Application of PDCA in improving hand hygiene compliance and nosocomial infection quality in orthopedics†

Abstract Objective To explore the effect of the Plan-Do-Check-Action (PDCA) cycle on hand hygiene and nosocomial infection quality of orthopedic medical staff. Methods The whole year of 2021 was selected to monitor the quality of hand hygiene and hospitalization. Follow-up monitoring and real-time recording during the period of morning shift and medical operation concentration time, and compare the compliance of hand hygiene before and after implementation, and evaluate the quality of nosocomial infection. Results The hand hygiene compliance of doctors and nurses in stage P was 82%. The compliance of medical staff in stage D was 93%. The compliance of stage C was 94% and that of stage A was 95%. The quality score of hospital self-examination nosocomial infection was also significantly increased. Conclusions The PDCA management cycle can effectively improve the compliance of hand hygiene and the nosocomial infection quality, which is worthy of circulatory application in orthopedic nosocomial infection quality control, especially improving the quality of hand hygiene.

The Genesis of the Crypto-economy: Application of the Institutional Theory

This study investigates the institutionalization of the crypto-economy from an institutional theory perspective and also examines the role of existing institutional pillars in its development. The path analysis using global institutional indicators reveals that the development of the crypto-economy is significantly affected by regulatory, cognitive, and normative factors of a country. Further, the regulative pillar fully mediates the relationship between the social pillar and the crypto-economy. While we find convincing evidence that a country’s social pillar endorses crypto-economy, its indirect effect is more prominent and significant than the direct effect.

Enhancing machine failure prediction with a hybrid model approach

This transformative shift involves the incorporation of machine learning techniques to refine the accuracy of predicting machinery failures. In this article, we delve into an in-depth exploration of machine failure prediction, employing a hybrid model amalgamating long short-term memory (LSTM) and support vector machine (SVM). Our comprehensive study meticulously assesses the hybrid model’s performance, comparing it with standalone LSTM and SVM models across three distinct datasets. The results showcase that the hybrid model outperformed, providing the modest dependable, and highest F1-score values in our evaluation.</span>

Gene and cell therapy of human genetic diseases: Recent advances and future directions.

Disruptions in normal development and the emergence of health conditions often result from the malfunction of vital genes in the human body. Decades of scientific research have focused on techniques to modify or substitute defective genes with healthy alternatives, marking a new era in disease treatment, prevention and cure. Recent strides in science and technology have reshaped our understanding of disorders, medication development and treatment recommendations, with human gene and cell therapy at the forefront of this transformative shift. Its primary objective is the modification of genes or adjustment of cell behaviour for therapeutic purposes. In this review, we focus on the latest advances in gene and cell therapy for treating human genetic diseases, with a particular emphasis on FDA and EMA-approved therapies and the evolving landscape of genome editing. We examine the current state of innovative gene editing technologies, particularly the CRISPR-Cas systems. As we explore the progress, ethical considerations and prospects of these innovations, we gain insight into their potential to revolutionize the treatment of genetic diseases, along with a discussion of the challenges associated with their regulatory pathways. This review traces the origins and evolution of these therapies, from conceptual ideas to practical clinical applications, marking a significant milestone in the field of medical science.

My Holistic Data Share: A WEB3 Data Share Application: Extending Beyond Finance to Privacy-Protected Decentralized Share of Multi-Dimensional Data to Enhance Global Healthcare

Abstract WEB3.0 technologies on network architectures, distributed ledgers, and decentralized artificial intelligence represents a transformative shift in how data are handled, stored and shared. These innovations promise to significantly enhance consumer data privacy rights by addressing fundamental vulnerabilities associated with traditional centralized systems and self-custody wallets. Data breaches in traditional systems operated mainly by third parties are more common, resulting in significant data leaks because of centralized storage and excessive data movement, sometimes unnecessarily. Healthcare data breaches have been a growing concern globally. Several hospitals faced operational halts on account of the impact of ransomware on patient care and privacy. WEB3 Wallets are a vital component emerging as a significant force for global financial inclusion, especially in developing economies. They promote inclusion, reduce costs and empower individuals through self-custody. Though major improvements are needed in these wallets, their use is rising steadily. The global cryptocurrency user base is expected to reach over 500 million by 2025, with substantial growth in emerging markets, according to a report by Statista in 2023. This paper introduces a concept beyond cryptocurrencies and finance into everyday real-world use cases that need combinatorial access to a person’s holistic data, including financial and health records, genomic data, and advanced directives, among others, that need to be privacy protected and shared with specific actors identified for their roles in the WEB3 ecosystem through decentralized identifiers and non-fungible token badges identifying particular recipients. The author introduced the concept at ETHBoston in April 2024, won accolades for a primitive implementation using underlying threshold cryptography technologies, and enhanced it into a conceptual holistic data share application for global healthcare, as presented in this paper.

