Three-year evaluation of different adhesion strategies in non-carious cervical lesion restorations: a randomized clinical trial.

:Objective:To evaluate non-carious cervical lesions (NCCLs) restored with different adhesion strategies.Methodology:This is a prospective, randomized, double-blind, split-mouth study. An adhesive restorative system (Single Bond Universal/Filtek Z350XT – SBU) was evaluated both without and with selective enamel conditioning (E-SBU), resin-modified glass-ionomer cements (Vitremer; RMGIC), and ethylenediaminetetraacetic acid pretreatment (EDTA; E-RMGIC). In total, 200 restorations, placed in 50 patients, were evaluated at baseline and at a 3-year follow-up using the modified United States Public Health Service (USPHS) criteria. Data were analyzed using the two-proportion equality test, multinomial logistic regression, Wilcoxon test, and Kaplan-Meier survival curves.Results:In total, 42 (84%) patients returned for the 3-year follow-up. SBU showed restoration losses statistically different from RMGIC. Retention was also statistically different in SBU between baseline and the 3-year follow-up. Marginal defects and surface texture were statistically significant for all groups in the period studied, except for the surface texture of SBU and the marginal integrity in E-RMGIC. We observed no statistically significant difference in wear, secondary caries, anatomical form, surface staining, and color over time. Recession degree was the only factor to influence retention rates. Cumulative survival (%) was 89, 98, 98, and 95.3, for SBU, SE-SBU, RMGIC, and E-RMGIC, respectively, without significant differences among them. There was a statistically significant difference between survival curves; however, multiple comparison procedures found no statistical differences.Conclusion:Selective enamel etching affected the retention of non-carious cervical restorations. Adhesion using EDTA and resin-modified glass-ionomer cements delayed marginal defects over time. The degree of gingival recession influenced retention rates. Resin composite restorations showed initial marginal defects, and ionomer restorations, reduced surface luster. EDTA pre-treatment followed by resin-modified glass-ionomer cements may be a promising adhesion strategy for NCCL restorations.

A prospective and randomized clinical trial evaluating the effectiveness of ART restorations with high-viscosity glass-ionomer cement versus conventional restorations with resin composite in Class II cavities of permanent teeth: two-year follow-up.

Objective To compare the effectiveness of ART restorations using High Viscosity Glass-ionomer cement (HVGIC) with conventional restorations using resin composite in Class II cavities of permanent teeth, in a 2-year follow-up.Methodology Seventy-seven restorations were made with each restorative material, Equia Fil-GC Corporation (ART restorations) and Z350-3M (conventional restoration), in 54 participants in this parallel and randomized clinical trial. Restorations were evaluated at 6 months, 1 and 2 years using the ART and the modified United States Public Health Service (USPHS) criteria. Chi-square test and Survival Analysis (p<0.05) were used for statistical analysis.Results The success rates for ART restorations were 98.7% (6 months) and 95.8% (1 year) for both criteria. At 2 years, success rate was 92% and 90.3% when scored by the modified USPHS and ART criteria (p=0.466), respectively. The success rates for conventional restorations were 100% (6 months), 98.7% (1 year) and 91.5% (2 years) for both assessment criteria. ART restorations presented a lower survival rate by the criterion of ART (83.7%) when compared to the modified USPHS criterion of (87.8%), after 2 years (p=0.051). The survival of conventional restorations was 90.7% for both evaluation criteria.Conclusion At the 2-years follow-up evaluation, no statistically significant difference was observed between the success rate of ART restorations with HVGIC compared to conventional restorations with resin composite in Class II cavities of permanent teeth.

What is the best caries removal strategy for primary molars?

