A Persuasive in the Management of Indralupta (Alopecia areata) with Ayurvedic approach

The aim of the case study is to evaluate the efficacy of Leech therapy along with ayurvedic treatment in Alopecia areata (INDRALUPTA). It is prospective interventional case report on alopecia areata treated with leech therapy and ayurvedic formulations. Using a subjective evaluation, an improvement in growth of hair on bald patches over scalp region was observed. An 18-year-old female patient, resident of Delhi, presented with complaints of two bald patches (right parietal & vertex) over the scalp without any secondary changes. With these complaints, the patient visited Shalya Tantra, Outpatient department (OPD No 16) at Chaudhary Brahm Prakash Ayurved Charak Sansthan, New Delhi. She took treatment for six weeks and got complete remission in her symptoms. Line of treatment adopted was tridosha shamaka, rakta-shodhaka and raktamokashana (Leech application). Leech therapy supplemented with ayurvedic formulations have definite role in Alopecia Areata (Indralupta) especially of recent origin. A case series or pilot study may further strengthen above case report and open new treatment arena for such cases. This case report can also be used to formulate hypothesis for management of baldness from an ayurvedic perspective.

Bone impaction grafting with trabecular metal augments in large defects in young patients: unravelling a new perspective in surgical technique

BackgroundAcetabular reconstruction with bone impaction grafting in large defects has yielded conflicting results.MethodsThis was a retrospective study of a case series of five patients with a young age (≤50 years) at the time of surgery who had large acetabular defects reconstructed by bone impaction grafting and trabecular metal augments. The mean follow-up was 79 months. We describe the surgical technique in detail.ResultsImprovement was significant on the WOMAC and SF-36 scales (p < 0.05). The radiographs taken at the last follow-up examination showed no migration of the polyethylene cup (p = 0.31) or differences in the abduction angle (p = 0.27) compared to the radiographs from the immediate postoperative period. One patient presented two dislocation episodes as a complication.ConclusionThe combination of trabecular metal augments with the bone impaction grafting technique in young patients with large acetabular defects provides satisfactory results in the long term and restores the bone stock.

A novel trans-muscular approach for microsurgical bilateral decompression via unilateral approach for degenerative lumbar stenosis: a series study

Objective: To compare clinical outcomes of microsurgical bilateral decompression via unilateral approach (MBDU) between trans longissimus and iliocostalis approach, a novel lateral paraspinal approach(LPA), and classic Wiltseapproach for single level degenerative lumbar stenosis. Methods: LPA approach was researched by imaging measurement and cadaver anatomy. Retrospective analysis of a total 124 cases(male/female 75/49, aged(55±14) years) received single-level MBDU due to degenerative lumbar stenosis from 2016 to 2018 in Beijing Tiantan Hospital. Patients were categorized into two groups according to spinal canal morphology. Group A: trefoil shape (n=48), received MBDU via LPA; Group B: round & oval shape (n=76), received MBDU via Wiltse approach. Clinical parameters, including visual analogue scale (VAS) of pain, OswestryDability Index (ODI), satisfaction rate were assessed before the operation and at the latest follow-up. The internal fixation and intervertebral fusion were evaluated by X-ray and CT.The data were compared with single sample t test between the two groups. Results: The feasibility of trans longissimus and iliocostalis approach was proved by imaging measurement and anatomic study. The tube's maximum tilt angle in LPA group was 65°, and it was 40°in Wiltse group. There was no significantly difference in the operating time, bleeding and drainage volume between the two groups((120±27) min vs (115±32) min, (104±31) ml vs (110±41) ml, (50±15) ml vs (47±18) ml, respectively, t=1.246,-1.917,1.730,all P>0.05). In both groups, VAS and ODI scores significantly improved at the latest follow-up when compared with those before the operation (all P<0.05), but those were all comparable between the two groups(all P>0.05). The excellent rate of subjective satisfaction for the clinical efficacy in group A and B was 89.6%(43/48) and 86.8%(66/76) respectively (χ(2)=0.208,P>0.05). Conclusions: LPA approachfor microsurgical bilateral decompression via unilateral approach (MBDU) in degenerative lumbar stenosis is safe and effective, it has advantages in direct vison of contralateral nerve root canal decompression for trefoil shaped spinal canal.

