Statistics documenting the relentless rise of women in American medicine, primary care specialties, and surgery are presented. Representation of women in many surgical societies, on journal editorial boards, and in surgical texts does not appear to have kept pace. The participation of women surgeons in each of these activities ranges from 0.8% to 1.7%, equivalent to the percentage of women in general surgery before 1975. Comparison is made with women surgeons in an open national society (8.2%), an open regional society (6.9%), an open editorial board (7.5%), and a recently published textbook of surgery (8.2%). These proportions parallel the current 6.3% of women in general surgery and support the premise that women are at least as likely as their male colleagues to participate in the professional activities of the surgical establishment. During the waning years of the 20th century, ever-increasing numbers of qualified women will come forward to join the hierarchy of American surgery.

Variability of the Harris‐Benedict Equation in Recently Published Textbooks

The Harris-Benedict equation, as cited in a selection of textbooks of medicine, surgery, and nutrition, was compared with the original monograph published by the Carnegie Foundation. A significant amount of variation was found in published versions of this equation. Of 24 published versions, eight were sufficiently variant to produce errors of 7% to 55% in calculations of nutrition requirements. The error is usually in the constant term for one of the two Harris-Benedict equations. Either the constant term for men is increased to a value approximately the same as for women, or the constant term for women is reduced to be approximately equal to that for men. The primary reason appears to be the lack of availability in medical libraries of the original Carnegie Foundation publication by Harris and Benedict.

What's new in monitoring the coronary surgery patient?

Monitoring has been extensively reviewed in most textbooks of cardiothoracic surgery and anaesthesia, particularly in the recent textbooks on monitoring edited by Carol L Lake 1 and Casey D Blitt 2 and in the Journal of Clinical Monitoring. Although monitoring properly includes both pre- and postoperative periods, this review will concentrate exclusively on the operative period. I will also concentrate on new approaches or information which relate to more traditional approaches to monitoring. The emphasis in this review will not be on what we can monitor, but rather on what we should monitor. In this regard, I will analyse accuracy and identify sources of error and try to answer the following questions. Does the device or parameter measure (monitor) what we want to know? Does it improve patient outcome and safety? Is it cost-effective? Unfortunately, data are not always available to answer all these questions at present, but hopefully the discussions will make us aware of what we do and do not know, and what we should look for in the near future.

There must be a compelling reason for a 14th edition of a textbook to be published only five years after its predecessor. Neither the publisher, editor, nor chapter authors will invest the enormous resources necessary unless each considers the effort worthwhile. In the case of Sabiston's text, it seems clear that the time and effort are well spent. As the subtitle suggests, emphasis is placed on the biologic basis of surgery. The basic science database on which clinical surgery depends has grown so fast during the past decade that an update on this basis alone is justified. New diagnostic and therapeutic
