Results of surgical treatment of gastric cancer--special reference to pathological findings.

During 16 year period from 1967 to 1982, a total of 1,471 patients with gastric cancer were surgically treated at the authors' clinic and 1,347 of them underwent gastric resection. The 5-year survival rate for 692 of curatively operated patients was 73.8%, in contrast to 11.8% for non-curatively operated cases. A correlation study disclosed the depth of cancer infiltration and the extent of lymphnode metastasis to be most reliable prognostic factors. The 5-year survival rate for stage I cases was 91.0%, 70.0% for stage II, 46.7% for stage III, and 8.6% for stage IV. An improvement in the end-results was attributed mainly to the increased incidence of early cancer in surgical cases, and partially to proper adjuvant chemotherapy for the advanced cancer cases during or long-term after surgery. In fact, the cases of early gastric carcinoma whose depth invasion was limited within the mucosa or submucosa constituted 41.9% of the authors' surgical series in 1967-1982, and 5-year survival rates of patients in stages II, III and IV receiving cancer chemotherapy strikingly improved to 80.6%, 57.3%, and 13.2%, in contrast to 59.7%, 35.2% and 2.4% for control groups not treated with anticancer agents.

Possible role of lymph node dissection in the surgical treatment of gastric cancer with disseminating peritoneal metastasis.

In palliatively gastrectomized patients, there was a close relationship between the extent of cancer invasion to the gastric serosa and postoperative survival; the less extensive the serosal invasion, the longer was the survival time. The relationship between the extent of lymph node dissection and postoperative survival showed that, in the presence of metastasis to Group 1 and 2 lymph nodes, many of the long-term survivors had undergone dissection of these lymph nodes. Although a sweeping conclusion should be avoided since retrospective analysis forms the basis of this report, it is assumed that in patients with gastric cancer and peritoneal metastasis, surgery should not be confined to resection of the primary focus, but should include regional lymph node dissection.

Adjuvant studies in gastric cancer in the United States.

Early optimism for the chemotherapeutic augmentation of standard surgical treatment of gastric cancer has thus far generally not come to pass. First attempts to destroy circulating cancer cells in small metastatic foci by giving drugs at the completion of operation and in the immediate postoperative period were disappointing. Numerous large cooperative groups and individual institutions are now studying drugs and drug combinations in various treatment regimens and have introduced the use of radiotherapy and immunotherapy in an effort to improve long-term survival following surgical resection of gastric cancer. Resurgence of interest in surgical adjuvant studies, and especially recent encouraging results from drug and immunotherapy, generates optimism that the theoretical potential of this approach to cancer control may soon be realized.

Guided hemodilution in radical surgery for gastric cancer

The use of sub-narcotic controlled hemodilution in the surgical treatment of gastric cancer creates conditions for improving the indicators of central and peripheral blood circulation, reducing intoxication and preventing complications in the operated patients and preventing complications in the operated patients.

Histologically positive esophageal margin in the surgical treatment of gastric cancer

Histologically positive esophageal margins caused by transection of the esophagus in an area involved by disease were encountered in 20 percent of 350 patients undergoing total or proximal subtotal gastrectomy for gastric cancer. There was a significant increase in the incidence of positive margins in patients with neoplasms of the cardia and in those who were managed without frozen-section examination of the resected esophagus. Positive margins were avoided only with resection of 12 cm or more of macroscopically tumor-free esophagus above the primary. Despite the presence of tumor at the margin, anastomotic recurrences appeared in only 23 percent of the patients at risk. Recurrences affected mainly those who had TNM stage I and II lesions. Patients with more advanced disease usually died from metastasis without developing anastomotic recurrence. Positive margins were associated with a poor prognosis that was not influenced by adjuvant postoperative treatment. The avoidance of positive margins by wide esophagectomy is important in patients with TNM stage I and II disease, particularly if the primary lesion is located in the cardia. Patients with positive margins should be watched closely rather than subjected to further treatment.

The role of lymphadenectomy in curative surgery for gastric cancer.

AbstractA total of 530 cases of gastric cancer treated by curative surgery was analyzed with regard to lymph node metastases and survival rate. During this period, curative resections made up 75.9% of all resections (530/698) for gastric cancer, and the 5‐year survival rate was 50.6% (268/530). A total of 15,739 regional lymph nodes (an average of 30 per specimen) removed at surgery were examined histologically for metastases, and 16.0% were found to be positive. Of 339 patients with advanced, transmural cancers, 74.9% had lymph node métastases; those with negative nodes had a 5‐year survival rate of 63.5%, while those with positive nodes had a 5‐year survival rate of 29.9%. The 5‐year survivors with positive nodes made up 58.5% of all 5‐year survivors. The almost complete removal of at least the primary and secondary lymph node groups draining a gastric neoplasm is an essential part of the curative surgical treatment of gastric cancer. For lesions in the upper and middle portions of the stomach that invade the serosa of the posterior wall, total gastrectomy with a caudal hemipancreatectomy and splenectomy should be aggressively performed, so as to accomplish complete en bloc removal of the lymph nodes in these regions.

Clinical and experimental studies of oral 5-FU emulsion as an adjuvant to the surgical treatment of gastric cancer.

AbstractBased on the propensity of fat emulsion to be absorbed mainly into lymphatic capillaries and regional lymph nodes, we studied experimentally and clinically the effectiveness of oral administration of a 5‐fluorouracil (5‐FU) emulsion as an adjuvant to surgery in the treatment of gastric cancer, especially when there were lymph node metastases. Experiments in rats showed that drug concentration in the regional lymph nodes was higher and remained longer after oral administration of 5‐FU emulsion than after oral administration of an aqueous solution of 5‐FU. Furthermore, measurement of the concentration of 5‐FU in the regional lymph nodes of 13 patients with gastric cancer who had been given the 5‐FU emulsion or 5‐FU in water orally 2 hours before surgery showed that the mean 5‐FU level was higher in the patients that received the emulsion than in those who received the aqueous solution. In the past 3 years, we have administered 5‐FU emulsion to 123 patients with gastric cancer for a period of 1–3 weeks before surgery, and examined histologically the effect of this therapy on the met astatic foci in the lymph nodes. A positive change, such as marked necrosis or marked degeneration, was found in 62% of the metastatic lesions. The results suggest that preoperative oral administration of 5‐FU emulsion may be effective as an adjuvant to surgery for gastric cancer, particularly in patients with metastases to lymph nodes.
