Risk factors of pulmonary tuberculosis in Indonesia: A case-control study in a high disease prevalence region.

Tuberculosis (TB) remains one of the most widespread infectious diseases in the world, with high mortality and morbidity rates, contributing to a significant disease burden, particularly in developing countries. The aim of this study was to determine the risk factors of TB among the population aged 17 years (late adolescent) and above in one of the high TB prevalence countries, Indonesia. A case-control study was conducted in Banda Aceh, Indonesia, involving newly diagnosed pulmonary TB patients who visited one of the eleven community health centers in Banda Aceh. The controls were individuals matched by age and sex who lived near the cases and had no TB infection. A total of 16 risk factors were assessed. Hierarchical multivariable logistic regression models were used to examine the association between the risk factors and the occurrence of TB. A total of 196 cases and 196 controls were included in the study. Multivariable logistic regression analysis identified 11 independent predictors for the occurrence of TB after controlling possible confounders. Patients who lived in houses with insufficient light exposure were 77 times more likely to develop TB compared to those living in houses with sufficient light exposure (adjusted odds ratio (AOR): 77.69; 95%CI: 27.09, 222.79). The study also showed that people who had close contact with TB patients (AOR: 25.39; 95%CI: 2.10, 306.52), had poor knowledge of TB (AOR: 24.2; 95%CI: 6.89, 85.17), had comorbidities (AOR: 4.49; 95%CI: 1.35, 14.89), insufficient food utilization (AOR: 3.41; 95%CI: 1.51, 7.71), negative preventive behavior (AOR: 3.39; 95%CI: 1.49, 7.72), low education level (AOR: 3.08; 95%CI: 1.26, 7.55), and insufficient housing humidity (AOR: 2.89; 95%CI: 1.18, 7.12) were associated with increased the odds of having TB. Being employed, having income above minimum wage, and having good nutritional intake were protective factors for developing TB. In conclusion, the determinants of TB among people aged 17 years and older are a clear indication of the need for TB prevention programs targeting not only individual risk factors but also environmental risk factors, particularly providing adequate housing conditions. This study provided useful information that might help to develop and adopt effective policies for TB control in Indonesia.

PROBLEM ANALYSIS OF CHILD TUBERCULOSIS PREVENTION AND CONTROL PROGRAM LAMONGAN DISTRICT, EAST JAVA

Tuberculosis (TB) is an infectious disease caused by the bacterium Mycobacterium tuberculosis and is one of the unresolved health problems in Lamongan District. The purpose of this study was to analyze the problems in the Child Tuberculosis Prevention and Control Program in Lamongan Regency. This study was a descriptive observational study conducted at the Lamongan District Health Office from January 02 to January 27, 2023. Problem identification was conducted using brainstorming techniques and then prioritizing the problems using the CARL method (Capability, Accessability, Readiness, and Leverage). Furthermore, identification of the causes of problems that have been prioritized is carried out with a fish bone diagram using a process approach. Based on the root causes of the problems that have been identified, recommendations are given to the health department to overcome the problems found. Suboptimal cross-program/sector coordination with a CARL value of 505.440 became the top priority health problem in the child tuberculosis control program. Therefore, it is necessary to optimize the discovery of child TB suspects and education through integration with other activities, improving the competence of field officers, ensuring budget availability, data management, good recording and reporting.

Ubuntu Is a Critical Component in the Fight against Human Immunodeficiency Virus and Tuberculosis Stigma: Nursing Students' Perceptions.

Stigma is one of the documented barriers to achieving universal access to human immunodeficiency virus (HIV) and tuberculosis (TB) prevention, treatment, care, and support programs. The lack of African social theories to emphasize these issues may be the cause of the continent's failure to reduce stigma. We can use Ubuntu, an African philosophy that emphasizes sociability and ethics, to deepen our understanding of how to reduce HIV- and TB-related stigma in South Africa. In many African regions, Ubuntu values and principles were found to assist in reducing problems related to HIV. Ubuntu is a comprehensive phrase used by Africans to signify the characteristics that incorporate the core human virtues of compassion and humanity. We explored the perceptions of nursing students regarding the use of Ubuntu in the fight against HIV and TB stigma. Qualitative participatory research was used to engage all first- and second-level nursing students enrolled for the 2023 academic year at the selected university in South Africa. Purposive sampling was used to recruit the participants. An interactive workshop was used to gather data. The data from the recordings and flip charts were analysed together. Throughout the study, trustworthiness and ethical principles were upheld. Two primary themes emerged as expressions of Ubuntu in relation to a reduction in HIV and TB stigma and recognition of Ubuntu as a tool to combat stigma associated with HIV and TB. This confirms that within Ubuntu, there are elements that might be applied to reduce stigma attached to HIV and TB.

