The value of quantitative and a new qualitative color pattern shear wave elastography for the differentiation of ACR TI-RADS 4 or 5 category thyroid nodules measuring ≤10 mm.

This study aims to evaluate the diagnostic performance of quantitative shear wave elastography (SWE) and a new qualitative color pattern SWE for the differentiation of benign and malignant American College of Radiology Thyroid Imaging, Reporting, and Data System (ACR TI-RADS) 4 or 5 category thyroid nodules measuring ≤10 mm. From May 2020 to July 2022, a total of 237 patients with 270 thyroid nodules were enrolled, and conventional ultrasound and SWE examinations were performed for each patient. Each ACR TI-RADS 4 or 5 category thyroid nodule measuring ≤10 mm was evaluated by quantitative SWE and a new qualitative color pattern SWE. The diagnostic performance of quantitative SWE parameters, the new qualitative color pattern SWE, and the combination of SWE with ACR TI-RADS, respectively, for the differentiation of benign and malignant ACR TI-RADS 4 or 5 category thyroid nodules measuring ≤10 mm was evaluated and compared. Among 270 thyroid nodules in 237 patients, 72 (26.67%) thyroid nodules were benign and 198 (73.33%) thyroid nodules were malignant. The qualitative color pattern SWE showed better diagnostic performance than the quantitative SWE parameters. When combining the qualitative color pattern SWE with ACR TI-RADS scores, with the optimal cutoff value of the total points ≥8, the thyroid nodules were considered malignant. The sensitivity, specificity, accuracy, and AUC were 89.90%, 56.94%, 81.11%, and 0.820 (95% CI: 0.768-0.864), respectively. Compared with using qualitative color pattern SWE alone, the combination of qualitative color pattern SWE and ACR TI-RADS had better diagnostic performance, which was significantly different (p < 0.05). The combination of qualitative SWE color patterns and ACR TI-RADS had high sensitivity and accuracy, which might be a convenient and useful method to differentiate benign and malignant ACR TI-RADS 4 or 5 category thyroid nodules measuring ≤10 mm. It would be helpful for the management of thyroid nodules and improving prognosis.

The value of microvascular Doppler ultrasound technique, qualitative or quantitative shear-wave elastography of breast lesions for predicting axillary nodal burden in patients with breast cancer.

The status of the axillary lymph node (ALN) in patients with breast cancer can critically inform clinical decision-making and prognosis. Preoperative evaluation of limited nodal burden (0-2 metastatic ALNs) and high nodal burden (≥3 metastatic ALNs) is vital for individual treatment in patients with breast cancer. Thus, this study aimed to evaluate the value of Angio-PLUS (AP; Aixplorer, SuperSonic Imagine) and the qualitative and quantitative shear-wave elastography (SWE) of breast lesions to predict limited or high axillary nodal burden and to develop a model for predicting limited or high axillary nodal burden. From March 2020 to November 2022, a total of 232 consecutive patients with breast cancer comprising 232 breast lesions were enrolled retrospectively from Yueyang Central Hospital. The sensitivity, specificity, negative predictive value (NPV), positive predictive value (PPV), accuracy, and area under the receiver operating characteristic curve (AUC) of AP, qualitative SWE, quantitative SWE, and the predictive model for evaluating limited or high axillary nodal burden were compared. There was no significant difference in AP patterns between the limited nodal burden group and high nodal burden group. The best cutoff values of Emin (the minimal value of the first Q-box), Emean (the mean value of the first Q-box), Emax (the maximum value of the first Q-box), Eratio (ratio of the first Q-Box and the second Q-Box) and standard deviation for predicting limited or high nodal burden were 80.85 KPa, 133.45 KPa, 153.40 KPa, 9.95, and 19.25 KPa, respectively. The Emax had the highest AUC, and its sensitivity, specificity, PPV, NPV, accuracy, and AUC were 71.64%, 56.36%, 40.00%, 83.04%, 60.78%, and 0.640 [95% confidence interval (CI): 0.575-0.702], respectively. The sensitivity, specificity, PPV, NPV, accuracy, and AUC of seven color patterns for qualitative SWE were 71.64%, 74.55%, 53.33%, 86.62%, 73.71%, and 0.731 (95% CI: 0.669-0.787), respectively, which was significantly higher than all the other quantitative SWE parameters. ALN evaluation in ultrasound and qualitative SWE were independent risk factors for predicting limited or high nodal burden according to a binary logistics regression analysis. The AUC of the predictive model based on independent risk factors was 0.820 (95% CI: 0.765-0.867), which was significantly higher than that of the other independent risk factors. The seven color patterns in the qualitative SWE of breast lesions were valuable for predicting limited or high nodal burden for patients with breast cancer. Compared with quantitative SWE, qualitative SWE exhibited a better diagnostic performance. Breast lesions present no findings, vertical stripes, and spot patterns were important indicators for limited nodal burden. The predictive model developed in this study could be a simple, noninvasive, and convenient method for predicting limited or high nodal burden, which would be beneficial for clinical decision-making and individual treatment to improve prognosis.

