Bad news: The Substance Abuse and Mental Health Services Administration (SAMHSA) has no information on “who drafted the questions and when,” according to an agency spokesman. He was answering our question about the “sex questions” that must be asked of patients to satisfy Government Performance and Results Act (GPRA) requirements for recipients of SAMHSA grants (see “Sex questions SAMHSA grantees must ask cause hubbub: GPRA,” ADAW, March 28; https://onlinelibrary.wiley.com/doi/10.1002/adaw.33387). These questions include how many times and what kind of sex with how many partners, including questions about oral, anal and vaginal sex and masturbation patients have engaged in. The entire GPRA tool, including its questions, must be used until the current tool expires Oct. 1, the spokesman told us last week.

Factors related to self-rated health: a survey among patients and their general practitioners

Objective To explore associations between general practice patients’ SRH and symptoms, diagnoses, chronic conditions, unexplained conditions, and life stressors. Design A cross-sectional study. Data were collected from GP and patient questionnaires. Setting General practices in Southeast Norway. Subjects 47 general practitioners (GPs) who included 866 consecutive patients. Main outcome measures SRH was measured with a single question from the COOP-WONCA overall health chart and dichotomized into good/poor SRH. Binary logistic regression models were used in the analyses. Results Poor SRH was reported by 48% of the patients in the past week. A higher prevalence of poor SRH was found for women, middle-aged, recipients of social security grants, patients diagnosed with asthenia, lower back pain, and depression/anxiety, and for patients with reported life stressors and unexplained conditions. We found an almost linear association between the number of symptoms and the likelihood of reporting poor SRH. The probability of reporting poor SRH increased along with an increasing number of symptoms for common diagnoses. In a multivariate analysis, the only number of symptoms, being in receipt of social security grants and being retired was associated with poor SRH. Conclusion The likelihood of reporting poor SRH increased with an increasing number of symptoms, partly independent of the diagnosis given by GPs. This result coincides with our previous findings of a strong association between the number of symptoms, function, and health. The symptom burden thus appears to be an important factor for SRH among patients in general practice. KEY POINTS There is a high prevalence of poor SRH in general practice patients. The likelihood of reporting poor SRH is partly independent of the diagnosis given. The number of symptoms was the factor strongest associated with poor SRH.

Development and Assessment of Early Utilization of the Standardized Letter of Recommendation for Use in the Radiology Residency Match

Letters of recommendation are essential for residency applications. Traditionally, narrative letters have been used. Standardized letters of recommendation (SLOR) have been developed and have benefits to the writer and reader. The goal was to develop an informative, meaningful, and efficient SLOR for the radiology residency Match and to assess its early use. An Association of University Radiologists (AUR) Research and Education Venture Fund Grant was awarded for the development of a SLOR for use in the radiology residency Match. Grant recipients developed the letter and modifications were based on feedback from the Alliance of Medical Student Educators in Radiology (AMSER) SLOR task force and a larger task force including additional AUR and Association of Program Directors in Radiology (APDR) members. AUR and APDR members were surveyed in 2020 to assess the radiology SLOR. The radiology SLOR became available for use in September 2018. It highlights the top six traits or abilities deemed most valued in a resident candidate and guides the writer to develop a concrete narrative. Top perceived benefits of the Radiology SLOR are ease of reading and interpretation. Top perceived drawbacks are an impersonal format and concerns regarding program directors' perceptions of the new SLOR. SLOR utilization increased in the second year of availability. The radiology SLOR was developed and first used in 2018. Its use increased over two years and will likely continue to increase given the perceived benefits and increasing awareness of the SLOR.

Influence of Society for Academic Emergency Medicine Foundation's Research Training Grant on postaward academic federal funding.

