Особенности проявлений климактерического синдрома у женщин в постменопаузальный период в современных условиях

The objective: investigation was to study the expression of climacteric syndrome in postmenopausal women in Azerbaijan. Materials and methods.There were 120 postmenopausal women examined the average age of the women was 60.29±0.69 years. The duration of postmenopausal period was within 10.33±0.67 years. In 45 (37.5%) of women the duration of postmenopausal period varied between 2 and 5 years, 16 (13%) it was between 6 and 9 years, while in 59 (49.2%) of women it was more than 10 years. Results. As a result of the conducted studies, the frequency and severity of vegetative, metabolic-endocrine, psychoemotional manifestations of menopausal syndrome in the examined women in the postmenopausal period were determined. Conclusion. It was that autonomic nervous expressions of climacteric syndrome comprised 28.8±0.46, endocrine symptoms 22.6±0.8, emotional expressions were 7.8±4.6. The average evaluation of climacteric syndrome was 59.2±2.11, that corresponds to the moderate degree of expression of climacteric syndrome. Menopausal disorders in obese women

Aim. To study menopausal disorders in women suffering from obesity. 
 Methods. The study included 165 women in physiological postmenopausal state. Women were divided into two groups: group 1 (study group) included 87 women with obesity, group 2 (control group) consisted of 78 women without obesity. With the use of modified menopausal index (the Kupperman index) the severity of climacteric disorders was assessed. Severity of neurovegetative, metabolic-endocrine and psychoemotional symptoms was assessed according to 4-point scale (from 0 to 3 points). The number of points in each group was summarized and a modified menopausal index was obtained. 
 Results. It was revealed that in women with obesity severe (in 24 females, 27.5%) and moderate (in 39 patients, 44.5%) climacteric syndrome was prevalent, in patients from the control group such forms were 1.5-2 times less frequent - in 11 (14.2%) and 28 (36.2%) patients, respectively. A deeper analysis of the entire symptom complex of climacteric syndrome revealed that severe and moderate forms of neurovegetative manifestations were 2 times more common in patients with obesity, in contrast to healthy women (in 7.8% and 4.5%, 21.5% and 12.3%, respectively). Severe forms of psychoemotional manifestations were also observed 2.6 times more often in women with obesity. 
 Conclusion. Severity of menopausal disorders depends on obesity presence, the data obtained demonstrated that, against the background of obesity, climacteric syndrome tends to have prolonged course with predominance of severe and moderate forms.

Аналіз клініко-епідеміологічних аспектів гіперпластичних процесів ендометрію жінок із застосуванням багато параметричної нейромережевої кластеризації

З метою підвищення ефективності діагностики гіперпластичних процесів ендометрія у жінок<br />перименопаузального віку було проведено ретроспективний аналіз 52 карт стаціонарного хворого жінок, які<br />перебували на лікуванні в гінекологічних відділеннях Тернополя і області. У відповідності з патогістологічним<br />висновком 1 групу склали 28 жінок з простою гіперплазією ендометрія, 2 групу – 24 жінки з комплексною гіперплазією<br />ендометрія. В групу порівняння увійшли 12 соматично здорових жінок. Середній вік хворих 1 групи – (47,0±1,0)<br />років, 2 групи – (56,0±1,5) років. З метою поглибленого аналізу показників застосовано багатопараметричну<br />нейромережеву кластеризацію. Встановлено, що до факторів ризику розвитку гіперпластичних процесів<br />ендометрія у жінок перименопаузального періоду належать неблагополучні соціально-економічні фактори:<br />проживання в сільській місцевості, тяжкі умови праці, шкідливі звички та обтяжений акушерсько-гінекологічний<br />анамнез, 3 і більше вагітностей, велика кількість (4 і більше) акушерсько-гінекологічних втручань, використання<br />внутрішньоматкової контрацепції. У жінок з різними видами гіперплазії ендометрія клімактеричний синдром<br />перебігав середньотяжко чи тяжко з нейровегатативними, ендокринно-обмінними та психоемоційними проявами.<br />Серед коморбідних станів найчастіше спостерігали дисгормональну патологію молочних залоз та порушення<br />гепатобіліарного тракту.<br />Ключові слова : гіперпластичні процеси ендометрія, клімактеричний синдром, перименопаузальний вік,<br />дисфункційні маткові кровотечі, коморбідні стани, нейромережева кластеризація.</p>

PROBLEMATIC ISSUES OF DIAGNOSTICS AND THERAPY OF NON-PSYCHOTIC MENTAL DISORDERS IN FEMALE PATIENTS OF CLIMACTERIC AGE WITH HYSTERICAL SYMPTOM COMPLEX (LITERATURE REVIEW)