Aim This systematic review and meta-analysis of randomised controlled trials (RCTs) aimed to compare the efficacy of different caries removal techniques: complete caries removal (CCR), selective caries removal (SCR) and stepwise caries removal (SWR) for deep carious lesions in vital primary teeth.Data sources The design of this review followed the PRISMA guidance ( http://www.prisma-statement.org/ ). Relevant studies were identified using electronic databases (PubMed [Medline], Cochrane Library, EMBASE) and finally reference lists were screened. The US National Institutes of Health Trials Register (NIHTR; http://clinicaltrials.gov ) and World Health Organisation International Clinical Trials Registry Platform (WHO ICTRP; http://apps.who.int/trialsearch ) were used to help assess publication bias, as it was not possible to test funnel plot asymmetry. Duplicates were located and eliminated using EndNote X7 programme.Study selection To be included, studies had to be published RCTs comparing SCR or SWR with CCR as caries removal strategies for deep carious lesions in vital primary teeth. The outcomes were pulp exposure, pulpo-periodontal complications (clinical and radiological failures) and/or restorative failures. RCTs applying these caries removal techniques were excluded if one of the other trial arms did not involve caries removal (that is, Hall Technique, therapeutic sealing of cavity lesions). Initially, 1,374 potentially eligible articles were identified, out of which 15 (English or French language only) were selected for full-text screening, which included ten relevant references corresponding to eight studies.Data extraction and synthesis Two authors independently extracted data using a piloted data extraction sheet, with a third reviewer resolving any disagreements. The authors performed conventional intention-to-treat and per-protocol meta-analyses, and calculated odds ratios (ORs) as effect estimates in the random-effects model, using Revman5.Results The eight included RCTs were conducted between 1977 and 2018. They include 669 patients and 824 teeth, with follow-up ranging from four weeks to 24 months. Collective results showed reduced risk of pulp exposure after SCR (OR: 0.10, 95% CI [0.04, 0.25]) or SWR (OR: 0.20, 95% CI [0.09, 0.44]), compared with CCR. There was a higher risk of composite restorative failure (OR: 2.61, 95% CI [1.05, 6.49]) using United States Public Health Service (USPHS) criteria, after SCR was compared with CCR only in intention-to-treat analysis. However, when comparing the risk of clinical or radiographic failure of pulpo-periodontal complications, no difference was found between SCR, CCR or SWR.Conclusion The conclusions of the paper are that there is a significant decrease in pulp exposure risk with SCR and SWR in comparison with CCR. However, there is a need for further studies with less risk of bias powered to report on the long-term outcomes of pulpo-periodontal health and restoration longevity.

Long-term results of a randomized clinical trial of 2 types of ceramic crowns in participants with extensive tooth wear

Statement of problemEvidence is sparse regarding the long-term outcomes of restorative treatment of patients with extensive tooth wear. PurposeThe purpose of this long-term prospective randomized clinical trial was to evaluate the performance and success rate of pressed lithium disilicate (LD) and translucent zirconia (TZ) crowns in participants with extensive tooth wear. Material and methodsA total of 62 participants with extensive tooth wear (17 women, 45 men; mean age 44.8 years; range 25-63 years) received a total of 713 crowns, LD=362 and TZ=351. Both types of crowns had chamfer preparations and were adhesively luted with dual-polymerizing composite resin cement (PANAVIA F 2.0; Kuraray Noritake Dental Inc). The restorations were clinically reevaluated on average 14, 31, 39, 54, and 65 months after insertion of the crowns according to the modified United States Public Health Service (USPHS) criteria. ResultsAfter an observation period of up to 6 years, the survival rate for both types of crowns was 99.7%, with 1 lost LD crown after 1 year as a result of loss of retention and 1 lost TZ crown after 3 years because of tooth fracture at the cemento-enamel junction. The success rates were similar for both types of crowns: 98.6% for LD and 99.1% for TZ. Reasons for failures were that 3 participants in each group developed apical lesions, minimal ceramic fractures, or their crowns were rebonded after loss of adhesion. Assessment of color at baseline was significantly different with a better match for LD (84.8% Alfa, 15.2% Bravo) than for TZ crowns (36.5% Alfa, 63.5% Bravo), including TZ crowns with veneered porcelain (P<.001). Secondary caries and cracks did not occur. A post hoc analysis of clinical performance did not indicate any significant differences between extensive tooth wear with primarily mechanical or chemical factors. ConclusionsNo differences were found between the 2 types of ceramic materials concerning the long-term success and clinical performance, except that TZ crowns were rated by a blinded clinician as less esthetic than LD crowns. The use of high-strength ceramic materials, as well as reliable adhesive bonding, are probably the key factors in the long-term success of ceramic crowns in participants with extensive tooth wear independent of the specific etiology.

