The Role of Physical Activity in the Relationship between Satisfaction with Life and Health-Related Quality of Life in School-Age Adolescents.

(1) Background: Adolescence is a critical stage in the development of healthy habits. In this regard, physical activity has emerged as a useful tool to improve satisfaction with life and health-related quality of life in adolescents. Therefore, the aim of the present study was to examine the mediating role of physical activity in the relationship between satisfaction with life and health-related quality of life in adolescent boys and girls. Also, we aimed to investigate the differences between sexes in the HRQoL, physical activity level, and satisfaction with life. (2) Methods: A total of 297 adolescents, ranging in age from 11 to 12 years (11.46 ± 1.63), participated in this cross-sectional study. The Satisfaction with life scale, Physical activity Questionnaire for Adolescents and the KIDSCREEN-10 questionnaires were employed. (3) Results: The estimated indirect effect showed that physical activity level was a mediator of the positive effect of satisfaction with life on health-related quality of life (β = 0.105, 95% CI = 0.031; 0.202). However, the index of moderated mediation showed that sex is not a significant moderator of the mediating role of physical activity in the relationship between satisfaction with life and HRQoL (β = −0.033, 95% CI = −0.023, 0.136). Furthermore, significant differences in satisfaction with life were found, with girls manifesting lower values (p-value = 0.026). (4) Conclusion: This study shows the importance of physical activity during adolescence and the association of this behavior with the health-related quality of life of adolescents.

Rodeo Thumb: To Replant or Not.

This study investigated patient-reported outcomes after surgical treatment of rodeo thumb amputation to guide clinical decision-making. A retrospective review was performed for rodeo thumb amputations from 2009 to 2019. Patient-reported outcomes were collected and compared by injury level, age, and treatment. Two-sided t test was used to compare continuous variables, and Pearson chi-square test was used to compare categorical data. The study included 37 patients. Patients with interphalangeal injuries treated with replantation had Quick Disabilities of the Arm, Shoulder and Hand questionnaire scores similar to those treated with amputation (1 versus 8; p = 0.07). There was no significant difference in percentage of patients with similar or better roping ability after treatment (40 percent versus 79 percent; p = 0.26), and similar percentages were satisfied (80 percent versus 71 percent; p = 1.00). Patients with metacarpophalangeal injuries treated with replantation and those treated with amputation had similar questionnaire scores (7 versus 10; p = 0.47). Both groups had similar roping ability after treatment (67 percent versus 56 percent; p = 1.00), and there was no statistically significant difference in satisfaction (79 percent versus 44 percent; p = 0.34). Pediatric patients had questionnaire scores similar to those of adults (6 versus 8; p = 0.42). A significantly higher percentage of pediatric patients had similar roping ability following injury than adults (100 percent versus 54 percent; p = 0.02). Most patients in both groups were satisfied (89 percent versus 61 percent; p = 0.22). For both interphalangeal and metacarpophalangeal injuries, patient-reported outcomes were similar regardless of treatment. Children were able to return to roping and perform at a similar or higher level at a greater rate than adults, but had similar questionnaire scores and satisfaction.

Rapid Review on the Effectiveness of Telehealth Interventions for the Treatment of Behavioral Health Disorders.

The Psychological Health Center of Excellence conducted a rapid review of the literature to investigate the effectiveness of behavioral health treatments (i.e., evidence-based psychotherapy and psychiatry) delivered in person compared to telehealth (TH; i.e., video teleconference and telephone). The rapid review methods included a systematic search of a single database (PubMed), hand-searching of relevant systematic reviews, dual screening, single-person data abstraction verified by a second person, and dual risk of bias assessment. Due to heterogeneity across studies, no quantitative analyses were conducted. Twenty-two randomized controlled trials (RCTs), eight of which were non-inferiority trials, evaluated the effectiveness of TH via telephone or video teleconferencing compared to in-person (IP) delivery for patient populations with posttraumatic stress disorder, depression, and mixed diagnoses. The majority of RCTs and all but one of the eight non-inferiority trials found that clinical outcomes did not differ between TH and IP treatment delivery. Two studies found that subgroups with higher symptom severity (hopelessness and anxiety disorders, respectively) in the TH group had worse treatment-related outcomes than IP participants with similar symptom profiles. The majority of studies found no significant differences in satisfaction with care, quality of the therapeutic alliance, or study discontinuation between TH and IP groups. Based on evidence from 22 RCTs, the use of TH platforms, including video conference and telephone modalities, generally produces similar outcomes as face-to-face provision of psychotherapy and psychiatry services.