In the article, problematic questions of diagnostics and therapy of non-psychotic mental disorders (NPMD) in female patients of climacteric age with hysterical symptom complex are considered. Efficacy of psychotherapy (PT) in NPMD, hypnopsychotherapy in hysterical states: hysterical neurosis, neurasthenia and obsessive-compulsive neurosis is indicated. In treatment of NPMD, PT by creative selfexpression is successfully used. It is highlighted that PT forms conscious-critical attitude of patients toward themselves. Combination of PT with physiotherapy in hysterical conversional symptoms has been described. In hysterical manifestations neuroleptics are recommended, in neurotic depressions – antidepressants of mild action. In severe hysterical state, psychopharmacotherapy (PPhT) with tranquilizers and neuroleptics is applied on long-term basis. Stable recovery in dissociative and hysterical disorders has been shown. In vegetovascular disorders in structure of climacteric syndrome (CS) vinpocetine, in psychoemotional manifestations phenibut was administered. In therapy of hysterical neurosis, “minor neuroleptics”, hypnosuggestive therapy, social rehabilitation were applied. Effective group PT of psychogenically conditioned disorders in asthenicand anxiety-depressive symptoms is effective. Complex therapy of NPMD in hysterical and asthenic neurosis, obsessive-compulsive neurosis has been suggested. Organization of specialized preventive examinations for early revealing of persons with personality pathology is based. Efficacy of a number of medications in periand post-menopause – SSRIs and gabapentin, during menopause paroxetine, in depressions of non-psychotic level – pyrazidol, coaxil, in neurotic hypochondriasis sulpiride and quetiapine, diazepam, in climacteric vegetative and mental disorders hormone replacement therapy (HRT), hormonal therapy, PPhT and PT, in neurovegetative symptoms of CS – antidepressants, in psychovegetative syndromes – SSRIs, in dysthymic disorders – tianeptine (coaxil), in involutional hysteria – psychotropic medications and medications of neurometabolic action, in psychoemotional disorders of mild and moderate degree – deprim, in depressive disorders of mild and mean severity – valdoxan has been shown. In climacteric depression, hormonal therapy stops climacteric symptoms.

Psycho-emotional Manifestations of Valvular Heart Diseases: Prospective Assessment in Mitral Regurgitation

ObjectiveTo define the prevalence and consequences of post-traumatic stress disorder (PTSD) as an emotional response to cardiac diseases in patients with mitral regurgitation. MethodsWe prospectively enrolled 186 patients with moderate or severe organic mitral regurgitation, presenting class I (absent) or II (minimal) dyspnea, who were compared with 80 controls of similar age (38 with completely normal cardiac function; 42 with mild mitral-valve prolapse; all with no, or at most mild, mitral regurgitation). Mitral-regurgitation severity and consequences were comprehensively measured, simultaneously with PTSD, anxiety, and depression. ResultsPTSD prevalence was higher in mitral-regurgitation patients vs controls (23% vs 9%, P <.01). Although mitral-regurgitation objective severity (regurgitant volume 77.8 ± 28.9 vs 79.0 ± 27.5 mL, P = .8) and objective consequences (left-atrial volume 59.1 ± 20.9 vs 54.02 ± 15.6 mL, P = .1; right-ventricular systolic pressure 34.1 ± 11.4 vs 32.9 ± 7.2 mm Hg, P = .6) were similar with and without PTSD (all P ≥.1), patients with PTSD were more symptomatic (class II 74 vs 38%; fatigue 71% vs 38%, both P <.0001) and had higher anxiety and depressions scores (P <.0001). ConclusionsPTSD is prevalent in organic moderate or severe mitral-regurgitation patients but is not determined by objective mitral-regurgitation severity or consequences. PTSD is linked to anxiety and depression and to symptoms usually considered cardiac, such as dyspnea. Thus, PTSD and psycho-emotional manifestations, linked to symptoms, represent important responses to chronic-valve disease that may affect clinical outcomes.

Specific Affectivity and the Relationships in Preadolescents with Different IQ

We intend to reveal by this study the particularities of the emotions and emotional manifestations, of the relationship types, in preadolescents with intellect deficiencies and with normal intellect. In this study, we assume that the affectivity will have specific characteristics, in reference to the levels of intellectual development. The conclusions, the psycho-emotional manifestations and characteristics appear present in all the categories of subjects, almost without a difference, but the share they have in each differs in reference to the level of intellectual development.