Clinical Performance of Bulk-Fill Resin Composite Restorations Using the United States Public Health Service and Federation Dentaire Internationale Criteria: A 12-Month Randomized Clinical Trial.

Objective This study was aimed to compare the 12-month clinical performance of two full-body bulk-fill resin composites Filtek bulk fill/3M ESPE (FBF) and Tetric EvoCeram bulk fill/Ivoclar Vivadent (TBF) and a conventional microhybrid resin composite Filtek Z250/3M ESPE (Z250) using the modified the United States Public Health Service (USPHS) and Federation Dentaire Internationale (FDI) criteria. Also, the agreement between the two evaluation criteria was evaluated at baseline and after 12 months of follow-up. Materials and Methods A total of 138 class I and II restorations were placed in posterior teeth (split-mouth design) of 46 volunteers following manufacturer’s instructions and bonded with a self-etching bonding agent (Clear fill SE Bond/Kuraray). The restorations were evaluated at baseline and after 12 months of follow-up by three previously calibrated dentists (Cohen’s K = 0.84). Statistical Analysis Fisher’s exact test and Pearson’s Chi-squared test were used to evaluating the homogeneity of distribution of the clinical characteristics. Friedman’s test was applied to evaluate differences among the resin composites. The results obtained for the USPHS and FDI criteria at the different observation times were compared using the Wilcoxon test. A level of significance of 0.05 was adopted for all tests. Results After 12 months (recall rate, 78.3%, n = 36 patients), the overall success rate was 99.07% for both criteria. Only one failed restoration (0.93%) was detected for each system during follow-up in the TBF group. Conclusion The bulk-fill resin composites showed satisfactory clinical performance compared with conventional resin composite after 12 months. The percentage of the acceptable scores was significantly higher for the USPHS criteria, due to discrepancies in the score description for each criterion.

One-year clinical evaluation of two high-viscosity glass-ionomer cements in class II restorations of primary molars.

Little information exists on the clinical performance of restorative materials on primary teeth of preschool children. This study aimed to evaluate clinical performance of compomer, glass-hybrid-added high-viscosity glass-ionomer cement and zinc-added high-viscosity glass-ionomer cement materials in class ΙΙ restorations of primary molars. The study included 251 teeth of 57 patients aged 4-7years with proximal caries in primary molars. The teeth were divided into three groups, and each restorative material was randomly distributed. Dyract XP, Equia Forte and ChemFil Rock materials were placed after cavity preparation and clinically evaluated at 3, 6, 9 and 12months using modified United States Public Health Service criteria. Statistical analyses were performed using chi-square and z tests. At the end of 12months, a total of three restorations in Dyract XP group, 22 in Equia Forte group and 11 in ChemFil Rock group failed in retention criteria. No statistically significant difference existed between retention, colour match and surface texture criteria of all groups at 3 and 6months, but a statistically significant difference was found at 12months (P<0.05). For class ΙΙ restorations of primary molars, the success of compomer material was superior to high-viscosity glass-ionomer materials.

Implant-Supported Three-Unit Fixed Dental Prosthesis Using Coded Healing Abutments and Fabricated Using a Digital Workflow: A 1-Year Prospective Case Series Study