The Effectiveness of Implementation of Online Lectures on Student Satisfaction in Higher Education Institutions (Case Study of the President University)

This study aims to identify and obtain information about the effectiveness of the implementation of online lectures during the COVID-19 pandemic at Higher Education Institutions, especially President University. Data was collected by looking at the results of student satisfaction assessments of lecturers during the teaching and learning process, namely in the PUIS (President University Information System) system in the "Lecturing Evaluation" menu. The research used quantitative methods and data analysis used SPSS version 21 software. After the data were analyzed, the researchers conducted online interviews with 12 (twelve) lecturers. The results of this study are that there is a significant difference in satisfaction between offline and online lectures, namely, when offline class students can get knowledge directly without any technical constraints and face-to-face teaching and learning process there are values ​​that can be taken such as the process of social, cultural, ethical maturity. and morals that can only be found in the area of ​​education. In contrast to online lectures, apart from technical problems such as the unsupported internet, online classes also make students lose motivation to study because no chemistry can be built. There are different characteristics in each faculty, this is a challenge for lecturers to explore their abilities and provide new knowledge, and of course, it can be well accepted by students. Online classes will run more effectively if both lecturers and students live it with a sense of responsibility.

Functional Outcomes Following Total Knee Arthroplasty Without Patellar Resurfacing: A Minimum Two-Year Follow-Up Retrospective Cohort Study.

This paper seeks to address the effectiveness of total knee arthroplasty (TKA) when performed without patellar resurfacing. The objective of this article is to investigate the effect of total knee arthroplasty without patellofemoral resurfacing on postoperative outcome.All patients with degenerative knee osteoarthritis (OA) that underwent TKA without patellar resurfacing were included in the study. The clinical data of 163 patients, including 98 females and 65 males with a mean age of 63 years (range 54-78 years) were retrospectively analyzed from April 2008 to April 2011. Intraoperative cartilage degeneration according to Outerbridge classification criteria was as follows: 22 cases of grade I, 38 cases of grade II, 64 cases of grade III, 39 cases of grade IV. There were no significant differences in gender, age, and side differences between the patients at all levels (P > 0.05). The duration of tourniquet use and related complications were recorded. Knee function was assessed using the American Knee Society Scoring System (KSS) and the patellar score (PS). Patient satisfaction and knee pain were assessed by the pain visual analog scale (VAS). The evaluation was conducted using routine X-ray film to observe the position of the prosthesis and the patella. Statistical analysis used included a comparison between groups by analysis of variance (ANOVA) using the Student-Newman-Keuls (SNK) test and comparison of grade data using the rank-sum test.The average tourniquet time was 125 minutes, with a range of 90-150 minutes. All the incisions healed with primary intention without early complications. All patients were followed for two to five years with an average of 3.6 years. At six months and at the last follow-up, the KSS and PS scores were significantly higher than those before surgery (P < 0.05). There was no significant difference between the sixth month and the last follow-up (P > 0.05). There were significant differences in preoperative KSS and PS scores between patients with different grades of cartilage degeneration (P < 0.05), but there was no significant difference at the last follow-up (P > 0.05). At the last follow-up, seven patients had persistent anterior knee pain, five patients had mild pain, and two patients had moderate pain according to the VAS assessment criteria. Patient satisfaction evaluation was as follows: 90 patients were very satisfied, 66 patients were satisfied, five patients were uncertain, and two patients were unsatisfied. There were no significant differences in satisfaction and knee pain between patients with different grades of patellofemoral degeneration (P > 0.05).In conclusion, at six months and at the last follow-up, outcome measures for patients were significantly higher than before surgery for TKA without the use of patellar resurfacing and the majority of patients were satisfied with the outcome of the procedure. TKA continues to be a successful procedure without the use of patellar resurfacing.

