VALSARTAN IN EVERYDAY CLINICAL PRACTICE IN RUSSIA: ANTIHYPERTENSIVE EFFICACY AND INFLUENCE ON SEXUAL FUNCTION IN PATIENTS WITH ARTERIAL HYPERTENSION

Aim. To study antihypertensive efficacy and safety of valsartan-based therapy (Diovan, Novartis Pharma) as well as patient’s compliance and influence of treatment on several aspects of sexual function. Material and methods. 114 doctors from 81 medical institutions of Russia participated in this prospective multicenter observation study. 650 hypertensive patients (average age 53,9±0,4 y.o.) were enrolled. The evaluation of therapy efficacy was based on analysis of systolic (SBP) and diastolic (DBP) blood pressure (BP) changes. Safety and compliance of treatment was also analyzed. The evaluation of sexual function was performed with 5 universal questions selected form the International Index of Erectile Function. These questions could be asked in both men and women. The valsartan dose was 80-320 mg OD. A combination of the valsartan with hydrochlorothiazide (12,5-25 mg/d), amlodipine (5-10 mg/d) or any other antihypertensive was allowed. Results. Significant similar decrease of SBP and DBP was observed in smoking and non-smoking patients (37,5/18,5 and 37,6/15,9 mm Hg respectively, р<0,01 vs baseline). 312 patients (56,9% men, 43,1% women) completed sexual function survey. After 12 weeks of treatment the number of patients without sexual activity and with 1-2 successful sexual attempts in the last 4 weeks significantly decreased from 22 to 16% and from 44 to 30% respectively (р<0,05). Significant increase in the number of patients with 5-6 and 7-10 successful sexual attempts was observed (from 7 to 20% and from 0 to 7% respectively, р<0,05). The treatment resulted in significant increase in the number of patients, who characterized their sexual life satisfaction as “very satisfied” (from 11 to 25%, р<0,01). Conclusion. During the course of effective antihypertensive treatment there was a significant increase in sexual function and general satisfaction with sexual life in patients with arterial hypertension, which could have favorable long-term consequences in increased compliance to the antihypertensive therapy.

Voiding and Sexual Function after Autonomic-Nerve-Preserving Surgery for Rectal Cancer in Disease-Free Male Patients

PurposeWe evaluated the effects of surgery for rectal cancer on postoperative voiding and sexual function over the course of time.Materials and MethodsData from 28 patients who underwent autonomic nerve preserving rectal cancer surgery were retrospectively analyzed. Operations were performed between October 2005 and July 2007 and all patients were followed-up for more than 3 years. Preoperatively, all patients underwent urodynamic studies including uroflowmetry, and filled out the International Prostate Symptom Score (IPSS). The evaluation of sexual function consisted of Erectile Function domain score in International Index of Erectile Function (IIEF-EFD) and Ejaculation domain score in Male Sexual Health Questionnaire (MSHQ-EjD). Data from uroflowmetry and questionnaires were examined.ResultsAt 3 years postoperatively the prostate volume was similar to the preoperative value (p=0.727). There were no statistically significant postoperative changes in the average maximum flow rate (15.9 ml/s vs. 16.2 ml/s, p=0.637) and post-void residual urine volume (34.7 ml vs. 36.8 ml, p=0.809). No statistically significant differences were observed in the IPSS (13.2 vs. 12.2, p=0.374). However, although pelvic autonomic nerve preservation have been performed, a significant proportion of rectal cancer patients suffer from sexual dysfunction and the average of IIEF-EFD and MSHQ-EjD scores was decreased postoperatively until 3 years (25.1 vs. 16.1 and 28.3 vs. 14.2 respectively, p<0.001).ConclusionsVoiding function was not affected after autonomic nerve-preserving rectal cancer surgery, however sexual function was significantly aggravated. We recommend that the baseline genitourinary function should be evaluated before the treatment for male rectal cancer patients, and penile rehabilitation is necessary for their quality of life after treatment.