Utility of shear wave elastography for diagnosing carpal tunnel syndrome with psoriatic arthritis.

Carpal tunnel syndrome (CTS) is a type of peripheral entrapment neuropathy and common for the patients with psoriatic arthritis (PsA). Shear wave elastography (SWE) is a new ultrasonography technique that can be used for diagnosing CTS, but not studied in PsA patients. The aim of this study to measure the stiffness of median nerve and hand muscles by quantitative SWE to identify whether SWE can be used for diagnosing CTS in patients with PsA or not. To diagnose CTS, all patients had electrodiagnostic study. The stiffness values of the median nerve, abductor pollicis brevis, and abductor digiti minimi were determined using SWE. Muscle stiffness ratio was also calculated. Consideration is given to 48 patients with PsA (93 wrists) and 29 healthy volunteers (57 wrists). Median nerve stiffness was found to be significantly higher, and abductor pollicis brevis' stiffness and muscle stiffness ratio were significantly lower in PsA patients' wrists compared to control group (p = 0.002, p < 0.001, p = 0.001, respectively) and in CTS wrists compared to others (p < 0.001, p < 0.001, p = 0.001, respectively). Receiver operating characteristic analysis identified 28.2 kPA as the median nerve stiffness cut-off point for differentiating CTS in PsA patients (p = 0.001). We found that SWE has a good diagnostic value for CTS with PsA patients; hence, we can conclude that SWE could diagnose CTS in PsA patients.

Convolutional neural network based on automatic segmentation of peritumoral shear-wave elastography images for predicting breast cancer.

Our aim was to develop dual-modal CNN models based on combining conventional ultrasound (US) images and shear-wave elastography (SWE) of peritumoral region to improve prediction of breast cancer. We retrospectively collected US images and SWE data of 1271 ACR- BIRADS 4 breast lesions from 1116 female patients (mean age ± standard deviation, 45.40 ± 9.65 years). The lesions were divided into three subgroups based on the maximum diameter (MD): ≤15 mm; >15mm and ≤25 mm; >25mm. We recorded lesion stiffness (SWV1) and 5-point average stiffness of the peritumoral tissue (SWV5). The CNN models were built based on the segmentation of different widths of peritumoral tissue (0.5mm, 1.0mm, 1.5mm, 2.0mm) and internal SWE image of the lesions. All single-parameter CNN models, dual-modal CNN models, and quantitative SWE parameters in the training cohort (971 lesions) and the validation cohort (300 lesions) were assessed by receiver operating characteristic (ROC) curve. The US + 1.0mm SWE model achieved the highest area under the ROC curve (AUC) in the subgroup of lesions with MD ≤15 mm in both the training (0.94) and the validation cohorts (0.91). In the subgroups with MD between15 and 25mm and above 25mm, the US + 2.0mm SWE model achieved the highest AUCs in both the training cohort (0.96 and 0.95, respectively) and the validation cohort (0.93 and 0.91, respectively). The dual-modal CNN models based on the combination of US and peritumoral region SWE images allow accurate prediction of breast cancer.

Contrast-enhanced ultrasound and shear wave elastography in the diagnosis of ACR TI-RADS 4 and 5 category thyroid nodules coexisting with Hashimoto's thyroiditis.