Forecasting Hierarchical Time Series in Power Generation

Academic attention is being paid to the study of hierarchical time series. Especially in the electrical sector, there are several applications in which information can be organized into a hierarchical structure. The present study analyzed hourly power generation in Brazil (2018–2020), grouped according to each of the electrical subsystems and their respective sources of generating energy. The objective was to calculate the accuracy of the main measures of aggregating and disaggregating the forecasts of the Autoregressive Integrated Moving Average (ARIMA) and Error, Trend, Seasonal (ETS) models. Specifically, the following hierarchical approaches were analyzed: (i) bottom-up (BU), (ii) top-down (TD), and (iii) optimal reconciliation. The optimal reconciliation models showed the best mean performance, considering the primary predictive windows. It was also found that energy forecasts in the South subsystem presented greater inaccuracy compared to the others, which signals the need for individualized models for this subsystem.

Tamis sin Cierre de la Brecha Quirúrgica

Antecedentes: La cirugía mínimamente invasiva transanal (TAMIS), es una técnica que resulta especialmente útil en lesionesde recto extra peritoneal difíciles para el abordaje por vía anterior.Actualmente sigue siendo controversial el cierre del defecto rectal por debajo de la reflexión peritoneal. La suturaendoluminal es uno de los aspectos más desafiantes del procedimiento y la literatura es discordante con respecto al cierrede la brecha quirúrgica. Nuestro objetivo es realizar un estudio descriptivo de una serie consecutiva de pacientes conlesiones de recto extraperitoneal sometidos a cirugía de TAMIS, dejando el lecho quirúrgico abierto.Objetivo: Descriptivo prospectivo longitudinal.Material y Método: Entre agosto de 2013 y julio de 2019 se incluyeron, en forma consecutiva, todos los pacientes conlesiones de recto extra peritoneal, operados con técnica de TAMIS, que presentaron lesiones benignas ≥ a 3 centímetros dedifícil resolución endoscópica, tumores neuroendocrinos ≤ de 2 cm, adenocarcinomas T1N0 sin signos histológicos de malpronóstico, adenocarcinomas T2N0 con elevado riesgo quirúrgico o que rehusaron la cirugía radical y pacientes con dudasde la remisión completa luego de la neoadyuvancia. Todos los procedimientos fueron realizados por el mismo cirujano y entodos se dejó el lecho quirúrgico abierto.Resultados: En un período de 6 años se trataron 35 pacientes con técnica de TAMIS, con una edad promedio de 61 años±12.El tamaño promedio de las lesiones fue de 3.68±2.1 cm (0.7- 9 cm) y la distancia desde el margen anal al extremo distal de lalesión de 5.7±1.48 cm (3 a 8.5 cm). El tiempo operatorio fue de 39.2±20.5 min y el tiempo de internación de 33±28.2 horas.La anatomía patológica de las lesiones resecadas fue: 15 adenomas, 3 tumores carcinoides y 17 adenocarcinomas. 1 ypT0,4 carcinomas in situ, 4 tumores T1 y 8 tumores T2. En todos los casos se dejó el defecto rectal abierto, con un seguimientomínimo post operatorio de 9 meses. No hubo mortalidad y la morbilidad global fue de 14.2% (5 pacientes). Dos pacientesfueron re internados para el tratamiento del dolor (Clavien-Dindo II) y 3 pacientes (8.5%) presentaron sangrado postoperatorio, de los cuales solo 1 (2.8%) requirió re intervención (Clavien-Dindo III). No hubo complicaciones infecciosas,estenosis de recto postquirúrgicas ni perforaciones hacia la cavidad abdominal.Conclusión: Nuestros resultados permiten describir a la técnica de TAMIS dejando la brecha quirúrgica abierta, como unatécnica con buenos resultados, alta factibilidad y baja tasa de complicaciones.

Should Lopinavir/Ritonavir combination be used in the treatment of COVID-19?

Background. Lopinavir/Ritonavir is an oral combination agent being used to treat Human Immunodeficiency Virus. It was demonstrated to have clinical and in vitro activity against coronaviruses. There are no specific anti viral preparation or biologic agents currently recommended to treat Severe Acute Respiratory Syndrome Corona Virus&#x0D; 2(SARS-CoV-2) infection. There are no in-vitro studies published as well, for Lopinavir/ritonavir against SARS- CoV-2. However, recent case series and cohort studies showed earlier clinical improvement and shorter duration of viral shedding were noted especially if the combination drug is administered early in the course of the disease.&#x0D; Objective. The objective of this review is to search, retrieve, appraise and summarize existing studies and clinical trials regarding the efficacy and safety of Lopinavir/Ritonavir combination in the treatment of SARS Cov-2 infection. The evidenced based recommendations may be used as a guide by clinicians and practitioners in their current practice.&#x0D; Methods. Electronic databases were searched for evidences (Medline, CENTRAL, ISRCTN registry, ClinicalTrial. gov and Chinese Clinical Trial Registry) and articles were selected based on a pre-defined criteria according to population, intervention, outcome and study designs.&#x0D; Results. Two randomized controlled trials were included in this review. Lopinavir/ritonavir treatment did not significantly accelerate clinical improvement, reduce mortality, or shoten the viral RNA detectability in patients with serious COVID-19.&#x0D; Conclusion. There is little evidence to conclude on the effectiveness of lopinavir/ritonavir in patients with COVID-19. More high-quality randomized controlled trials are needed.