The objective was to measure the impact of the Society for Academic Emergency Medicine Foundation's (SAEMF) Research Training Grant (RTG) by comparing academic success in grant recipients versus non-recipient applicants. Our primary outcome was subsequent federal funding as a principal investigator (PI) or multiple principal investigator (MPI). Our secondary outcomes included subsequent K-award funding, R-series funding, R01 funding, and academic productivity measured by first author peer-reviewed publications. The authors examined all SAEMF RTG applicants from 2002 through 2019 (n=109). Data were collected using the National Institutes of Health RePORTER database, a literature search using PubMed, and an online survey sent to all RTG applicants. Relative risks (RRs) with 95% confidence intervals (95% CI) were calculated. Over 18years, 18 of 109 (16.5%) RTG applicants were awarded by SAEMF. Subsequent federal funding as PI or MPI was obtained by 11 of the 18 RTG recipients compared to 29 of the 91 nonrecipients (61% vs. 33%, RR=1.9; 95% CI=1.2-3.1). The RTG award was also associated with increased probability of receiving a federal Career Development Award (K-series) (RR 2.0; 95% CI 1.1-3.9) and R-series award (RR 2.0; 95% CI 1.1-3.9) but not an R01 award (RR 2.1; 95% CI 0.8-5.3). The median number of first-authored peer reviewed manuscripts did not differ between RTG award recipients (14, IQR 8,44) and nonrecipients (14, IQR 6,30) (p=0.5) though RTG recipients had a higher percentage of their publications as a first author (49% vs. 33%, p=0.04). SAEMF RTG awards were associated with increased probability of future federal funding, including career development awards and R-series awards but not R01 awards. RTG recipients also had a higher percentage of their peer reviewed publications as first author.

Accountability and Collaboration for Intergovernmental Performance Management: Communicating Goals through the HUD Grantmaking Process

This paper presents an exploratory case study of a performance-based intergovernmental grant program, U.S. Department of Housing and Urban Development's (HUD) homeless assistance grants to local Continuums of Care (CoC). The goal is to illuminate the mechanisms by which a federal agency may communicate its priority goals to local grant recipients and seek to promote local actions that further those goals. The mixed-methods case study finds that meeting HUD performance targets does not lead to larger awards in the competitive grant process, casting doubt on the strength of incentives provided by the competitive grants. Nevertheless, the grant making process plays an important role in communicating federal priorities. The system performance metrics emphasized in the grant process coupled with targeted technical assistance appear to promote a focus on performance at the local level and play a role in system reform and capacity building.

Ethnic and Sex Diversity in Academic Orthopaedic Surgery: A Cross-sectional Study.

Introduction:Although the diversity in orthopaedic residency programs has been studied, the diversity within academic orthopaedics has not.Methods:The board of specialty societies, five leading journals and the National Institutes of Health RePORTER tool, and three accreditation organizations were explored.Results:The board of directors comprised 220 (72%) Caucasians, 36 (12%) Asians, 4 (1%) Hispanic/Latinos, 29 (9%) African Americans, and 18 (6%) Other individuals; 250 (81%) were men, and 57 (19%) were women. The editorial boards comprised 288 (77%) Caucasians, 62 (16%) Asians, 14 (4%) Hispanic/Latinos, 8 (2%) African Americans, and 4 (1%) Other individuals; 341 (91%) were men, and 35 (9%) were women. The National Institutes of Health grant recipients comprised 117 (64%) Caucasians, 58 (32%) Asians, 4 (2%) Hispanic/Latinos, and 3 (2%) African Americans; 128 (70%) were men, and 54 (30%) were women. On average, Caucasians, Asians, Hispanic/Latinos, and African Americans received $776,543, $439,600, $420,182, and $494,049, respectively. On average, men and women received $759,426 and $419,518, respectively. The accreditation boards comprised 45 (82%) Caucasians, 6 (11%) Asians, and 4 (7%) African Americans; 45 (82%) were men, and 10 (18%) were women.Conclusions:Academic orthopaedic surgery does not resemble the United States. Residency, fellowship committees, and professional organization boards need to become aware of these disparities.

