Hyaluronic acid, as a widely used polymer material in clinical practice, has been proven to have numerous biological functions. In Oral medicine, the application of hyaluronic acid also has certain advantages, which can be used to treat oral diseases. However, there are still many issues that need to be explored in research. Therefore, this article aims to explore the current situation and problems of hyaluronic acid in Aphthous stomatitis, and further test the effectiveness and safety of hyaluronic acid in Aphthous stomatitis, so as to propose a more reasonable and effective treatment plan for oral Aphthous stomatitis.

Starfruit extract (Averrhoa bilimbi) as an alternative ingredient in the treatment of aphthous stomatitis and gingivitis

Background: Stomatitis is a lesion of the oral mucosa, inner cheek, inner lip, tongue, gums, and palate. Although it is not dangerous, canker sores are very annoying. Gingivitis is inflammation of the gingival area and is the most common disease in the community. The content of starfruit (Averrhoa bilimbi) includes saponins, tannins, glucosides, and calcium, one of the best known as thrush drugs. In addition, the use of herbal plants such as star fruit (Averrhoa bilimbi) to reduce the risk of caries because this plant has antibacterial ability. Objective: This study aimed to analyze the efficacy of starfruit on the healing of aphthous stomatitis and gingivitis. Methods: the type of research used is quasi-experimental, with a pre and post-test design. The sampling technique used purposive sampling. The data collection instrument used to measure the diameter of stomatitis was using a caliper and measuring gingivitis using the WHO standard measuring instrument, namely the gingival index. Analysis of the data used was a paired sample test to compare stomatitis and gingivitis before and after the intervention. Result: the application of starfruit (Averrhoa bilimbi) showed p= 0.000 and the gingival index showed p=0.016, meaning that starfruit extract (Averrhoa bilimbi) was effective in treatment of aphthous stomatitis and gingivitis. Conclusion: starfruit extract (Averrhoa bilimbi) is effective as an alternative ingredient in the treatment of aphthous stomatitis and gingivitis.

CLINICAL EFFICACY AND SAFETY OF HERBAL MEDICINE THERAPY IN RECURRENT APHTHOUS STOMATITIS: A SYSTEMATIC REVIEW

Recurrent aphthous stomatitis is the most common oral mucous ulcerative lesion with challenging treatment. Herbal medicine therapy can propose clinical efficacy and safety due to its large biological activities. The objective was to review the clinical efficacy and safety of herbal medicine therapy in terms of ulcer size, pain score, healing duration, and adverse effects in recurrent aphthous stomatitis. A systematic was conducted based on the PRISMA statement. The search was performed using four electronic databases, namely PubMed, Cochrane, Science Direct, and Google Scholar for articles published from 2016 until 2021 using specific keywords. The search was limited to randomized controlled trials (RCTs), in English, full text, and study in humans. The main outcome is expected to be ulcer size, pain score, healing duration, and adverse effects. Quality assessment of selected articles was conducted using the Quality Appraisal of Randomized Trials Checklist (Cochrane Risk of Bias tool). The methodology quality of studies was evaluated using the Cochrane Handbook for Systematic Review of Interventions and Rev Man software. Five articles were eligible for analysis. The population of the sample study ranged from 34-70 patients of 15-65 y old. The herbs used were Aloe vera (Aloe barbadensis Miller), curcumin (Curcuma longa), licorice (Glycyrrhiza glabra), tobacco (Nicotiana tabacum L.), and pomegranate (Punica granatum Linn.). This review showed that there is some evidence of the clinical efficacy and safety of herbal medicine therapy in improved outcomes of recurrent aphthous stomatitis treatment with minimum adverse effects.

"STINGLESS BEE HONEY INCORPORATED CELLULOSE HYDROGEL/POLY(LACTIC-CO-GLYCOLIC ACID) PATCH AS AN ALTERNATIVE TREATMENT FOR APHTHOUS STOMATITIS

Aphthous stomatitis is a disease that often reappears, causing irritation and pain. Common topical medications to treat aphthous stomatitis are fast-dissolving synthetic drugs, sometimes with limited therapeutic effectiveness. In this study, a patch, composed of a stingless bee honey incorporated cellulose hydrogel layer and a poly(lactic-co-glycolic acid) (PLGA) layer, was fabricated as an alternative treatment for aphthous stomatitis. The composition of the honey patches was verified by the presence of a distinct physical structure, considerable wettability records and lower degradation percentages on the layers containing higher PLGA concentrations. The honey patches were capable to retard Eschericia coli in the early hours (0.5-2 hours) and Staphylococcus aureus in the late hours (2-4 hours) of application, with tolerable cell viability and cell closure. The therapeutic values of the honey patches in retarding bacterial growth and inducing cell closure recommend the developed patches to be used in aphthous stomatitis treatment.

