Clinical experiences with untreated homologous vein grafts in reconstruction of arteries (author's transl)

50 transplantations of homologous vein grafts in reconstruction of arteries are reported on. Vein grafts were either transplanted immediately or used after deep freezing. This procedure has proved to be effective in the replacement of arteries during our observation period of four years. Results of homologous vein transplants are similar to those of autologous transplants.

Frühverschlüsse in 1,5% ohne Frühamputation, bisher Spätverschlüsse in 3%, deutlich verbessertes Oszillogramm in 2,5% und einwandfrei tastbarer peripherer Puls in 93%. Kombinierte ein- oder zweizeitige aorto-iliacale und Femoralis-Rekonstruktion bei 51 Patienten. Postoperative Letalität 1,0% durch akuten Myokardinfarkt bei 2 Patienten.

Partial anomalous pulmonary venous drainage associated with mitral stenosis: Report of a case with surgical correction of both lesions

1. 1. A case of partial anomalous pulmonary venous drainage into the left innominate vein associated with mitral stenosis is reported. 2. 2. Mitral commissurotomy and transplantation of the anomalous vein into the left atrial appendage were successfully performed at one operation. 3. 3. A follow-up of one year is presented.

The collateral circulation is established within 1-2 weeks in experimental animals after obstruction of the portal vein. These animals had no operation except on splenic vein and superior pancreatico-duodenal vein. In patients subjected to pancreaticoduodenectomy for carcinoma of the pancreas or bile duct, the collateral formation was relatively limited by resection of the tumor with half of the stomach, most part of the duodenum, half of the pancreas and common bile duct. As already being shown by Edwards, 2) accessory portal veins in human beings are important in establishment for collaterals toward the liver. It seems important to preserve the vessels around the hepatic portae as far as no carcinomatous infiltration exists. Though (12), (13) & (14) in table I are not described in the literature, they do not seem to play an important role as portal collaterals. Veins described by Retzius1) is important when superior mesenteric vein is obstructed. (9) seems to have been built by the pressure difference of the bed of gastrosplenic vein and mesenteric veins during the operation. The collateral vessels (superior haemorrhoidal vein middle haemorrhoidal vein-venules in the pelvis-spermatic vein) described by Retzius were not discovered by the author. The collaterals (a) Portal vein -paraumbillical vein in the abdominal wall, i.e. Caput Medusae; (b) Sappey's vein3) -diaphragmatic veins did not appear. The reason that varix in the esophagus was not noticed is probably due to the small amount of blood flow in the esophageal vein, because the veins leading to it are cut at the operation when the portal vein is transplanted.

DEBAKEY and Simeone, 1 Hufnagel, 2 Schloss and Shumacker, 3 and others have reviewed the development of techniques for restoring continuity of severed blood vessels by direct anastomosis and methods for bridging of arterial defects with the use of transplants of veins and arteries or tubes of glass, metal, or plastic. Transplantation of veins was recommended by Blakemore, Lord and Stefko, in 1942, 4 as an emergency method for bridging defects in severed primary arteries. This technique has been helpful for certain emergency problems. Vein grafts as tried experimentally for defects of the aorta by Shumacker, Schloss, Freeman, Wayson, and Stahl, 5 Nabatoff, Touroff, and Gross, 6 Sako and Varco, 7 Johnson, Kirby, and Hardy, 8 Schmitz and co-workers, 9 and others have usually dilated. THE RECENT experimental and clinical contributions in vascular surgery have stimulated an interest in this field which may lead to further widening of the scope of surgery. The soluble rod for arterial anastomosis which was described by Smith 1 was designed to simplify the suture technic which was developed chiefly by Carrel, 2 Horsley, 3 and others, for blood vessel anastomosis. The vitallium tubes which were introduced by Blakemore, Lord and Stefke 4 for vascular anastomosis and for restoring arterial defects by vein transplants represent a promising technical development in vascular surgery. The absorbable tubes composed of fibrin which were devised by Swenson and Gross 5 for blood vessel anastomosis represent another important development which overcomes a disadvantage of the vitallium tube in children's surgery where it would obviously be desirable for the vascular channel to increase in size with the growth of the child. The first recorded surgical attempt to reestablish the seminiferous channels was that of Bardenheuer, 1 who in 1886, following excision of the epididymis for tuberculosis, implanted the cut distal end of the vas deferens in the testis. Since then many operations have been devised for the relief of male sterility due to obstructive lesions in the epididymides. These fall into three main categories: 1. Vaso-epididymostomy 2 2. Vaso-orchidostomy 3 3. Miscellaneous operations ( a ) Vaso-orchidocystostomy 4 ( b ) Sac epididymostomy 5 ( c ) Transplantation of the saphenous vein as a substitute for the vas deferens 6 ( d ) Vaso-urethrostomy 7 There are valid objections to most of these operations on anatomic and physiologic grounds. 8 It appears from a review of the literature that Martin's vaso-epididymostomy with technical modifications is the most popular and the most successful operation for the reestablishment of the seminiferous channels following bilateral obstructive epididymitis.
