Medical-legal partnership (MLP) integrates the unique expertise of lawyers into collaborative clinical environments. MLP teams meet the needs of individual patients while also detecting structural problems at the root of health inequities and advancing solutions at the institutional, community, and system levels. Yet MLPs today operate in limited settings and survive on scant budgets. Expanding their impact requires secure funding. Financing MLPs as health care can do the following: (1) help address inequity at the point of care; (2) enable expert diagnosis and treatment of nonmedical drivers of health; (3) enhance team-based practice in health care organizations; (4) offer another way for clinicians to participate in advocacy; and (5) bolster a broader movement to increase access to justice.

Strategic alignment in multispecialty hospitals: Implementing a balanced scorecard approach for optimal performance

The study investigates the relationship between strategic alignment, balanced scorecard (BSC) implementation, and optimal performance (OP) in multispecialty hospitals. Data from 530 healthcare professionals in Lucknow, Varanasi, Kanpur, and Prayagraj were collected using a cross-sectional design. Descriptive statistics, correlation analysis, and regression analysis were employed for data analysis. The results demonstrate significant positive relationships between strategic alignment, balanced scorecard, and optimal performance. Both strategic alignment and balanced scorecards were identified as significant predictors of optimal performance, indicating that higher scores in these constructs are associated with higher levels of performance. The findings underscore the critical role of strategic alignment and performance measurement frameworks in enhancing organizational performance in healthcare settings. However, the study faces limitations, including reliance on self-reported data and limited generalizability to other healthcare settings and regions. Future research should explore longitudinal or experimental designs and incorporate objective performance measures to enhance the validity and generalizability of the findings. Overall, the study contributes to understanding strategic management practices in healthcare organizations and highlights avenues for further research in this area.

Assessing health care leadership and management for resilience and performance during crisis: The HERO-36.

Whereas organizational literature has provided much insight into the conceptual and theoretical underpinnings of organizational leadership and management during emergencies, measures to operationalize related effective practices during crises remain sparse. To address this need, we developed the Healthcare Emergency Response Optimization survey, which set out to examine the leadership and management practices in health care organizations that support resilience and performance during crisis. We administered an online survey in April to May 2022 to health care administrators and frontline staff intimately involved in their hospital's emergency response during the COVID-19 pandemic, which included a sample of 379 respondents across nine rural and urban hospitals (response rate: 44.4%). We used confirmatory factor analysis and quantile regressions to examine the results. Applying confirmatory factor analysis, we retained 36 items in our survey that comprised eight measures for formal and informal practices to assess crisis leadership and management. To test effectiveness of the specified practices, we regressed self-reported resilience and performance measures on the formality and informality scores. Findings show that informal practices mattered most for resilience, whereas formal practices mattered most for performance. We also identified specific practices (anticipation, transactional and relational interactions, and ad hoc collaborations) for resilience and performance. These validated measures of organizational practices assess emergency response during crisis, with an emphasis on the actions and decisions of leadership as well as the management of organizational structures and processes. Organizations using these measures may subsequently modify preparedness and planning approaches to better manage future crises.

Mapping sustainability practices in the healthcare sector: A systematic literature review and future research agenda

AbstractSustainable healthcare means providing affordable, quality medical care without causing undue environmental damage, which is critical for a world just emerging from a debilitating pandemic. This article contends that it is now an opportune time to map the existing knowledge to guide future sustainability research in healthcare. Therefore, this article aims to systematically review and comprehensively analyse the extant literature on sustainability practices in healthcare organizations and propose future research directions based on gaps in the literature. A corpus of 79 articles sourced from five databases and published between 1995 and 2022 was identified for an in‐depth review. Following a framework‐based systematic literature review, this article offers an organized, retrospective view of the performance using antecedents, decisions, and outcomes (ADO) and Theories–Contexts–Methods (TCM) integrated framework. These frameworks are used to evaluate and understand sustainability practices in the healthcare sector. Additionally, the article shows the thematic evolution of sustainability research in healthcare with the help of five clusters. While the systematic literature review results highlight the large theoretical base, it also reveals the lack of consensus on defining sustainability. The analysis of 79 articles yielded six valuable categories of antecedents with 27 constructs, five categories of decisions with 11 constructs, and two categories of outcomes with 15 constructs. Results also reveal a lack of consensus regarding the influence of sustainability practices on organizational performance across the three sustainability pillars (social, environmental, and economic dimensions). The present review has implications for healthcare service delivery systems and policymakers to help them redesign their services to promote sustainable development goals. Before concluding the article, knowledge gaps and future research directions are structured using the ADO‐TCM framework through pertinent ideas for sustainability research in healthcare organizations.