Longitudinal Evaluation of Sexual Function in a Male Cohort: The Olmsted County Study of Urinary Symptoms and Health Status among Men

The presence of erectile or ejaculatory dysfunction may indicate physical problems; however, individual perceptions (e.g., sexual satisfaction) may reflect the degree of concern about these changes. Long-term data showing how changes in multiple sexual function domains track together may be useful in understanding the importance of physical declines vs. sexual satisfaction. The aim of this study was to describe changes in sexual function among a population-based sample of aging men. A population-based cohort study using data from the Olmsted County Study of Urinary Symptoms and Health Status among Men. Sexual function was assessed biennially from 1996 to 2004 using a previously validated questionnaire in a random sample of 2,213 men. Changes in erectile function, libido, ejaculatory function, sexual problems, and sexual satisfaction. Overall, we observed declines in all of the sexual function domains, ranging from an annual decrease of 0.03 point per year for sexual satisfaction to an annual decrease of 0.23 point per year in erectile function. Moderate correlations were observed among all longitudinal changes in sexual function (range in age-adjusted r(s) = 0.14-0.43); however, significantly smaller correlations between changes in the functional domains and changes in sexual satisfaction and problem assessment were observed among older men (range in age-adjusted r(s) = 0.03-0.29). Overall, these results demonstrate that longitudinal changes in five sexual function domains change together over time in our community-based cohort. Erectile function, ejaculatory function, and sexual drive decrease over time with greater rates of decline for older men. However, older men may be less likely to perceive these declines as a problem and be dissatisfied. These data may prove helpful to patients and clinicians in understanding and discussing changes in multiple aspects of sexual function.

Prospective evaluation of sexual function after open and laparoscopic surgery for rectal cancer

Sexual function may be harmed after treatment for rectal cancer. This study aimed to evaluate prospectively the incidence of sexual dysfunction after rectal cancer treatment and to compare the effects of laparoscopic and traditional open approaches in terms of postoperative sexual function. Baseline and 3-, 6-, and 12-month assessments of sexual dysfunction using the International Index of Erectile Function (IIEF) and its specific domains prospectively took place for 56 patients who underwent rectal cancer surgery (38 open vs. 18 laparoscopic procedures, 38 low anterior vs. 18 abdominoperineal resections). The preliminary results are presented. The average total IIEF and isolated IIEF response domain scores were significantly decreased after surgery (p < 0.01) except for the intercourse satisfaction and overall satisfaction scores at 12 months. An improvement in IIEF scores was observed between the 3- and 6-month assessment points (p < 0.01) except for the erectile function and orgasmic function scores. No significant differences were observed between the open and laparoscopic groups in the total IIEF and domain scores preoperatively and at the 3- and 6-month assessment points. The rates of sexual dysfunction did not differ significantly preoperatively or at 3 months postoperatively when open and laparoscopic procedures were compared, although there was a trend in favor of laparoscopic surgery at 6 months (p = 0.076). The baseline IIEF score and the baseline, 3-, and 6-month sexual desire scores were better (p = 0.035, 0.004, 0.017, and 0.061, respectively) in the low anterior resection group than in the abdominoperineal resection group. Rectal cancer resections were postoperatively associated with a significant reduction in IIEF scores and high rates of sexual dysfunction at 3 and 6 months. The IIEF and domain scores at different assessment points were comparable between the laparoscopic and open surgery groups. Extending the monitoring period and adding more patients in this ongoing prospective study will further elucidate postoperative sexual dysfunction after rectal cancer surgery.

Time of Decline in Sexual Function After External Beam Radiotherapy for Prostate Cancer

Erectile dysfunction is one of the most concerning toxicities for patients in the treatment of prostate cancer. The inconsistent evaluation of sexual function (SF) and limited follow-up data have necessitated additional study to clarify the rate and timing of erectile dysfunction after external beam radiotherapy (EBRT) for prostate cancer. A total of 143 men completed baseline data on SF before treatment and at the subsequent follow-up visits. A total of 1187 validated SF inventories were analyzed from the study participants. Multiple domains of SF (sex drive, erectile function, ejaculatory function, and overall satisfaction) were analyzed for < or =8 years of follow-up. The median follow-up was 4.03 years. The strongest predictor of SF after EBRT was SF before treatment. For all domains of SF, the only statistically significant decrease in function occurred in the first 24 months after EBRT. SF stabilized 2 years after treatment completion, with no statistically significant change in any area of SF >2 years after the end of EBRT. These data suggest that SF does not have a continuous decline after EBRT. Instead, SF decreases maximally within the first 24 months after EBRT, with no significant changes thereafter.

