Testicular Cancer Incidence among Obstructive Sleep Apnea (OSA) Patients: South Korean National Health Insurance Data.

Obstructive sleep apnea (OSA) has been linked to an increased risk of acquiring many types of cancer. No data on the prevalence of testicular cancer in OSA patients have been reported in the literature. The goal of the present investigation is to find out the impact of OSA on the incidence of testicular cancer based on the Korea National Health Insurance Service (KNHIS) dataset. A cohort of adult male patients newly registered with OSA in the KNHIS data from 2007 to 2014 who had no history of any previous cancer diagnosis was included. The main outcome measure was newly diagnosed testicular cancer in the National Medical Expenses Support Program. The control group was set at five times larger than the OSA group, and it was matched with age and sex. The cumulative incidence and hazard ratio (HR) for the development of testicular cancer were compared between the OSA and control groups. Further subgroup analysis was conducted in the three different age groups. In the study period, a total of 152,801 male adult patients newly diagnosed with OSA were included, whereas 764,005 individuals were recruited as the control group. The HR of OSA for developing testicular cancer was 1.58 (95% confidence interval [CI]: 0.92-2.60), showing no significant HR regardless of confounding adjustment. However, the subgroup analysis revealed a significantly increased HR to develop testicular cancer of 3.39 (95% CI: 1.08-10.06) in groups aged more than 65, whereas the age ranges of 20-40 and 40-64 showed no significance (1.19 (0.44-2.75) and 1.27 (0.50-2.80), respectively). OSA may not influence the incidence of testicular cancer in the general adult population. However, compared to younger males, males over 65 may be more susceptible to OSA when it comes to developing testicular cancer.

Awareness of testicular cancer and testicular self-examination in young adults, and the efficacy of education-based media.

e17022 Background: The prevalence of testicular cancer in adolescents and young adults creates a unique opportunity for education through social media. We investigated this populations’ baseline knowledge of testicular cancer (TC) and testicular self-examination (TSE) and identified the efficacy of various forms of media in educating young individuals. Methods: A survey was designed to assess participants’ baseline knowledge of TC and TSE. Participants were randomly shown one of three forms of educational media created by the Testicular Cancer Society and the National Cancer Institute. These were an infographic, a short (60 second) and a long (6 minute) video. Participants were then asked to correctly identify facts regarding TC and TSE. Baseline knowledge was analyzed by age (< 24 vs. > 25), and post-media knowledge was analyzed to determine the efficacy of each media form. Results: Responses from 328 participants were analyzed, with a mean age of 24.3 years, 40.9% male, 72.0% white, and 70.7% graduated college or a graduate program. Only 30.7% of participants reported being educated about TC, 25.9% correctly identified the 15-35 age group as having the highest prevalence of TC, and 26.3% correctly identified the TC survival rate as being 75-100%. The > 25-year-old group were significantly more likely to have heard of TSE (p < 0.001), performed a TSE if male (p = 0.043), encouraged males to perform TSE if female (p < 0.001) and correctly identify TC survival rate (p = 0.006) and treatability (p = 0.007). Long video viewers were significantly less likely to watch it in its entirety compared to infographic and short video viewers, and they were less likely to report an understanding of TSE compared to infographic viewers. Short video viewers were less likely to have properly identified the TC survival rate compared to both infographic (P < 0.001) and long video viewers (p = 0.001). Conclusions: Overall there is poor awareness and education regarding TC and TSE, with adolescents performing significantly worse than young adults. The infographic performed the best overall in completion and education about TC and TSE, with the short video performing similarly well, and the long video performing the worst. This suggests that easily dispersed and viewable forms of media may be the most effective tool for educating young people.[Table: see text]

Incidence of Testicular Cancer in Transfeminine Patients following Vaginoplasty with Orchidectomy.