A nationwide survey assessing the satisfaction of dental colleges graduates with their undergraduate experience in Saudi Arabia

ObjectiveTo date, there are no publications on the Saudi graduates’ perceived value of their dental curriculum. The aim of this nation-wide survey was to investigate graduates’ satisfaction with specific disciplines of their dental education and training. The first part of this study addressed findings according to the respondents’ gender and the number of years of postgraduate experience (Graduate-related factors). MethodsAn electronic questionnaire was emailed to registrants of the Saudi Dental Society, specifically dental interns, general dentists, and specialized dentists who graduated from private and governmental dental colleges in Saudi Arabia (n = 2185). ResultsThe response rate was 36.4 %. The number of male respondents was (n = 352, 44.2 %), the remainder being female (n = 444, 55.8 %). The number of recent dentists (≤ 5 years since graduation) was (n = 428, 53.8 %) and experienced dentists (n = 368, 46.2 %). There were statistically significant gender variations in satisfaction levels, and among dentists with different post-graduation experiences. Although male and female dental students are taught similar curriculums and have similar graduation requirements, there were statistically significant differences in various subjects covered. Similarly, recent and experienced dentists reported statistically significant differences in satisfaction in various subjects. ConclusionThis study provides graduates’ perspectives on the strengths and weaknesses of their dental education in Saudi Arabia. The findings help in curriculum reform and collaborations between the dental colleges nationally. Ultimately this can lead to formulating a national dental accreditation and assessment body. Clinical significanceThe findings of this study provide graduates’ perspective on the dental education received in Saudi Arabia. This will help stimulate discussion and collaboration within and amongst the colleges on improving their curriculum in this fast-paced, evolving field of dentistry.

Hearing Their Voices: Parents’ Perceptions of Preschool Special Education Evaluations With Dual-Language Learners

Advancing equity in early childhood education is crucial as our broader society faces issues of inequities across systems, including education. Professionals must honor children’s home language and cultural contexts, coming from a perspective of value and respect rather than from a traditionally dominant Eurocentric perspective. Through this study, we share the perspectives of 22 U.S. parents of dual-language learners (DLLs) as they experienced an evaluation for preschool special education. Through a mixed-methods approach using survey and interview data, three primary themes emerged: (a) relationships and interactions, (b) home language and cultural context, and (c) the evaluation process. Significant differences in satisfaction emerged between those who had participated in Part C Early Intervention services versus those who had not.

Race and satisfaction with pain management among patients with HIV receiving long-term opioid therapy

IntroductionManagement of chronic pain is an essential aspect of HIV primary care. Previous literature in the general population has elucidated racial disparities in the evaluation and treatment of pain. This study examined racial/ethnic differences in patient satisfaction and barriers to pain management among a cohort of PWH receiving LTOT. Materials and methodsPatient-reported survey and EMR data were compared between non-white (n = 135; 81.3 %) and white (n = 31; 18.7 %) patients in a cohort of 166 PWH receiving LTOT in two clinics in Atlanta and Boston. Quantile and linear regression were used to evaluate the association between race and pain management outcomes: 1) satisfaction with pain management (0–10) and 2) patient-related barriers to pain management, including patient perceptions of pain medications, fatalism, and communication about pain. Models were adjusted for sex, age, clinical site, and baseline general health. ResultsNon-white participants were noted to receive chronic opioids for a shorter mean duration of time than white participants (6.0 versus 11.0 years, p < 0.001) and lower mean morphine equivalent daily dose (MEDD) than white participants (28.1 versus 66.9 mg, p < 0.001). In adjusted analyses, there was no significant difference in satisfaction with pain management among non-white and white participants (p = 0.101). There was no significant difference in barriers to pain management in unadjusted (p = 0.335) nor adjusted models (p = 0.397). ConclusionWhile non-white PWH were noted to have received lower doses of chronic opioids and for shorter duration than white PWH, satisfaction with pain management was similar. Patient-related barriers to pain management were similar among non-white and white PWH.

