Healthy nutrition – prevention of chronic non-communicable diseases. Overview of recommendations

The article provides an overview of the National Recommendations on healthy nutrition available in Europe, Asia (China), the USA and the Russian Federation, assesses the compliance of the presented guidelines with the principles of a healthy diet according to the recommendations of the World Health Organization, and analyzes their role in the prevention of the development of non-infectious diseases, including number of diseases of the cardiovascular system. A search of relevant literature for the review was conducted in PubMed (www.pubmed.gov) using English keywords (“national dietary guidelines”, “WHO diet”, “FBDG”, “dietary guidelines”, “cardiovascular factors”), the website of the National Project of the Russian Federation “Healthy Nutrition” (https://здоровое-питание.рф), eLIBRARY.RU (https://elibrary.ru/defaultx.asp) using keywords in Russian (“ESSE-RF”, “risk factors for non-infectious diseases”, “nutrition”). Publications published between 1995 and 2022 were considered. The review included the National Recommendations in force for 2023, as well as the results of cohort and multicenter studies, the purpose of which was to assess the impact of current recommendations on the development of traditional cardiovascular risk factors, associated clinical conditions (myocardial infarction, stroke, etc.), as well as the impact on general mortality. Publications with completely duplicate data and conclusions were excluded, and in case of partial overlap, both sources were indicated. As a result, a list was compiled of articles, newsletters, and links with up-to-date information on the National Guidelines.

СРАВНИТЕЛЬНАЯ ОЦЕНКА ДИАГНОСТИЧЕСКИХ КРИТЕРИЕВ МЕТАБОЛИЧЕСКОГО СИНДРОМА У ВЗРОСЛЫХ ПАЦИЕНТОВ С ИЗБЫТОЧНОЙ МАССОЙ ТЕЛА/ОЖИРЕНИЕМ

Background. Diagnostic criteria for metabolic syndrome (MetS), which is a cluster of risk factors for noninfectious diseases, continue to be discussed and refined by various professional communities. The aim of the study was determination of the incidence of MetS in overweight/obese adult cardiac hospital patients according to the IDF (2005), AHA/NHLBI (2004)/AHA (2023) diagnostic criteria and criteria agreed by IDF/AHA/NHLBI and other scientific societies (2009). Material and methods. 222 patients (103 men and 119 women) with overweight/obesity aged 18 years and older were examined using clinical, laboratory, and instrumental research methods. Results. Among adult patients (46.4% men, 53.6% women), 85.6% (95% CI 80.4-89.6) were found to have MetS according to the IDF (2005) criteria, 82.4% (95% CI 76.9-86.9) according to the AHA/NHLBI (2004) and AHA (2023) criteria, and 87.4% (95% CI 82.4-91.1) according to the criteria agreed by IDF/AHA/NHLBI and other scientific communities (2009). Metabolic syndrome established on the basis of the IDF criteria (2005) and the criteria agreed by the IDF/AHA/NHLBI and other scientific communities (2009) was more often four-component, while that based on the AHA/NHLBI (2004) and AHA (2023) criteria was three-component. Arterial hypertension occurred in 100% of patients with MetS. Conclusion. The presence of four and five components of MS, in particular arterial hypertension in all MetS patients included in the study, indicates the severity of cardiometabolic disorders.

Харчування як чинник ризику та втручання при ревматичних захворюваннях

Мета публікації — огляд ефектів харчування як чинника ризику та втручання в аспекті ревматичних захворювань. З використанням методів інформаційного аналізу досліджено понад 100 джерел спеціальної літератури та комп’ютерних ресурсів доказової медицини. У великих обсерваційних дослідженнях, систематичних оглядах і мета­аналізах, присвячених профілактичному, терапевтичному потенціалу та безпеці застосування різних харчових продуктів, дієт, мікронутрієнтів і пробіотиків, у тому числі за наявності пов’язаних провідних чинників ризику неінфекційних захворювань (вживання тютюну, алкоголю, підвищеного індексу маси тіла тощо), встановлено, що значущість харчування як чинника ризику розвитку та кінцевих результатів доведено насамперед для ревматоїдного артриту та подагри. Патогенетичну роль харчування розглянуто в контексті його впливу на кишкову мікробіоту. Показано необхідність проведення рандомізованих контрольованих досліджень для оцінки протективного потенціалу різних моделей харчування й ефектів дотримання модифікованої середземноморської дієти та вживання певних дієтичних добавок і пробіотиків при ревматичних захворюваннях, а в кінцевому результаті — для розробки спеціальних настанов із харчування для хворих ревматологічного профілю. Відзначено достатню доказову базу профілактичного консультування таких пацієнтів з питань харчування з метою зниження кардіоваскулярного ризику.