Modern tuberculosis prevention and control programs at the regional level

The article presents an assessment of the effectiveness of measures to prevent the spread of tuberculosis in the Omsk region, where modern approaches and directions of the national strategy of tuberculosis elimination are realized. The qualitative and quantitative changes in the cohorts of first-detected cases of tuberculosis for the analyzed period of 2015 - 2021 are noted. Actual directions of tuberculosis prevention for the Omsk region are revealed.

A proposal to evaluate the management of tuberculosis programs: a qualitative, evaluability assessment in the border region of Brazil and Venezuela.

This study aims to analyze the feasibility of building an evaluative model for the management of the Tuberculosis Prevention and Control Program in the State of Roraima, located on the border between Brazil and Venezuela. This is an evaluability assessment, a type of study used as a pre-evaluation of the development and implementation stages of a program, as well as throughout its execution. The study was developed in stages comprising the: (i) definition of the intervention to be analyzed and its objectives and goals; (ii) construction of the intervention logical model; (iii) screening of parties interested in the evaluation; (iv) definition of the evaluative questions; and (v) design of the evaluation matrix. Four priority components were defined for the evaluation: management of the organization and implementation of tuberculosis (TB) prevention and control policy; epidemiological surveillance management; care network management; and management of expected/achieved results. In this model, and based on theoretical references, we defined the necessary resources, activities, outputs, outcomes, and the expected impact for each of the policy management components. The management of the TB control program is feasible for evaluation based on the design of its components, the definition of structure and process indicators, and relevant results for the analysis of the management of TB prevention and control actions, as well as its influence on compliance with the agreed indicators and targets aiming at eradicating the disease by 2035.

The community as an active part in the implementation of interventions for the prevention and care of tuberculosis: A scoping review.

Interventions involving direct community stakeholders include a variety of approaches in which members take an active role in improving their health. We evaluated studies in which the community has actively participated to strengthen tuberculosis prevention and care programs. A literature search was performed in Pubmed, Scopus, ERIC, Global Index Medicus, Scielo, Cochrane Library, LILACS, Google Scholar, speciality journals, and other bibliographic references. The primary question for this review was: ¿what is known about tuberculosis control interventions and programs in which the community has been an active part?. Two reviewers performed the search, screening, and selection of studies independently. In cases of discrepancies over the eligibility of an article, it was resolved by consensus. 130 studies were selected, of which 68.47% (n = 89/130) were published after 2010. The studies were conducted in Africa (44.62%), the Americas (22.31%) and Southeast Asia (19.23%). It was found that 20% (n = 26/130) of the studies evaluated the participation of the community in the detection/active search of cases, 20.77% (n = 27/130) in the promotion/prevention of tuberculosis; 23.07% (n = 30/130) in identifying barriers to treatment, 46.15% (n = 60/130) in supervision during treatment and 3.08% (n = 4/130) in social support for patient. Community participation not only strengthens the capacities of health systems for the prevention and care of tuberculosis, but also allows a better understanding of the disease from the perspective of the patient and the affected community by identifying barriers and difficulties through of the tuberculosis care cascade. Engaging key community stakeholders in co-creating solutions offers a critical pathway for local governments to eradicate TB.

Latent tuberculosis infection treatment completion in Biscay: differences between regimens and monitoring approaches.