Satisfaction of patients in pain management delivering in Reunion Island with or without neuraxial analgesia

BackgroundNeuraxial analgesia (NA) rate during labour in Reunion Island is below the average rate in continental France. Our study objective was to know if the patients that deliver with or without NA were satisfied. We also wanted to identify the reasons of this lower rate. MethodThis was a multicentre prospective observational study led in the seven Reunion Island maternity hospitals including all patients who delivered after 24 weeks of gestation onwards during two weeks in 2019 and excluding scheduled caesarean sections. Included patients received a satisfaction questionnaire the day after delivery and returned it before leaving. ResultsOf the 497 patients who met the inclusion criteria, 303 questionnaires were analysable. 216 patients had NA (71.3 %) and 87 patients (28.7 %) delivered without. A total of 88.0 % of patients with NA were satisfied with pain management compared to 83.9 % without. There was no significant difference between the groups (p=0.68). Among patients who delivered without NA, it was their choice in 55.2 % of cases. They wanted the most natural birth possible in 87.5 % of cases. Birth without NA in opposition to their wish was related to too rapid dilation in 90.0 % of lowerecases. DiscussionNo significant difference in satisfaction between women who delivered with or without NA was found. The lower regional NA rate seems to be linked to a choice to give birth as naturally as possible and to too fast dilation at the time of requesting NA.

Quality in Higher Education and Satisfaction among Professors and Students.

The aim of this study was to analyze the significant differences in satisfaction with educational quality in higher education in Italy (Naples) among students and professors. The sample consisted of 501 higher education students and 121 professors, resulting in a total sample size of 622 subjects. Once the quality parameters of the instrument were determined, reliability was confirmed, and data collection was initiated. In order to analyze the results, a test of independent means (Student’s t-test) was performed, interrelating the variables of educational quality, concerning both management and satisfaction with higher education. Based on the results, we concluded that there are significant differences between the group of students and the group of professors, highlighting a higher level of satisfaction with quality reported by students regarding the coordination of teachers and staff in the educational process; therefore, the inclusion of students in the direct management of the center should be more active and an indicator to be taken into account in self-evaluation. Despite the limitations in the sample at the regional level, it offers many possibilities for future research.

Comparing two models of outpatient specialised palliative care

BackgroundIdeally, patients with life-threatening illness who are suffering from multiple symptoms and reduced quality of life should receive palliative care that addresses their specific needs. The many well-defined clinical pathways may not always leave room for a person-centred and individual approach with respect to symptom control, psychosocial and spiritual support, and practical issues. In deciding how to organize outpatient specialist palliative care (SPC), it is relevant to include the perspectives of both patients and families. Thus, the aim of this study was to compare two models for outpatient SPC: first contact between patient, next-of-kin and doctor/nurse in the form of a home visit; and first visit in the hospital setting.MethodThe study was a comparative mixed methods study with follow-up at one and 3 months. It started with a quantitative strand in the form of a 38-item questionnaire. Data were analysed using linear mixed effects models, with maximum likelihood estimation for each outcome variable. The repeated measurements on patient level were modelled by including random intercepts of patients in the mixed model.ResultsIn total, 190 participants were enrolled, of whom 102 answered the first questionnaire.No differences were found between the two SPC interventions when development in satisfaction with care, communication or overall quality of life were compared. At baseline, a significantly higher score for satisfaction was found, in favour of first visit taking place in the hospital setting (65.91 vs. 55.83; p = 0.03) measured by FAMCARE-P16, and more patients were satisfied with availability of nurses and their abilities to listen and communicate than of doctors.ConclusionSpecialist palliative care is in request for many patients in the late phase of their disease. We found no significant differences in satisfaction with care, communication with health professionals or in overall quality of life between the two models. This may imply that access to SPC is more important than the model that is applied, and that a person-centred approach together with time available may matter more than the context. These two factors should be considered when implementing SPC.

The effects of a leaflet-based health guide on health literacy, self-efficacy, and satisfaction among older Japanese-Brazilian adults living in Brazil: A quasi-experimental study