Background:Little is known about the prevalence of testicular cancer in the transfeminine population. Only six cases have been reported in the literature. This case series reports six additional cases of various testicular cancers found in transfeminine patients who underwent vaginoplasty with orchidectomy in our institution.Methods:In our institution, all specimens are routinely sent to pathology following vaginoplasty with orchidectomy. This permitted the identification of all positive cases of testicular cancer. A chart review was conducted to retrieve patient demographics, duration of hormonotherapy, type of neoplasm, the context of its discovery, and cancer follow-up.Results:A total of 2555 patients underwent vaginoplasty with orchidectomy between January 2016 and January 2021. All specimens were sent to pathology for analysis. A total of six (0.23% of patients) specimens revealed malignant lesions.Conclusions:Increased societal awareness toward the transgender population encourages recourse to gender-affirming procedures. Little is known about the incidence of testicular cancer in the transfeminine population. In total, 0.23% of patients in our cohort presented with positive pathology findings indicative of testicular cancer. All cancers were found to be only locally invasive, and all patients were successfully treated. We therefore encourage routine pathology examination for all specimens following vaginoplasty with orchidectomy.

P–061 Protective effect of melatonin against bleomycin, etoposide, and cisplatin (BEP) chemotherapy-induced testicular toxicity in Wistar rats: A biochemical, immunohistochemical and apoptotic genes based evidence

Abstract Study question Does exogenous melatonin (MLT) attenuate BEP-induced damage in testicular cells and spermatogenesis in a dose-dependent manner? Summary answer Melatonin protected the testes against BEP-induced testis damage through ameliorating nitro-oxidative stress, apoptosis, and inflammation. However, there was no significant difference between melatonin-treated groups. What is known already Recently, the prevalence of testicular cancer (TC), accounting for the most common cancer among young people of reproductive age (15–40 years), has risen internationally. BEP chemotherapy has increased the 5-year survival rate of TC patients at all stages of testicular germ cell tumors to 90–95%. However, BEP creates a high incidence of male infertility and even long-term genotoxic effects, which emerges as a critical health issue. Melatonin is a well-known potent antioxidant with widespread clinical applications that recently has been giving increasing attention to its role in male sub/infertility. Study design, size, duration 60 Adult male Wistar rats were randomly assigned to six groups (n = 10/group). Group 1, 3, and 4 were injected with vehicle, 10 and 20 mg/kg of melatonin, respectively. Other groups received one cycle of bleomycin, etoposide, and cisplatin for a total of 3 weeks with or without melatonin. Melatonin administration started daily one week before BEP initiation continued on days 2, 9, and 16; and one week after the completion of the BEP cycle. Participants/materials, setting, methods Bodyweight, testes weight, Sperm parameters (count, motility, viability, and morphology), testosterone hormone level, testicular histopathology, stereological parameters, testicular level of malondialdehyde (MDA), nitric oxide (NO), and total antioxidant capacity (TAC), the expression of Bcl–2, Bax, Caspase–3, p53, and TNF-α (Real-time PCR and immunohistochemistry) were evaluated at the end of the study (day 35). Main results and the role of chance Our findings showed that melatonin restores the BEP-induced reduction in the body and testes weight (P<.05). the evaluation of quantitative analysis of the testes stereological procedures, QRT-PCR examination and immunohistochemical (IHC) staining revealed that melatonin reverses the BEP-induced impaired spermatogenesis (P<.05). Furthermore, melatonin rectifies BEP-induced disturbance on sperm count, motility, viability, and morphology. The testosterone level in the BEP-treated group was decreased significantly by comparison with the control group (P<.01). By contrast, co-administration of 10 and 20 mg/kg of melatonin could enhance the serum testosterone level significantly (P<.05). Moreover, melatonin enhanced the antioxidant status of the testis by elevating TAC and ameliorating MDA and NO levels. More notably, QRT-PCR examination indicated that melatonin therapy suppressed BEP-induced apoptosis by modulating apoptosis-associated genes such as Bcl–2, Bax, Caspase–3, p53 in the testis (P<.01). Besides, Co-administration of 10 and 20 mg/kg of melatonin with BEP regimen decreased significantly the population of p53 (54.21 ±6.18% and 51.83±8.45, respectively) and TNF-α positive cells (42.91±9.92% and 33.57±2.97, respectively) by comparison to the BEP group. Also, melatonin with low and high doses could enhance the expression of Bcl–2 protein in spermatogenic cells line (59.19±10.18%, 63.08±5.23, respectively) compared to the BEP-treated group. Limitations, reasons for caution Owing to limited laboratory facilities we were not able to perform further studies to verify the mechanism of melatonin in the specific targets by using transfection technique and transgenic. Wider implications of the findings: These findings can draw attention to the clinical application of melatonin and also suggest that melatonin may be an attractive agent for attenuating chemotherapy-associated male sub/infertility. This indolamine also may shorten the fertility recovery period in patients undergoing chemotherapy with the BEP regimen. Trial registration number N/A

