resilience, and ethical competence among them. To investigate the relationship between ethical climate, moral resilience, and ethical competence, and examine the mediating role of moral resilience between ethical climate and ethical competence among head nurses. A quantitative, cross-sectional study. A total of 309 Chinese head nurses completed an online survey, including ethical climate questionnaire, Rushton moral resilience scale, and ethical competence questionnaire. Inferential statistical analysis includes Pearson's correlation and a structural equation model. This study received ethical approval from the Institutional Review Board of Xiangya Nursing School of Central South University (No. E2023146). Head nurses' ethical climate score positively impacted ethical competence (r = 0.208, p < .001), and ethical climate could affect ethical competence through the mediating role of moral resilience. This study emphasized the value of ethical climate in moral resilience of head nurses, ultimately leading to an enhancement in their ethical competence.

Development of a Professional Practice Nursing Model for a University Nursing School and Teaching Hospital: A nursing methodology research.

The aim of the study was to describe the process of developing a Professional Practice Model by a Nursing School and Nursing Department of University Hospital. This is a descriptive nursing methodology research, developed along three stages: preliminary, empirical and validation. The empirical phase used qualitative and quantitative methodology. 28 teachers from the nursing school and nurses from the hospital participated. We defined the elements of the nursing meta-paradigm from narratives and focus group. Then, we extracted propositions regarding the nature of nursing from the relationship between the meta-paradigm elements, which concluded in the core elements. The core elements of this nursing professional practice model are nursing seeks the well-being of the person, family or group; nursing is total and global, person-centred; nursing is compassionate; nursing entails up-to-date, quality scientific, technical and human competencies; nursing is delivered in a teaching hospital environment, with a Christian ethical perspective.

Improving the Health of Public Housing Residents Through a Housing Authority and Nursing School Partnership.

The connection between health and housing is well-established. People who are precariously housed have worse health than those who have stable housing arrangements.- Persons moving into public housing have a higher illness burden than the general population, and public housing residents engage in less healthy behaviors, which contribute to public housing residents having poorer health than persons living in other housing situations. Public housing authorities and residents can benefit from authentic and constructive relationships with academic partners; academicians and students can benefit from engaging in partnerships with housing authorities and residents to better understand the connection between housing and health. This article describes the well-established relationship between the Duke University School of Nursing (DUSON) and the Durham Housing Authority (DHA), the evolution of that relationship, our collaborative work in improving the health of DHA residents while advancing nursing education and science, and lessons learned.

0229 Exploring the Relationships Between Sleep, Stress, and Performance in Simulation-Based Learning

Abstract Introduction Sleep deprivation and stress may affect performance among students. Simulation-based learning (SBL) in undergraduate nursing programs provides the opportunity for students to practice critical decision-making without fear of patient harm; however, students still report experiencing stress during SBL. Current research is unclear on the effect of sleep deprivation combined with stress on performance in SBL. The purpose of this study was to explore the association between stress, functional outcomes of sleep, and performance in SBL. Methods Elements of the Theory of Stress, Appraisal, and Coping and the National League for Nursing Jeffries Nursing Education Simulation Framework guided this study. Baccalaureate nursing students consented to participate in a 1-hour SBL experience that included the collection of one hair and 4 saliva samples for cortisol concentration. Participants completed the Functional Outcomes of Sleep- Short Form and the Perceived Stress Questionnaire. An experienced faculty member evaluated student performance using the Creighton Competency Evaluation Inventory. Results Participants (N=35) were mainly female (n=32, 91.4%), white (n=29, 82.9%), with ages ranging between 18–22 years (n=32, 91.4%), and employed outside of nursing school (n=32, 91.4%). Other ethnicities represented include Asian and African American. Kendall’s Tau correlations revealed a significant relationship between functional outcomes of sleep and perceived stress (r=-.281, p=.020). Although not significant, a small relationship was observed between functional outcomes of sleep and performance (r=.145, p=.236). No significant relationship between performance and perceived stress (r=-.099, p=.423) was identified. Conclusion This study suggests that daytime dysfunction related to sleep is related to perceived stress and performance in undergraduate nursing students during participation in SBL. Due likely to small sample size, the relationship between sleep and performance was unable to achieve significance. These findings support the need for future research exploring the effects of sleep on stress and performance with larger more heterogeneous samples of students. Using a Mentorship Approach to Address the Underrepresentation of Ethnic Minorities in Senior Nursing Leadership.