BackgroundThis study assessed the effects of a Japanese leaflet-based health guide for older Japanese-Brazilian adults living in Brazil, on health literacy, self-efficacy, and satisfaction with the health guide and participants’ subjective health status.MethodsThe study followed a one-group pretest-posttest design and was set in the Japanese-Brazilian community in South Brazil. The 21 participants were Japanese-Brazilian individuals aged over 65 years, living in Brazil, and able to converse in Japanese. During the annual health checkup of 2016, we provided about 20 min of health guidance in Japanese using our leaflet, which included information about lifestyle-related diseases, recommended salt and sugar intake levels, and graphical charts. Participants’ health literacy (HL) was the primary outcome; self-efficacy and satisfaction with the leaflet-based health guide and participants’ subjective health status were secondary outcomes. We assessed the effect after completion of the health checkups in 2016 and 2017. Data were analyzed using repeated measures of ANOVA and the Bonferroni multiple comparison test as required.ResultsThere were no statistical significant differences in HL (functional HL: p-value = 0.22; communicative HL: p-value = 0.17; critical HL: p-value = 0.40; total HL score: p-value = 0.12) and self-efficacy (p-value = 0.28) across the three assessment points. We detected a statistical significant difference in satisfaction with the health guide, post-intervention in 2016 and 2017 (baseline score: 86.7±20.4; post-intervention score in 2016: 92.5±12.2; post-intervention score in 2017: 76.2±21.9; p-value of repeated ANOVA < 0.01, ηp2 = 0.28; p-value of the multiple comparison in 2016 and 2017 = 0.01, 95% CI 4.09–28.51). However, the Bonferroni multiple comparison test did not show pairwise difference during multiple comparisons of participants’ satisfaction with their subjective health status (scores: baseline, 69.6±24.2; post-intervention in 2016, 78.5±21.1; post-intervention in 2017, 58.0±31.1; p-value of repeated ANOVA = 0.02, ηp2 = 0.21; p-values of the multiple comparisons> 0.05). Scores of all outcomes, except self-efficacy, increased from baseline to post-intervention in 2016, but declined at post-intervention in 2017.ConclusionsThe leaflet-based intervention appeared to have short-term effects. The findings suggest that direct intervention in older adults’ native language may improve their satisfaction when living in non-native countries.Trial registrationThe UMIN-CTR unique registration ID is UMIN000032443. Characteristics and Satisfaction Analysis of Eco-Tour Ships at Ecotourism Sites: Focusing on the Cases of the Nakdong River Estuary, Suncheon Bay Wetland, and Goesan Lake and Sanmagi-Yetgil in Korea

This study investigated eco-tour ships at Nakdong estuary, Suncheon wetland, and Goesan and Sanmagi-yetgil, 3 of the 26 ecotourism sites in Korea, by analyzing the characteristics and level of ecotourist satisfaction. The field investigation focused on eco-tour ship image, facilities, landscape observed from eco-tour ship, course, and operations. The results of the satisfaction analysis can be summarized as follows. First, there was no significant difference in satisfaction of eco-tour ship image between Nakdong estuary and Sanmagi-yetgil (p &gt; 0.01), while Suncheon wetland showed a significant difference (p &lt; 0.01). It is assumed that the fact that the ship was built using timber which better suits the natural environment and coastal ecosystem had a positive impact on the visual image. No significant difference in satisfaction with facilities on eco-tour ships was found between Nakdong estuary and Suncheon wetland, while Sanmagi-yetgil showed a significant difference. Second, satisfaction of the landscape observed from the eco-tour ships showed significant differences in the mean scores of landscape evaluation the three target sites: Sanmagi-yetgil (8.40), Suncheon wetland (7.20), and Nakdong estuary (4.67). These values are based on the qualitative evaluation of the landscape as seen from the eco-tour ship. It is presumed to have been influenced by the diversity of the landscape and the width of the river. The eco-tour ship course satisfaction also showed significant differences in the mean scores: Sanmagi-yetgil (7.37), Suncheon wetland (6.57), and Nakdong estuary (5.00). The landscape and tour course evaluations of the respondents seem to be correlated. In particular, the variety of courses available to visitors in Sanmagi-yetgil was relatively high compared to Nakdong estuary and Suncheon wetland. Third, satisfaction analysis for eco-tour ship operations found that Suncheon wetland and Sanmagi-yetgil did not differ significantly in terms of satisfaction (p = 0.634), but Nakdong estuary showed a significant difference. The relatively low satisfaction with the eco-tour ship operation at Nakdong estuary is due to the low scores for eco-tour ship commentary, reservation system, and with the facilities, while it is shown that the operation time as well as the landscape observed from the eco-tour ship and the tour course had a combined influence on satisfaction. In the comparative analysis of the impact of eco-tour ships on ecotourism, Nakdong estuary showed a significant difference. This finding indicates a greater influence of eco-tour ships on ecotourism in Suncheon wetland, Sanmagi-yetgil, and Nakdong estuary. Enhanced ecotourism satisfaction improves the probability of return visits. Thus, there is a need to incorporate the characteristics of ecological resources with well thought out operation and increased functional suitability of eco-tour ships to improve ecotourism satisfaction. The eco-ships in the three places in this paper have elements promoting and enhancing their potential. Our findings showed the satisfaction level of the users with the theme of ecotourism.

