Objectives This study is intended to explore nursing students’ experiences of practice in a long-term care hospital by analyzing the self reflective journals written by nursing students after practice. The intent is to describe and understand students’ perspectives on their major practice experiences.
 Methods A content analysis method was employed to analyze the reflective journals written by 97 fourth-year nursing students who completed five days in a long-term care hospital practicum between March and May 2023.
 Results Based on nursing students’ experiences of practice in long-term care hospital, 21 themes were placed into five categories: “Thoughts before practice,” “What I liked during practice,” “Difficulties during practice,” “What could have been better during practice,” and “What I learned through practice.”
 Conclusions This study’s results offered an insight into nursing students’ experiences of practice in a long-term care hospital. The findings underscore the necessity of efforts to standardize the gerontological nursing curriculum for nursing students along with the efficient operation of gerontological nursing practice.

Measuring gerontological nursing competencies among aged care nurses: Cultural adaptation and psychometric validation.

Existing gerontological nursing competencies include the requisite knowledge and skills required for aged care nursing. What were not previously considered were legal and ethical aspects and access to technology, e-health and social media. This study aimed to validate an Australian gerontological nursing competencies scale and assess the factors associated with for aged care nurses in Taiwan. A methodological study design was used to validate the scale with a sample of 369 aged care nurses from aged care settings including nursing homes, long-term care facilities and aged care wards in Taiwan. The cultural adaptation and psychometric validation were evaluated. The content validity, construct validity through exploratory factor analysis and the internal consistency of the scale were assessed. The exploratory factor analysis extracted two levels of gerontological nursing practice, 'essential' and 'enhanced', which accounted for 80.8% of the total variance. The internal consistency, split-half reliability and test-retest reliability were excellent. Aged care nurses with higher levels of education, aged care education as their highest degree and continued education within 6 months, and certified long-term care education had higher gerontological nursing competencies scores than those who did not. This validated gerontological nursing competencies scale is a reliable and valid tool that can be implemented in future workforce planning, research and postgraduate and undergraduate curriculum and education in Taiwan and other Mandarin-speaking locales. Using validated gerontological nursing competencies scales to explain different levels of specialist practice is crucial for addressing negative views about aged nursing and explicitly demonstrate the possibilities for career pathways in gerontological nursing.

Developing a competence framework for gerontological nursing in China: a two-phase research design including a needs analysis and verification study

BackgroundChina faces a serious shortage of competent nurses who can address the healthcare needs of older people in an ageing society. Chinese higher education institutes face serious challenges when it comes to developing new curricula that are capable of educating sufficient numbers of competent gerontological nurses. Therefore, the aim of this research study was to identify and verify competencies for gerontological nurses in China that are needed to provide nursing care for the growing number of older people in all care settings. This study takes into account the possible opportunities that trends and developments may offer in the near future.MethodsIn this study, a two-phase research design was used. The first phase concerned needs analysis, including a situational analysis, a trend analysis and a competence analysis. This process resulted in a draft competence framework. The second phase addressed the verification of the competence framework through a two-round Delphi study with a panel of Chinese and European experts. This process led to the final competence framework.ResultsThe final competence framework for gerontological nursing in China included six competencies divided into 13 essential and five relevant learning outcomes. The competencies are: ‘providing gerontological care’, ‘communication and collaboration’, ‘organization of gerontological nursing care’, ‘health promotion’, ‘evidence-based nursing and lifelong learning’ and ‘professional behaviour’.ConclusionThe framework comprehensively covers the six core competencies that nurses who care for older people should possess. These competencies are well-embedded in a Chinese context. The framework therefore offers concrete, practical suggestions for the competencies and skills that nursing graduates will need to work in current and future professions related to gerontological nursing education and practice.