Evaluation of a systems-level technical assistance program to support youth with complex behavioral health needs

The National Technical Assistance Network for Children’s Behavioral Health (TA Network) supports the development and implementation of Systems of Care (SOC) for youth with serious emotional disorders and their families in states, tribes, territories, and communities throughout the United States. The purpose of the current research was to conduct an evaluation of the TA Network to assess: The degree to which it has deployed research-based elements of TA; levels of participant satisfaction; types and scope of TA services provided; and systems-level outcomes. Study participants were drawn from a stratified random sample of SOC grant recipients who received technical support from the TA Network between 2013 and 2017. Results suggest that the TA Network has encompassed research-based elements of effective TA. Participants rated their interactions with the network very highly, and they accessed a wide variety of resources from the network. Finally, participants reported a variety of systems-level outcomes associated with TA Network support. Together, these findings underscore the importance of structuring TA systems to tailor support to fit with recipients’ needs, build positive, proactive relationships, and offer services of sufficient dosage. Given the lack of rigorous evaluations on provision of TA, future studies can confirm the degree to which such tailored approaches to TA result in increased satisfaction, more effective implementation of SOC principles, and ultimately improved outcomes for youth and families.

‘I know what I should be feeding my child’: foodways of primary caregivers of Child Support Grant recipients in South Africa

ABSTRACT Background Despite South Africa being an upper middle-income country producing enough food to sustain its population, and having an advanced social welfare system, it has high levels of food insecurity at the household-level. Food insecurity is linked to malnutrition and undernutrition in children. This manuscript addresses gaps in knowledge about food choices and practices of primary caregivers of children in receipt of South Africa’s largest cash transfer programme, the Child Support Grant (CSG). Objective The main objective of the study was to explore CSG caregivers’ foodways and the choices they made about what food to buy, where to buy it and for what reasons, in Langa in the Western Cape and Mt Frere in the Eastern Cape. Methods We conducted a total of 40 in-depth interviews and 5 focus group discussions with primary caregivers of Child Support Grant recipients younger than 5 years in the Eastern and Western Cape provinces. Results Caregivers’ food choices were less influenced by cultural practices and personal preferences, than by financial and physical constraints in terms of what and where to access food. Constraints in food choices were chiefly a consequence of the small amount of the grant, as well as a food environment that only availed foods of a certain quality and type in these low-income communities Conclusions The foodways of recipients of social assistance can only be better aligned with nutrition messaging and policy if there are changes in the monetary value of cash transfers, and the food environments of low-income households which determine access to, availability and affordability of nutritious food. Local informal food enterprises play an important role in the food system of CSG recipients and need to be considered in any strategies that seek to reform the food system of low-income communities in South Africa and similar settings

A Ten-Year Analysis of Recent National Institutes of Health Funding for Anesthesiology Research in United States Medical Schools

The characteristics of recent National Institutes of Health (NIH) grant funding to anesthesiology researchers in United States (US) medical schools have not been systematically quantified. NIH funding to cardiac anesthesiologists has also not been estimated. The author conducted an internet-based analysis of NIH awards to anesthesiology researchers from 2011-2020 to identify the types, duration of funding, and amount of grants, and the terminal degree(s), faculty rank, gender, board certification status, and type of appointment of the grant recipients including those with an interest in cardiac anesthesiology. Observational study. Internet analysis. NIH grants recipients. None. NIH grant recipients affiliated with anesthesiology departments were identified from the Blue Ridge Institute for Medical Research website. The number of grants, years of support, and total amount of funding were quantified for research project grants (R series), mentored career development awards (K series), and other grants (U and P series) using NIH Research Portfolio Online Reporting Tools. The terminal degree(s), faculty rank, gender, and type of appointment of grant recipients were identified using department web pages. American Board of Anesthesiology (ABA) certification, National Board of Echocardiography Advanced Perioperative Transesophageal Echocardiography (TEE) certification, and previous or current Foundation for Anesthesia Education and Research (FAER) awards to NIH grant recipients were obtained from each organization's website. A total of 532 researchers received 1250 grants with 3844 cumulative years of funding amounting to $1,676,482,440. R series grants accounted for three-quarters of all funding. PhDs were awarded more than one-half of NIH grants. MDs had lower median numbers of projects, R01 grants, and total R series grants than their colleagues with PhD or MD PhD degrees, but MDs received more K awards. One hundred ninety-eight MD and MD PhD NIH grant recipients were ABA diplomates. These physician-scientists received 26.0% and 53.1% of R and K series grants, respectively. Thirty physician-scientists also held TEE certification; these individuals with an interest in cardiac anesthesiology were awarded 4.8% of all NIH grants. Full Professors were awarded more than three-quarters of R grants and amassed more than $1.3 billion in funding, whereas assistant and associate professors received the majority of K series grants. Male investigators received greater median R grants but fewer median K awards than female researchers. One hundred-fifteen previous or current holders of FAER grants were identified; these individuals earned a total of 240 NIH awards totaling $357.7 million. PhDs, Professors, and male researchers receive the majority of R01 and other R series grants to anesthesiology departments at US medical schools. Physician-scientists, including those interested in cardiac anesthesiology, are awarded a minority of R series grants. FAER continues to provide an important stimulus for subsequent NIH funding of physician-scientists in anesthesiology.