360 Student Satisfaction Towards an Academic Research Program

The emergency department is a unique environment that co-exists within a larger organization where a wide range of medical issues are treated. As a means to reduce costs and increase visibility for prospective physicians, departments will frequently use voluntary undergraduate student support. The University of California, San Francisco Fresno Emergency Department’s (UCSFF) Academic Research Associates (ARA) program was evaluated to help guide volunteer placement decisions for future implementation. The evaluation measured each volunteer’s relationship within the organization, personal gain, and satisfaction with the program based upon the student’s grade point average (GPA). To measure volunteer satisfaction in a psychometrically sound way, Galindo-Kuhn and Guzley (2001) developed the Volunteer Satisfaction Index (VSI). Responses are recorded using a seven-point Likert Scale. Satisfaction and other volunteering aspects are measured through participant responses. The purpose of this study was to investigate volunteer satisfaction in a level 1 trauma emergency department within a teaching hospital. Furthermore, this study sought to determine best practice for program placement; satisfaction of volunteers based upon their GPA. Participants included 40 (55% female, SD=0.28) undergraduate students enrolled in Natural Science 110 at California State University, Fresno during a period of three semesters from Spring 2019 to Fall 2019. Of those participants, 95% were majoring in pre-medical studies. The initial assessment of each student contained basic demographic questions including GPA and sex. VSI questionnaires were administered to students. Participants indicated relationship within the organization, personal gain, and relationships with peers. Responses were used to identify satisfaction. Volunteers use a seven-point Likert Scale where 1 (very dissatisfied) to 7 (very satisfied) to indicate their level of satisfaction. Utilization of the VSI identifies specific problems experienced by volunteers. The survey scores of the top one-third of GPA will be compared against the students in the bottom-third of GPA during the course of four semesters. All the students participated in the survey. Of those participants, 95% were majoring in pre-medical studies. Their responses were averaged and their responses were tested against various variables such as sex, and major. A reliability analysis was carried out on the perceived task values scale comprising 26 items. Cronbach’s alpha showed the questionnaire to reach acceptable reliability, α = 0.94. All items appeared to be worthy of retention, resulting in a decrease in the alpha if deleted. There was no significant difference in satisfaction based on students who were paired versus students who worked individually. These variables did not show any significant variation. However, Pearson's correlation was computed to assess the relationship between GPA and volunteer satisfaction, r (18) =-.559, p=.005. There was a strong negative correlation between GPA and volunteer satisfaction. An increase in GPA correlates with a decrease in volunteer satisfaction.

Scores of the Cleft Hearing, Appearance and Speech Questionnaire (CHASQ) in Swedish Participants With Cleft lip and/or Cleft Palate and a Control Population.

Objective:The primary aim of this study was to investigate whether there was any difference in scores of the Cleft Hearing, Appearance and Speech Questionnaire (CHASQ) between patients with cleft lip and/or cleft palate (CL/P) and a control population. The second aim was to compare CL/P and control population scores in this study with a British norm CL/P population.Design:Single-site, cross-sectional study with an age-matched control population.Setting:Participants were recruited from a hospital, a school, and a sports club. They answered the CHASQ in the hospital or at home.Participants:Sixty-four participants with CL/P (7-19 years of age) and a control population of 56 participants without CL/P (9-20 years of age).Main Outcome Measure:CHASQ.Results:There was no statistically significant difference in satisfaction with cleft-related features between the CL/P and the control population. Participants with CL/P were significantly more satisfied with non-cleft-related features than the control population. Cleft Hearing, Appearance and Speech Questionnaire scores were also similar to earlier established British normative data of a CL/P population.Conclusion:The results indicated that children and young people with CL/P were as satisfied with their appearance, hearing, and speech as children and young people without CL/P. Swedish CHASQ scores were also similar to British scores.

Stability of Long-Term Outcomes in Implant-Based Breast Reconstruction: An Evaluation of 12-Year Surgeon- and Patient-Reported Outcomes in 3489 Nonirradiated and Irradiated Implants.