Prospective evaluation of sexual function in patients receiving cryosurgery as a primary radical treatment for localized prostate cancer

To evaluate prospectively the sexual function of patients undergoing cryosurgery as a primary radical treatment for localized prostate cancer, as the development of 17 G cryotherapy probes has improved the delivery of this treatment, but one of the side-effects of cryosurgery is the development of erectile dysfunction (ED). Between July 2003 and May 2008, 53 patients were treated using an argon-based third-generation cryotherapy system (Oncura, Arlington Heights, IL, USA). Prospective data were collected at 6 weeks, 3 months, then 3-monthly up to 1 year and subsequently 6-monthly. Patients were followed up for up to 54 months, with a median (mean) follow-up 36 (30.5) months. All 53 patients were followed after receiving cryosurgery as primary treatment for prostate cancer; 51 (96.3%) had ED at 6 weeks while two (3.7%) were experiencing partial erections. By 9 months one (2.4%) of 42 patients was fully potent using phosphodiesterase type-5 inhibitors, and six (14.3%) were experiencing partial erections. By 18 months eight (21%) of 39 patients followed up had regained full potency and by 24 months eight (24%) of 33 patients were fully potent and three (9%) experienced partial erections. While ED is a significant side-effect of cryotherapy, a considerable proportion of patients who have no ED before treatment (39%) recover full sexual function afterward. Focal nerve-sparing cryosurgery might be the way forward in an attempt to preserve erectile function in men who had no ED before treatment. Erectile aids should be made available for those patients for whom sexual dysfunction compromises the quality of their life and relationships.

Prospective Evaluation of Functional Outcome After Laparoscopic Sigmoid Colectomy

To prospectively determine the functional impact of elective laparoscopic sigmoidectomy after prior acute diverticulitis. Decision-analysis for elective colonic resection after acute diverticulitis is predicated on future risk estimates of disease recurrence and complication rates. Quality of life alone is rarely countenanced as sufficient to warrant the supposed risk of intervention. Consecutive patients undergoing laparoscopic sigmoid colectomy for prior diverticulitis over 18 months were studied at baseline (preoperative) and again subsequently (3, 6, and 12 months postoperatively) regarding quality of life and bowel symptomatology [gastrointestinal quality of life index (GIQLI)]. All patients also had enquiry made into urinary and sexual function at the same time points (men underwent international prostate symptom severity and international index of erectile function, respectively, whereas for women the urinary dysfunction index and a modified questionnaire for the evaluation of sexual function were used). Forty-six patients (mean age 58.3, 26 men) were studied, 45 of whom had their surgery completed laparoscopically. Quality of life significantly improved in the group overall, with 36 patients having an increase in their GIQLI >10 points (23 patients >20 points). This improvement was evident by 3 months and maintained thereafter. The improvement was due to increments in gastrointestinal symptomatology subdomain and was most significant in those with lowest preoperative scoring. Five patients' GIQLI score deteriorated without obvious predictive event. Not one of the patients presented evident alteration in urinary and sexual functioning postoperatively. Significant improvement in quality of life and social functioning accrues from elective sigmoid resection in the majority of patients. Avoidance of subsequent episodic attacks therefore should not represent the sole reason for operating but instead patient-centered issues should be prioritized.

Evaluation of sexual function in women with type 2 diabetes mellitus

Sexual health is an important, but often neglected, component of diabetes care. In contrast to erectile dysfunction among men with diabetes, female sexual dysfunction has not been well studied among diabetic women. The aim of this study was to assess the prevalence of sexual dysfunction in women with type 2 diabetes compared to that in an age-matched control group. In all, 50 married women with type 2 diabetes attending the outpatient endocrine clinic of Ghaem Hospital between April 2007 and March 2008 were selected. Fasting plasma glucose and glycosylated haemoglobin were measured and sexual function was assessed by questionnaire. Scores in each domain of sexual function were compared with those of 40 non-diabetic controls. Sexual function scores for the sexual drive, arousal, vaginal lubrication, orgasm and overall satisfaction domains were all lower in the diabetic women (p value < 0.05). Duration of diabetes and age correlated negatively with all domains of sexual function. There was no significant relationship between sexual function and body mass index (BMI), glycaemic control, education or employment status. Diabetes significantly impairs the sexual performance of diabetic women. Determinants of sexual function include age and duration of diabetes.