Objectives The purpose of this study was to explore the experiences and perceptions of nursing students on their potential as gerontological nursing practice educational medium that can enhance nursing students’ caring competency in using the storytelling platform for the elderly in nursing homes before developing and applying an intergenerational storytelling platform for the elderly in nursing homes. Methods A total of 6 groups were composed of two 2nd grade groups of nursing university and four 3rd grade groups, and the number of people per group was 4-5. A total of 28 participants conducted focus group interviews and the results were derived through qualitative data analysis. Qualitative data were obtained using the Zoom application through six focus group interviews consisting of 28 nursing students finishing 2-3 grade courses at a national university in Gyeongsang province. Participants completed interviews from January 16 to February 21, 2022. All interviews were recorded, transcribed, and analyzed by employing the systemic qualitative analysis method. Results The current study used the six phases of thematic analysis proposed by Braun & Clarke. The analysis revealed a total of six topics, seventeen categories, and 36 subcategories. The main results of the study showed that the use of a storytelling platform could increase creativity and decrease feelings of isolation among the elderly, resolute ageism among students, and improve empathy, and the possibility of change in nursing home culture. Also, a storytelling platform with digital technology would be used as an educational medium connected and shared between nursing schools, nursing homes, and the community. Conclusions When constructing a storytelling platform for the elderly in nursing homes, based on connectivity, familiarity, common experience, and interest, with being archived, re-created, and connected, this storytelling platform could not only bridge the generation gap but also contribute to the development of students’ gerontological nursing competency.

Nursing Students' Experiences of Virtual and Hybrid Simulation in Gerontological Nursing: A Mixed-Methods Study

Purpose: This study aimed to examine the experiences of nursing students who took a clinical practicum in geriatric nursing through virtual and hybrid simulation.Methods: The study employed a mixed-method design. Participants in quantitative research were 94 nursing students (Virtual simulation group: 45, Hybrid simulation group: 49). Qualitative research was conducted with 4 focus groups (7 students per group). Quantitative data were analyzed using x2 and t-test with SPSS/WIN 23.0. Qualitative data were analyzed using content analysis.Results: The hybrid simulation group had significantly more interest in gerontological nursing (t=10.45, p=.002), sharing opinions (t=6.93, p=.010), and recognition of one's own strengths and weaknesses (t=15.90, p<.001) than the virtual simulation group. The two simulation training experiences were shown in five themes: (1) space and relationship, (2) role differences as learners, (3) higher-order thinking skills, (4) embodied knowledge by acquisition, and (5) differences in perspectives on patients. Overall, the learning attitude according to the hybrid simulation was reported to be higher than that of the virtual simulation.Conclusion: This study is meaningful in that it suggests an effective method for nursing practice for older adults by comparing the experience of virtual simulation and hybrid simulation methods in clinical practice of gerontological nursing.

Advancing Gerontological Nursing at the Intersection of Age-Friendly Communities, Health Systems, and Public Health.

Mounting efforts to improve care and promote healthy aging throughout society and across the care continuum have created unique opportunities for gerontological nursing practice. Population aging has invoked a multitude of responses among all levels of international and national organizations, foundations, health care, and government to meet the needs and promote preferences of older adults. Large-scale programs by the World Health Organization, The John A. Hartford Foundation, Institute for Health-care Improvement, and Trust for America's Health have galvanized to advance the momentum of age-friendly communities, health care, and public health. Gerontological nurses can leverage this growing interest in aging by enhancing their knowledge about age-friendly movements, influencing these movements with their expertise in evidence-based practices, and advancing their own competencies in caring for older adults in any setting. Older adult care in nursing education: How have curricula been developed?

Given the rapidly growing older adult population, future nurses should increase their knowledge and skills in gerontological nursing to deliver high-quality care to older adults. The aim of this national survey (n = 40 nursing schools) was to analyse the status of gerontology education in Portuguese baccalaureate nursing programmes. Data were collected using a 51-item questionnaire about baccalaureate nursing education programmes and gerontology-related topics. Descriptive analysis was used. A total of 18 nursing schools returned the questionnaire (response rate = 45%). Results showed that 66.7% (n = 12) of nursing schools integrated gerontology content into several courses, 38.9% (n = 7) of them had stand-alone courses, and 11.1% (n = 2) of them had both options. The most significant factor inhibiting the development of the gerontological nursing curriculum was the negative image of gerontological nursing (44.4%). Gerontological-related competencies were identified in only two nursing programmes. Thirteen schools reported needing help to strengthen the gerontological content in the nursing curriculum. This study has demonstrated that gerontological content is covered in the nursing curriculum of all nursing schools. The increase of knowledge and skills in gerontological nursing and the development of a standard gerontological curriculum could contribute to enhancing gerontological nursing education and practice.