This study aims to evaluate the value of contrast-enhanced ultrasound (CEUS), shear wave elastography (SWE), and their combined use in the differentiation of American College of Radiology (ACR) thyroid imaging reporting and data system (TI-RADS) 4 and 5 category thyroid nodules coexisting with Hashimoto's thyroiditis (HT). A total of 133 pathologically confirmed ACR TI-RADS 4 and 5 category nodules coexisting with HT in 113 patients were included; CEUS and SWE were performed for all nodules. The sensitivity, specificity, negative predictive value (NPV), positive predictive value (PPV), accuracy, and the area under the receiver operating characteristic curve (AUC) of the SWE, CEUS, and the combined use of both for the differentiation of benign and malignant nodules were compared, respectively. Using CEUS alone, the sensitivity, specificity, PPV, NPV, and accuracy were 89.2%, 66.0%, 81.3%, 78.6%, and 80.5%, respectively. Using SWE alone, Emax was superior to Emin, Emean, and Eratio for the differentiation of benign and malignant nodules with the best cutoff Emax >46.8 kPa, which had sensitivity of 65.1%, specificity of 90.0%, PPV of 91.5%, NPV of 60.8%, and accuracy of 74.4%, respectively. Compared with the diagnostic performance of qualitative CEUS or/and quantitative SWE, the combination of CEUS and SWE had the best sensitivity, accuracy, and AUC; the sensitivity, specificity, PPV, NPV, accuracy, and AUC were 94.0%, 66.0%, 82.1%, 86.8%, 83.5%, and 0.80 (95% confidence interval: 0.713, 0.886), respectively. In conclusion, CEUS and SWE were useful for the differentiation of benign and malignant ACR TI-RADS 4 and 5 category thyroid nodules coexisting with HT. The combination of CEUS and SWE could improve the sensitivity and accuracy compared with using CEUS or SWE alone. It could be a non-invasive, reliable, and useful method to differentiate benign from malignant ACR TI-RADS 4 and 5 category thyroid nodules coexisting with HT.

Viscoelasticity assessment for in vivo quantification of muscle contusion injury in rats using shear wave elastography.

The aim of the study described here was to investigate the role of viscoelasticity in assessing muscle fibrosis and inflammation in a rat model of contusion using quantitative shear wave elastography (SWE). Unilateral gastrocnemius muscle contusion was induced in 32 male rats using an impactor apparatus. The contralateral muscles served as the control group. SWE was applied to the control group and rats 1, 3, 14 and 21 d after successful modeling (each time point group, n=8). Histologic features were used as reference standards. The degree of fibrosis was moderately correlated with shear wave speed (r=0.53), whereas the degree of inflammation was well correlated with shear wave dispersion (SWD) slope (r=0.74). The area under the receiver operating characteristic curve (AUC) for the dispersion slope for muscle inflammation and fibrosis assessment was 0.87 (95% confidence interval: 0.705-0.963), which exceeded that of the shear wave speed (0.68, 95% confidence interval: 0.494-0.834). The larger decline in dispersion slope in the fibrotic stage than in the inflammation stage (1-d group vs. 14-d group or 21-d group, p &lt; 0.05) indicated better predictive performance than the shear wave speed.

Convolutional Neural Network for Predicting Thyroid Cancer Based on Ultrasound Elastography Image of Perinodular Region.

We aimed to develop deep learning models based on perinodular regions' shear-wave elastography (SWE) images and ultrasound (US) images of thyroid nodules (TNs) and determine their performances in predicting thyroid cancer. A total of 1747 American College of Radiology Thyroid Imaging Reporting & Data System 4 (TR4) thyroid nodules (TNs) in 1582 patients were included in this retrospective study. US images, SWE images, and 2 quantitative SWE parameters (maximum elasticity of TNs; 5-point average maximum elasticity of TNs) were obtained. Based on US and SWE images of TNs and perinodular tissue, respectively, 7 single-image convolutional neural networks (CNN) models [US, internal SWE, 0.5 mm SWE, 1.0 mm SWE, 1.5 mm SWE, 2.0 mm SWE of perinodular tissue, and whole SWE region of interest (ROI) image] and another 6 fusional-image CNN models (US + internal SWE, US + 0.5 mm SWE, US + 1.0 mm SWE, US + 1.5 mm SWE, US + 2.0 mm SWE, US + ROI SWE) were established using RestNet18. All of the CNN models and quantitative SWE parameters were built on a training cohort (1247 TNs) and evaluated on a validation cohort (500 TNs). In predicting thyroid cancer, US + 2.0 mm SWE image CNN model obtained the highest area under the curve in 10 mm < TNs ≤ 20 mm (0.95 for training; 0.92 for validation) and TNs > 20 mm (0.95 for training; 0.92 for validation), while US + 1.0 mm SWE image CNN model obtained the highest area under the curve in TNs ≤ 10 mm (0.95 for training; 0.92 for validation). The CNN models based on the fusion of SWE segmentation images and US images improve the radiological diagnostic accuracy of thyroid cancer.