Translating evidence-based knowledge objects into practice.

This paper aims to show how organizational translation theories and models may supplement implementation science with a new process perspective on how knowledge objects such as Cochrane reviews, clinical guidelines and reference programs are implemented in practice in healthcare organizations. They build on Bruno Latour's idea about translation that states that the spread in time and space of anything-including knowledge objects-is in the hands of people and that each of these people may act in many different ways, letting the token drop, modifying it, deflecting it, betraying it, adding to it, or appropriating it. Implementation science theories, models and frameworks often try to identify general aspects of processes and variables that influence implementation processes. In contrast, translation theories and models build on a process view that uses the sequence of events, activities and choices by translators situated in time as well as in space to explain how outcomes of translation/implementation processes came about. The paper develops some implementation relevant propositions about translation of knowledge objects in healthcare organizations that may inform further research. Moreover, it discusses how organizational translation studies and implementation science may supplement each other.

Gender Equity in Hospitals: An Italian case Study

The topic of gender equity is increasingly cited as a goal of health policy despite the rise in the number of female doctors around the world, and attempts to enhance gender awareness have gained prominence. Recently, initiatives to increase gender awareness have been numerous nevertheless, the topic of gender equity in the medical profession has not been adequately assessed. In order to fill this gap, this paper examines the current state of gender equity literature in hospitals, with a systematic literature review on Scopus, and, then, it focuses on an Italian case study to identify the characteristics that obstacle or support gender equity in terms of salary, career, and professionalism. The finding highlighted some reflections regarding the roles in the Italian hospitals and obstacles to gender equity, such as the stereotyped judgments, as well as shed light on new considerations regarding the actions aimed at managing gender disparity in such working environments. This has important implications for practice in health care organizations and human resource management, which are highlighted in conclusion, together with the limitations and future research avenues.

Intersectional Antiracist Advocacy Practice in Health Care Organizations.

Health care organizations, like individuals, can evolve to become antiracist and promote racial equity within and beyond the organization. In this brief article, we introduce an intersectional antiracist advocacy practice framework applicable to health care organizations that seek restorative and transformative change, as well as participation in social and economic justice action. Becoming an antiracist organization requires an acknowledgment that no organization is impervious to racist and other oppressive ideologies. Organizations can then begin to interrogate, interrupt, and address how racism permeates agency policies, procedures, and culture. The implementation of an intersectional antiracist advocacy practice framework within organizations involves a multifaceted approach, including both internal and external practices. Internally facing practices include providing mandatory antiracist trainings to all employees; promoting a representative and equitable workplace; and developing an organizational power structure based on inclusion, transparency, and accountability. Externally facing practices include fostering nonexploitative, reciprocal community partnerships; contributing to social and economic justice movements; and demonstrating transparency and accountability for the negative impact of operations in Black, Indigenous, and People of Color (BIPOC) communities and lands. We conclude with key questions for health care organizations to consider in regard to their racial equity efforts, specifically around organizational readiness, risk tolerance, and long-term commitment.

Nurses' Perceptions of the Clinical Nurse Specialist Role Implemented in a Highly Specialized University Hospital in Spain: A Qualitative Study.