Contact tracing and treatment of latent tuberculosis infection (LTBI) is a key element of tuberculosis (TB) control in low TB incidence countries. A TB control and prevention program has been active in the Basque Country since 2003, including the development of the nurse case manager role and a unified electronic record. Three World Health Organization-approved LTBI regimens have been used: isoniazid for 6 months (6H), rifampicin for 4 months (4R), and isoniazid and rifampicin for 3 months (3HR). Centralized follow-up by a TB nurse case manager started in January 2016, with regular telephone follow-up, telemonitoring of blood test results, and monitoring of adherence by electronic review of drugs dispensed in pharmacies. To estimate LTBI treatment completion and toxicity of different preventive treatment regimens in a real-world setting. Secondary objective: to investigate the adherence to different approaches to preventive treatment monitoring. A multicentre retrospective cohort study was conducted using data collected prospectively on contacts of patients with TB in five hospitals in Biscay from 2003 to 2022. A total of 3,066 contacts with LTBI were included. The overall completion rate was 66.8%; 86.5% of patients on 3HR (n = 699) completed treatment vs. 68.3% (n = 1,260) of those on 6H (p < 0.0001). The rate of toxicity was 3.8%, without significant differences between the regimens. A total of 394 contacts were monitored by a TB nurse case manager. In these patients, the completion rate was 85% vs. 67% in those under standard care (p < 0.001). A multivariate logistic regression model identified three independent factors associated with treatment completion: being female, the 3HR regimen, and nurse telemonitoring. 3HR was well tolerated and associated with a higher rate of treatment completion. Patients with nurse telemonitoring follow-up had better completion rates.

Interferon-gamma release assay for the diagnosis of latent tuberculosis infection in the prison population with a positive tuberculin test: a descriptive study in a prison (Burgos, Spain).

A high prevalence of prison inmates have a positive tuberculin skin test (TST) and sometimes unnecessary treatment for latent tuberculosis infection (LTBI) is prescribed. The prison tuberculosis prevention and control program has not generalized the use of QuantiFERON (QFT) in prisons. We set out to describe the implementation and usefulness of QFT in a population of inmates with positive TST, and to detect false positives and avoid unnecessary treatments. We also analysed the sociodemographic variables of the inmate population. All the positive TST tests between December 2020 and December 2021 from an average population of 300 inmates in Burgos prison were analysed. The QFT value was measured in all the positive cases. Sociodemographic variables were analysed and finally the number of inmates with positive TST, but with a negative QFT result and therefore not requiring LTBI treatment, was evaluated. A total of 41 inmates were included in the study, with a mean age of 44 years. The proportion between Spanish inmates and foreigners was similar. Of all the positive TST, 48.8% were QFT negative. It was observed that QFT is a safe method for the diagnosis of LTBI in prisons and that its use would contribute to a more specific selection of inmates who actually need chemoprophylactic treatment for LTBI.

Parent's Stigma Towards Childhood Tuberculosis: A Multicenter Survey from Eastern Bandung Regency

Aims: Tuberculosis-related stigma negatively influences tuberculosis prevention programs such as delayed diagnoses, poor treatment adherence, and decreased quality of life of patients. This descriptive quantitative study aimed to describe the stigma towards tuberculosis among parents. Method: A total of 30 samples taken through accidental sampling were parents who had children (0-14 years) with tuberculosis who had been treated at the public health centers. A questionnaire adapted from Van Rie TB Stigma Scale (VTSS) was used and analyzed using descriptive analysis. Result: Most parents (76.6%) had lower levels of stigma, while the others scored higher levels. Lower stigma levels can be caused by a mature mindset, good knowledge of TB information, and indicates higher hopes from parents to increase their care level for children’s development, health-seeking behavior, and support for children’s TB treatment until it is completed. Accordingly, higher stigma levels from parents might be a result of several external factors that can increase the lack of motivation for health-seeking behavior in their children’s TB treatment. Conclusion: The role of health workers is very important in reducing stigma. Health education about prevention and treatment of childhood tuberculosis is still needed because it is a source of fear and high stigma against childhood tuberculosis in society.

