Late-onset puerperal ovarian vein thrombophlebitis treated laparoscopically

Puerperal ovarian vein thrombophlebitis is an uncommon condition that usually develops in the immediate postpartum period. Computed tomography, magnetic resonance imaging, Doppler ultrasound, and exploratory laparotomy may be performed to diagnose it. A woman developed worsening abdominal pain 6 weeks postpartum. Laparoscopic intervention allowed treatment of late-onset, left-sided puerperal ovarian vein thrombophlebitis. The disorder may develop much later after delivery than expected and can be successfully treated laparoscopically.

Puerperal ovarianvein thrombophlebitis

Puerperal ovarian vein thrombophlebitis is a rare, but life-threatening complication. It often occurs at term after cesarean section or after operative vaginal delivery. According to the puerperal uterine drainage, the predominant location is the right ovarian vein. There is a lack of specific symptoms and signs. The diagnosis of ovarian vein thrombosis should be considered in all women with prolonged and unexplained fever postpartum. If prompt defervescence does not occur with aggressive intravenous antibiotic therapy, a computed tomography scan should be obtained in a timely manner. In uncomplicated cases a treatment trial with antibiotics and heparin intravenously in a therapeutic dosage should be undertaken. If this treatment is not successful within 48 h or if the patient exhibits thromboembolic complications or sepsis, operative therapy is recommended. The aim of the operation is to remove the clotted and infected foci. Early diagnosis and treatment can improve the prognosis of this disease significantly.

Postpartum ovarian veins thrombophlebitis

Puerperal ovarian vein thrombophlebitis (POVT) is a rare postpartal complication. Its incidence is about 0.05%. POVT can follow a term pregnancy, a premature delivery, an abortion or an ectopic pregnancy. POVT usually presents as a syndrome consisting in lower abdominal pain and fever which does not respond to adequate antibiotics. We present a case report, in which the diagnosis was based upon computed tomography.

Diagnosis of puerperal ovarian vein thrombophlebitis by computed tomography

A case of puerperal ovarian vein thrombophlebitis diagnosed by computed tomography is presented. Resolution after treatment with antibiotics and anticoagulation is demonstrated. The obstetrician must be aware of the role of computed tomography in making a prompt and accurate diagnosis in such cases.

A case of renal vein thrombosis following puerperal ovarian vein thrombophlebitis is reported. We review the syndrome of puerperal ovarian vein thrombophlebitis and emphasize the potential for renal vein and vena caval involvement. The utility of computed tomographic scanning for diagnostic confirmation of this postpartum complication is described.

Puerperal ovarian vein thrombophlebitis: A syndrome

Puerperal ovarian vein thrombophlebitis is a condition which usually presents as a syndrome consisting of lower abdominal pain, fever, and a characteristic abdominal mass. This syndrome occurs in about 0.18 per cent of puerperas, usually has its onset in the first week postpartum, and is usually best treated with anticoagulants and antibiotics. It occurs more often on the right than on the left. We offer a theory to explain the etiology of this condition and its dextropreponderance.