To test the applicability of coded healing abutments, intraoral scanners, and monolithic zirconia for the fabrication of three-unit fixed dental prostheses (FDPs) on two dental implants. Patients with three missing teeth in the posterior region of either the maxilla or mandible received two dental implants. After healing, coded healing abutments were placed. Full-arch intraoral scans were made to produce individual titanium abutments and a three-unit FDP. Peri-implant tissues were assessed 2 weeks after placement of the FDP and again after 1 year. Patient-reported outcome measures were registered prior to treatment and after 1 year. The quality of the FDPs was assessed using modified United States Public Health Service criteria after 1 year of service. A total of 54 patients were treated with 60 restorations, and 51 patients with 56 restorations were available at the 1-year follow-up. Implant survival was 99.1%, and prosthesis survival was 100%. The peri-implant tissues remained healthy, and patient satisfaction was high. However, the USPHS evaluation showed that some prostheses exhibited fit or color issues that needed to be addressed, although most were rated as successful (80.4%). The use of coded healing abutments and intraoral scanners to produce full-zirconia three-unit FDPs on two dental implants proved to be a feasible technique, with promising objective and subjective results. However, technical challenges still impacted the treatment results, resulting in a number of restorations having clinical or radiographic marginal gaps or reduced color match.

Clinical Evaluation of Wear Behavior of Human Enamel and Chipping of Veneered Zirconia against Monolithic Zirconia (Randomized Controlled Clinical Trial)

Objective: To evaluate the wear behavior of human enamel and chipping of veneered and monolithic zirconia for posterior full coverage restorations. Material and methods: Thirty-four zirconia full coverage restorations (seventeen in each group) were fabricated. The patients were divided into two groups according to the type of zirconia used; group 1 (comparator group) veneered zirconia crowns and group 2 (intervention group) monolithic zirconia single crowns. All crowns were lab fabricated and polished. For opposing teeth wear measurements 3D non-contact profilometer was used where epoxy resin replicas were constructed for opposing arch immediately after crowns cementation, three, six and twelve months. Restoration chipping was measured using modified United States Public Health Services (USPHS) criteria. Results: All restorations were reported as alpha with no chipping. There was no statistically significant difference between (Group 1) and (Group 2) for wear test. Conclusion: Both monolithic and veneered restorations revealed satisfactory mechanical properties with no chipping after one year of clinical use. Five-year clinical evaluation of a universal adhesive: A randomized double-blind trial

ObjectiveTo evaluate the five-year clinical performance of Scotchbond Universal Adhesive (SU; 3M Oral Care, St. Paul, MN, USA) in non-carious cervical lesions (NCCLs) using two evaluation criteria. MethodsThirty-nine patients participated in this study. Two hundred restorations were assigned to four groups: SU-ERm: etch-and-rinse + moist dentin; SU-ERd: etch-and-rinse + dry dentin; SU-Set: selective enamel etching; and SU-SE: self-etch. A nanofilled composite resin was placed incrementally. The restorations were evaluated at baseline and after 5 years using both the World Dental Federation (FDI) and the United States Public Health Service (USPHS) criteria. The survival rates (retention/fractures) were calculated with the Kaplan-Meier and the log-rank test. For the secondary outcomes, Friedman repeated measures analysis of variance by rank was applied (α = 0.05). ResultsAfter 5 years the recall rate was 86%. The retention/fracture rates were 93% for Erm and ERd, 88.4% for SEet and 81.4% for SE. A significant difference was observed for SE vs. ERd and SE vs. ERm (p = 0.01). Also, marginal discoloration and adaptation showed significant differences with ERm and ERd resulting in fewer marginal discrepancies than SE (p < 0.05). SignificanceAfter 5 years, the clinical behavior of the universal adhesive in the etch-and-rinse strategy was better when compared to the self-etch strategy. The use of selective enamel etching is highly recommended for the self-etch strategy. The FDI and USPHS evaluation criteria showed similar results after 5 years.

Clinical outcomes of minimally invasive ceramic restorations executed by dentists with different levels of experience. Blind and prospective clinical study.