PREVALENCE AND RISK FACTORS ASSOCIATED WITH TUBERCULOSIS OCCURRENCE AMONG PATIENTS ATTENDING MUHIMA DISTRICT HOSPITAL, NYARUGENGE DISTRICT, RWANDA

Tuberculosis (TB) is one the serious public health problems globally and locally and killed at least eight million in 2018 (WHO, 2019). TB is a curable disease if well treated and followed. One of the most key considerations for tracking and evaluating TB prevention and control programs is the prevalence and related risk factors. The study adopted a quantitative approach and a cross-sectional retrospective design to collect data from the respondents. A systematic sampling technique was used and secondary data recorded in the laboratory were collected. Specific objectives were: to determine the prevalence of tuberculosis among patients attending Muhima District Hospital in Nyarugenge District, and to identify behavioral factors associated with TB occurrence among patients attending Muhima District Hospital in Nyarugenge District. Descriptive and analytical statistics were used to analyze the data. The total number of respondents was 2161 patients. The majority, were male 1524(70.5%) and female were 637(29.5%). For the age, those less than 9 years were 19(.9%), between 10-17 years 76(3.5%), between 18-35 years 1185(54.8%), between 36-59 years 843(39.0%), and above 60 years 38(1.8%). Those who were married were 1014(47%), singles were 977(45.2%), and widower were 57(2.6%). For employment, 883(40.9%) were unemployed, 845(39.1%) were employed, 206(9.5%) were students, and 227(10.4%) were prisoners. Based on location, 1181(54.7%) were from urban areas, and 980(45.3%) were from rural areas. The prevalence of TB was 7.2%, out of which the females were 53(34.0%) and males were 103(66.0%). The prevalence of TB among HIV patients was 32.6%. For behavior practices, drinking alcohol was found among 908(42.0%) and smokers were 186(8.6%). For all the predictors of TB occurrence, none of them was statistically associated with TB occurrence. These include demographic characteristics, behavioral factors including use of alcohol, smoking and use of tobacco products, and serology. The prevalence of TB among patients attending Muhima District was higher than the National TB prevalence estimates reported by the World Health Organization (WHO, 2019), respectively 7.2% versus 5.2%. All predictors of TB occurrence including demographic characteristics and behavioral factors none of them was associated with TB occurrence. Impact of the COVID-19 Pandemic on tuberculosis management in India: A Brief Overview

Chronicles suggests that emerging and re-emerging viral infections disrupting the normal lifestyle of humankind, whether in the form of HIV, Ebola, Influenza, Nepah, or the current SARS-CoV-2 pandemic. Such viral infections disrupt the healthcare system along with the prevention and control of epidemics and pandemics, resulting in an increased burden of such diseases in the post-pandemic period. Tuberculosis (TB) routine services are interfered with by severe lockdowns due to the new COVID-19 virus. This article tried to measure the long-term epidemiological effects of such interruptions on TB prevalence in high-burden countries. The participating facilities performed a comprehensive review based on modifications to the care of TB patients during the COVID-19 pandemic. Retrospectively, clinical factors and household contact information were collected from a literature survey. Researchers looked at numerous strategies over the following five years to see whether they might lessen the effects on TB incidence and death. Present comprehensive literature was collected and analyzed using suitable keywords such as "COVID-19," "Pandemics," "Tuberculosis," and "India" during the current COVID-19 pandemic to investigate the influence of COVID-19 on tuberculosis management. The present article looks at the effects of the breaks in the delivery of TB care in hospital and primary care settings. Lockdown, social isolation, measures to prevent viral transmission, and public health guidelines impacted tuberculosis care. The present study revealed that the COVID-19 pandemic has adversely affected numerous TB prevention, monitoring, and treatment programs. Still, these adverse effects are diminished by the prompt restoration of TB services and the application of particular therapies as soon as restrictions are lifted.

A Qualitative Study on the Implementation of the Workplace TB Program in the Philippines: Challenges and Way Forward.

Tuberculosis (TB) is a chronic infectious disease that remains to be a primary health concern globally. The Philippines is among the top TB-burdened countries. Workplace TB prevention and control programs are essential to ensure the health and safety of workers and economic security. There remains a knowledge gap regarding the Philippine workplace TB prevention and control program implementation. This qualitative study involving key informant interviews reviewed the implementation of the workplace TB program in selected companies in a high TB burden region in Eastern Philippines. Results were presented under four themes in accordance with the components of the workplace TB policy: preventive strategies, medical management, data recording and reporting, and social policy. Various good practices, opportunities, and challenges in the implementation of the workplace TB program were identified. There is a need to strengthen the enforcement of policy across different components. Compliance with guidelines on preventive strategies and recording and reporting schemes needs to be intensified. Coordination across different levels and agencies may also be enhanced to allow more efficient implementation. Increased awareness of corporate decision-makers may improve company ownership of the program leading to improved implementation while increased awareness of employees on their rights and entitlements may likewise enhance compliance.

