This document was developed by clinical psychologists working in community older adult mental health services during Covid-19. Born out of discussions about what the profession should expect during the recovery phase, the present authors decided to write a document to inform and guide those working in psychological services on identifying and working with changes in the population’s mental health needs. This document focuses on the likely role of clinical psychology and is structured in two parts. The second part addresses the potential for patients to experience post-traumatic stress disorder (PTSD) and how we can assess and treat PTSD in these populations.

The role of clinical psychologist in modern healthcare system

The article analyses current understanding of clinical psychology as a field of professional activity of psychologists in medicine and public health. It reveals the main traditional and new areas of activity of a medical psychologist in clinical and preventive medicine. Main directions and forms of clinical psychologist’ work in healthcare system is reviewed. The role of clinical psychologist as part of a multidisciplinary team is outlined and the main models of multidisciplinary teams in the field of medical rehabilitation are analyzed. We identified the main problems and the goals in this area, which solution is necessary to create an effective system for the provision of medical, psychological and social assistance to the population. The article shows that clinical psychology plays a key role within the multidisciplinary approach to assistance of patients and their relatives in specialized psychiatric or addiction treatment, psychotherapy, as well as in general medical practice. One of the main tasks of clinical psychology is considered humanization and personalization of the entire process of medical care, protection from inconvenient impact of technization of medicine on patient and doctor, narrow specialization of health services. Some of the organizational and legal barriers hindering full integration of medical psychologist into the healthcare system and effective realization of the goals of medical and psychological activities were highlighted. The prospects for the development of clinical psychology in healthcare in the theoretical, methodological, substantive and organizational-legal aspects are determined. It is emphasized that in order to realize the potential of clinical psychology, constant improvement of theory and methodology, the development of effective medical and psychological technologies for diagnosis, correction, and rehabilitation are necessary. And at the same time, it is important to create organizational and legal conditions for the full cooperation of clinical psychologist with specialists in other areas.

‘Is it time to decolonise neuropsychology?’ Critical reflections on colonial structures, neuropsychology and the role of clinical psychologists

There is a longstanding history of psychological services being inaccessible for individuals from a racially minoritised background, including continuing to pathologise individuals from these communities (Fernando, 2010). There has been significant evidence that clinical psychology continues to perpetuate racism, discrimination, and marginalisation, with Pilgrim and Patel (2015) stating that clinical psychology remains ‘anything but for all’. A further exploration into the theory and practice of neuropsychology highlights a history of systematic and scientific racism which includes the inappropriate use of ‘race’ in neuropsychological assessment and testing to further marginalise, infantilise and create a narrative of individuals from racially minoritised backgrounds as ‘inferior’ (Hiermeier & Verity, 2022). In this article the authors explore the experiences of navigating neuropsychology as racially minoritised individuals. The authors reflect on their experiences of witnessing and navigating racism in the practice and application of neuropsychology. The paper also highlights that due to the significant evidence of scientific racism in neuropsychology that Whiteness, power, racism and colonisation should be a core and compulsory component of the training of clinical and neuropsychologists. As a result, there is a need to move away from centring conversations on equality, diversity and ‘cultural competence’ with a clear focus on equity, justice, Whiteness and racism.

Orthognathic surgery is a major elective procedure that can pose a significant amount of psychological distress to patients before, during and after surgery. These patients tend to have higher levels of state anxiety and a poor (facial) body image of themselves. Patients less satisfied by their outcomes tend to have had particularly high expectations of surgery beforehand. Therefore, it is important to ensure that there are effective and supportive psychology services in place. Here, we assess the utility and the role of clinical psychology in modern, holistic orthognathic surgery and the tools used to assess patients for further psychological intervention. CPD/Clinical Relevance: Clinicians should be aware of the services available to patients who are transitioning through the orthognathic surgery pathway.