To evaluate the clinical outcomes of minimally invasive ceramic restorations executed by dentists with different levels of experience. Sixteen professionals were divided into 4 groups according to their experience levels. These included G1: up to 2 years since graduation, G2: 2-5 years, G3: 5-10 years, and G4: more than 10 years. All professionals were trained to follow the same standardized clinical protocol, but were unaware of the research objectives. A single evaluator followed the clinical treatments and recorded the complications and errors that occurred during the execution of the protocol. Ninety-one full crowns, 137 veneers, and 46 no-preparation veneers were prepared from lithium disilicate. Follow-ups were performed immediately and at 30, 180, and 360 days after the cementation and the evaluation based on the modified United States Public Health Service criteria. Ceramic chipping/fracture and debonding were considered failures. Fractures were replicated and submitted to fractographic analysis. The highest number of failures were found in G1 followed by G3 and the lowest number of failures were found in G2. The level of success was 94% after 360 days. The fractographic analysis demonstrated the external surface of restorations as the critical area and suggested that failures might occur due to noncompliance with the clinical protocol. There was no correlation between professional experience and number of failures or patient satisfaction. Professional experience was not a decisive for patient satisfaction and success of minimally invasive ceramic restorations, and noncompliance with the clinical protocol was associated with early ceramic restoration failures.

Fourteen-year evaluation of posterior zirconia-based three-unit fixed dental prostheses: A Prospective clinical study of all ceramic prosthesis

Objectivesthe purpose of the present prospective trial was to evaluate the clinical performance of posterior 3-unit zirconia-based fixed dental prostheses (FDPs) after 14 years of clinical function. Methodsthirty-seven patients needing to replace either premolars or molars were involved and 48 FDPs were fabricated (Procera Zirconia, Nobel Biocare AB). Frameworks with a9 mm2 cross section of the connectors and 0.6 mm minimum thickness of the retainers were made by means of Procera Forte CAD-CAM System (Nobel Biocare AB). The patients were recalled after 6 and 12 months and then yearly up to a total follow-up of 14 years. Two independent survival curves for patients wearing 1 or 2 FDPs were calculated by means of Kaplan-Meier analysis and a log-rank test was performed in order to compare these curves. The United States Public Health Service criteria were used to examine technical and esthetic outcomes. The biological examination was performed evaluating plaque control, pocket depth, attachment level, bleeding on probing at both abutments and contralateral teeth and evaluated by means of the Wilcoxon test (p < 0.05) between the baseline and the 14-year follow-up. Resultsdescriptive statistics resulted in 91 % and 99 % cumulative survival rates for patients wearing 1 and 2 FDPs, respectively. There were no significant differences in periodontal parameters between control and test teeth. Both function and esthetic results were successful for FDPs over a 14-year follow-up period. Conclusionsthe results of this prospective clinical study confirmed the effectiveness of zirconia as a clinical option to fabricate short-span posterior FDPs.Clinical significance: within the limitations of the present prospective clinical study, zirconia-based three-unit fixed dental prostheses perform satisfactorily on long term, in posterior areas and in patients with standard biomechanical conditions.

Evaluation of Clinical Outcomes of Veneers Made at Kurdistan Board of Restorative Dentistry/ Sulaymaniyah Center; A Retrospective Study

Objective: To evaluate veneers made at Kurdistan board of restorative dentistry/ Sulaymaniyah center, regarding their mechanical (debonding, marginal adaptation, and fracture of restorations), biological (gingival index, gingival recession, postoperative sensitivity, and secondary caries), and esthetical (color matching, with marginal discoloration) qualities. &#x0D; Methods: One hundred and ten units of veneers applied to 15 patients were selected and evaluated according to clinical evaluation parameters from modified United States public health service criteria (USPHS) and FDI criteria. After signing informed consent, each patient was recalled and had the mechanical, biological, and esthetic aspects of their veneers clinically examined by the dental mirror, sharp explorer, periodontal probe, and visual inspection.&#x0D; Results: Overall, the veneers made at the Kurdistan board of restorative dentistry/ Sulaymaniyah center were considered to be successful restorations with a high survival rate (97.2%) as only three veneers were found to have failed. And most of the patients, n= 94 (85.4%), were very satisfied with the color of their restorations. A total of 45 units of veneers (40.9%) were found to have mild gingivitis, while 14 units of veneers (12.7%) were found to have postoperative sensitivity. Only seven units of veneers (6.3%) were found to have secondary caries. &#x0D; Conclusions: Veneers made and fitted in the Kurdistan board of restorative dentistry/ Sulaymaniyah center were found to have an acceptable prognosis and longevity regarding their success rate and patient satisfaction.