Estudio de asociación entre la estirpe histológica y el estadio clínico de los tumores testiculares con los marcadores tumorales

Antecedentes: El tumor testicular es el tumor sólido más frecuente en hombres entre 20 y 34 años. Los tumores testiculares pueden dividirse histológicamente en células germinales que se presentan en 90–95% y no germinales en alrededor del 5%. Los marcadores tumorales son cruciales en el diagnóstico, clasificación, pronóstico y vigilancia posterior al tratamiento de los tumores testiculares.
 Objetivo: Conocer la prevalencia puntual de cáncer de testículo en nuestro servicio. Determinar las características clínico-patológicas del cáncer de testículo. Correlacionar los niveles de marcadores tumorales con la estirpe histológica y el estadio clínico del cáncer testicular
 Metodología: Es un estudio transversal en el que se analizó el total de expedientes de pacientes atendidos en el servicio Urología con diagnóstico de cáncer de testículo, durante el período entre el 1° de enero de 2014 al 31° de diciembre de 2018.
 Resultados: Se incluyeron 99 pacientes, la media de edad fue de 29±8.5 años. Los tumores que con mayor frecuencia se registraron fueron: tumor de células germinales en el 99%: germinal mixto 50 (50.51%), seguido del seminoma con 42 (42.42%), teratoma 5 (5.05%). La combinación TNM mas comúnmente encontrada fue T1 N0 M0. El estadio clínico mas frecuente fue estadio I, seguido del II y finalmente el III. La alfafetoproteína es el único marcador tumoral que se asocia a extirpe histológica y podría estar asociado a enfermedad diseminada.
 Limitaciones: Las limitaciones de nuestro estudio es un tamaño de muestra pequeño con una incidencia de algunas variedades de tumores también muy baja.
 Originalidad: El valor de nuestro estudio es que aporta información sobre la materia en población latinoamericana que es escasamente reportada. 
 Conclusiones: La estirpe histológica que se encontró mayor frecuencia fueron los tumores germinales mixtos y la clasificación TNM más comúnmente encontrada en nuestros pacientes fue T1a, N0, M0 y estadio clínico I. La AFP es el único marcador predictor de la estirpe histológica y de una posible enfermedad diseminada.

The prospective correlation between GGN and CAG repeat polymorphisms of androgen receptors and testicular cancer

Worldwide testicular cancer (TC) is a contributing factor towards male infertility at reproductive age. A gradual increase had been observed in TC especially in Western countries. Prevalence of TC was found to be more common in Caucasian populations as compared to Africans and Asians due to variation among genetic makeup of individuals that varies their susceptibility to anomalies like cryptorchidism, hydroceles and inguinal hernias (1).