To explore nurses' perceptions of clinical nurse specialist practice as implemented in a highly specialized university hospital in Spain. A descriptive qualitative study was carried out in 3 inpatient wards, with a clinical nurse specialist within the team, at a high specialized university hospital in Spain. Semistructured interviews were conducted by purposive sampling with 17 selected nurses with at least 2 years of professional experience who voluntarily agreed to participate and signed the informed consent form. Analysis of the qualitative data was conducted according to Burnard's method of content analysis. The Consolidated Criteria for Reporting Qualitative Research checklist was used. Four main categories emerged from the analysis of the data: "qualities of the role-holder," "role competencies in practice," "integration with the team," and "impact of the role" on nursing, the patient, and the organization. The findings of this study have interesting implications for the development and implementation of clinical nurse specialist practice in healthcare organizations. They also provide evidence of the benefit of implementing clinical nurse specialist practice for improving the quality of care, patient outcomes, and healthcare efficiency.

Organizational culture and readiness for evidence-based practice in the Kingdom of Saudi Arabia: A pre-experimental study.

This study aims to establish postgraduate students' perceptions of the organizational culture and readiness for evidence-based practice of their workplaces in the Kingdom of Saudi Arabia. Nurse shortages and a reliance on a transient nurse workforce have long been a challenge in the Kingdom of Saudi Arabia. Developing a home-grown nurse workforce, a key objective of the Government of Saudi Arabia, can help to address this. Evidence-based practice offers a mechanism to address this. Evidence-based practice implementation is heavily reliant on the prevailing organizational culture. Establishing the organizational culture and readiness for evidence-based practice is crucial for sustainable evidence-based practice implementation. A pre-experimental pilot study collected data from the same participants at three different points. As part of this, a questionnaire measuring organizational culture and readiness for evidence-based practice was administered twice. Descriptive, inferential and correlational statistics were employed to analyse the data. Results demonstrated improved participant perceptions of the organizational culture and readiness for evidence-based practice of their workplaces between the first (M= 76.58, SD= 19.2) and second (M= 92.10, SD= 23.68) data collection points, indicating moderate movement towards a culture of evidence-based practice. Strengths, challenges and opportunities for improvement were identified. This study established participants' perceptions of the organizational culture and readiness for evidence-based practice of their workplaces, affording insight into context-specific strategies to embed evidence-based practice in health care organizations. Assessing an organization's culture and readiness for evidence-based practice (EBP) can afford insight on the strengths, challenges and opportunities that exist to equip nurse managers to advance evidence-based practice at individual, professional and organizational levels. This study demonstrated the importance of promoting an environment conducive to EBPand putting in place the necessary resources to support evidence-based practice implementation. Nurse managers can play a central role in this.

Prediction of the complicated early neonatal period in large for gestational age newborns

The aim of the study is to develop a method for predicting the complicated course of the early neonatal period in the large newborns for gestational age in order to optimize and increase the efficiency of medical care for this category of children.A survey of 157 large newborns large for gestational age was carried out. The study group consisted of 105 large newborns for gestational age. The control of the correct operation of the regression model was carried out on an examination sample of children with large birth weight (n = 52). The control group consisted of 221 newborns with the physical development corresponding to the gestational age (appropriate for the gestational age). The anamnestic, clinical, laboratory, instrumental, morphological data were analyzed with a subsequent determination of the most significant factors associated with early adaptation disorders in newborns. The predicted event was considered to be a complicated course of the early neonatal period, which was determined by the presence of one or more diseases in a newborn.Based on the multivariate regression analysis, it was found that the most significant prognostic factors associated with the disadaptation risk of large newborns were the pregravid maternal body weight of more than 70 kg, the delivery mode, the harmonicity coefficient (ponderal index) at a birth of 26.5 kg/m3 or more, the neutrophils level in the complete blood count on the 1-2 days of life. A mathematical model was developed for determining the probability of a complicated course of the early neonatal period in the large newborns large for gestational age. The threshold value was calculated and a classification scheme was created, allowing one to calculate the infant's belonging to the risk group of the complicated course of the early neonatal period based on the calculation of points for timely correction. It is shown that the developed multivariate mathematical model and the classification scheme based on it work steadily on the examination sample and can be used in practice in health care organizations at all levels of perinatal care.