A garment-integrated textile stitch-based strain sensor device, IoT-Enabled for enhanced wearable sportswear applications

The field of wearable technology is undergoing a transformative shift with the integration of IoT-enabled, AI-assisted sensors into sportswear, bringing sophisticated performance analysis within reach for a broader audience. Despite advancements in stitch strain sensors, there is a significant research gap in developing high-performance, stable, and IoT-connected complete stitch strain wearable sensor systems. These advanced sensors are essential for enhancing wearable technology and meeting the growing demands for improved user experience and functionality. In this study by utilizing conductive sewing threads and innovative stitching techniques, we fabricated textile strain sensors that offer high sensitivity, flexibility, and durability. Extensive calibration tests revealed strong linear relationships between resistance and tensile strain, with highest sensor sensitivity (2.08 Ω/mm during loading and 2.72 Ω/mm during unloading). These sensors demonstrate reliable performance with minimal hysteresis, particularly in shorter lengths. The wearable sportswear device, tested in gym environments, demonstrated the sensor's capability to provide real-time feedback on knee flexing/bending and body movement during exercise. This functionality supports injury prevention and enhances workout efficiency. The sensor system is IoT-enabled, powered by an ESP8266 microcontroller, facilitating data transmission to the Thing Speak platform for advanced analysis and remote monitoring. This comprehensive approach not only advances wearable technology but also opens new avenues for remote health monitoring and smart e-textiles, contributing significantly to the fields of sports science and fitness technology.

QuickPic AAC: An AI-Based Application to Enable Just-in-Time Generation of Topic-Specific Displays for Persons Who Are Minimally Speaking.

As artificial intelligence (AI) makes significant headway in various arenas, the field of speech-language pathology is at the precipice of experiencing a transformative shift towards automation. This study introduces QuickPic AAC, an AI-driven application designed to generate topic-specific displays from photographs in a "just-in-time" manner. Using QuickPic AAC, this study aimed to (a) determine which of two AI algorithms (NLG-AAC and GPT-3.5) results in greater specificity of vocabulary (i.e., percentage of vocabulary kept/deleted by clinician relative to vocabulary generated by QuickPic AAC; percentage of vocabulary modified); and to (b) evaluate perceived usability of QuickPic AAC among practicing speech-language pathologists. Results revealed that the GPT-3.5 algorithm consistently resulted in greater specificity of vocabulary and that speech-language pathologists expressed high user satisfaction for the QuickPic AAC application. These results support continued study of the implementation of QuickPic AAC in clinical practice and demonstrate the possibility of utilizing topic-specific displays as just-in-time supports.

Temporal multilayer structures in discrete physical systems towards arbitrary-dimensional non-Abelian Aharonov-Bohm interferences

Temporal modulation recently draws great attentions in wave manipulations, with which one can introduce the concept of temporal multilayer structure, a temporal counterpart of spatially multilayer configurations. This kind of multilayer structure holds temporal interfaces in the time domain, which provides additional flexibility in temporal operations. Here we take this opportunity and propose to simulate a non-Abelian gauge field with a temporal multilayer structure in the discrete physical system. Two basic temporal operations, i.e., the folding/unfolding operation and the phase shift operation are used to design such a temporal multilayer structure, which hence can support noncommutative operations to realize the non-Abelian Aharonov-Bohm interference in the time domain. A two-/three-dimensional non-Abelian gauge field can be built, which may be further extended to higher dimensions. Our work therefore provides a unique platform enabling generalization of non-Abelian physics to arbitrary dimensions and offers a method for wave manipulations with temporal band engineering.