The Effect of Carbon Dioxide Laser on Aphthous stomatitis Treatment: A Double-Blind Randomized Clinical Trial

Introduction: This randomized clinical trial was designed to investigate the effect of carbon dioxide laser on the treatment of minor recurrent aphthous stomatitis (miRAS) as a painful and common oral ulcer. Methods: Fifteen patients with miRAS entered into this study after compliance with the inclusion and exclusion criteria. The patients were assigned to three groups of pulsed CO2 laser, continuous CO2 laser, and control. A pulsed CO2 laser with power of 261 W and pulse duration of 180 microseconds and continuous CO2 lasers with the power of 1 W and radiation duration of 5-10 seconds were used. In the control group, topical triamcinolone ointment of 0.1% was used. The degree of pain was measured using the visual analogue scale (VAS). The patients were evaluated in terms of pain intensity before and immediately after the treatment and at follow-up hours, 12, 24, 48, and 72 hours after the intervention, and changes in the size of the ulcers were measured before the treatment and on third, fifth, and seventh days. Results: The effect of CO2 laser with pulsed and continuous radiation on the pain intensity of aphthous lesions, immediately after the treatment and at subsequent follow-up hours were similar and even better than the control group (P<0.05). All three treatments had a similar effect on the size of the ulcer, which was not statistically significant (P>0.05). Conclusion: The results of this randomized controlled trial (RCT) showed that the carbon dioxide laser, whether with pulsed radiation or continuous radiation compared to the control group, had a better impact on the pain relief of aphthous lesions.

Apremilast in dermatology: A review of literature.

Apremilast is an orally administered small molecule that specifically inhibits the phosphodiesterase-4 enzyme and modulates the immune system by increasing the levels of intracellular cyclic adenosine monophosphate (cAMP) and inhibiting IL-2 & 8, interferon-γ and tumor necrosis factor (TNF) production. It is FDA approved for the treatment of psoriasis, psoriatic arthritis, and oral ulcers of Behcet's disease. More recently, apremilast has been used off-label to treat varied dermatological diseases where systemic corticosteroids or immunosuppressive agents were not effective. We review the efficacy and safety of apremilast in the treatment of aphthous stomatitis, Behçet's disease, chronic actinic dermatitis, atopic dermatitis, cutaneous sarcoidosis, hidradenitis suppurativa, lichen planus, and discoid lupus erythematosus in cases where standard treatment has failed.

Nanocellulose-Based Patches Loaded with Hyaluronic Acid and Diclofenac towards Aphthous Stomatitis Treatment.

Nanostructured patches composed of bacterial nanocellulose (BNC), hyaluronic acid (HA) and diclofenac (DCF) were developed, envisioning the treatment of aphthous stomatitis. Freestanding patches were prepared via diffusion of aqueous solutions of HA and DCF, with different concentrations of DCF, into the wet BNC three-dimensional porous network. The resultant dual polysaccharides-based patches with a nanostructured morphology present thermal stability up to 200 °C, as well as good dynamic mechanical properties, with a storage modulus higher than 1.0 GPa. In addition, the patches are non-cytotoxic to human keratinocytes (HaCaT cells), with a cell viability of almost 100% after 24 h. The in vitro release profile of DCF from the patches was evaluated in simulated saliva, and the data refer to a diffusion- and swelling-controlled drug-release mechanism. The attained results hint at the possibility of using these dual polysaccharides-based oral mucosal patches to target aphthous stomatitis.

Doxycycline: An option in the treatment of ulcerated oral lesions?