Practicing patients' rights in Iran: a review of evidence.

Protection of patients' rights is critical in improving healthcare quality, and hence this study aimed at reviewing patient rights’ practices in healthcare organizations of Iran.Using systematic search, this review was conducted based on Preferred Reporting Items for Systematic Review and Meta-Analysis Protocols (PRISMA-P). Several keywords, including "patient rights", "patient bills of rights" and "patients rights’ charter" were searched bilingually in the databases of SID, Magiran, PubMed, Scopus, and Web of Science from 2010 to 2021, and then, following a three-tier screening using the Critical Appraisal Skills Program (CASP) checklists, 76 articles were extracted. The degree of compliance with the Patients' Rights Charter (PRC) in healthcare organizations was 60.88% on average. As to the observance of the PRC dimensions, respectively, the highest and lowest scores were related to the "right to privacy and confidentiality" (70.16%) and "right to access an efficient complaining system" (53.01 %).Respect for patients' rights in organizations was assessed at a moderate level, and some aspects of patients' rights should be attended to immediately. Therefore, discrepancies in the dimensions of patients' rights and their implementation by organizations should be on the agenda of healthcare managers and policymakers.

Managerial Responses to the Onset of the COVID-19 Pandemic in Healthcare Organizations Project Management.

The goal of this study was to identify and assess the impact of the COVID-19 pandemic on project management practices in healthcare organizations, taking into account, in particular, risk analysis, project performance, organization of the work of project teams, and tendencies in future developments in project management. In order to achieve this goal, a study was conducted on 20 project managers in selected healthcare organizations in Poland. The results indicate that a systematically conducted risk analysis as part of the implemented projects enabled rapid and effective reactions during the COVID-19 pandemic. The pandemic has also significantly altered the organization of the work of project teams and, in the opinion of the research subjects, this will significantly impact future solutions for project management in healthcare organizations. The obtained results form a basis for the increase in professionalization in project management in healthcare organizations.

Exploring how key performance indicators influence nursing and midwifery practice: A mixed-methods study.

To scope the key performance indicators (KPIs) used in nursing and midwifery across the United Kingdom and Republic of Ireland and explore how they influence practice in healthcare organizations. The study adopted a sequential, exploratory mixed-methods design. Phase 1 incorporated a multiple-choice questionnaire completed by 77 Directors of Nursing recruited using voluntary response sampling. In phase 2, 35 nurses and midwives who were working at executive, senior manager and clinical levels, participated in semi-structured interviews. Data collection of both phases was conducted from January 2016 to October 2016. Quantitative data revealed over 100 nursing and midwifery-specific KPIs. National requirements were a deciding factor in KPI selection, while clinical involvement was mainly through data collection. Respondents stated that they used patient experience KPIs, but only one was assessed as valid. Thematic analysis identified two themes: The leadership challenge (including 'voiceless in the national conversation', 'aligning KPIs in the practice context' and 'listening to those who matter'); and taking action (including 'establishing ownership and engaging staff', 'checks and balances' and 'closing the loop'). The large volume of KPI measurement taking place makes meaningful evaluation of performance and quality of care difficult, both in and across organizations. Nurses and midwives require enhanced knowledge of the nature and purpose of KPIs, as evidence gained from KPI data collection is insufficient to lead to improvements in practice. A practice context that encourages collective leadership, where multiple sources of evidence are gathered and everyone is included in KPI evaluation and subsequent decision-making, is key. This study adds to the body of evidence on KPI understanding. It informs the future effective management of indicators that will facilitate the delivery of meaningful care and reduce the cost, time and effort invested in the implementation of KPIs and data management.