Monolithic Screw-Retained Lithium Disilicate Implant Crowns: Preliminary Data of a Prospective Cohort Study.

To analyze the clinical behavior of screw-retained monolithic lithium disilicate (LDS) implant-supported single crowns (ISSCs) over a 1-year follow-up. A total of 28 patients were restored with 45 screw-retained monolithic LDS (press-fabricated) ISSCs bonded to titanium bases in posterior sites. Modified United States Public Health Service criteria and Kaplan-Meier survival and success rates were evaluated. A total of 27 patients with 44 ISSCs were evaluated 12 months (mean ± standard deviation: 13.02 ± 2.28) after prosthesis delivery. No implant or crown failures occurred. The Kaplan-Meier survival and success rates were each 100% after 1 year. Minorly increased surface roughness was noted in the occlusal contact point areas of 9 ISSCs (20.5% Bravo rating). Marginal adaptation, color match, and anatomical form stayed favorable over time. Press-fabricated LDS monolithic screw-retained implant crowns appear to be a promising treatment option for posterior implants under short-term observation.

Ten-Year Clinical Survival of Anterior Cantilever Resin-Bonded Fixed Dental Prostheses: A Retrospective Study.

To assess the 10-year clinical retention and survival rates of metal-ceramic (MC) and all-ceramic (AC) cantilever resin-bonded fixed dental prostheses (CRBFDPs). Forty CRBFDPs constructed from either cobalt-chromium ceramic (MC group; n = 20) or glass-infiltrated alumina ceramic (AC group; n = 20) were placed in 40 patients between August 2007 and December 2009. After baseline recordings, the patients were followed up using modified United States Public Health Services (USPHS) criteria after 6 months and thereafter annually for 10 years. Data were statistically analyzed using Kaplan-Meier estimation with log-rank (Mantel-Cox) test. The 10-year clinical retention rate was 95.0% in the MC group and 70% in the AC group. The difference was statistically significant (P = .02, log-rank test). Three all-ceramic CRBFDPs fractured at 6, 12, and 84 months after insertion. No statistically significant difference in survival rate was observed between the MC and AC groups over the 10 years of clinical observation (MC: 100%; AC: 85%; P = .075) (Kaplan-Meier method, confidence interval = 92.5% to 97.5%). Although glass-infiltrated alumina ceramic anterior CRBFDPs exhibited a lower clinical retention rate compared to metal-ceramic CRBFDPs, the debonded prostheses were recemented and continued in function over the observation period. Additionally, an acceptable 10-year clinical longevity was recorded.

Monolithic Polymer-Infiltrated Ceramic Network CAD/CAM Single Crowns: Three-Year Mid-Term Results of a Prospective Clinical Study

To evaluate the clinical outcomes of monolithic CAD/CAM-fabricated polymer-infiltrated ceramic network (PICN) single crowns (SC) after 3 years. A total of 34 patients who received 76 single crowns were included in this cohort study. Full-coverage crown preparation with reduced thickness (1.0 to 1.5 mm) was applied. All monolithic PICN SCs (VITA Enamic) were manufactured using a CAD/CAM system and adhesively seated. Clinical follow-up evaluations were performed at 6, 12, 24, and 36 months after insertion using modified United States Public Health Service (USPHS) criteria. Kaplan-Meier survival and success methods were applied to calculate absolute and relative failures. To determine effects of time and restoration, a mixed logit model was adjusted for the modified USPHS criteria (P < .05). PICN SCs revealed an estimated Kaplan-Meier survival rate of 93.9% after 3 years. Four clinically unacceptable fractures occurred, which resulted in replacement of the affected SCs. Neither secondary caries nor debonding were recorded. The 3-year Kaplan-Meier success rate was 92.7%. Significant increases in marginal discoloration (P = .0002) and surface roughness (P < .0001) were noted over time. Color match, anatomical form, and marginal adaptation remained favorable over the given observation period. PICN CAD/CAM crowns with reduced thickness showed acceptable survival and success rates over a service time of 36 months. However, extended clinical follow-up periods are needed to evaluate the long-term performance.