Effectiveness of Quantitative Shear Wave Elastography for the Prediction of Axillary Lymph Node Metastasis

Objective Invasive breast cancer can be metastasized through axillary lymph nodes (LNs). This study was to evaluate the effectiveness of quantitative shear wave elastography (SWE) to predict axillary LN metastasis, which also provides prognostic implication of SWE as a histopathologic element of invasive breast cancer. Methods 72 prospectively enrolled patients received B-mode ultrasound (BUS) and SWE, and the elasticity index (EI) of SWE at the stiffest part of lymph nodes (LNs) was measured. EI of SWE was closely associated with pathologic results and the histopathologic elements. The receiver operating characteristics (ROC) curve was drawn to evaluate the optimal cut-off value for the assessment of disease severity. Results A significantly longer short-axis diameter and a larger maximal cortex were observed in malignant LNs than that in healthy LNs. The absence of the hilum was associated with metastatic LNs. The EI of SWE varied markedly between the benign and malignant LNs. The combination of Emax and BUS showed higher area under the curve (AUC) than BUS alone to predict metastatic LNs (0.7762 vs. 0.7230). EI of SWE in malignant lymph nodes those with extranodal extension are higher than those without extranodal extension. Conclusions Quantitative SWE provides a viable alternative for the assessment of axillary LN and shows great potential to predict pathological prognostic elements of metastatic axillary LNs in invasive breast cancer. Joint use of SWE and BUS allows examination of the predictive outcome of BUS for axillary lymph node metastasis in invasive breast cancer.

The usefulness of quantitatively assessing temporomandibular joint disk stiffness with shear wave elastography in adolescents with bruxism

The objectives of this study were to quantitatively measure temporomandibular joint (TMJ) disk stiffness in adolescents with bruxism using shear wave elastography (SWE) and to examine the relationship between elastography values, patient age, and duration of bruxism. This prospective study evaluated 120 TMJ disks of 60 adolescents (30 patients with bruxism and 30 controls). The stiffness of the anterior, intermediate, and posterior parts of the disk was measured. The patient and control groups' respective quantitative SWE values of elasticity (kilopascals [kPa]) and velocity (meters/second [m/s]) were compared. The elasticity and velocity values of the anterior and intermediate parts were significantly higher in the patients than in the controls (P ≤ .013), with no significant difference in the size of the joint space (P=.886). A receiver operating characteristic analysis resulted in sensitivity for the anterior part of 0.80 for kPa and 0.83 for m/s, with specificity of 0.57 (kPa) and 0.60 (m/s). For the intermediate part, the sensitivity was 0.80 for kPa and 0.86 for m/s, with specificity of 0.64 (kPa) and 0.57 (m/s). No correlations were found between the SWE values and patient age (P ≥ .098) or duration of bruxism (P ≥ .134). SWE may be useful in the evaluation of TMJ disk stiffness in patients with bruxism.

Quantitative shear wave elastography assessment of tibial nerve in diagnosis of diabetic peripheral neuropathy

BackgroundDiabetic peripheral neuropathy (DPN) is one of the most common complications of diabetes mellitus. Diagnosis of DPN is very important in the prognosis of disease and treatment as early treatment of DPN decreases both short-term and long-term morbidities. SWE elastography is a noninvasive and reproducible method for the precise evaluation of nerve stiffness.ResultsTibial nerve stiffness is notably high at SWE in diabetic patients with DPN (mean shear wave elastography value of RT tibial SWE 75.3 ± 15.1 kPa) compared to patients without DPN (mean shear wave elastography value of RT tibial SWE 37.8 ± 11.6 kPa) and nerve stiffness in healthy control subjects (mean shear wave elastography value of RT tibial SWE 24.9 ± 6.3 kPa). There is a significant increase in the cross-sectional area (CSA) among diabetic patients with DPN (mean cross-sectional area of the right tibial nerve of 17 ± 1.9 mm2) and without DPN (mean cross-sectional area of the right tibial nerve of 14.5 ± 3.8 mm2) in comparison with control subjects (mean cross-sectional area of the right tibial nerve of 13.2 ± 3.1 mm2) in the right side. Borderline significance of the CSA parameters of the tibial nerve study on the left side in different groups. The cutoff point to determine DPN among diabetic patients in the right lower limb is more than 63.8 kPa. With 89% sensitivity and 100% specificity in the detection of DPN on the right side, the SWE has 100% PPV and 95.5% NPV in the detection of DPN on the right side.ConclusionSWE is an effective assistant method in the diagnosis of DPN and is useful when a suspected neuropathy is not detectable by electrophysiology.