Green human resources management and practices in healthcare organizations

Sağlık kurumları tarafından sunulan sağlık hizmetlerinin temel hedefi insan ve toplum sağlığının geliştirilmesidir. Bu hedef, dışsal fayda olarak insanın yaşam çevresine de çok yönlü katkı sağlamaktadır. Sağlıklı bireyler, sağlıklı toplum ve çevre için olumlu katkı sağlamaktadırlar. Sağlık kurumlarında üretilen ve sunulan hizmetin ana yürütücüsü ise sağlık personelidir. Her ne kadar çok yüksek teknoloji kullanılsa da sağlık hizmeti emek yoğun bir ortamda sunulmaktadır. Sağlık işgücünün verimli ve etkili hizmet sunumu, kurumun uyguladığı insan kaynakları politikaları ile doğrudan ilişkilidir. Son yıllarda yeni uygulama alanı bulan yeşil insan kaynakları politikaları sürdürülebilir yönetim için uygulanmakta ve geliştirilmektedir. Sağlık kurumları ise faaliyetleri gereği; atık yönetim sistemleri ile çevre ve insan sağlığına duyarlı, çevre yönetim sistemlerini geliştirmektedir. Sağlık kurumlarında çevre yönetim sisteminin yeşil insan kaynakları uygulamaları ile geliştirilmesi ve uygulanması, sağlık hizmetlerinin dışsal faydalarının daha belirgin olmasına sebebiyet verecektir. Nitekim yeşil insan kaynakları politikalarının amaçlarından birisi de dış paydaşlar için olumlu faydalar sunabilmektir. Sağlık personelinin istihdamından başlanmak üzere, yeşil hedefler ve duyarlılık ile kurumda çalışmasının sağlanması, insan kaynakları yönetiminin önemli bir başarısı olacaktır. Bu başarı sağlık kurumlarının olağanüstü durumlara da (salgın, afet, kriz vb.) uyumlaşmasını kolaylaştırabilecektir. Aynı zamanda geleneksel İKY uygulamaları ile beraber yeşil İKY uygulamalarının sağlık kurumları tarafından benimsenmesi, personelin motivasyonunu yükselterek, örgütsel sürdürülebilirliği sağlamaya yardımcı olabilecektir. Çevresel duyarlılık ve faaliyet ekseninde hizmet sunulması da sağlık kuruluşlarının toplumsal imajı ve yararlılığı açısından son derece önemlidir. Bu çalışmada daha çok üretim işletmeleri tarafından benimsenmekte olan yeşil insan kaynakları politikalarının, sağlık kurumları tarafından uygulanabilirliği incelenmiştir. Aynı zamanda Türkçe yazına teorik bir çerçevede, yeşil insan kaynakları politikalarının sağlık kurumlarında uygulanabilirliği noktasında katkı sağlamak ve farkındalık oluşturmak amaçlanmıştır.

A Systematic Review on Lean Applications' in Emergency Departments.

This article presents the state of the art of Lean principles applied in Emergency Departments through a systematic literature review. Our article extends previous work found in the literature to respond to the following questions: (i) What research problems in emergency departments can Lean principles help overcome? (ii) What Lean approaches and tools are used most often in this environment? (iii) What are the results and benefits obtained by these practices? and (iv) What research opportunities appear as gaps in the current state of the art on the subject? A six-step systematic review was performed following the guidance of the PRISMA method. The review analysis identified six main research problems where Lean was applied in Emergency Departments: (i) High Waiting Time and High Length of Hospital Stay; (ii) Health Safety; (iii) Process redesign; (iv) Management and Lessons Learned; (v) High Patient Flow; (vi) Cost Analysis. The six research problems’ main approaches identified were Lean Thinking, Multidisciplinary, Statistics, and Six Sigma. The leading Lean tools and methodologies were VSM, Teamwork, DMAIC, and Kaizen. The main benefits of applying Lean Principles were (a) reductions in waiting time, costs, length of hospital stay, patient flow, and procedure times; and (b) improvements in patient satisfaction, efficiency, productivity, standardization, relationships, safety, quality, and cost savings. Multidisciplinary integration of managers and work teams often yields good results. Finally, this study identifies knowledge gaps and new opportunities to study Lean best practices in healthcare organizations.

Linkage between performance of healthcare professionals and management practices in health care organizations.