Trends in incidences of newly notified tuberculosis in Jeju Province, Korea, 2017-2021

In 2021, the incidence of newly notified tuberculosis in Jeju Province decreased by 23.0% compared to that in the previous year. This was the largest decline among the 18 municipalities studied. This study aimed to examine trends in the incidence of tuberculosis among different age and sex groups in Jeju Province between 2017 and 2021. This study observed the number of new cases in the annual report from 2017 to 2021, published by the Jeju Center for Infectious Diseases Control and Prevention. The average annual percentage change (AAPC; %) was calculated to determine the trend in crude incidence rates over the past 5 years by sex and age. While a &lt;i&gt;P&lt;/i&gt;-value of &lt;0.1 was considered marginally significant, a &lt;i&gt;P&lt;/i&gt;-value of &lt;0.05 was considered statistically significant. The AAPC of all age groups in Jeju Province had negative values. In men, there was a statistically significant decrease in ages 30-34, 40-44, 45-49, and 50-54 years and a marginally significant decrease in ages 35-59, 55-59, 60-64, 65-69, 75-79, and ≥80 years. In women, a statistically significant decrease was observed among patients aged 30-34, 35-39, and ≥80 years, and a marginally significant decrease was observed among patients aged 45-49, 50-54, 60-64, 65-69, and 75-79 years. A statistically significant decrease in tuberculosis was observed among patients in their 30s and 40s, and elderly patients aged ≥80 years comprised the group vulnerable to tuberculosis. Based on these findings, tuberculosis prevention and screening programs conducted in Jeju Province were effective.

Exploration of inter-jurisdictional TB programming and mobility in a Canadian First Nation community

BackgroundColonially imposed jurisdictional boundaries that have little meaning to Indigenous peoples in Canada may confound tuberculosis (TB) prevention and care activities. This study explores how inter-jurisdictional mobility and the current accommodation of mobility through policies and programming sustain a regional TB epidemic in northwestern Saskatchewan, and northeastern Alberta.MethodsA qualitative instrumental case study was performed using a community based participatory approach. Semi-structured interviews were conducted with First Nations peoples from a high-incidence community in Canada including community-based healthcare workers. These interview data are presented in the context of a multi-level document analysis of TB program guidelines.ResultsThe location of the community, and related lack of access to employment, services and care, necessitates mobility across jurisdictional boundaries. There are currently no formal federal or provincial guidelines in place to accommodate highly mobile patients and clients within and across provincial TB prevention and care programs. As a result, locally developed community-based protocols, and related ad-hoc strategies ensure continuity of care.ConclusionIndigenous peoples living in remote communities face unique push/pull factors that motivate mobility. When these motivations exist in communities with increased risk of contagion by communicable infectious diseases such as TB, public health risks extend into increasingly large areas with competing jurisdictional authority. Such mobility poses several threats to TB elimination. We have identified a gap in TB services to systematically accommodate mobility, with specific implications for Indigenous peoples and reconciliation. We recommend clearly defined communication paths and inter-jurisdictional coordination to ensure maintenance of care for mobile populations.

Prevalence and Onset of Hepatotoxicity Caused by Anti-Tuberculosis Drugs on Pulmonary TB Patients in Wangaya General Hospital Denpasar – Bali in 2016

Introduction: As tuberculosis (TB) endemic country, TB becomes a community health problem in Indonesia. Data from the Global TB Report in 2016 showed that Indonesia is the second most country with TB burden after India. WHO and the Ministry of Health issue a TB prevention program based on the End TB Strategy aimed to reduce morbidity, mortality, and disability due to TB. One of the challenges faced in the management of TB is the side effects of the anti-tuberculosis drug. Hepatotoxicity is the most common side effect. Aims: This study aims to determine the prevalence and onset of anti-TB drug-induced hepatotoxicity in TB patients after receiving the anti-TB drug in Wangaya Hospital Denpasar Bali. Method: This study used a cross-sectional method by obtaining secondary data of pulmonary TB patients visiting the pulmonary clinic in Wangaya General Hospital from January to December 2016. Result: Of 77 subjects, fifty-six percent of them were men. The highest prevalence was found in the 41-50 years age group (26.9%), while the lowest was found in the 81-90 years age group (1.2%). Anti-TB drug-induced hepatotoxicity occurred in 6.5% of subjects. The most frequent onset of hepatotoxicity occurred within the second week of medication. Conclusion: Based on this study, we suggest a liver function test before and after two weeks of an anti-TB drug.