Quantitative shear wave elastography compared to standard ultrasound (qualitative B-mode grayscale sonography and quantitative power Doppler) for evaluation of achillotendinopathy in treatment-naïve individuals: A cross-sectional study.

Achillotendinopathy is reported as an overuse disorder and/or degeneration change of the tendon. The diagnosis of tendinopathy is not always easy through imaging modalities. The B-mode grayscale sonography and power Doppler are well-established methods aimed at visualising tendon structure, but have limited sensitivity and lack conventional sonographic characteristics in symptomatic patients. Shear wave ultrasound elastography quantitatively assesses tissue stiffness. To compare the diagnostic accuracy of shear wave ultrasound elastography to that of standard ultrasound (combined B-mode grayscale sonography and power Doppler) for diagnosis of achillotendinopathy, considering clinical symptoms as the reference standard. Standard questionnaires regarding medical history and the Chinese version of a Victorian Institute of Sports Assessment - Achilles Questionnaires (VISA-AC) score were evaluated for a total of 14 treatment-naïve patients with complaints of localized swelling, and reduced force and/or flexibility of the Achilles tendon(s). The irregular thickening around the Achilles tendon, heterogeneity of echotexture of the tendon and calcification of the calcaneal attachment were considered incidences of tendinopathy in B-mode grayscale sonography. Tendinopathies were considered if tendons were >0.60 cm thick in power Doppler. Shear wave elasticity <350 kpa (10 m/s) was considered tendinopathy. A total of 28 conditions of both sides were evaluated through standard ultrasound examinations. Eighty-four tendons were assessed using shear wave ultrasound elastography. Asymptomatic tendons were visible as red and symptomatic (VISA-AC score <80) tendons were visible as blue or turquoise in the images. The rigidity of symptomatic tendons was lower than that of asymptomatic tendons (p < 0.0001). Sensitivity and accuracy for standard ultrasound were increased by the addition of shear wave ultrasound elastography for both symptomatic and asymptomatic tendons. The VISA-AC score was strongly correlated with the elasticity values (p = 0.000, Kendall's tau-beta (τβ) = 0.71) of Achilles tendons. Shear wave ultrasound elastography augments diagnostic confidence of standard ultrasound for the treatment of tendinopathies of Achilles tendons.

Prediction of high nodal burden in invasive breast cancer by quantitative shear wave elastography.

Axillary imaging has been earmarked to forecast high nodal burden [≥3 metastatic axillary lymph nodes (ALN)] instead of lymph node metastasis since the Z0011 trial period. We aimed to ascertain the possibility of utilising quantitative shear wave elastography (SWE) to forecast high nodal burden in invasive breast cancer (IBC). In our hospital, 324 patients with clinical T1-T2N0 IBC who underwent surgery from June 2020 to October 2020 were analyzed retrospectively. A total of 273 patients (84.3%) were categorized as having a limited nodal burden, while 51 patients (15.7%) had a high nodal burden. The two groups were compared in terms of clinicopathological traits, ultrasonic features, and SWE values. The diagnostic performance for prediction of high nodal burden with the optimal cutoff values was drawn by SWE value. The optimal cutoff values for forecasting high nodal burden were as demonstrated: 119.52 kPa for tumor Emax, 97.31 kPa for tumor Emean, 19.38 for tumor Esd, 26.22 kPa for ALN Emax, 19.79 kPa for ALN Emean, 2.32 for ALN Eratio, 3.34 for ALN Esd. Combined with the ratings of sensitivity and specificity, ALN Emax could be chosen as the optimal index if the best diagnostic achievement was contemplated (AUC: 0.856; 95% CI: 0.802-0.909). An Emax cutoff 26.22 kPa of ALN, 72% of women with a high nodal burden of axillary disease would be detected, but if used for clinical decision making, 13% of women with a limited nodal burden disease would be potentially over treated. This data can allow us to appropriately ascertain this subgroup and can be used as one of the therapeutic implementation resources for patient decision support.