Outcomes following prosthetic breast reconstruction have been well studied. However, the majority of studies are limited by short-term follow-up and a lack of aesthetic and patient-reported outcomes. This study objectively examines long-term surgeon- and patient-reported outcomes following two-stage prosthetic breast reconstruction. Consecutive patients undergoing two-stage prosthetic breast reconstruction from 1994 to 2016 performed by the senior author (P.G.C.), with at least 1-year follow-up after implant exchange, were reviewed retrospectively. Long-term surgeon-reported outcomes, including aesthetic and capsular contracture scores, and patient-reported outcomes using the BREAST-Q, were recorded at each outpatient visit and analyzed over the 12-year follow-up period. Retrospective review revealed 2284 patients, or 3489 breasts, that fit the inclusion criteria. Aesthetic scores and capsular contracture rates remained stable over the entire follow-up period. Subset analysis demonstrated that bilateral and nonirradiated reconstructions consistently had the highest aesthetic scores, whereas unilateral irradiated breasts had the lowest. Irradiated breasts consistently had high rates of capsular contracture, although the extent of contracture improved over time in all patients. Patient-reported BREAST-Q scores showed either stability or improvement over time in all patients. Irradiated and nonirradiated patients demonstrated comparable long-term satisfaction with outcomes despite significant differences in satisfaction with their breasts. The authors' study, the largest of its kind, demonstrates that prosthetic breast reconstruction outcomes do not deteriorate over time. This stability is apparent in both long-term surgeon- and patient-reported outcomes data measured in the same patients. These results contradict the surgical dogma surrounding prosthetic breast reconstruction and therefore should be given significant consideration when counseling patients. Therapeutic, IV.

Impacts of Converting Courses to Virtual Instruction Midsemester at a Hispanic-Serving Institution

The sudden transition from in-person to virtual learning during the spring 2020 semester posed challenges for students and faculty alike. This paper explores the techniques used by faculty to transform the chemistry and biochemistry curriculum at a Hispanic-serving institution with a significant proportion of first-generation students and the response of students to these changes. Faculty utilized an assortment of techniques, including synchronous lectures by video conference, asynchronous prerecorded lectures, online examinations with varying timing policies, and virtual office hours. A survey was completed by 332 students across 26 different courses taught by 11 faculty regarding their preferences for course delivery format and the academic and nonacademic challenges they faced. A paired samples t-test indicated a statistically significant difference in satisfaction with face-to-face instruction (M = 4.13, SD = 0.765) and satisfaction with virtual instruction (M = 3.57, SD = 0.961), t(440) = 13.30, p < 0.001, Cohen’s d of 0.63, and Cronbach’s α of 0.767. The results indicated a preference for face-to-face instruction. Open-ended survey questions identify that the primary academic challenges faced by students involved technology, understanding material, and difficulty obtaining help, while the primary nonacademic challenges were work issues, focus/motivation, and family issues. These results may inform future practices for virtual instruction.

A seven-year surveillance study of the epidemiology, antifungal susceptibility, risk factors and mortality of candidaemia among paediatric and adult inpatients in a tertiary teaching hospital in China

BackgroundThere are no current national estimates of the candidaemia burden in China, and epidemiological candidaemia data from the underdeveloped region of China are lacking.MethodsA 7-year retrospective study was carried out to analyse the prevalence, species distribution, antifungal susceptibility, risk factors and inpatient mortality of candidaemia among paediatric and adult patients in a regional tertiary teaching hospital in China.ResultsDuring the seven-year study period, a total of 201 inpatients with candidaemia were identified. The median age of the patients was 65 years (range, 1 day to 92 years), and 114 of the patients (56.7%) were male. The mean annual incidence of candidaemia was 0.26 cases per 1000 admissions (0.42 cases per 1000 paediatric admissions vs 0.24 cases per 1000 adult admissions, P < 0.05). Candida albicans was the most common fungal species (81/201, 40.3%) in all patients, Candida glabrata was the most common fungal species (18/35, 51.4%) in paediatric patients. Most isolates were susceptible to flucytosine (99.0%) and amphotericin B (99.0%), and the activity of antifungal agents against Candida species was no significant difference in satisfaction between paediatric and adult patients (P > 0.05). The all-cause mortality rate was 20.4% (paediatric patients: 11.4% vs adult patients:22.3%, P > 0.05). Fewer univariate predictors of poor outcomes were identified for paediatric patients than for adult patients (4 vs 11 predictors). Respiratory dysfunction and septic shock were independent predictors of 30-day mortality for all patients.ConclusionsThe epidemiological data of candidaemia in paediatric and adult patients are only different in the distributions of Candida species and the mean annual incidence of candidaemia. Flucytosine and amphotericin B can be used as first-choice agents when no antifungal susceptibility test results are available.