In addition to its antimicrobial effect, doxycycline has potent anti-inflammatory activity. In view of these pharmacological characteristics, its use in the management of inflammatory, autoimmune and granulomatous diseases has been proposed. The objective of this study was to investigate, through a systematic literature review, the effect of doxycycline on pain and healing of ulcerated lesions of the mouth. An electronic search was performed in accordance with PRISMA guidelines in PubMed, Cochrane Central Register, Web of Science, Bireme/LILACS and Scopus databases. Controlled, randomized clinical trials were selected. The concentration of doxycycline, frequency of application, pain relief and clinical remission of the lesions were analysed. According to the inclusion criteria, five articles were selected. In four of these studies, doxycycline was used in the treatment of aphthous stomatitis, and in one study, it was used in the treatment of herpes labialis. In all studies, the drug was used topically, both as a hydrogel and as a crushed tablet (along with a prosthetic adhesive). The groups treated with doxycycline showed faster healing of lesions and lower pain scores compared to placebo. The present study suggests that topical doxycycline has a positive effect on the treatment of recurrent aphthous ulceration and herpes labialis. Experimental animal studies and double-blind randomized clinical trials should be performed on other oral lesions, such as traumatic ulcers and mucositis.

A comparison of the effect of triamcinolone ointment and mouthwash with or without zinc on the healing process of aphthous stomatitis lesions.

Background. Recurrent aphthous stomatitis (RAS) is one of the most common ulcerative diseases of the oral mucosa. Definitive etiology of RAS has not been conclusively established. There is no certain treatment for aphthous stomatitis but some drugs such as steroids are commonly used for the treatment of RAS. Regarding the effect of zinc on the healing process of epithelial layer and cell division, in this research the effect of triamcinolone (with orabase) in combination with a zinc-containing mouthwash and triamcinolone alone on the healing process of RAS lesions was assessed.Methods. The present study consisted of 20 patients diagnosed with RAS. The patients were instructed to rinse the mouth-wash or placebo three times a day and triamcinolone ointment twice a day for two weeks. The largest dimension of the ulcer was measured by a digital caliper and the severity of pain was assessed by visual analogue scale (VAS). Number, size, duration, ulcer-free period and pain of the lesions were evaluated twice a week for twomonths. Data were analyzed by SPSS 16 using Mann-Whitney U test and t-test.Results. A decrease was seen in the mean pain severity score (P = 0.631) and the size of the lesions but it was not statistically significant (P = 0.739). Also the difference between the number of lesions (P = 0.739), duration and ulcer-free period (P = 0.873) were not statistically significant.Conclusion. Zinc mouthwash seems to be as effective on wound healing process as typical treatment modalities for RAS.

An Alternative Approach for the Treatment of Major Aphthosis: Case Report

An effective treatment for aphthous stomatitis is not still available and its treatment consists only of therapeutic measures to suppress its symptoms. We report a case of a 60-year caucasian woman presented a 16-year history of episodes of multiple major recurrent ulcers in the oral mucosa. Topical application of Oralmedic® caused an improvement of pain, discomfort and healing of the aphthous ulcer. No adverse events or side effects were recorded. Oralmedic® is an effective topical device for the major aphthous ulcers treatment.

Treatment of Aphthous Stomatitis with Topical Alchemilla vulgaris in Glycerine

Recurrent aphthous ulceration is the most common oral mucosal disease known. It presents as three types: minor (most prevalent), major and herpetiform. However, there are no well established effective and reliable treatments of this condition. Alchemilla vulgaris (Lady's Mantle) has traditionally been used in oral hygiene and was recently shown to accelerate wound healing when used in combination with glycerine. The objective of this study was to determine whether this combination is effective in the treatment of the most prevalent form of aphthous ulcers. An open-label study was conducted in 48 otherwise healthy male and female patients aged 4-44 years to determine the putative healing properties and tolerability of a standard 3% extract of A. vulgaris in glycerine (Aphtarine) on common minor oral ulcers. Patients with major or herpetiform ulcers were excluded from the study. Topical application three times daily of Aphtarine gel to minor mouth ulcers relieved discomfort and produced complete healing in the majority of patients (60.4%) within 2 days and in 75% within 3 days, compared with 10.4% and 33.3%, respectively, without treatment and 15% and 40%, respectively, with commonly available treatments. Most patients appreciated the product's ease of application, taste and texture. Aphtarine was well tolerated locally and most patients rated the product good to excellent overall. Aphtarine is a safe, well tolerated and highly effective promising new treatment for healing common mouth ulcers.