Education on Disease Risk Factors in Adapting New Habits to Prospective Hajj Pilgrims at KBHI Bisri-Pusri Palembang City

Health istitha'ah is the health ability of the pilgrims physically and mentally with a measurable health assesment. It is essential for every citizen who will perform the hajj pilgrimage. The Indonesian Ministry of Health through the Non-Infectious Disease Integrated Service Post (Posbindu PTM) implements an early detection and prevention program for non-infectious diseases in sub-districts, schools or colleges, institutions or workplaces, and Hajj guidance groups (KBIH) especially for pilgrims. The department of public health and community medicine (IKM-IKK) of the Faculty of Medicine, Sriwijaya University perform an education on the detection of risk factors for non-infectious diseases and physical fitness assesment of prospective pilgrims in the context of community service activities at KBIH Bisri Palembang. Examinations were done on 40 prospective pilgrims using the Rockport test method. The results of the examination showed that the 24 pilgrims had moderate fitness level (60%). The blood pressure test results showed that 9 pilgrims had hypertension (22%) and the nutritional status test showed that 20 pilgrims were overweight and obese (38.5%). In addition, there is an assesment for prospective hajj pilgrims in order to understanding risk factors for non-infectious diseases. The result shows that there is a significant increase in understanding about non-infectious diseases in the adaptation of new habits.
 
 

Prognostic value of risk factors for non-infectious diseases in workers of a poultry enterprise (according to an 8-year prospective study)

<h3>ЦЕЛЬ ИССЛЕДОВАНИЯ</h3> Провести комплексную оценку факторов развития неинфекционных заболеваний (НИЗ) по данным 8-летнего проспективного исследования и на основе полученной информации разработать прогностическую шкалу. <h3>МАТЕРИАЛ И МЕТОДЫ</h3> Проведено двухэтапное исследование: одномоментное сплошное исследование организованной популяции (400 пациентов) с последующим 8-летним проспективным наблюдением когорты практически здоровых (193 респондента). Изучаемые показатели I этапа: социодемографические характеристики; клинические и физикальные показатели; поведенческие факторы; семейный анамнез ранних сердечно-сосудистых заболеваний; профессиональное интервью и степень напряженности труда; психологические характеристики; лабораторные показатели; показатели ремоделирования сердца и сосудов. Изучаемые показатели II этапа: интенсивные и экстенсивные. <h3>РЕЗУЛЬТАТЫ</h3> За период 8-летнего наблюдения число заболевших НИЗ составило 146 (75,6%). За вышеуказанный период выявлены новые случаи заболеваний: один случай относился к классу МКБ-10 «Новообразования», что составило 0,5% от группы исследования; 10 (5,2% от группы исследования) — к классу «Болезни крови...»; 21 (10,8%) — к классу «Болезни эндокринной системы...»; 68 (36,2%) — к классу «Болезни системы кровообращения»; 11 (5,7%) — к классу «Болезни органов дыхания»; 20 (10,4%) — к классу «Болезни органов пищеварения»; 1 (0,5%) — к классу «Болезни кожи и подкожной клетчатки»; 2 (1%) — к классу «Болезни костно-мышечной системы и соединительной ткани»; 12 (6,2%) — к классу «Болезни мочеполовой системы». Значимыми факторами изучаемых исходов были: семейное положение (разведен/разведена), индекс курящего человека (пачка/лет), уровень суммарных фракций динитрофенилгидразонов при длине волны 270 нм, наличие атеросклеротической бляшки брахиоцефальных артерий, скорость кровотока по средней мозговой артерии. <h3>ЗАКЛЮЧЕНИЕ</h3> Факторами риска установленных НИЗ по данным 8-летнего проспективного исследования являются: социальная изоляция, основанная на результатах самооценки; курение; продукты свободнорадикального окисления белков; морфологические, гемодинамические изменения брахиоцефальных артерий и сосудов головного мозга. Разработанная на основе полученной многофакторной модели программа-калькулятор позволяет определять индивидуальный суммарный прогноз риска НИЗ, наметить план лечебно-профилактических мероприятий в конкретной популяции.

DESCRIPTION OF RISK FACTORS OF NON-INFECTIOUS MORBIDITY AMONG POPULATION OF SURGUT CITY

The objective was to study behavioral risk factors of non-infectious diseases, such as physical activity (PA), smoking, food, use of alcoholic beverages in case of a sampled unity of permanent Surgut-dwellers. 2 256 people have been interrogated in an ecological-sociological research by means of telephone interviews. 2 000 people’s samples were processed - 1 000 men and 1 000 women of five age categories: 18 29 years, 30-39 years, 40-49 years, 50-59 and older than 60. Dependence (F = 50.274; p &lt; 0.001) between the indicators of health and PA of the respondents, between PA and presence of children in a family (F = 45.549; p &lt; 0.001) has been revealed. The PA level depends on education (F = 17.130; p &lt; 0.001) and profession (F = 60.273; p = 0.012). Dependences between PA and a marital status (F = 1.128; p = 0.288) have not been revealed. Gender distinctions have not been revealed in PA (F = 2.903; p = 0.313), between PA and heart insufficiency (F = 1.376; p = 0.241), ischemic illness (F = 0.554; p = 0.457), smoking (F = 2.239; p = 0.265), myocardial infarction (F = 4.249; p = 0.047).

Theorizing About Causes at the Individual Level While Estimating Effects at the Population Level

The dominant philosophy of modern epidemiology is individualism, despite the limitations of epidemiologic tools and methods when considering the individual level. We pursue information on increasingly reductionist causes in our search for knowledge of causes of specific cases. Philosophical reasoning and empiric evidence suggest that this search may not be as fruitful as proponents claim. I argue that using individualism to guide our search for causes of disease hinders our effectiveness in contributing to disease prevention, because the positive predictive values of most established genetic and environmental risk factors for noninfectious diseases are too low to be quantitatively convincing to an individual.