OTRA MIRADA A LA REPRESENTACIÓN DE FUNCIONES EN SERIE DE FOURIER

Aprender matemáticas, requiere de razonamiento, de desarrollo del pensamiento lógico, algorítmico y ordenado. Algunos estudiosos de estas ciencias duras la identifican como la ciencia que necesita de los patrones. Cuando se trabaja con sucesiones y series, se reconocen patrones, por eso es tan importante hacerlo desde una posición constructivista, involucrando al aprendiz en la construcción y reconstrucción de su propio aprendizaje, proporcionando la importancia que requieren las conexiones entre conocimientos ya adquiridos y los nuevos por aprender, demostrando la utilidad práctica y la significación social del contenido. En el estudio de series, en la práctica se presentan problemas cuyos modelos son funciones que no son continuas o son de una naturaleza que no permiten el uso de las series de potencias. En el análisis de funciones, esencialmente en la Ingeniería Civil, se ha profundizado en el estudio de funciones que pueden escribirse como una serie de potencias, en muchas ocasiones se obvian otras funciones que describen con frecuencia, fenómenos vivenciales, los cuales por su complejidad no son abordados. En consecuencia, esta investigación se enfoca a estudiar otro tipo de serie que permite representar una clase más amplia de funciones: la serie de Fourier, a partir de la aplicación de métodos tales como: inducción-deducción y análisis-síntesis, se analizan las principales definiciones, teoremas y sus demostraciones desde una mirada diferente a las tradicionales, métodos que provocaron una asimilación significativamente superior a etapas anteriores. Mapping Urbanization as an Anthropedogenetic Process: A Section through the Times of Urban Soils

Current trends of spatial planning policies give a strategic role to soils, the multifunctionality of which must be considered as a crucial driver facing cities’ forthcoming social-ecological transition. However, soils within urban areas are insufficiently studied as a long-term record of environmental history and heavy anthropization. This article investigates the extreme qualitative variability of urban soils by presenting a conceptual model and cartographic workflow highlighting soil evolution processes as a value which co-variates with urbanization. Based on a case study in West Lausanne (Switzerland), the layers and map series of an atlas underscore the applicability of different types of information and spatial analysis for documenting the influence of anthrosediments and land cover changes. Combined with empirical profile descriptions, such a consolidated concept map defines a template, in the form of a complex spatio-temporal figure, on which to apply the state factor approach. Instead of using a simple spatial transect or gradient, the increasing anthropic dominance over original landscape conditions is explained using a section through time. An urban anthroposequence consequently retraces contrasting soil development pathways as a coherent bundle of historical trajectories. Such a narrative integrates various facets of land use, including one-off construction techniques and recurring maintenance practices, planning tools, and morphologies, into a specific ‘project for the ground’ which brought forth the mixed mesh of the Swiss Plateau ‘cityterritory.’ Ultimately, the dynamic vision conveyed by these intertwined soil–urbanization coevolution trajectories outlines opportunities for the regeneration of the resource deposit made up of both West Lausanne’s urban fabric and its soils as a palimpsest.

An overview of 30 days of confirmed COVID-19 cases in the state of Ceará

Objective: to analyze the evolution of the number of confirmed COVID-19 cases in the 30 first days of transmission in the state of Ceará. Methods: ecological study of a temporal series. Measures of position and dispersion were calculated, as well as the coefficient of incidence for the first 30 days. The evolution of the number of cases was compared to the trend of advance of the disease. Results: in the 30th day there were 1,800 confirmed cases, peaking in the 23rd day. This study assumes that the measures implemented in the state started to influence the evolution of the disease in the 26th day after the first confirmed case. The incidence coefficient increased 146.5% between the 21st and the 30th days. Conclusion: the mean number of new cases consistently increased in the period analyzed, suggesting that the epidemic has not yet been controlled in the state, but is decelerating.

Coronary artery double stenting techniques and their results in complex left main bifurcation disease.