Quality of life and prevalence of osteoarticular pain in patients submitted to bariatric surgery

To analyze quality of life and observe the prevalence of musculoskeletal pain in patients submitted to bariatric surgery. A prospective, observational and comparative study with 26 individuals aged 18 to 60 years, 25 women, which included two evaluations, one preoperative and the other approximately 42 months after surgery. The Medical Outcomes Study 36-Item Short Form Health Survey (SF-36) and the Human Body Diagram with Visual Analogue Scale were employed. The individual samples showed grade III obesity, with a predominance of postoperative overweight, hypertension and diabetes in 65.4% and 42.3% of the samples, with remission of hypertension in 50% and of diabetes mellitus in 38.5% (p < 0.001). The SF-36 demonstrated improved quality of life, especially in aspects related to motricity; vitality and mental health showed no significant changes. Osteoarticular pain was reported and identified in various sites by the subjects; however, 87.5% of patients in the preoperative period and 88.5% in the postoperative period reported not having any physical therapy orientation, while 65.4% reported being engaged in some type of regular physical activity after surgery (p<0.001). Morbidly obese individuals have a high probability of suffering from clinical, psychic, and musculoskeletal alterations, compromising their quality of life and showing improvement after bariatric surgery; on the other hand, the psycho-emotional manifestations did not progress in the same way.

A Low Brain Serotonergic Neurotransmission in Children With Type 1 Diabetes Detected Through the Intensity Dependence of Auditory-Evoked Potentials

OBJECTIVE—To determine in children with type 1 diabetes the plasma free fraction of l-tryptophan (FFT) and the intensity-dependent auditory-evoked potentials (IDAEPs) as indicators of possible changes in brain serotonergic neurotransmission. RESEARCH DESIGN AND METHODS—A prospective and comparative study was performed in children with type 1 diabetes and normal control subjects. We measured FFT, bound and total plasma l-tryptophan, neutral amino acids (NAAs), albumin, free fatty acids (FFAs), glucose, and HbA1c(A1C) and recorded IDAEPs with four intensities (40, 60, 90, and 103 dB). RESULTS—The glycemia, A1C, FFAs, and NAAs in plasma were significantly elevated. The FFT and the FFT-to-total l-tryptophan and FFT-to-NAA ratios were reduced. The latencies of N1 and P2 increased at all intensities and the slope of the amplitude/stimulus intensity function (ASF slope) of the N1/P2 component significantly increased. CONCLUSIONS—The decrease of the FFT in plasma and increase in the N1/P2 component amplitude may reflect a functional relationship between the brain serotonergic activity with the N1/P2 changes. The increase of the ASF slope in children with type 1 diabetes suggests that the response of the auditory cortex to sound intensity stimulus may be regulated by the serotonergic tone and that decreased serotonergic neurotransmission may provoke a different behavior of sensory cortices. Therefore, the IDAEP (N1/P2 component) may be an electrophysiological indicator of brain changes of serotonergic neurotransmission in children with type 1 diabetes. These changes may be related to psychoemotional manifestations observed in diabetic children such as anxiety and depression.

Succinate-Based Preparation Alleviates Manifestations of the Climacteric Syndrome in Women

Clinical placebo-controlled study of Enerlit-Clima (bioactive succinate-based food additive) a showed positive effect of the preparation on general clinical and psychoemotional manifestations of the climacteric syndrome. A trend to an increase in estradiol level in early pathological climacteric and normalization of the endometrial status were observed.

Psychoemotional manifestations in hippocampectomized rats

The effects of lesioning of the dorsal hippocampus on the psychoemotional state of Wistar rats were studied. Hippocampal lesions did not affect the learning process, but affected mainly the pattern of psychoemotional manifestations in all animals regardless of the typology of higher nervous activity. All animals showed reductions in the extremes of types of passive and active stress reactions. Individual effects depended on the behavioral phenotypes of the animals. Hippocampal lesions in rapidly learning rats (10%) led to sharp decreases in irritability (active/passive = 0.35/0.5 instead of 1.0/0.4), while passive-defensive manifestations were not altered. In slowly learning animals (30%), hippocampal lesions were not accompanied by qualitative changes in the patterns of psychoemotional manifestations, while non-learning rats (60%) showed decreases in passive and increases in active manifestations (active/passive = 0.45/1.4 instead of 0.3/1.9). It is suggested that the hippocampus is involved in producing psychoemotional reactions, individual patterns of which depend on the morphofunctional characteristics of the system responsible for organizing the psychoemotional state.