Healthcare organizations must be intentional and purposeful in creating diversity programs. A nursing leader mentorship program for racial and ethnic minority nurse managers was introduced at a large academic medical center to meet this need. The program design was based on the successful Leadership Institute for Black Nurses, first conducted at a university school of nursing. The participants in the 4-month program were 16 nurse managers from 2 city hospitals and their mentors.

MUSE pilot study

Increased levels of stress and anxiety have negative impacts on nursing students’ mental health and education. The purpose of the MUSE pilot study is to assess the feasibility and impact of two methods of technology-assisted meditation on stress and anxiety: the MUSE headband and the Headspace meditation application. These forms of meditation have been shown in the literature to decrease levels of stress and anxiety and improve mental wellbeing.&#x0D; This pragmatic randomized control trial (RCT) occurred over a five-week time frame during fall 2019. In order to test the effectiveness of these interventions, eight nursing students at Queen’s University were recruited, three of whom withdrew. Individuals were eligible for this study if they were 2nd, 3rd, or 4th year Queen’s nursing students in the four-year stream or advanced standing track (AST) program. The study intervention consisted of two 10 to 15 minute meditation sessions per week for five weeks, which were conducted at Queen’s University School of Nursing. Participants completed validated self-reported pre- and post-surveys to evaluate stress and anxiety levels before and after the study. After the participants completed 10 mediation sessions, qualitative data was collected in order to gather feedback on participant experience.&#x0D; In this presentation the study purpose, procedures and outcomes will be discussed, while additionally highlighting the experience and perspective of novice undergraduate researchers. Future directions for data analysis and hopes of sharing the findings within the School of Nursing will be addressed.

‘Thinking like a nurse’. Changing the culture of nursing students′ clinical learning: Implementing collaborative learning in practice

This article reports a study evaluating the implementation of Collaborative Learning in Practice models at a university School of Nursing and Midwifery with practice partners across the South West of England. We conducted four focus group interviews with 40 students with experience of Collaborative Learning in Practice placements, and two focus groups with eight clinical practice staff with responsibility for implementing and supporting such models in their areas. Data were transcribed and analysed using the Framework Method. Key themes were ‘Real time’ Practice of Collaborative Learning Implementation, Collaborative Learning as Preparation for Registrant Practice, and the Student/Mentor Relationship. We conclude that Collaborative Learning in Practice utilising models of coaching and peer support, offers benefits to students who are exposed to the reality of nursing practice from the beginning of their placement experiences, enabling them greater responsibility and peer support than under normal mentoring arrangements. Furthermore, there are benefits to the registrants because the burdens of supervising students are spread more widely. This is timely given the review of Nursing and Midwifery Council standards for programmes and student support and the need to increase placement capacity as a response to global nursing shortages.

CUSTOMIZING STAKEHOLDER ENGAGEMENT TO SUPPORT AGING IN PLACE

Geriatrics infusion and transformation of community-based settings to support “aging in place” is complex. It requires a customized approach that engages multiple stakeholders who are invested in systems redesign and process change. Rowan University School of Osteopathic Medicine’s NJ Geriatrics Workforce Enhancement Program, in partnership with an affordable housing facility operated by Fair Share Housing/Northgate II, and Rutgers University School of Nursing (RSoN), implemented a Resident Health Risk Assessment (RHRA) tool as part of an interprofessional community-based training experience for health professions students. The goal was to identify health risks that impact a resident’s ability to age in place and implement a person-centered intervention plan. Multi-level stakeholder engagement and ongoing rapid cycle quality improvement catalyzed changes in structure and process for all partners. Results included refinement of an interprofessional clinical rotation, introduction of competency attainment into the orientation process, and resource reallocation to support data collection.

A randomized comparison trial of two and four-step approaches to teaching Cardio-Pulmonary Reanimation.

Background and aim of the work: The treatment of cardiac arrest in an extra-hospital environment improves with the increase in the number of people able to establish an early Cardio-Pulmonary Reanimation (CPR). The main aim of the study was to assess the validity of the two-step method in case of prolonged CPR. Methods: A randomized comparison study was conducted in the University Nursing School of a Northern Italian town, during the 2015/16 academic year, among 60 students, to teach them CPR techniques, through two different teaching methods (4-step and the 2-step of CPR training). The effectiveness of the maneuvers performed on mannequins equipped with skill-meter was verified. Results: Our study did not highlight any significant difference between the two methods of CPR training. The comparison between the two methods regarding their efficacy in practical teaching of CPR, highlighted by this study, proved the validity of both the 4-minute continuous method (1st method) and the 30:2 method (2nd method). Conclusions: The results of the study showed no differences between the 2-step and the 4-step methods, in the effectiveness of cardiac massage. The correct execution of chest compressions during a CPR is the key to increase the patient’s chances of rescue. Research has shown that any interruption in the execution of chest compressions, leads to a progressive reduction of the effectiveness of cardiac massage, with negative consequences on the prognosis of the patient undergoing at CPR.