Comparative effects of conventional hormone replacement therapy and tibolone on climacteric symptoms and sexual dysfunction in postmenopausal women

To compare the effects of tibolone with those of conventional hormone replacement therapy on climacteric symptoms and sexual function in postmenopausal women. In a randomized, controlled trial, 140 postmenopausal women were allocated into three groups. Of the subjects included, 47 women received 2.5 mg tibolone + one Cal+D tablet (500 mg calcium and 200 IU vitamin D) daily; 46 women received 0.625 mg conjugated equine estrogen + 2.5 mg medroxyprogesterone (CEE/MPA) + one Cal+D tablet daily; and 47 women received only one Cal+D tablet as the control group. The Greene Climacteric Scale (GCS) questionnaire was used to detect the efficacy of treatment on climacteric symptoms. Rosen's Female Sexual Function Index (FSFI) was used for sexual function evaluation. Sex hormone binding globulin (SHBG), free estradiol index (FEI) and free testosterone index (FTI) were measured before and after treatment. The women were followed up for 6 months After treatment, all subscores in the GCS improved in the tibolone and CEE/MPA groups (p < 0.01), except the sexual subscore in the CEE/MPA group, compared with baseline. There were significant differences in the FSFI in the tibolone and CEE/MPA groups in comparison to the control group after treatment. Tibolone, in comparison to CEE/MPA, significantly lowered SHBG levels and increased the FTI and FEI and improved the desire, arousal and orgasm sexual domains of the FSFI (p < 0.001). Tibolone may be an alternative to conventional hormone replacement therapy in the treatment of climacteric symptoms and sexual dysfunction in postmenopausal women.

Sexual Pleasure and Motivation

Individuals engage in behaviors or partake of specific substances because it pleases them. Indeed, without the sensations of pleasure and displeasure we would be unlikely to repeat or restrict behaviors. Since repetitive sexual activity over the millennia has ensured the continuance of the human race, it is perhaps surprising that pleasure has received little attention in the research literature on sexual function and behavior. By exploring sexual pleasure and motivation theories and studies relevant to the evaluation of sexual function and dysfunction, a goal response model is presented. This model proposes a route from sexual stimuli, via cognitive and emotional processing to behavior and subsequent feedback, which illustrates how eudemonic goals could influence treatment outcomes.

There are very few studies which have focused on the ability to attain orgasm after radical prostatectomy. Peyronie's disease: a critical appraisal of current diagnosis and treatment

Peyronie's disease is a fibrotic disorder of the tunica albuginea of the penis resulting in varying degrees of penile curvature and sexual dysfunction. Diagnosis of the disorder is made by detailed sexual history and focused physical examination. A wide range of medical treatments has been employed to treat the disorder. Verapamil treatment administered by intraplaque injection is supported by generally consistent randomized controlled trials (RCT). Some RCT evidence exists to support the use of colchicine, potaba, L-carnitine and liposomal superoxide dismutase. Future studies of medical efficacy should focus on improving RCT design, implementation and results reporting to improve the quality of evidence generated by each study. The surgical treatment of Peyronie's disease is a viable and recommended alternative for men with compromised sexual function due to severe curvature or lesions causing penile instability. The choice of corrective procedure should be tailored to each patient after a detailed evaluation of disease severity and sexual function.