Aim: Complex left main (LM) bifurcation disease seems to be better approached by a planned double stent technique. Materials & methods: Medline search for articles including randomized trials, prospective series, large registries and retrospective studies >50 patients has been performed. Results: Double kissing crush demonstrated its superiority over culotte stenting and cross over, while other techniques such as the T-stenting and T-stent and Protrusion have not been extensively reported in LM setting. The nano inverted-T-stenting has provided evidences that the use of ultrathin strut stents and very minimal crush is beneficial for both the physiological and rheological properties. Conclusion: The double stenting techniques used in LM should be evaluated in terms of procedural differences and technical simplicity.

Este trabajo tiene como objetivo el estudio de una serie artística de representaciones de diversos pazos y fortalezas gallegas, emplazados en dominios vinculados a los marqueses de Camarasa, que se conservan en la sección de “Mapas y Planos” del Archivo Ducal de Medinaceli, no hace mucho catalogada. La serie se compone de siete bellos planos paisajísticos realizados en 1805 por Pedro Peinador Castrillo, cuya figura aquí descubrimos. El autor de esta planimetría histórica demuestra, por un lado, un profundo conocimiento de los dominios gallegos vinculados a la Casa nobiliaria titular de esos inmuebles (Amarante, Ribadavia, San Miguel das Penas, Puebla de Parga, Oca...) por su condición de administrador general entre 1779 y 1817. Pero, por encima de ello, Peinador en estos bellos planos manifiesta sus dotes de gran dibujante e ilustrador, que domina la técnica de la composición sobre las artes cartográficas, desvelando una faceta suya no del todo potenciada hasta ahora ni tampoco en toda su extensión.&#x0D; &#x0D; [gl] Este traballo ten como obxectivo o estudo dunha serie artística de representacións de diversos pazos e fortalezas galegas, emprazados en dominios vinculados aos marqueses de Camarasa, que se conservan na sección de “Mapas e Planos” do Arquivo Ducal de Medinaceli, non hai moito catalogada. Atomic elastic stiffness analysis to predict twinning in Fe single crystal under shear

As a series study that discusses local instability by atomic elastic stiffness, Bija=Δσia/Δεj, molecular dynamics simulations of simple shear on Fe perfect lattice are performed to discuss twin deformation. In the simulation of extremely low temperature of 0.1K, many twin boundaries are nucleated in a periodic slab cell of stacked (1¯1¯2) planes just after the stress drop or elastic limit under shear. The 1st eigenvalue ηa(1) of the 6×6 matrix Bija shows fluctuation in each (1¯1¯2) planes just before the elastic limit. At the stress-strain peak, ηa(1)<0 or unstable layers emerge and twin deformation occurs at these unstable “band” in the simulation cell. The period of the ηa(1) fluctuation is almost constant even if the cell size is expanded in the [1¯1¯2] and [11¯0] direction, while the unstable band arrays tilt normal to the diagonal line of the simulation cell, under the cell expansion in the [111] direction. At the elevated temperature of 300K, the simultaneous periodic band never appears but a few unstable bands appear locally in time sequence and bring twinning there.

Theoretical and Experimental Studies of Subnanoscale Molecules and Clusters

A brief review is given of theoretical and experimental studies of a series of subnanoscale molecules and clusters performed by the authors with the staff of the Institute of Atomic and Molecular Sciences and the Genomics Center, Academia Sinica (Taipei, Taiwan) using quantum chemical calculations and experimental means of laser spectroscopy and mass spectrometry in gas phase. Several systems are considered: (a) the fragmentation and association of heme, hemin, and beta-hematin; (b) the structure, stability and chirality of the isomers of the protonated octamer of serine; (c) the stability and structure of Li- and Na-substituted saccharides, fullerols, and fullerol-like polyhydroxyl clusters; and (d) the theoretical prediction and experimental synthesis and study of the endohedral methane–fullerene cluster CH4@C60.