The performance of healthcare professionals is now becoming a great apprehension for health care organizations to deliver quality services. Therefore, organizations are considering management practices to effectively manage their human resource, as they are an essential resource to deliver services through their performance. The objective of the current paper was to describe the link between management practices and performance of healthcare professionals in healthcare setup. In addition, based on the review, it has been found that recruitment & selection, training & development, performance appraisal along with compensation, practices of health and safety, as well as, flexible working hours play a crucial role in the enhancement of employee performance, whereas ineffective execution of these practices has a negative influence on the performance of healthcare professionals. Thus, the management of the health care segment should focus on the timely and fair implementation of the above-mentioned management practices in order to improve the performance of their employed personnel.

Improving healthcare: a guide to roll-out best practices.

Best practices comprise recent, relevant, and helpful nursing practices, methods, interventions, procedures or techniques based on high-quality evidence. Best practices should be implemented to improve individual patients' health outcomes the overall quality of health care, and to strengthen the health system at large. Four facilitators (benefit levers) to effectively roll-out best practices in healthcare organizations were identified: alignment, permeation plans, leadership for change, as well as supporting and reinforcing structures. To develop and review a guide for an operational plan to facilitate the roll-out of best practices in healthcare organizations. The content of the guide was based on findings of an integrative literature review and interviews. This article discusses the development of the guide using the logic model as format (phase One) as well as its review including five key informants using the Delphi method (phase Two). The Logic Model provided a feasible format for the guide. Two Delphi rounds were required to review the guide's design and content. This guide addresses pre-roll-out resources; pre-roll-out activities, best practices (outcomes) during and after roll-outs; and impact. The guide should be tested in different healthcare organizations and adapted accordingly to have an impact on improved patient and health outcomes. The guide can be used by managers wishing to roll-out best practices in their healthcare organizations to achieve improved health outcomes for individual patients.

Quality management in healthcare organizations: Empirical evidence from the baldrige data

The purpose of this paper is to investigate the determinants of customer satisfaction and quality results in the healthcare industry using the Baldrige data. We use publicly available data on quality assessment of healthcare organizations that applied for the Baldrige award to examine two research questions: 1) whether the Baldrige model is a reliable and valid model for assessment of quality practices in healthcare organizations, and 2) to determine the relationship between quality practices and their impact on quality results in the healthcare organizations. Using structural equation modeling, the findings suggest that the Baldrige model is a valid and reliable quality assessment model for healthcare organizations. Consistent with previous studies, the findings suggest that the main driver of the system is leadership, which has a significant effect on all quality practices in the healthcare industry. Controlling for the applicants’ year, the findings indicate that 1) Information analysis and knowledge management has a significant impact on Quality results, 2) Workforce development and human resource management has a significant impact on both Customer focus and satisfaction and Quality results, and 3) Strategic planning for quality has a significant impact on Customer focus and satisfaction. The study provides insights and suggestions for healthcare organizations on how to improve their quality systems using the Baldrige model.

Assessment methodology for Lean Practices in healthcare organizations: case study in a Brazilian public hospital

Abstract Paper aims This article aims at proposing a methodology for assessing lean practices in healthcare organizations that are undergoing a lean implementation. Originality The proposed methodology evaluates healthcare organizations and help managers identify the relationships intensities between their problems and lean practices. This application occurs in a case study in an Emergency Department from a Brazilian public hospital. Research method The proposed methodology provides a theoretical framework that addresses lean practices according to fundamental aspects of the organization, such as people, partnerships, results, processes and leadership, corroborating to a systemic view of lean implementation and facilitating its long-term sustainability. Main findings Our findings provide guidelines for the public hospital leadership, allowing a clear focus on the improvement efforts that can mitigate problems. Implications for theory and practice Due to contextual diversity, the understanding of the critical success factors for lean implementation under such conditions deserves attention. Hence, the proposed method allows practitioners and academics to better comprehend the lean implementation within healthcare organizations. Such enhanced comprehension allows to anticipate potential issues throughout the lean implementation, entailing more assertive efforts.
