Selection of patients--critical aspects.

Proper selection of patients is the most critical aspect in designing clinical trials in migraine research. We establish specific research diagnostic criteria for the definition of migraine. Additional factors in patient selection, migraine frequency, duration, medications and previous treatments, are reviewed. Exclusion criteria and the mixed headache syndrome patient are discussed.

ABSTRACT For the past several years, Clifford Rose has edited a series of remarkable volumes dealing with a variety of topics all characterized by the astute selection of authors and the high quality of contents. Since the late 1930s and the first demonstration by Graham and Wolff that the ergot drugs were cranial vasoconstrictors, the emphasis in migraine research has been on vasomotion. This is as it should be, and these changes have been well investigated. With the aid of regional cerebral blood-flow measurements using radioactive techniques, the classical theory of preheadache intracranial vasoconstriction and subsequent intracranial and extracranial vasodilation has been proved accurate, though the recent studies of Edmeads 2 and Otis et al 3 suggest that the vascular changes in migraine may be more persistent than heretofore suspected. Migraine is characterized by vasodilation, but vasodilation of itself does not produce painful headache. During the last decade, a series of observations has been reported that suggests that in migraine, vasoactive substances are liberated locally about painful and distended blood vessels. 4 This combination of vasodilation and sterile inflammation is needed if the clinical syndrome

THE PRESENT DAY STATUS OF MIGRAINE THERAPY1

FOR THE PAST five years, we have been carrying on research in migraine on the Neurological Service at Bellevue Hospital, under the direction of Dr. Foster Kennedy. During this time, we have treated three hundred patients. The following is a clinical resume of our therapeutic methods and result. It is our belief that to satisfactorily treat migraine, one must accept the concept of the conglomerative character of the syndrome. We must treat the condition not as a clinical entity with one cause and one specific cure, but as a clinical symptom that is indicative of some underlying pathological process. This underlying pathological process, whatever it may be, must be determined and treated before one can expect to relieve the suffering of migraine. Although it is generally admitted that migraine is a syndrome, this concept is neglected by investigators in their therapeutic discussions. The medical literature contains many reports of successful migraine therapy.