PENGARUH ZAKAT, INFLASI DAN PERKEMBANGAN USAHA MIKRO KECIL MENENGAH TERHADAP PERTUMBUHAN EKONOMI TAHUN 2011-2018 (STUDI KASUS DI INDONESIA)

This research aims to analyze the influence of zakat, inflation and the development of micro small and medium enterprises (UMKM) to economic growth in the Indonesia period 2011-2018. This research uses quantitative approach methods. This research is a secondary study of data time series that was subsequently analyzed using the EVIEWS 09 application. The data used ate quarterly data on zakat, inflation and the development of micro small and medium enterprises and gross domestic product (PDB) period 2011-2018. The results of this research show that zakat has a positive and insignificant effect, Inflation has negative and insignificant, and the development of micro small and medium enterprises (UMKM) has a positive and insignificant effect to economic growth in Indonesia. Caracterización de lidocaína y articaína mediante técnica troncular en exodoncias de terceros molares inferiores incluidos

Objetivo: Caracterizar los efectos de tiempo de latencia, tiempo de profundidad y tiempo de duración total de articaína al 4% y lidocaína 2% ambas con epinefrina 1:100.000, mediante técnica troncular para la exodoncia con odontosección de terceros molares inferiores incluidos. Materiales y Métodos: Se realizó un estudio descriptivo de tipo de serie de casos transversal seleccionada por un muestreo no probabilístico por conveniencia de 27 sujetos, con terceros molares inferiores incluidos con necesidad de odontosección donde durante el procedimiento hayan utilizado Articaína 4% o Lidocaína al 2%, ambas con epinefrina 1:100.000. Se recopilaron datos en relación al procedimiento. Resultados: El tiempo total de duración del efecto anestésico fue mayor con el uso articaína al 4% versus lidocaína 2%, siendo esto estadísticamente significativo, no así en el tiempo de latencia y el tiempo de profundidad anestésica que no presentaron significancia estadística. Conclusión: La articaína al 4% presenta el mejor tiempo de duración total del efecto anestésico, por lo que sería preferente para procedimientos de larga duración, permitiendo mayor comodidad post-operatoria.

Synthesis, biological activity, molecular docking studies of a novel series of 3-Aryl-7H-thiazolo[3,2-b]-1,2,4-triazin-7-one derivatives as the acetylcholinesterase inhibitors

The acetylcholinesterase inhibitors play a critical role in the drug therapy for Alzheimer’s disease. In this study, twenty-nine novel 3-aryl-7H-thiazolo[3,2-b]-1,2,4-triazin-7-one derivatives were synthesized and assayed for their human acetylcholinesterase (hAChE) inhibitory activities. Inhibitory ratio values of seventeen compounds were above 55% with 4c having the highest value as 77.19%. The compounds with the halogen atoms in the aromatic ring, and N,N-diethylamino or N,N-dimethylamino groups in the side chains at C-3 positions exhibited good inhibitory activity. SAR study was carried out by means of molecular docking technique. According to molecular docking results, the common interacting site for all compounds were found to be peripheral anionic site whereas highly active compounds were interacting with the catalytic active site too. HIGHLIGHTS A novel series of 3-aryl-7H-thiazolo[3,2-b]-1,2,4-triazin-7-one derivatives were synthesized and assayed for their human acetylcholinesterase (hAChE) inhibitory activities. The SAR study of the target 3-aryl-7H-thiazolo[3,2-b]-1,2,4-triazin-7-one derivatives was summarized. The active sites in the acetylcholinesterase were analyzed by molecular docking technique. UTILIZAÇÃO DE ESTATÍSTICA DESCRITIVA E DE MODELO SARIMA NO ESTUDO DE PRECIPITAÇÃO NA REGIÃO SUDESTE DE MATO GROSSO

Os modelos da classe ARIMA mostram ter grande potencial nos estudos de séries temporais de dados de chuva. Presente no Sudeste do Estado de Mato Grosso, o município de Rondonópolis apresenta grande destaque no cenário agropecuário e é um dos principais polos econômicos mato-grossenses. Diante da importância da aplicação de modelos de bons desempenhos, e da escassez de trabalhos utilizando esse método nessa região, este trabalho teve como objetivo a obtenção de um modelo do tipo SARIMA para o fenômeno de precipitação pluviométrica entre os anos de 2004 e 2015 no município de Rondonópolis-MT. Analisaram-se também parâmetros estatísticos descritivos, referentes à população de dados, para identificação das estações climáticas nesse período. Para isso, utilizaram-se dados referentes ao período de 2004 a 2015, pertencentes ao INMET (Instituto Nacional de Meteorologia). Para aplicação do modelo SARIMA, foram adotados a metodologia e os indicadores de desempenho propostos na literatura. A média da precipitação acumulada anual nesse período (2004 a 2015) foi de 1330 mm com desvio padrão de 145,6 mm, sendo as estações de chuva e seca definidas pelas faixas de meses outubro-março e abril-setembro, respectivamente. Obteve-se o modelo SARIMA (2,0,0) x (0,1,2) como o mais adequado, apresentando melhor performance em relação aos dados observados.