Carotid Plaque Characterization Using Quantitative Shear Wave Elastography: A Pilot Study

Objective: To assess the feasibility and reliability of incorporating quantitative shear wave elastography (SWE) data as an additional means of characterizing focal atheromata during routine carotid sonographic examinations when compared with conventional gray-scale Gray-Weale (GW) methods. Methods: Inpatients and outpatients undergoing routine carotid examinations at a medical center were included in the pilot study. Patients with prior history of endarterectomy, interventional therapeutic procedures, or absence of plaque formation were excluded. All examinations were performed by registered vascular technologist using a Mindray Resona 7 ultrasound equipment system with an L9-3U transducer in a carotid examination mode. The images were later reviewed by two independent, blinded observers and correlated to GW characterization. Results: There were 41 atheromatous lesions evaluated during the pilot study. The range of absolute (low to high) average and median Young’s modulus (YM) kPa values vary widely across each plaque type; they nonetheless still exhibit evidence of increasing stiffness as the gray-scale sonographic appearance becomes denser. The same observation is true of the average kPa and median kPa values as increasing YM values indicate increasing stiffness of interrogated tissues. Conclusion: This pilot study affirms the feasibility and reliability of incorporating quantitative SWE data as an additional means of characterizing focal atheromata during routine carotid sonographic examinations; further large-scale studies are needed to determine the sensitivity and specificity of individual kPa values associated with specific plaque types.

Qualitative and quantitative strain and shear wave elastography paradigm in differentiation of breast lesions

BackgroundBreast cancer is the most common life-threatening cancer in women worldwide. A high number of women are going through biopsy procedures for characterization of breast masses every day and yet 75% of the pathological results prove these masses to be benign. Ultrasound (US) elastography is a non-invasive technique that measures tissue stiffness. It is convenient for differentiating benign from malignant breast tumors. Our study aims to evaluate the role of qualitative ultrasound elastography scoring (ES), quantitative mass strain ratio (SR), and shear wave elasticity ratio (SWER) in differentiation between benign and malignant breast lesions.ResultsAmong 51 female patients with 77 histopathologically proved breast lesions, 57 breast masses were malignant and 20 were benign. All patients were examined by B-mode ultrasound then strain and shear wave elastographic examinations using ultrasound machine (Logiq E9, GE Medical Systems) with 8.5–12 MHz high-frequency probes. Our study showed that ES best cut-off point > 3 with sensitivity, specificity, PPV, NPP, accuracy was 94.7%, 85%, 94.7%, 85%, 90.9%, respectively, and AUC = 0.926 at P < 0.001, mass SR the best cut-off point > 4.6 with sensitivity, specificity, PPV, NPP, accuracy was 96.5%, 80%, 93.2%, 88.9%, 92.2%, respectively, and AUC = 0.860 at P < 0.001, SWER the best cut-off value > 4.9 with sensitivity, specificity, PPV, NPP and accuracy was 91.2%, 80%, 92.9%, 76.2%, 93.5%, respectively, and AUC = 0.890 at P < 0.001. The mean mass strain ratio for malignant lesions is 10.1 ± 3.7 SD and for solid benign lesions 4.7 ± 4.3 SD (p value 0.001). The mean shear wave elasticity ratio for malignant lesions is 10.6 ± 5.4 SD and for benign (solid and cystic) lesions 3.6 ± 4.2 SD. Using ROC curve and Youden index, the difference in diagnostic performance between ES, SR and SWER was not significant in differentiation between benign and malignant breast lesions and also was non-significant difference when comparing them with conventional US alone.ConclusionES, SR, and SWER have a high diagnostic performance in differentiating malignant from benign breast lesions with no statistically significant difference between them.

Shear Wave Elastography and Thyroid Imaging Reporting and Data System (TIRADS) for the Risk Stratification of Thyroid Nodules—Results of a Prospective Study

Purpose: To compare the diagnostic performance of thyroid imaging reporting and data system (TIRADS) in combination with shear wave elastography (SWE) for the assessment of thyroid nodules. Methods: A prospective study was conducted with the following inclusion criteria: preoperative B-mode ultrasound (US) including TIRADS classification (Kwak-TIRADS, EU-TIRADS), quantitative SWE and available histological results. Results: Out of 43 patients, 61 thyroid nodules were detected; 10 nodules were found to be thyroid cancer (7 PTC, 1 FTC, 2 HüCC) and 51 were benign. According to Kwak-TIRADS the majority of benign nodules (47 out of 51, 92.2%) were classified in the low-risk- and intermediate-risk class, four nodules were classified as high-risk (7.8%). When using EU-TIRADS, the benign nodules were distributed almost equally across all risk classes, 21 (41.2%) nodules were classified in the low-risk class, 16 (31.4%) in the intermediate-risk class and 14 (27.4%) in the high-risk class. In contrast, most of the malignant nodules (eight out of ten) were classified as high-risk on EU-TIRADS. One carcinoma was classified as low-risk and one as intermediate-risk nodule. For SWE, ROC analysis showed an optimal cutoff of 18.5 kPa to distinguish malignant and benign nodules (sensitivity 80.0%, specificity 49.0%, PPV 23.5% and NPV 92.6%). The addition of elastography resulted in an increase of accuracy from 65.6% to 82.0% when using Kwak-TIRADS and from 49.2% to 72.1% when using EU-TIRADS. Conclusion: Our data demonstrate that the combination of TIRADS and SWE seems to be superior for the risk stratification of thyroid nodules than each method by itself. However, verification of these results in a larger patient population is mandatory.