A systems psychodynamic description of clinical psychologists’ role transition towards becoming organisational development consultants

Orientation: Clinical psychologists’ transition from practising in a clinical context to a large organisation implies an intensely experienced professional identity shift.Research purpose: To provide a systems psychodynamic description of the lived experiences of a group of clinical psychologists’ role transition towards becoming organisational development (OD) consultants.Motivation for the study: Although the role of clinical psychologists in organisations is theoretically explicated, limited research exists on their role transition experiences. Their lack of theoretical knowledge about and experience in organisational psychology make them vulnerable for exclusion and isolation.Research approach/design and method: A hermeneutic phenomenological research design using a collective case study, consisting of eight clinical psychologists, was used. The data gathering methods included a Listening Post and socio-analytic interviews. Systems psychodynamic data analysis was performed.Main findings: The manifesting themes related to experiences of diverse and intense anxiety, defensive structures, role, task, boundary and authorisation conflicts as well as solitary pilgrimages. Participants experienced an attack on their personal and professional identities, a sense of being overwhelmed, excluded and isolated from their colleagues and peers.Practical/managerial implications: The findings will facilitate clinical psychologists’ transition into OD roles and organisations’ awareness of their identity challenges.Contribution/value-add: Although employing clinical psychologists in OD roles is practised in many large organisations, the findings suggest that their transition into this role is underestimated in terms of emotional intensity.

Clinical Psychologist in the System of Complex Rehabilitation

The research substantiates the role of clinical psychologists in the system of comprehensive rehabilitation within the conditions of bio-psychosocial model of rehabilitation. Practical experience and the analysis of scientific literature made it possible to describe peculiarities of modern complex rehabilitation and the problems experienced by clinical psychologists. The authors determined difficulties and characteristics of professional work of rehabilitation psychologist within modern legal terrain. One problem is the lack of professional standard for the profession of clinical psychologist. The other problem is the inconsistency in the use of terms denoting professional specifics. Тhe paper focuses on the specificity of psychodiagnostic and rehabilitative work of a psychologist in the light of changing theoretical basis of rehabilitation and the transition from the International Classification of Disability Disorders and Social Insufficiency to the International Classification of Functioning, Disability, and Health. The authors showed the discrepancy between the theoretical approaches of domestic and foreign psychology to the assessment of mental disorders. New professional tasks of the psychologist are defined taking into account multidisciplinary orientation of rehabilitation process. The paper describes the problems of professional training of clinical psychologists and specifies new educational tasks are specified proceeding from the modern approach to rehabilitation. Positive dynamics in the solution of the declared problem can be found in new legislative initiatives, development of General clinical recommendations for rehabilitation psychologists, scientific publications on psychological rehabilitation, update of educational programs for training clinical psychologists.

The Emerging Role of Clinical Pharmacopsychology

Clinical pharmacopsychology is an area of clinical psychology that is concerned with the application of clinimetric methods to the assessment of psychotropic effects of drugs on psychological functioning, and the interaction of such drugs with specific or non-specific treatment ingredients. Clinical pharmacopsychology derives its data from observational and controlled studies on clinical populations and refers to the therapeutic use of medical drugs, not to the effects of substances used for other purposes. Domains and operational settings of clinical pharmacopsychology are illustrated. The domains of clinical pharmacopsychology extend over several areas of application which encompass the psychological effects of psychotropic drugs (with particular emphasis on subclinical changes), the characteristics that predict responsiveness to treatment, the vulnerabilities induced by treatment (i.e., side effects, behavioral toxicity, iatrogenic comorbidity), and the interactions between drug therapy and psychological variables. A service for clinical pharmacopsychology is here proposed as an example of the innovative role of clinical psychology in medical settings. Clinical pharmacopsychology offers a unifying framework for the understanding of clinical phenomena in medical and psychiatric settings. Its aim is to provide a comprehensive assessment of the clinical important changes that are concerned with wanted and expected treatment effects; treatment-induced unwanted side effects; and the patient's own personal experience of a change in terms of well-being and/or quality of life. It is now time to practice clinical pharmacopsychology, creating ad hoc services in Europe. Clinical psychology services and peer to peer support can both contribute in increasing the psychological wellbeing of patients with chronic medical conditions. In this perspective paper, indications are given about the provision these services for the specific case of adults with congenital heart disease. These patients are at an increased risk of psychological distress, neurocognitive deficits, and social challenges. The psychosocial characteristics and mental health treatment preferences of these patients are briefly described, followed by guidelines and indications for the implementation of clinical psychology services. The most structured peer to peer program available for this population is subsequently illustrated and finally, specific benefits and challenges when it comes to the integration of both services are reported.