Patient Satisfaction with the Work of Nurses in Primary Health Care

Introduction. Patient satisfaction with provided health care is an important indicator of health care quality. Research on patient satisfaction has been very frequent in recent years. Aim. A prospective, cross-sectional study was conducted at the Health Centre of the Ministry of the Interior of the Republic of Croatia between April and June 2019. The aim of this study was to examine the satisfaction of patients with the work of nurses in primary health care. Methods. A standardized Nursing Practitioner Satisfaction Survey questionnaire, for which we received the author’s approval, was used. The study involved 200 patients from the Health Centre of the Ministry of the Interior of the Republic of Croatia. Patients who came to the Health Centre’s outpatient clinics requiring only the services of nurses were included in the study. Out of 200 patients surveyed, 185 questionnaires were filled out correctly, while 15 were filled out incorrectly. Results. Patients from the Health Centre of the Ministry of the Interior of the Republic of Croatia are satisfied with the work of nurses in primary health care. There is no statistically significant difference in the satisfaction with the work of nurses in primary health care according to gender, education, marital or working status. There is a statistically significant difference in patient satisfaction with the work of nurses according to the respondents’ age, where respondents in the age group between 26 and 40 years are less satisfied (Kruskal-Wallis test, Me=3.8). There is a statistically significant difference in the satisfaction of patients with the work of nurses in primary health care according to the respondents’ annual income, where satisfaction is more pronounced among respondents with an annual income between 26.000 to 35.000 HRK and in the range between 36.000 to 45.000 HRK (Kruskal-Wallis test, p&amp;lt;0.01). Conclusion. The patients who took part in the study are satisfied with the work of nurses in primary health care. There are significant differences in terms of age and annual income.

İlahiyat Fakültesi Öğrencilerinin Mobil Öğrenmeye Yönelik Tutumları

Bu araştırma, İlahiyat Fakültesi’nde okuyan öğrencilerin mobil öğrenme teknolojilerine yönelik tutumlarını incelemeyi amaçlanmıştır. Bu temel amaç doğrultusunda katılımcıların “Mobil Öğrenmeye Yönelik Tutum Ölçeğinden” aldıkları puanların cinsiyet/sınıf değişkenine göre anlamlı bir farklılık gösterip göstermediğine bakılmıştır. Araştırma yöntemi olarak nicel araştırma yöntemlerinden nedensel karşılaştırma tercih edilmiştir. Araştırmanın örneklemi; Hatay Mustafa Kemal Üniversitesi (MKÜ) İlahiyat Fakültesi’nde 1. Sınıfta (55) ve 2. Sınıfta (46) okuyan ve yaşları 18-30 arasında değişen toplam 101 gönüllü öğrenci oluşturmaktadır. Araştırmaya katılan öğrencilerin mobil öğrenmeye yönelik tutumlarına ait veriler nicel olarak Demir ve Akpınar tarafından 2016 yılında geliştirilen “Mobil Öğrenmeye Yönelik Tutum Ölçeği” ile toplanmıştır. Araştırmada elde edilen nicel veriler SPSS 20.0 veri analizi programı ile çözümlenmiş ve bağımsız gruplar t-testi ile analiz edilmiştir. Araştırmanın ilk sonucuna göre çalışmaya katılan İlahiyat Fakültesi öğrencilerinin cinsiyet değişkenine göre ölçek alt faktörlerinde (memnuniyet, öğrenmeye etki, motivasyon, kullanışlılık) ve ölçek toplam puanlarının anlamlı bir şekilde farklılaşmadığı sonucuna ulaşılmıştır. İkinci olarak katılımcılar arasında sınıf değişkenine göre kullanışlılık alt ölçeğinde anlamlı bir farklılık olmadığı; fakat memnuniyet, öğrenmeye etki, motivasyon ve ölçek toplam puanı açısından anlamlı bir farklılık olduğu ortaya çıkmıştır.