Shear wave elastography and pulsed doppler for breast lesions: Similar diagnostic performance and positively correlated stiffness and blood flow resistance

PurposeTo compare the diagnostic performance of shear wave elastography (SWE) and pulsed Doppler ultrasound in breast lesions, and to explore whether the quantitative SWE parameters correlated with pulsed Doppler ultrasound parameters. Materials and methodsSeventy-nine patients with 79 breast lesions who had undergone conventional ultrasound, pulsed Doppler ultrasound and SWE examination were included. All of them underwent core needle biopsy or surgery within one week. Parameters including Emax (the maximum elastic modulus), Emean (mean elastic modulus), Emin (minimum elastic modulus), Esd (elastic modulus standard deviation), and RI (resistive index), PI (pulsatility index), PSV (peak systolic velocity) and EDV (end diastolic velocity) were obtained for statistical analysis. ResultsAlmost all SWE parameters were significantly different between benign and malignant breast lesions (P＜0.05). There was no significant difference between Esd and PI (P＞0.05), which had the best AUC among SWE and vascular parameters respectively (0.877 vs. 0.871). Emax showed a moderate correlation with PI (P = 0.000, r = 0.552) and RI (P = 0.000, r = 0.544), and Esd moderately correlated with PI (P = 0.000, r = 0.567) and RI (P = 0.000, r = 0.546). For the benign group, no parameters showed any significant correlation (P＞0.05), while for the malignant group, Emax and Esd also significantly correlated with PI or RI. ConclusionsSWE and pulsed Doppler ultrasound had similar diagnostic efficacy for breast lesions. SWE and pulsed Doppler parameters were significantly correlated in breast lesions, especially in malignant ones, indicating the potential association between elastographic and vascular characteristics of breast tumors.

Role of Shear Wave Elastography of Synovium to Differentiate Rheumatoid and Tubercular Arthritis.

Background:Synovitis is the underlying pathology in various arthritis, and sometimes, it is difficult to differentiate various arthritis clinically or even by imaging. The purpose of our study was to use shear wave elastography (SWE) to evaluate rheumatoid arthritis (RA) and tubercular (TB) arthritis and to differentiate them using synovial stiffness.Methods:The prospective study was performed on Supersonic Imagine Aixplorer Ultrasound (USG) machine using a linear array probe SL10-2 (2–10 MHz). A total of 29 participants, 15 of RA (ACR/EULAR criteria) and 14 of proven TB arthritis were included. Region of interest of 1 mm was applied on the hypertrophied synovium and quantitative SWE data in form of elasticity (kPa) and velocity (m/s) were measured. Discrete categorical data were presented as n (%). Mean values were recorded along with standard deviation and the range of values. To find a maximal cutoff value of elasticity and velocity - receiver operating characteristic curve were plotted.Results:The mean elasticity and velocity values were 54.81 ± 10.6 kPa and 4.2 m/s ± 0.42 for RA and 37 ± 10 kPa and 3.4 ± 0.47 m/s for TB group. Significant difference (P < 0.001) was seen in elastic modulus values between rheumatoid and TB group with cutoff of 43.6 kPa to differentiate the two groups (sensitivity – 86.7% and specificity – 80%). Similar significant (P < 0.001) results were seen with velocity values, with cutoff of 3.76 m/s (sensitivity – 86.7% and specificity – 80%).Conclusion:SWE shows the potential to be a useful adjunct to gray scale and color Doppler USG in differentiating various arthritis on the basis of elastic properties of the synovium. Elastic modulus and velocity are useful SWE quantitative parameters for synovial evaluation and can differentiate RA and TB arthritis.