AbstractPurposeThe present study aimed to reveal trends in simulated education in Japanese gerontological nursing by conducting a text mining analysis of the Igaku Chuo Zasshi databank.MethodsSimulated education is focused by field of Japanese nursing education in recently. Therefore, we searched for original articles containing the terms “Simulated education” and “gerontological nursing” which were published between 2014and 2019. Articles containing these terms, as well as thesaurus words in the articles, were analyzed using Trend Search. The analysis provided a conceptual map of related words, with the strength of the relationship reflected in the thickness of lines and the distance between them.Igaku Chuo ZasshiA literature search was conducted using Igaku Chuo Zasshi, the Japanese medical literature database provided by the NPO Japan Medical Abstracts Society. This database comprises approximately 5000 journal titles and 6,300,000 articles.Text mining softwareTrend Search is a commercially available software developed by FUJITSU. Articles were anonymized prior to analysis.Ethical considerationsOnly anonymous articles were selected.Results and ConclusionsThe search identified 15 articles. Mapping yielded the two wedges of [Learning] and [Simulated game]. The [Learning] wedge had three groups ( Introduce, Communications, and Aging ). Introduce refers to nursing processes via simulated patients or role play. Communications refers to practice in gerontological nursing. Aging refers to role playing with group works in fundamental nursing. The [Simulated game] wedge had two groups (Effects and On-the-job training). Effects refers to leaving bed or psychographics. On-the-job training refers to simulation of elderly because of understanding.

Nurses' provisions for self-determination when caring for clients with cognitive impairment who live in a residential aged care facility: a scoping review protocol.

The objective of this scoping review is to examine and map knowledge of nursing provisions for self-determination while providing care to clients with cognitive impairment in residential aged care facilities. Maintaining the ability to have choices and to make decisions about daily activities is important for older adults. In residential aged care facilities, nurses' can be challenged to preserve clients' self-determination in favor of ritualistic care routines and a perceived duty to care. Moreover, nurses may perceive that their professional responsibilities to protect clients requires them to guard against decisions that are considered unwise or pose a risk to clients' health or safety. Insight into how nurses negotiate choice with clients with cognitive impairment who are living in a residential care facility will provide an in-depth understanding of the role self-determination plays in clients' lives. The scoping review will consider research and narrative reports on nursing provisions for self-determination in clients with cognitive impairment who are living in a residential aged care facility. The concepts of interest are self-determination and nursing provisions. Self-determination is defined as client choice and nursing provisions are the intentional reactions to clients' expressed choices. This scoping review will aim to locate published and unpublished literature employing a three-step search strategy. Only papers published in English from 1995 onward will be included. Data extracted from included papers will outline details on the participants, context, strategy, activity, and outcomes. Extracted data will be reported in a tabular form and presented narratively to address the review objective.

Student nurses' career motivation toward gerontological nursing: A longitudinal study

BackgroundThe intensity of student nurses' motivation toward a certain career can influence their behaviors for career choice and decision making. However, little is known about the career motivation among student nurses toward gerontological nursing and their influencing factors. ObjectivesThe objectives of this study were to examine student nurses' career motivation toward gerontological nursing and to explore potential predictors using Bandura's reciprocal determinism. DesignA longitudinal design was used in this study. Settings and participantsA total of 305 student nurses were recruited from six hospitals in Shandong province, and they completed the questionnaires over three stages. MethodsStudent nurses' career motivation toward gerontological nursing, geriatrics attitudes, aging anxiety, empathy, and clinical practice environment were measured using the motivation questionnaire, the Geriatrics Attitudes Scale, the Anxiety about Aging Scale, the Interpersonal Reactivity Index, and the clinical practice environment questionnaire, respectively. Measurements were performed at the first month, fourth month, and seventh month of student nurses' clinical practice. A series of Pearson correlation, multiple linear regression, and generalized estimating equations were applied to analyze the data. ResultsAmong 305 student nurses, the mean scores for career motivation toward gerontological nursing over three stages were 66.11 (SD = 8.98), 67.55 (SD = 8.53) and 66.78 (SD = 9.51), sequentially. There were significant differences in scores for career motivation over time (P < 0.05) and the lowest score was observed at the first month. Geriatric attitudes, aging anxiety, empathy, clinical practice environment and stage of clinical practice were notable predictors of student nurses' career motivation toward gerontological nursing. ConclusionsStudent nurses' career motivation toward gerontological nursing needs to be improved, especially at the first month of clinical practice. Targeted interventions should cultivate positive geriatric attitudes, relieve anxiety about aging, promote empathy, and enrich the clinical practice environment to improve student nurses' career motivation toward gerontological nursing.