Purpose The purpose of this paper is to discuss the role of clinical psychologists in promoting compassionate cultures within intellectual disability services. Design/methodology/approach This is a commentary paper which draws on Albee’s (1983, 2000) social justice model as a theoretical framework for the primary prevention of the mental health consequences of social injustice. Findings Albee’s model conceptualises three interconnected areas including; the individual or micro-level, the organisational or meso-level and the level of social structures, policies and ideology or macro-level. Where possible, specific examples were used to highlight how the authors are currently contributing to this agenda through work that is already underway and areas for development within the service. Originality/value As a commentary paper this discusses current practice and ideas for future practice but does not present original data.

The psychological impact of asbestos, an invisible killer of body and mind

Introduction: Exposure to asbestos causes the onset of various diseases, including lung plaques, asbestosis and malignant pleural mesothelioma (MPM). Living in a Contaminated Site can therefore represent for the entire community a real cumulative trauma that brings into the field disease and death due to a kind of "invisible killer": asbestos. In such circumstances, it is necessary to implement multidisciplinary protocols that can offer an integrated approach to care for both patients and their caregivers. Methodology: There is now a large body of scientific evidence in the literature substantiating the importance of the role that clinical psychology can play in oncology. The objective of this work is to describe specifically the situation in Casale Monferrato, the research carried out in the area and to outline the psychological-clinical intervention proposed there to respond to the quality of needs highlighted in patients suffering from asbestos-related diseases, including mainly MPM, and their caregivers. Results: The results of the research carried out in Casale Monferrato highlight what emerges in the international literature, namely that patients with asbestocorrelated diseases frequently manifest anxiety, depression, somatic disorders and a certain tendency to social withdrawal. The psychological-clinical work within the intervention of short-term group psychotherapy offers them a setting in which they can historicize the disease and process the many facets of pain that an inauspicious diagnosis brings to the field: from the one more related to the body, to the more purely psychic. Conclusion: The role of clinical psychology in oncology is of fundamental importance both for the patient and for his family, as it can facilitate the processing of suffering and anger related to the diagnosis of cancer disease or having to take care of a loved one who has been given an inauspicious diagnosis. Psychological intervention also allows to give name and meaning to the fear related to exposure to asbestos: a silent and invisible killer, which otherwise risks remaining unthought of and unthinkable.

The Role of Clinical Psychologist as Perceived by Psychiatrists at the National Center for Mental Health

The present study aims at finding out the role of the clinical psychologist as perceived by the psychiatrists at the National Center for Mental Health in Jordan. The study sample consisted of (33) male and female working psychiatrists at NCMH. The researcher has designed an instrument to measure the main functions of the clinical psychologist represented by diagnosis, psychotherapy, and psychological counseling. The instrument was applied to the current study sample. The results of the study show differences between male and female psychiatrists. Results indicate that there are differences in the role of the clinical psychologist as perceived by the psychiatrists, the differences are in favor of experienced psychiatrists, besides indicating that the experience(over 10 years) obtains the highest rate for the role of the clinical psychologist. The results show that the age (greater than 45 years ) is the highest rated for the role of the clinical psychologist. The results show no statistically significant differences at the level of significance (α ≤ 0.05) in the role of clinical psychologist as perceived by the psychiatrists at the National Center for Mental Health attributed to the rank of career where the value (V) = 0.314. In light of these results the researcher put forward a number of recommendations and suggestions.