Clinical Performance of Anterior Full Veneer Restorations Made of Lithium Disilicate with a Mean Observation Time of 8 Years.

To evaluate the survival and complication rates of full veneer restorations after up to 11 years of clinical service. Six patients (four men, two women, median age 42.3 ± 4.7 years) were restored with a total of 40 adhesively luted anterior full veneers (maxilla: 36; mandible: 6; mostly canine to canine) made of lithium disilicate ceramic. Patients were treated between July 2007 and January 2014. All restorations were examined during annual recall visits using the modified United States Public Health Service criteria for color match, marginal discoloration, secondary caries, marginal integrity, surface texture, and restoration fracture, rated as Alpha, Bravo, or Charlie. Data were statistically analyzed using Kaplan-Meier estimation with log-rank test. Time of clinical service was 68 to 139 months (median: 8.1 ± 2.0 years) without any dropouts. Full veneer restorations in the anterior dentition presented a survival rate of 100% and a complication rate of 12.5% due to reparable minor chippings (technical complication/restoration fracture rated Bravo) of four restorations (one after 11 months, one after 20 months, and two after 66 months) and a crack in one restoration (after 38 months) due to trauma. No further technical (debonding or discoloration) or biologic (secondary caries) complications occurred. Based on the present results, minimally invasive anterior full veneer restorations might be considered as a reliable treatment option, but further clinical data are essential.

Clinical evaluation of bulk-fill resins and glass ionomer restorative materials: A 1-year follow-up randomized clinical trial in children.

This prospective study aimed to evaluate the clinical performance of different restorative materials in primary molars with class II carious lesions. A total of 160 class II carious lesions (with radiographic involvement of the outer half of dentin) in 30 patients were randomly divided into four groups and restored with a glass ionomer restorative system (Equia™), two different bulk-fill composites (Sonicfill™ and X-tra fil™), and a nanohybrid composite (Filtek Z550™). The restorations were clinically and radiographically evaluated at the baseline, and 3, 6, and 12 months according to the modified United States Public Health Service criteria. Statistical analyses were performed using Pearson's Chi-square and McNemar tests. After 1 year, 134 restorations were evaluated in 26 patients. Equia was statistically less successful than the other restorative materials in marginal adaptation and retention criteria (P < 0.05). However, no material was found to be superior to the others over the study period in marginal discoloration, color matching, secondary caries, anatomical form, and postoperative sensitivity (P > 0.05). The bulk-fill and conventional composites exhibited good clinical performance, and Equia exhibited minor changes over the 1-year trial period.

Quality of Fixed Dental Prostheses and Associated Biological Complications in a Saudi Population

The interrelationships between dental prostheses, abutments, and supporting periodontal tissue are dynamic. Clinical studies assessing the quality and associated complications of fixed dental prostheses (FDPs) in the Saudi population are scarce. The aims of this project were to assess the location and accuracy of marginal adaptation and proximal contact quality of FDPs provided by dentists in the Kingdom of Saudi Arabia and to assess the impact of these factors on the health of the periodontium and caries susceptibility. This retrospective cohort study collected demographic, dental, and social history data from patients with FDPs. Fixed dental prostheses quality was assessed using the United States Public Health Service Criteria, and periodontal health indices were measured. Descriptive and inferential statistics were used for data analysis. Logistic and multiple linear regression analyses were performed to assess predictors of caries risk and periodontal disease, respectively. Sixty-two patients with 62 FDPs were assessed. The mean patient age was 32.45 ± 9.0 (19-61) years with a male-to-female ratio of 3:1. On the assessment, 74.2% had marginal discrepancy, 54.8% had subgingival margins, 22.6% had open or tight proximal contacts, and 8.1% had marginal caries. Well-adapted margins promoted periodontal health, and inadequate proximal contact increased the risk of marginal caries. This study suggests that 26% of FDPs provided by dental practitioners in the Western province, Kingdom of Saudi Arabia, were of high marginal fit and proximal contact quality. Constructing FDPs with high-quality marginal fit and proximal contact promotes periodontal health and reduces caries risk, thereby improving FDP outcome.