Educating Young Men About Testicular Cancer: Support for a Comprehensive Testicular Cancer Campaign

Despite the prevalence of testicular cancer among men 15–39 years of age, little has been done to increase awareness of this disease or educate males about its prevention. To fill this gap, the Standard Model of Health Communication was incorporated to design and implement a comprehensive testicular cancer campaign among male college students. To test the effectiveness of these messages, college students (N = 220) completed measures before and after the campaign. In addition, the authors obtained a control group of male college students (N = 52) who were not exposed to the messages. Survey items assessed awareness of testicular cancer and behaviors related to testicular cancer. Participants' knowledge of testicular cancer and likelihood of conducting a testicular self-exam increased significantly after being exposed to the campaign information. Men who were exposed to testicular cancer messages were more knowledgeable about testicular cancer and were more likely to conduct testicular self-examinations than were men in the control group.

Testicular Microlithiasis: Prevalence and Clinical Significance in a Population Referred for Scrotal Ultrasonography

PurposeTesticular microlithiasis (TM) is an uncommon pathologic condition that is commonly diagnosed by scrotal ultrasonography. Indirect evidence suggests that this syndrome may be associated with an increased risk of testicular malignancy and infertility.Materials and MethodsA total of 1,439 patients undergoing scrotal ultrasound during a 6-year, 5-month period (January 2003 to May 2009) were retrospectively reviewed. Any possible association of TM with pathologic findings was assessed. Among patients with TM, further grading of TM with testicular cancer and semen analysis of the infertile group with TM were also performed.ResultsTM was diagnosed in 87 patients (6.0%) out of a total of 1,439. Of all established pathologic entities, only testicular malignancy and infertility were meaningfully associated with TM. There was no significant difference in the prevalence of testicular cancer between each grade. Seminal profiles (sperm count, motility, morphology, and white blood cell count) were not found to be statistically different between infertile men with and without TM.ConclusionsThe prevalence of TM in symptomatic men was found to be 6.0% with significant co-occurrence of TM, testicular cancer, and infertility. Further grading of TM does not seem to be essential with regard to the detection of patients with testicular cancer and TM. TM showed no significant effect on the seminal profiles of infertile men.

Grading of Classical Testicular Microlithiasis Has No Effect on the Prevalence of Associated Testicular Tumors

Aim: To evaluate the role of further grading of classical testicular microlithiasis (CTM) on the prevalence of associated testicular tumors. Methods: Patients diagnosed with CTM using scrotal ultrasound over a 5-year period from a referral radiology clinic were included in this study. Patients with CTM were categorized as group 1 (grade I; 5–10 microliths/image), group 2 (grade II 10–20 microliths/image), and group 3 (grade III > 20 microliths/image). Other pathological findings were also recorded. The prevalence of testicular cancer was compared statistically between groups using the χ² test. Results: Seventy-eight of 4,310 (1.8%) patients were found to have CTM. Of these, 17 (21.7%) had ultrasonographically detected testicular cancer. In patients without CTM (n = 4,232), 58 (1.1%) testicular cancers were found. This accounted to a 19.7-fold increase in the detection rate of testicular cancer in patients with CTM compared to patients without CTM. The rates of testicular cancer detected in groups 2 and 3 were 25 (6/24) and 26.6% (4/15), respectively (p > 0.05), while it was 17.9% (7/39) in group 1. Conclusions: CTM is an uncommon incidental finding in patients undergoing testicular ultrasonography, and grading of CTM plays no role in the prevalence of testicular cancer.

A history of cryptorchidism is the only risk factor for testicular cancer that can be considered unequivocally established. The rate of cryptorchidism at age 3 months appears to have increased over the 30 years from 1.7% to 3.2%. From a review of the literature, the overall proportion of testicular cancer patients with a history of cryptorchidism is approximately 10%. Case-control studies indicate that the relative risks of testicular cancer associated with a history of cryptorchidism are about 6. Prevalence of testicular cancer in patients with a history of cryptorchidism could range between 2% and 3%, as reported in studies from North European Countries. Orchidopexy itself or age at orchidopexy seem to have no effect on the subsequent risk of testicular cancer; however, data from the literature are somewhat discrepant. But orchidopexy does seem to influence the histologic type of malignancy, with a lower frequency of seminoma. The developement of non invasive techniques is badly needed to allow population screening for testicular cancer.