Correction of Erosion after Suburethral Sling Insertion for Stress Incontinence: Results and Related Sexual Function

BackgroundSuburethral slings are commonly used for the surgical treatment of female stress incontinence; occasionally they can cause erosion and dyspareunia. ObjectivesThe primary aim of this study is to determine the outcome after reclosure of the vaginal epithelium for suburethral sling erosion. Sexual function was assessed before and after intervention using the Female Sexual Function Index (FSFI) questionnaire. Design, setting, and participantsThis was a prospective case-controlled study in which, between December 2005 and December 2007, we included patients who were referred to the Department of Urogynaecology because of vaginal erosion after suburethral sling insertion for urinary stress incontinence. For evaluation of sexual function, all patients filled in an FSFI questionnaire before intervention and at follow-up.All patients underwent gynaecological examination including colposcopy, and the site and size of the defect were noted. InterventionThe edge of the vaginal epithelium was trimmed, mobilized, and closed with interrupted vertical Vicryl mattress sutures in a single layer. MeasurementsFSFI questionnaire and clinical findings. Results and limitationsTwenty-one patients were included in the study. Eighteen patients with larger defects were operated on, and three defects healed after topical application of estrogen cream.In 16 patients, the defect had healed at follow-up; two patients with persisting defects were brought back to surgery and the procedure was repeated, paying particular attention to tension-free adaptation of vaginal tissue. In one patient, partial sling removal was performed after the second failed intervention.The domains of desire (p<0.0001), arousal (p<0.0003), lubrication (p<0.0001), satisfaction (p<0.0130), and pain (p<0.0001) improved significantly. Orgasm remained unchanged (p=0.4130; all two-tailed t-test). ConclusionSuburethral erosion can be treated effectively by resuturing. Sexual function is improved in regard to desire, arousal, lubrication, satisfaction, and pain, but not orgasm.In septic patients and patients with a history of radiation, grossly infected tissue, or severe pain, excision of the mesh needs to be considered.

A Prospective Evaluation of Sexual Function and Quality of Life After Ileal Pouch-Anal Anastomosis

This study was designed to assess the effect of ileal pouch-anal anastomosis on sexual function and quality of life in men and women. Sexual function of patients undergoing ileal pouch-anal anastomosis from February 2005 to June 2006 was prospectively evaluated using the International Index of Erectile Function in men and Female Sexual Function Index in women. Quality of life was assessed using the Short Inflammatory Bowel Disease Questionnaire. Preoperative scores were compared with scores at 6 and 12 months postoperatively. Of 110 patients eligible for inclusion, 59 (53.6 percent) agreed to participate. Male sexual function and erectile function scores remained high 12 months after surgery (mean International Index of Erectile Function score 51.7 preoperative vs. 58.3 at 12 months postoperative; P = not significant (NS)). Abnormal sexual function decreased from 33.3 percent before surgery to 22.7 percent 12 months after surgery (P = NS). Female sexual function improved 12 months after surgery (mean Female Sexual Function Index score 19.2 preoperative vs. 27 at 12 months postoperative; P = 0.031). Abnormal sexual function decreased from 73.1 percent before surgery to 25 percent 12 months after surgery (P = 0.001). Quality of life significantly improved after ileal pouch-anal anastomosis in both sexes. In men, ileal pouch-anal anastomosis does not have an adverse effect on sexual function, whereas sexual function in women seems to improve 12 months after surgery.

T09-P-16 Evaluation of sexual function, quality of life and depressive aspects in women affected by type 1 diabetes mellitus

Sexual function in healthy and particularly in diabetic women has received little attention from clinical researchers. Aim of the study a) To define which is the most impaired sexual phase in a group of type 1 diabetic females. b) To study the role of biological, psychological, social and relational factors in the same population. Materials and methods We evaluated clinical and glycometabolic factors as well as the following psycho-cognitive and social variables: Zung Depression Scale, Diabetes Quality of Life, Body Image Scale, Female Sexual Function Index, Multidimensional Scale of Perceived Social Support. Inclusion criteria: type 1 diabetes mellitus, age >18 years, stable heterosexual relationship for at least 1 year. Results We enrolled 77 diabetic women (age [mean± SD]: 39.0±10.1 yr; duration of the disease: 16.8±10.1 yr; mean HbA1C: 8.1±1.1%). As expected we found a significant correlation between quality of life and: a) depressive aspects (p Conclusions In women affected by type 1 diabetes mellitus psychological factors seem to be predominant with respect to somatic ones; in particular we suppose that a good metabolic control could reflect an attitude to “control” both glucose monitoring and sexual life.