Application of informatization exam system in the objective structured clinical examination for nursing skills

Objective To explore the advancement and efficiency of informatization exam system and provide practice basis for its application in the nursing assessment through the construction and application of informatization exam operation system platform in the nursing skill assessment of the objective structured clinical examination (OSCE) . Methods OSCE for nursing skills was performed on nursing undergraduates from a university school of nursing in 2015 using the informatization exam system. Voice calls was used for calling students in order. The teachers used paperless PAD for evaluation. After the examination, students were required to finish the informatization evaluation questionnaire to assess the satisfaction rate. Results A total of 91 nursing students were included in the study. The highest dimension score was exam environment (4.20±0.86) , while the lowest dimension was the exam time (3.89±1.10) . The entry score ranked in the top three were reasonable location (4.31±0.89) , clear instructions content (4.25±1.01) and clear play sound (4.18±1.06) . 72.73% of the examinees thought that the combination of high-tech in the informatization exam system were advanced, and the exam arrangement was clear and time-saving. 27.27% of the examinees thought that the system response and test arrangement required improvement to increase the flow velocity and avoid nervous and anxiety caused by long waiting. Conclusions The application of the informatization exam operation system changes the traditional model of artificial operation examination. The examinees give the affirmation to exam environment. The examination schedule needs to be improved. The paperless scoring system improves the working efficiency of the evaluation and management in teaching management. Impact of a self-control promotion program on nursing students

ObjectiveTo describe the characteristics of positive mental health in nursing students, and to determine the impact of a self-control promotion program. MethodA quasi-experimental controlled trial including 72 second-year of Vitoria-Gasteiz University Nursing School. ResultsThe lowest scores in every measurement were for the self-control factor (F3). There were no statistically significant differences in self-control (F3) between the groups. ConclusionThis program shows higher scores in inter-personal relationship skills.

Utilization of communities of practice for ongoing learning and knowledge dissemination: Making the case for the Global Alliance for Nursing and Midwifery (GANM).

With the growing connectivity among countries and healthcare practitioners, nurses and midwives in low resource settings are connecting digitally to access information through online platforms. Ninety eight percent of adults online report visiting a social network in the past month, and spend almost two hours per day engaged with social media. In an increasingly interconnected world, innovative strategies are needed to translate knowledge into practice. The Global Alliance for Nursing and Midwifery (GANM), part of the Pan American Health Organization (PAHO)/World Health Organization (WHO) Collaborating Center (CC) at the Johns Hopkins University School of Nursing (JHSON) leverages its Knowledge Gateway to facilitate translating knowledge into practice in order to achieve the Sustainable Development Goals (SDGs). This paper explores the concepts of knowledge dissemination, communities of practice, and makes the case for further using the GANM as an exemplary model to build the capacity of nurses and midwives globally.

Student Excel! Program: Applying Structure and Vision to Clinical Learning Excellence

George Mason University School of Nursing has tried a variety of off-the-shelf commercial tracking products to collect, manage and communicate data associated with student clinical rotations. The products were each eventually removed from use because they did not meet the specific needs of either the faculty or students. Mason has developed an Excel-based clinical tracking program that facilitates rapid and accurate communication of the clinical experience and provides the flexibility to adapt across academic programs–at no cost to the students. This article describes the “Student Excel!” journal that was developed by Mason, how it is used by the School of Nursing and Health Informatics Program, response from students, and future implications.

Street Smarts ↔ Book Smarts

This article describes the development, implementation, and evaluation of an interdisciplinary undergraduate course embedded within a campus–community partnership initiative involving McMaster University School of Nursing, and three urban priority neighborhoods in Hamilton, Ontario, Canada. Students worked together with community residents and faculty to address selected priority community issues identified by neighborhood members. Using the qualitative interpretive description method, the evaluation explored different partners’ (students, community residents, and faculty) perceptions of the course (SWOT analysis: strengths, weaknesses, opportunities, and threats, as well as outcomes) as a community engagement and knowledge exchange intervention. Results provide lessons learned and recommendations for future campus–community engaged courses that can be transferred to similar contexts.