Idoso no mercado de trabalho: implicações para a Enfermagem Gerontológica

Objetivo: refletir sobre os aspectos determinantes à manutenção/reinserção do idoso no mercado de trabalho, considerando as implicações à enfermagem gerontológica. Método: estudo teórico-reflexivo sobre o idoso no mercado de trabalho e suas repercussões à prática da enfermagem gerontológica. Resultados: há uma presença crescente dos idosos no mercado de trabalho em decorrência da necessidade de manutenção do papel social, questões previdenciárias, perda do poder aquisitivo e necessidade de prover a família, assim, tornando maiores e mais específicas as demandas de cuidado na área da enfermagem gerontológica que devem contemplar aspectos relacionados ao ambiente de trabalho e ao processo de envelhecimento. Conclusão: a enfermagem gerontológica possui inúmeras possibilidades de prestar suas habilidades e competências na área da saúde do trabalhador voltadas às especificidades do trabalhador idoso com foco no fortalecimento do conhecimento relacionado à tríade envelhecimento/velhice/trabalho.

Practice development of gerontological nursing in the US and its enlightenment to China

Through the introduction of the gerontological nursing practice standards, the development course of practice, the scope of practice and gerontological nursing certification examination outline of American Nurses Credentialing Center in the US, the opinion about the development of gerontological nursing practice in our country is proposed. It is necessary to accelerate the cultivation of geriatric specialist nursing talent, and establishment of the scope and standards of gerontological nursing practice and geriatric specialized nurse certification system to cope with the development of aging society and improve gerontological nursing professional level in our country. Learning about the experience of living with chronic conditions: A framework analysis of nursing students’ reflections on their conversations with older adults

ABSTRACTPreparing nursing students to work effectively with older adults is an important element of undergraduate nursing education. Reflective journals written as course assignments represent a source of data about students’ learning. The aim of this study was to analyze nursing students’ reflective journals based on conversations with older adults who are community dwelling, to identify learning in relation to experiences of living with a chronic condition. This was one element of a wider study to evaluate learning partnerships between older adults who are community dwelling and nursing students as an alternative to traditional clinical placements in long-term care settings. Nursing students worked with faculty to complete a qualitative secondary analysis of 47 journals utilizing framework analysis. Students learned about three main aspects of living with chronic conditions: beliefs about chronic conditions, managing chronic conditions, and experiencing chronic conditions, with some evidence that these themes may be interconnected. Findings suggest that learning partnerships with older adults combined with reflective journaling enable nursing students to understand the experience of chronic conditions in later life as reflected within current literature and to identify interventions for gerontological nursing practice. Enabling nursing students to recognize the highly skilled nature of working with older adults with chronic illness has the potential to influence their future career choices.

Lifelong Health and Health Services Use: A New Focus for Gerontological Nursing Research and Practice.

It is commonly believed that older adults are often ill and therefore high users of health services. A pilot study involving adults 60 and older living in the Canadian province of Alberta was conducted to (a) raise interest in the concepts of lifelong health and health services use; (b) develop a lifelong health data collection tool; (c) obtain information about lifelong and recent health services use, and self-perceived lifelong and current health; and (d) reveal links (if any) between perceived health and lifelong health services use. A questionnaire was developed, pilot tested, and posted for completion by 100 volunteers. Most older adults reported good or very good current and lifelong health. Comorbidities and low finances were associated with higher health services use. These findings suggest community-dwelling older adults may be healthy currently and throughout their lives. Gerontological nursing research, practice, and advocacy are needed because myths about aging must be addressed to refocus attention on the importance of lifelong health promotion for older adults. 액션러닝을 활용한 노인간호학실습 수업 운영의 효과