Combination of shear wave elastography and BI-RADS in identification of solid breast masses

BackgroundTo explore the value of quantitative shear wave elastography (SWE) plus the Breast Imaging Reporting and Data System (BI-RADS) in the identification of solid breast masses.MethodsA total of 108 patients with 120 solid breast masses admitted to our hospital from January 2019 to January 2020 were enrolled in this study. The pathological examination served as the gold standard for definitive diagnosis. Both SWE and BI-RADS grading were performed.ResultsOut of the 120 solid breast masses in 108 patients, 75 benign and 45 malignant masses were pathologically confirmed. The size, shape, margin, internal echo, microcalcification, lateral acoustic shadow, and posterior acoustic enhancement of benign and malignant masses were significantly different (all P < 0.05). The E mean, E max, SD, and E ratio of benign and malignant masses were significantly different (all P < 0.05). The E min was similar between benign and malignant masses (P > 0.05). The percentage of Adler grade II-III of the benign masses was lower than that of the malignant masses (P < 0.05). BI-RADS plus SWE yielded higher diagnostic specificity and positive predictive value than either BI-RADS or SWE; BI-RADS plus SWE yielded the highest diagnostic accuracy among the three methods (all P < 0.05).ConclusionSWE plus routine ultrasonography BI-RADS has a higher value in differentiating benign from malignant breast masses than color doppler or SWE alone, which should be further promoted in clinical practice.

Tumor stiffness measured by shear-wave elastography: association with disease-free survival in women with early-stage breast cancer.

To determine whether shear-wave elastography (SWE)-measured tumor stiffness is associated with disease-free survival in females with early-stage invasive breast cancer. This retrospective study included 202 consecutive females (mean age, 52.9 years; range, 25-84 years) with newly diagnosed T1-two breast cancer who underwent preoperative SWE between April 2015 and January 2016. Tumor stiffness was assessed and quantitative SWE features of each breast lesion were obtained by a breast radiologist. Cox proportional hazard models were used to identify associations between SWE features and disease-free survival after adjusting for clinicopathologic factors. Fifteen (7.4%) patients exhibited recurrence after a median follow-up of 56 months. Mean (Emean), minimum, and maximum elasticity values were higher in females with recurrence than in those without recurrence (184.4, 138.3, and 210.5 kPa vs 134.9, 101.7, and 159.8 kPa, respectively; p = 0.005, p = 0.005, and p = 0.012, respectively). Receiver operating characteristics curve analysis for prediction of recurrence showed that Emean yielded the largest area under the curve (0.717) among the quantitative SWE parameters, and the optimal cut-off value was 121.7 kPa. Multivariable Cox proportional hazards analysis revealed that higher Emean (>121.7 kPa) [adjusted hazard ratio (HR), 10.01; 95% CI: 1.31-76.33; p = 0.026] and lymphovascular invasion (adjusted HR, 7.72; 95% CI: 1.74-34.26; p = 0.007) were associated with worse disease-free survival outcomes. Higher SWE-measured Emean was associated with worse disease-free survival in females with early-stage invasive breast cancer. Tumor stiffness assessed with shear-wave elastography might serve as a quantitative imaging biomarker of disease-free survival in females with T1-two breast cancer.

Ultrasound shear-wave elastography applicability in estimation of post-mortem time.

Post-mortem rigidity of the tissues is one of the basic principles in forensic medicine to estimate the time of death. Qualitative methods to determine the stiffness of the corpse may have some limitations. Methods that provide quantitative values may be useful. We intended to evaluate the applicability of ultrasound shear-wave elastography of the tissues to estimate the post-mortem interval (PMI). For 80 corpses, shear-wave elastography of the liver, sartorius muscle, testis, thyroid and parotid was performed before autopsy. Based on the forensic reports as the reference method to define post-mortem interval, the corpses were divided into four groups: group 0 (PMI < 24 hours), group 1 (PMI ≥24 hours and <48 hours), group 2 (PMI ≥ 48 hours and <72 hours), and group 3 (PMI ≥ 72 hours). There were 24, 38, 13, and 5 corpses, respectively, in groups 0, 1, 2, and 3. A significant rise in the elasticity values in comparison to elasticity of normal tissues in live adults was seen very early in the post-mortem period. Between-group comparisons showed that a significant difference in the liver elasticity was present among the groups. The mean (SD) liver elasticity was 10.29 (±0.83) in group 0, 14.98 (±1.56) in group 1, 12.49 (±1.09) in group 2, and 15.64 (±1.68) kilopascals (kPa) in group 3 (P = 0.035). Nevertheless, elasticity measurements in other tissues were not helpful in distinguishing post-mortem interval groups. It is possible to use liver quantitative shear-wave elastography to estimate the time of death.