A new treatment of aphthous stomatitis

Objectives: The objectives of this study were to prepare, characterize, and examine in human beings a new mucoadhesive tablet releasing natural active agents, for pain reduction and rapid healing of canker sores. Methods: Adhesive tablets were prepared by compression molding of mixed powders of crosslinked poly (acrylic acid) and hydroxypropyl cellulose, absorbed with citrus oil composition and magnesium salts. The tablet erosion and the rate of citrus oil and magnesium salt release were determined as well as the adhesiveness of the tablet by an Instron tensiometer using bovine gingival tissue. A human study was conducted on 248 volunteers suffering from canker sores. The volunteers were divided in 2 groups, one received tablet loaded with the active agents and the other without actives. Results: Tablets adhere well to the mucosal tissue and gradually erode for 8 hours while releasing the citrus oil at a zero order pattern with the magnesium salt being released for 2 hours. Both tablets were effective in reducing pain and decreasing healing time ( P < 0.05) without adverse effects. However, tablets loaded with actives were more effective. Erodable mucoadhesive tablets, releasing citrus oil and magnesium salts, are effective in pain relief and enhance healing of canker sores in human beings. Conclusion: Mucoadhesive tablet was found highly effective in pain reduction and healing time in human beings.

A Clinical Trial: In Vivo Studies on Dexamethasone Sodium Phosphate Liposomes in the Treatment of Human Aphthous Stomatitis

Recurrent Aphthous Stomatitis (RAS) is the most common form of nontraumatic oral mucosal ulceration. Numerous studies have suggested a wide variety of the probable etiologic mechanisms (1). However, various therapeutic approaches that have been made to date are controversial since the etiopathology has not been evidently elucidated yet. Corticosteroids are the most commonly used drugs in the treatment of Aphthous Stomatitis. However, they cause side effects when they are used topically in conventional dosage forms (2-4). In earlier study, we tested the potential usage of liposomes as drug carriers to the ulcerated rat oral mucosa by determining whether liposomes could increase local and decrease systemic concentrations of a corticosteroid, dexamethasone sodium phosphate (DSP). We showed that liposomal encapsulation increased drug concentrations at the desired site of action and decreased concentrations at the internal organs (5).In this study by using the combination of two types of dosage forms i.e. liposome dispersion and adhesive gel (liposome dispersion in gel; Gelosome), the therapeutic effect of DSP liposomes was investigated comparing with control groups. The results were evaluated from clinical point of view and histopathologically. In addition, in vitro release and its fitting to different release models was investigated.

Thalidomide in Patients With Acquired Immunodeficiency Syndrome

To the Editor. — The use of thalidomide as a sedative drug was abandoned as soon as its teratogenic potential was recognized. Its beneficial effects on diverse immunologic and dermatologic disorders have recently been demonstrated. 1-5 Thalidomide is being used successfully as an alternative in the treatment of aphthous stomatitis in the acquired immunodeficiency syndrome (AIDS) population. 6-9 We describe two patients with AIDS and with prolonged refractory fever, tuberculosis, and Mycobacterium avium-intracellulare (MAI) bacteremia that showed a dramatic response to thalidomide therapy. Two patients with major aphthosis and three with minor aphthosis which proved resistant to corticosteroids were treated with oral potassium iodide at doses ranging between 600 and 900 mg/day. Healing began after 3 days and was complete in a week. Tolerance was excellent despite the disagreeable taste of the drug.

A 27-year-old woman with recurrent aphthous stomatitis failed to improve with several forms of therapy, including the use of tetracycline compresses. Cultures of the aphthae yielded heavy growth of αhemolytic streptococci resistant to tetracycline but sensitive to cephalosporins. Application of 1% cephalexin monohydrate compresses to the oral lesions produced rapid relief of pain and noticeable healing at this time as well as during later attacks. Skin testing showed marked hypersensitivity to α-hemolytic streptococci with extension of the allergic reaction for 23 cm along the adjacent major lymphatic vessel. Intradermal injection of an autogenous streptococcal vaccine produced not only 50 mm of erythema and induration but also a transient flare-up of the patient's oral lesions. A series of vaccine injections was associated with a progressively diminishing cutaneous response as well as clinical amelioration.