Female Representation Among CORE Grant Recipients by Research Subspeciality.

This study evaluates the gender distribution of Centralized Otolaryngology Research Efforts (CORE) grants by research subspeciality over the past decade. Demographic information on CORE grant recipients from 2010 to 2019 was extracted through an Internet search, and research projects were categorized by subspeciality. Of 343 grants awarded during this period, 127 (37%) were awarded to women. Analysis demonstrated lower absolute rates of CORE funding for women in all fields except pediatrics, but women receive a proportional number of grants for most subspecialities based on their representation within the field.

Contraception information on the websites of student health centers in the United States

ObjectiveAs University Student Health Centers are considered reputable sources of information by many young adults, we evaluate the presence of contraceptive information on their websites. Study DesignWe used a software tool (Quantitative Measures of Online Health Information), designed for public health research to examine online information access on four broad categories of contraception and reproductive health (LARC/injectables, Contraception, Condom, Pap test) on student health center websites from all (591) public four-year universities across the United States between July to September 2020. Using a logistic regression model, we documented factors that are associated with information disparities. ResultsOur sample consisted of 545 public universities after excluding those for which information was unavailable. In 357 (66%) of the universities in our sample, we found evidence of some information related to contraception. A one percentage point increase in the student population that are Pell grant recipients, an indicator of the proportion of low-income students enrolled, is associated with a 3% to 6% (0.01<p<0.05) decrease in the odds of the presence of contraception information. A one percentage point increase in the student population that is female is associated with a 3% to 6% (0.01<p<0.1) increase in the odds of the presence of contraception information. Universities that offer medical degrees have an 82% (p<0.1) higher odds of providing information for LARC/injectables on their websites. Universities that have an institutional size of more than 10,000 students are almost four times (p<0.01) as likely to have contraception information on their websites relative to those with a smaller institutional size. ConclusionsInformation on contraception on University student health center websites is associated with on a host of factors related to student demographics, institutional characteristics, and university location. ImplicationsExamining student health center websites for contraception information can reveal important systemic gaps, especially at universities that serve nontraditional or vulnerable groups. An ongoing review is necessary to ensure equal access for all college students.

Price of Attending Universities with AMTA-Approved Undergraduate Music Therapy Degree Programs

The purpose of this study was to analyze tuition, student loan, and Pell Grant data from universities with AMTA-approved undergraduate music therapy degree programs. Data from the College Insight Tool was used to collect data on average tuition and fees, average loan debt of graduates, percentage of graduates with debt, and percentage of students who received a Pell Grant from four-year institutions offering an undergraduate music therapy degree during the 2016-2017 academic year. Results indicated that tuition and fees were slightly higher at public universities and slightly lower at private institutions than national averages. The data also suggested that students who attended schools with music therapy programs were more likely to have student loan debt. Percentages of Pell Grant recipients were similar to national averages, indicating similar representation of low- to middle-income students. Regional data suggested that the Southeast region was the least expensive area of the country for tuition and fees, and among the lowest for student loan debt. The inverse was found for schools in the Great Lakes region where student loan debt was among the highest. More research is needed to better understand the implications of student loan debt on the education, long-term financial security, and career choice for professional music therapists.