T09-O-34 The impact of the coronary artery bypass surgery on the sexual function

Introduction ED is related to atherosclerosis risk factors. Of great clinical importance is the potential link between ED and coronary artery disease. Cardiopulmonary bypass (CAB) has been used for coronary artery bypass grafting (CABG) for over 3 decades for surgical myocardial revascularization. The published articles discussing the effect of CABG on sexual function are few. Moreover these studies were based on questionnaires rather than Penile Duplex in the evaluation of sexual function. We herein evaluate the impact of CABG surgery on sexual function. Methods The present study included 100 patients who underwent CABG. The patients were evaluated by abridged form of International Index of Erectile Function questionnaire (IIEF-5), Pharmaco-Penile Duplex Ultrasound and finally by the European System for Cardiac Operative Risk Evaluation (EuroSCORE). Six months after surgery the erectile function of all patients was revaluated with the same preoperative procedures. Laparoscopic Pelvic Autonomic Nerve-Preserving Surgery for Sigmoid Colon Cancer

BackgroundTo test the feasibility of laparoscopic approach in performing the simultaneous pelvic autonomic nerve preservation during standard anterior resection of sigmoid colon cancer.MethodsPatients meeting appropriate eligibility criteria were recruited for the present study. The surgical procedures are shown in the video. The genitourinary function was evaluated on the basis of validated questionnaires including International Prostate Symptom Score (IPSS), International Index of Erectile Function (IIEF), and Female Sexual Function Index (FSFI).ResultsA total of 112 patients (tumor, node, metastasis system stage I, n = 8; stage II, n = 54; stage III, n = 50; male, n = 58; female, n = 54; age [mean ± standard deviation], 55.8 ± 6.4 years) with good baseline genitourinary function were operated on with the intent of total preservation of pelvic autonomic nerves and curative resection of sigmoid colon cancer. The patients were prospectively followed (median time of follow-up, 18 months; range, 6–30 months). In patients with a successful nerve-preserving surgery (96.4%, n = 108), 104 patients completed the evaluation of urinary function. The median duration for indwelling urine Foley catheter was 3.0 days (range, 1.0–7.0 days). The voiding function after removal of the urine Foley catheter was good (IPSS, 0–7) in 98 (94.2%) patients, fair (IPSS, 8–14) in 5 (4.8%), and poor (IPSS, 15–35) in 1 (1.0%). Before and after nerve-preserving surgery, there were no significant changes of IPSS scores (3.20 ± 1.72 vs. 3.68 ± 2.82, P = .075, paired t-test) in the present patient series. Forty-four male patients completed the postoperative evaluation of sexual function, and ejaculation was ranked as good in 40 (90.9%), fair (decrease in ejaculatory amounts) in 3 (6.8%), and poor (retrograde ejaculation, failure of ejaculation) in 1 (2.3%), whereas the potency was good (IIEF, 60–75) in 41 (93.2%), fair (IIEF, 44–59) in 2 (4.5%), and poor (IIEF, 5–43) in 1 (2.3%). Moreover, before and after a successful nerve-preserving operation, there were no significant changes of IIEF scores (72.4 ± 4.6 vs. 70.3 ± 8.4, P = .082, paired t-test). For female patients (n = 42), the postoperative sexual function was ranked as good (FSFI score, 76–95) in 36 (85.7%), fair (FSFI, 58–75) in 4 (9.5%), and poor (FSFI, 4–57) in 2 (4.8%). Furthermore, there were no significant changes of FSFI scores (89.0 ± 9.2 vs. 85.4 ± 16.4, P = .122, paired t-test) before and after successful nerve-preserving surgery.ConclusionsUnder laparoscopy, we can clearly identify and preserve the pelvic autonomic nerves to retain genitourinary function in most patients undergoing oncologic resection of sigmoid colon cancer.Electronic supplementary materialThe online version of this article (doi:10.1245/s10434-008-9861-x) contains supplementary material, which is available to authorized users.