Objective: The study aims to describe the current situation and analyze some factors related to treatment adherence in patients with pulmonary tuberculosis who are managed and treated in Cho Moi district, An Giang province in 2021. Research methods: By design of a descriptive cross-sectional study with analysis, using the whole sampling method with 216 participants, face-to-face interviews using pre-designed questionnaires, data analysis according to research objectives. The rate of patients with treatment adherence reached 47.2%. The factors that had a good influence on treatment adherence include as follows: the patient had good knowledge; the patient was supervised by family and relatives; the patient was supervised at home by medical staff; the patient was taken by their relatives for periodic re-examination and testing; the patient was satisfied with the supervision of medical staff; the patient had difficulty in accessing medical services. The Tuberculosis Prevention Program implements and maintains counseling and communication activities to improve patient knowledge. Health workers at the station increase supervision from 1 time per month to 4 times per month, improving their attitude towards patients. Patients should attend seminars on knowledge and practice understanding and properly implement the 6 principles of treatment. The patient's relatives need to coordinate with health workers in monitoring the implementation of treatment principles with the goal of helping the patient recover from the disease. Perception of social support and quality of life in cancer patients with tuberculosis who attend INEN outpatient clinics [Percepción del apoyo social y calidad de vida en pacientes oncológicos con tuberculosis que acuden a consultorios externos INEN

Objective: To determine the perception of social support and quality of life in cancer patients with tuberculosis attending outpatient clinics of the national institute of neoplastic diseases in 2021. Methodology: The study had a quantitative, correlational, cross-sectional, non-experimental design. The population consisted of approximately 80 oncology patients with tuberculosis attending the INEN tuberculosis prevention and control program. Two instruments were used; the MOS questionnaire of Social Support in Primary Care (De la Revilla et al., 2005), to measure social support with 4 dimensions: Emotional, positive social interaction, instrumental and affective with 14 items and the WHOQOL-BREF quality of life scale, which evaluates 4 dimensions: physical health, psychological health, social relationships and environment, with 26 items both instruments with a Likert scale that were validated by expert judgment and a pilot test was performed to obtain Cronbach's alpha. The results obtained were that there is a Spearman correlation indicating that there is a moderate relationship between perception social support and the quality of life in oncology patients with tuberculosis (Rho= ,582), which means that both variables are significant (p= ,000 &lt; ,05).

Evaluation of the Tuberculosis Prevention Program in the Discovery of Tuberculosis Cases in Nosara Public Health Center Palu City

BACKGROUNDS: Tuberculosis is an infectious disease caused by Mycobacterium Tuberculosis. The case in Indonesia rank third with the highest number of TB case in the world. There were 511,873 cases of tuberculosis of all types in 2018. Based on data from the Palu Health Office, the TB cases in Palu in 2018 were still high numbered 557 cases. In 2019, Nosarara Public Health Center had the lowest case finding among the 13 health centers in Palu with 17 TB case findings. The absence of a laboratory in Nosarara Public Health Center for examining sputum samples affected the output in program implementation.&#x0D; AIM: The aim of this study is to evaluate the Control of Tuberculosis in the Discovery of Tuberculosis Cases in Nosarara Public Health Center Palu.&#x0D; MATERIALS AND METHODS: This research used qualitative method with a case study approach. The research informants were 6 people selected through purposive sampling technique.&#x0D; RESULTS: The results summarizes the following : the input aspect in terms of human resources in quantity and quality is inadequate because TB program holders have multiple tasks so that they are not maximal in carrying out their duties and have never attended training, sufficient funds, inadequate facilities and infrastructure, and methods according to guidelines.&#x0D; CONCLUSION:&#x0D; The process of planning, organizing, implementing and supervising has not been maximal, meanwile the output of TB case detection (22%) has not yet reached the predetermined target (80%).It is expected that the Public Health Center will add more health personnel, complete facilities and infrastructure in order to maximize program implementation to achieve predetermined targets.