International Transfer Credit: Indonesia International MBKM Curriculum Based on Authenticity

Credit Transfer International is a prestigious program that is the hope of all Indonesian students so far. This activity provides an opportunity for Indonesian students to study on campuses abroad for one semester as grant recipients from the Ministry of Education, Culture, Research and Technology. Although a student successfully goes through the selection process and officially becomes a student on an international campus, it does not mean all problems have been resolved. Various problems often arise both regarding learning efforts, curriculum and intercultural surprises that often arise. This research is exploratory, based on the experience in lecturers while guiding students of Credit Transfer participants and participants of activities for two years in the period 2020 and 2021. The results of this study found elements forming students' competing abilities in the international learning process that are strongly influenced by local culture before the activities of their respective campuses. The local culture in question is something that needs to be developed under the concept of local learning that has been taught by the father of Education Ki Hajar Dewantara. The development of bloom's modern pedagogy theory has been shown to shape the Competency-Based Curriculum in students. This research proves that collaboration with local pedagogy a la Ki Hajar Dewantara has created a sustainable International MBKM Curriculum through strengthening IKU 2 (Students Get Off-Campus Experience) and IKU 6 (Study Program in collaboration with world-class Partners).

Nature, costs and benefits of clinical travelling fellowships

INTRODUCTION Many trainee doctors and consultants visit clinical units abroad for a period of specialised training. There are a number of grants available from professional and other bodies that provide a variable degree of financial assistance for these doctors but there is little information about the nature, costs and benefits of these training opportunities. METHODS This 11-year analysis of 385 applications to the Hospital Corporation of America International Foundation for financial support to train abroad was coupled with a detailed questionnaire to 127 UK doctors who received an award following an interview process. RESULTS There were an average of 11 annual awards, with a mean value of £5,600 (range: £1,000-£15,000). Trainees (predominantly ST7 and ST8 level) were the main applicants (60%) and award winners (71%). The applications were for variable time periods (from 1 month to over 24 months) and to clinical units throughout the world, the favoured locations being America, Canada and Australia. The surgical specialties were the most sought after for training (77%). There were 4.7 times more male than female applicants. CONCLUSIONS This paper discusses the benefits of travelling fellowships as recorded by grant recipients, three-quarters of whom were applying their overseas clinical experience back in the National Health Service. However, the overall costs of travel frequently exceed doctors' expectations and the need for extra financial support for overseas fellowships is clear.

Grant Expenditures must be managed in an orderly manner, obeying laws and regulations, efficient, economic, effective, transparent and accountable by taking into account the principles of justice, appropriateness, rationality and benefits for the community. This is intended to create administrative order, accountability, transparency in the management of grant funds and accuracy in the use of aid funds by grant recipients. The formulation of the problem discussed in this study is how to manage grants and social assistance for the Bekasi City Govemment. The research method used is descriptive qualitative research, which is the process of organizing and sorting data into patterns and categories as well as basic description units so that themes can be put forward as suggested by the data. The results of the research are that the management of grants and social assistance funds in Bekasi City was not properly implemented in 2018. It was explained that this study did not use population and samples but key informants and respondents. Researcher determines the Regional Work Unit (SKPD) for Grants and Social Assistance as Key Informants which in this study are represented by the Manager of Grants and Social Assistance in each SKPD. Meanwhile, the respondents were several people who were taken from the Advisory Team for grants and social assistance. Research results, researcherconcluded that the management of grants and social assistance at the Bekasi City Regional Financial and Asset Management Agency in 2018 had not been carried out properly. Some of the obstacles to the management of grant funds and social assistance in Bekasi City are the lack of available personnel, staff who have not received training regarding grant aid management, incomplete rules and procedures for managing grant funds, and overlapping allocations of funds in grant expenditure.