본 연구는 노인간호학실습 교육에 액션러닝의 팀 학습 방법을 활용한 교육과정을 운영하고 간호대학생의 문제해결능력, 의사소통능력 및 팀 효능감에 미치는 영향을 알아보고자 시도되었다. 연구대상은 2015년 3월 9일부터 2015년 6월 19일까지 Y대학 간호학과의 노인간호학 실습 교과목을 수강신청한 간호학과 3학년생 전체를 편의표집 대상으로 하여 실험군은 A, B병원으로 실습을 나가는 학생들 중에서 연구의 목적을 이해하고 참여에 동의한 학생 40명, 대조군은 동일 간호대학 3학년 학생들 가운데 실험군과 겹치지 않는 병원(C, D, E병원)에 실습을 낙가는 학생 40명을 선정하여 액션러닝을 2주간 적용하였다. 수집된 자료는 SPSS/WIN 21.0 program을 이용하여 하여 일반적 특성과 실험군과 대조군의 사전 동질성 검증은 <TEX>${\chi}^2$</TEX>-test, t-test, 실험처치 효과에 대한 검증은 independent t-test를 실시하여 분석하였다. 액션러닝을 적용한 결과 실험군은 대조군에 비해 문제해결능력, 의사소통능력 점수는 유의하게 향상되었으나, 팀 효능감은 유의한 차이가 없었다. 결론적으로 액션러닝은 간호학생들의 문제해결능력 및 의사소통능력의 향상을 가져올 수 있고 학습성과 기반 교육과정에서 매우 유용한 교수학습방법이 될 수 있을 것이라 본다. This study was conducted to examine the effects of a gerontological nursing clinical practice course using action learning on undergraduate nursing students. The subjects were 75 undergraduate nursing students from Y University. Data were analyzed using SPSS/Win 21.0. There were significant improvements in problem solving (t=3.58, p<.001) and communication (t=4.15, p<.000) in the experimental group compared to the control group. This study provides evidence that gerontological nursing courses improve undergraduate nursing students' problem solving and communication skills. Accordingly, this course would be a useful teaching and learning method in nursing programs of outcome based curriculum.

Analysis of the effect that nursing students acting as social volunteers to participate in gerontological nursing practice teaching

Objective To compare the effect of nursing students acting as social volunteers to participate in gerontological nursing practice teaching with traditional practice teaching mode, and promote the reform of teaching mode in nursing practice. Methods Students of two classes from department of nursing, medical college of Wuhan University of Science and Technology were selected as research objects. In class one nursing students acted as social volunteers to participate in gerontological nursing practice teaching (the experimental group), In class two nursing students took practice in geriatric wards according to traditional practice mode(the control group). The practice effect was compared between two groups. Results The average final grade of the experimental group was 86.50, and 78.51 in the control group. Ridit analysis revealed that, setting class one as the experimental group, R=0.796 8, 95% confidence interval was 0.715 1-0.878 4, in the control group, R =0.500 0. The investigation showed that the experimental group and the control group were different in moral elevation aspect, teaching level, capability improving aspect and employment promotion aspect, the difference was significant (P <0.01). Conclusions The mode of nursing students acting as social volunteers to participate in gerontological nursing practice teaching has advantage over traditional practice teaching, and it can gain satisfying teaching effect. Student nurses' motivation to choose gerontological nursing as a career in China: A survey study

The world's population is aging, and the need for nurses is increasing. Working with older adults, however, has always been an unpopular career choice among student nurses. It is important to understand student nurses' motivation for choosing gerontological nursing as a career. The purpose of this study was to examine the motivation for choosing gerontological nursing as a career and to identify the associated factors among student nurses. Cross-sectional survey. Participants were last-semester student nurses from 7 universities offering nursing undergraduate programs in Shandong, China. Of the 1290 student nurses, 916 completed the survey (a response rate of 71.0%). The outcome variable was the motivation to choose gerontological nursing as a career. This was measured using a motivation questionnaire that included expectancy and value subscales. Other instruments included the Chinese version of the Facts on Aging Quiz I, the Geriatrics Attitudes Scale, the Anxiety about Aging Scale, a clinical practice environment questionnaire and a self-administered general information questionnaire. Student nurses' expectancy and value aspects of motivation for choosing gerontological nursing as a career were both at a moderate level; the highest value they held was of personal interest. Clinical practice environment, anxiety about aging and the attitudes about geriatrics were the main factors influencing student nurses' motivation to choose gerontological nursing as a career in China. It is imperative for nurse educators to improve the gerontological nursing clinical practice environment for student nurses. Moreover, cultivating student nurses' positive attitudes about geriatrics and relieving anxiety about aging could be beneficial.