Implementation of 4-bit Universal Shift Register using Reversible Logic

The field of digital logic design is increasingly exploring reversible logic due to its potential in minimizing power dissipation, an essential criterion in low-power applications. This paper presents the design and implementation of a 4-bit universal shift register (USR) using reversible logic gates. The design leverages Fredkin gates, multiplexer (MUX), and D flip-flops to achieve functionality with minimal power consumption. Simulation results demonstrate the efficiency and effectiveness of the proposed design. Reversible logic is essential to low-power digital design and quantum computing because it makes it possible to reconstruct input signals from output signals, which reduces heat generation and information loss. The suggested shift register can be used for hold, parallel load, and left and right shift operations. To implement the functions of the shift register, we used a variety of reversible gates, including the Toffoli and Fredkin gates. When the architecture is contrasted with traditional non-reversible shift registers, it shows notable gains in reduced gate count and power efficiency

Critical review of clinical data and expert-based recommendations for the use of bosutinib in the treatment of chronic myeloid leukemia.

Chronic myeloid leukemia (CML), characterized by the presence of the BCR::ABL1 fusion gene, has undergone a transformative shift with the introduction of tyrosine kinase inhibitors (TKIs). The current availability of six different TKIs (imatinib, dasatinib, nilotinib, bosutinib, ponatinib, and asciminib) in clinical practice makes it important to know their efficacy and toxicity profile for treatment optimization. This review examines the latest insights regarding the use of bosutinib in CML treatment. Clinical trials have demonstrated the effectiveness of bosutinib, positioning it as a first-line treatment that can induce sustained molecular responses. Importantly, it can also be effective in patients who have experienced treatment failure or intolerance with prior TKIs, revealing the potential of bosutinib also in second- and later-line settings. Even in the advanced phase of CML, bosutinib has demonstrated its capacity to achieve molecular responses, expanding its usefulness. Real-world evidence studies echo these findings, emphasizing bosutinib's effectiveness in achieving deep molecular responses, maintaining remissions, and serving as an alternative for patients intolerant or resistant to other TKIs as a second-line therapy. Notably, one of the greatest strengths of bosutinib is its favorable safety profile, in particular the low incidence of vascular complications with its use, which is undoubtedly a comparative advantage over other TKIs. In summary, the latest research highlights the versatility of bosutinib in CML treatment and underscores its pivotal role in optimizing patient management in challenging cases. Continuing research and investigation will further establish bosutinib's place in the evolving landscape of CML therapy, offering an alternative for CML patients across different treatment stages.

Transformative potential of three-dimensional bioprinting technology for advanced organoid research

Organoid research has emerged as a transformative field in biomedicine, focusing on the in vitro development of 3-dimensional (3D) structures that mimic human organs. Derived from various types of stem cells, organoids closely replicate human organ structures and functions, offering significant advantages over 2-dimensional cell cultures and animal models, particularly for drug development, tissue engineering, and precision medicine. Recent innovations, including the integration of biofabrication technologies, have significantly increased the structural complexity and maturity of organoids, expanding their biomedical applications. A critical factor in organoid culture is the utilization of the extracellular matrix (ECM), particularly decellularized ECM hydrogels. These hydrogels are instrumental in organoid growth and development, effectively simulating in vivo environments and supporting organoid functionality across various organ systems. The integration of 3D bioprinting technology into organoid research marks a transformative shift that has enabled the creation of intricate and customized structures. This review demonstrates that these technological innovations have not only revolutionized tissue engineering and regenerative medicine, but also hold immense potential for pharmacology, disease modeling, and personalized medical interventions. The synergistic integration of these technologies presents a promising future for medical research, paving the way for significant advancements in disease modeling, drug discovery, and the development of patient-specific treatments, and marking our entry into a new era of precision medicine and personalized healthcare solutions.

Dynamic multifield continualization of multilayered lattice-like metamaterials

This work focuses on dynamic continualization of multifield multilayered metamaterials in order to obtain energetically-consistent models able to provide an accurate description of the dispersive behavior of the corresponding discrete system. Continuum models, characterized by constitutive and inertial non-localities, have been identified through a recently proposed enhanced continualization scheme. They are identified by governing equations both of the integro-differential and higher-order gradient-type, whose regularization kernel or pseudo-differential functions accounting for shift operators are formally expanded in Taylor series. The adopted regularization kernel exhibits polar singularities at the edge of the first Brillouin zone, thus assuring the convergence of the frequency spectrum to the one of the Lagrangian system in the entire wave vector domain. The validity of the proposed approach is assessed through the investigation of multilayered discrete lattices with an antitetrachiral topology, where local resonators act as rigid links among the layers. The convergence of dispersion curves of the continuum model to the ones of the Lagrangian model is proved in the whole first Brillouin zone as the adopted continualization order increases, both considering the propagation and the spatial attenuation of Bloch waves inside the metamaterial. A low frequency continualization is also provided, leading to the identification of a first-order medium.