Trabajo Final de Grado y Plan de Acción Tutorial en el currículum del Grado en Enfermería. Diseño, desarrollo y evaluación

&lt;p style="margin: 0cm 0cm 0pt; text-align: justify;"&gt;&lt;span style="font-family: Arial;"&gt;&lt;span style="font-size: medium;"&gt;La acción tutorial (PAT), en los estudios universitarios de Enfermería, se halla en el centro de la acción docente del profesorado. El tutor/a orienta en el proceso de aprendizaje y en la preparación del Trabajo Final de Grado (TFG). En este artículo presentamos los resultados de la investigación realizada con tutores y estudiantes, que tiene por objetivo evaluar el diseño y desarrollo del TFG, los resultados académicos y su vivencia y experiencia. Diseño mixto cuantitativo descriptivo y cualitativo de investigación acción participación. Muestra total de 124 estudiantes de 4º curso de Grado Enfermería y 37 tutores del TFG. Técnicas de recogida de datos: resultados académicos y evaluativos; 3 grupos focales con estudiantes y profesores; 3 entrevistas semiestructuradas abiertas a estudiantes. Los resultados constatan que las tipologías más elegidas fueron revisiones bibliográficas (38,75%) y Planes de cuidados (24,2%). El 63,9 % de la temática escogida es Enfermería Maternal, Pediátrica y Clínica. Estudiantes y tutores viven con dificultad la elaboración del TFG y se requiere una mayor cultura tutorial. Es un proceso complejo de organización y coordinación que, sin embargo, permite un trabajo colaborativo de todo el equipo docente. &lt;/span&gt;&lt;/span&gt;&lt;/p&gt;

The Operating Room Experiences of Nursing Students: A Focus Group Study.

The purpose of this study was to evaluate operating room (OR) experiences of student nurses. The focus group interview from qualitative research methods was used. This study was carried out between February and March 2011 in an OR practice at a university school of nursing. The grounded theory method was used to collect and analyze semistructured interview. Interviews were held with a total of 26 students in three focus group interviews. Each interview was tape recorded and was supported by taking notes. The audiotapes were listened and relistened by the researchers and transcribed. Four themes were determined as a result of this study. These are information, determination of career preference, period of internship and/or rotation, and fear and/or anxiety. The students stated that the period of OR practice was insufficient, the opportunities for being able to implement were limited, they mostly observed, and they experienced feelings of being alone and fear in the OR. Despite all of these, the students stated that the OR practice provided a major contribution to their education and was effective in the determination of their career preferences after graduation.

Regulation of Tissue Plasminogen Activator Expression in Human Epithelial Cells

RATIONALE: Prior intensive care unit (ICU) admission is a risk factor for fatal asthma. The purpose of this study was to examine characteristics of underserved children with a history of ICU admissions for asthma. METHODS: Baseline survey data, salivary cotinine and allergen specific IgE serology were obtained from underserved children with uncontrolled asthma enrolled in a randomized clinical trial of a multifaceted Emergency Department/ home-based asthma intervention. Baseline characteristics of children with and without a prior history of ICU admission were compared using chi-square and ANOVA. RESULTS: Subjects included 100 primarily African American (95%), Medicaid insured (97%) children (Mean age56.25 yrs; 56% male) with uncontrolled asthma. Atopy was high with 80.2% sensitized to > 1 environmental allergen. Most (54.1%) had detectable cotinine levels. Almost one-third (31%) had prior ICU admissions. ICU subjects were more likely to be sensitized to dustmite (p50.009) and ragweed (p50.002) and trended toward higher cockroach sensitization (p50.09) than non-ICU subjects. Allergen sensitization between the two groups did not differ for mouse, cat, dog, alternaria, aspergillus, grass or tree. Cotinine levels were similar. Albuterol overuse was prevalent (>70%); appropriate controller medication use was poor (32%). Although ICU subjects were more likely to have seen their PCP for asthma in prior 3 months, only 31% had seen an asthma specialist in the previous 2 years. CONCLUSIONS: Children at risk for fatal asthma should undergo asthma specialty evaluation to optimize controller medication use, assess environmental sensitivities/exposures and implement individualized trigger avoidance procedures to improve morbidity and mortality in this high risk population.