One-year evaluation of a new restorative glass ionomer cement for the restoration of non-carious cervical lesions in patients with systemic diseases: a randomized, clinical trial.

This randomized and clinical trial aimed to evaluate the performance of a new restorative Glass Ionomer Cement (GIC) for the restoration of non-carious cervical lesions (NCCLs) of patients with systemic diseases compared with a posterior resin composite after 12 months. 134 restorations were placed at 30 patients presenting systemic diseases by a single clinician. NCCLs were allocated to two groups according to restorative system used: a conventional restorative GIC [Fuji Bulk (GC, Tokyo Japan) (FB)] and a posterior resin composite [G-ænial Posterior (GC, Tokyo Japan) (GP)] used with a universal adhesive using etch&rinse mode. All restorative procedures were conducted according to manufacturer's instructions. Restorations were scored regarding retention, marginal discoloration, marginal adaptation, secondary caries, surface texture, and post-operative sensitivity using modified United States Public Health Service (USPHS) criteria after 1 week (baseline), 6, and 12 months. Descriptive statistics were performed using chi-square tests. Cochran Q and Mc Nemar's tests were used to detect differences over time. After 12 months, recall rate was 93% and the rates of cumulative retention failure for FB and GP were 4.9% and 1.6% respectively. Both groups presented similar alpha rates for marginal adaptation (FB 86.2%, GP 95.5%) and marginal discoloration (FB 93.8%, GP 97%) at 6-month recall, but FB restorations showed higher bravo scores than GP restorations for marginal adaptation and marginal discoloration after 12 months (p<0.05). Regarding surface texture, 2 FB restorations (3.1%) were scored as bravo after 6 months. All restorations were scored as alpha for secondary caries and postoperative sensitivity after 12 months. Although the posterior resin composite demonstrated clinically higher alpha scores than the conventional GIC for marginal adaptation and discoloration, both materials successfully restored NCCLs at patients with systematic disease after a year. Due to its acceptable clinical results, the tested conventional restorative GIC can be used for the restoration of NCCLs of patients with systemic diseases.

Clinical Performance of Short-fiber-reinforced Resin Composite Restorations vs Resin Composite Onlay Restorations in Complex Cavities of Molars (Randomized Clinical Trial)

Short-fiber-reinforced resin composite (SFRC) is a direct bulk fill resin composite specially indicated in large complex cavities. It is characterized by having high fracture toughness and load bearing capacity to decrease the incidence of fracture of the restoration. In two parallel groups (n = 38 restorations), 76 participants having complex proximal cavities with asymptomatic vital pulp were randomly enrolled in this trial and received either SFRC (Ever X Posterior, GC, Japan) covered by Gaenial posterior (GC, Japan) or chairside indirect restorations (Grandioso inlay system; VOCO, Germany) fabricated on a silicon die. Materials were applied according to the manufacturer instructions with the corresponding adhesive system. Only 67 participants completed the trial, which was assessed using the modified United States Public Health Service (USPHS) criteria by two independent blinded assessors at 6 months and 1 year follow-up visits. Fisher's exact and Cochran's Q tests were used to analyze inter- and intragroup comparisons, respectively. The significance level was set at p ≤ 0.05. No statistically significant difference was observed between both tested groups for all USPHS criteria at different follow-up periods except for marginal integrity favoring the SFRC at 12 months when the difference became significant (p < 0.001), and color match favoring the nanohybrid indirect resin composite restorations with significant difference in scores at all follow-up intervals (p < 0.001) was found. Cochran's Q test showed significant differences within the same technique during the follow-up period for some criteria. Direct SFRC and indirect nanohybrid resin composite complex proximal restorations showed an acceptable clinical performance along the 1 year follow-up period. Direct SFRC restorations could be a viable treatment option for complex restorative cases.
